Usefulness of X-ray Guided Biopsy and Ultrasound Guided Biopsy in Breast Microcalcification Biopsy (유방 미세석회화 조직검사에서 X선 유도 하 조직검사와 초음파 유도 하 조직검사의 유용성)

  • Social interest in breast cancer has increased. The most basic exams for diagnosis include breast X-ray and breast ultrasound. In particular, breast microcalcification requires histological diagnosis, and breast microcalcification biopsy is commonly performed. Therefore, this study aimed to analyze and assess X-ray guided biopsy (needle localized open biopsy) and ultrasound guided biopsy (sono guided core needle biopsy), which are basics in diagnosis of microcalcification. Targeting 241 cases in which magnification mammography was performed for patients who visited the hospital due to breast microcalcification, age distribution and the location of lesions were analyzed in X-ray guided biopsy and ultrasound guided biopsy. By classifying exams performed after magnification mammography, the frequencies of X-ray guided biopsy and ultrasound guided biopsy were analyzed, and malignant and benign results were confirmed. The results showed that 64 cases(26.6%) were X-ray guided biopsy, which was 5.4 times higher than 12 cases(4.9%) of ultrasound guided biopsy. Due to development of ultrasound equipments, stereotactic vacuum-assisted biopsy, etc. the methods of histological diagnosis of microcalcification have become diverse, but when considering characteristics and limitations of each exam, X-ray guided biopsy is thought to be most accurate and useful.

Factor analysis using S-detect Method in Breast Ultrasound (유방 초음파 검사 시 S-detect 방법을 활용한 인자 분석)

  • This study aimed to investigate the performance of the S-detect method in breast ultrasonography and to determine how to reduce unnecessary biopsy by comparing the results of the S-detect method and biopsy. Thirty patients who had undergone breast ultrasonography between August and October 2018 and were scheduled to undergo biopsy because of the presence of breast nodules were retrospectively analyzed. The McNemar test was performed to determine whether detection of a malignant breast mass significantly differed between the S-detect method and biopsy. The following results were obtained from the analysis of the S-detect method: sensitivity, 90.9%; specificity, 84.21%; validity, 86.66%; positive predictive value, 76.92%; and negative predictive value, 94.11%. Analysis of the degree of agreement between the S-detect method and biopsy revealed a kappa value as high as 0.724 (p < 0.05), exhibiting good agreement between the two methods. The S-detect method in breast ultrasonography is diagnostically valuable in terms of distinguishing between malignant and benign breast masses, and if used properly before breast biopsy, unnecessary biopsy can be reduced.

  • The magnetic valve of the breast tissue expander generates imaging artifacts during MRI examination, so MRI examination is limited. To evaluate the effect of imaging artifacts on the diagnosis area for patients with breast tissue expander who need MRI examination. Imaging artifacts were measured using self-made phantoms and actual clinical conditions. Imaging artifacts were measured differently depending on the environment of 1.5 Tesla and 3.0 Tesla, and the effects of imaging artifacts were less in the C-spine and L-spine tests. If MRI due to breast cancer metastasis is absolutely necessary, head & neck examination and L-spine can be examined mainly at 1.5 Tesla, but some sequences may cause distortion due to image artifacts. In terms of safety, MRI scans of patients with breast tissue expanders can be performed conditionally at 1.5T, avoiding 3.0T.

The Variation of Scan Time According to Patient's Breast Size and Body Mass Index in Breast Sentinel lymphangiography (유방암의 감시림프절 검사에서 유방크기와 체질량지수에 따른 검사시간 변화)

  • Purpose : At this time, the sentinel lymph node mapping using radioisotope and blue dye is preceded for breast cancer patient's sentinel lymph node biopsy. But all patients were applied the same protocol without consideration of physical specific character like the breast sizes and body mass indexes. The purpose of this study is search the optimized scan time in breast sentinel lymphangiography by observing how much the body mass index and breast size influence speed of lymphatic flow. Materials and Methods : The Object of this study was 100 breast cancer patients(Female, 100 persons, average age $50.34{\pm}10.26$ years old)at Severance hospital from October 2011 to December 2011. They were scanned breast sentinel lymphangiography before operation. This study was performed on Forte dual heads gamma camera (Philips Medical Systems, Nederland B.V.). All patients were intra-dermal injected $^{99m}Tc$-Phytate 18.5 MBq, 0.5 ml. For 80 patients, we have scanned without limitation of scan time until the lymphatic flow from the lymph node since injection. We measured how long the lymphatic flow time between departures from injects site and arrival to lymph node using stopwatch. After we calculated patient's Body mass Index and classified as 4 groups. And we measured patient's breast size and classified 3 groups. The modified breast lymphangiography that changing scan time according to comparison study's result was performed on 20 patients and was estimated. Results : The mean scan time as breast size was A group 2.48 minutes, B group 7.69 minutes, C group 10.43 minutes. The mean scan time as body mass index was under weight 1.35 minutes, normal weight 2.56 minutes, slightly over 5.62 minutes, over weighted 5.62 minutes. The success rate of modified breast lymphangiography was 85%. Conclusion : As the Body mass index became higher and breast size became bigger, the total scan time is increased. Based on the obtained information, we designed modified breast lymphangiography protocol. At the cases applying that protocol, most of sentinel lymph nodes were visualized as lymphatic pool. In conclusion, we found that the more success rate in modified protocol considering physical individuality than study carrying out in the same protocol.

Observation with Calcifications of Breast Tissue Phantoms Using Acoustic Resonance (공명현상을 이용한 유방조직 팬텀의 석회화 관찰)

  • Diagnosis of breast ultrasound is better than mammography in the early detection of breast cancer, but, it is difficult to detect microcalcification. We studied on detection for calcification of breast tissue using acoustic resonance and power doppler with 7.5 MHz linear probe in breast ultrasound. We first constructed breast tissue phantom made of gelatin and saw breast, and then observed calcification by the change of external vibration. Calcification injected breast tissue phantom visualized the difference for brightness and region of color in ROI regions of power doppler. Acoustic resonance almost never visualized in low frequency regions, plateau constituted in about 300-400 Hz and colors vanished according to the increase of frequency.

Association Between Body Mass Index and Breast Density Using Digital Mammography (체질량지수와 디지털유방촬영술상의 유방밀도와의 연관성)

  • It is well known that low body mass index and younger age are associated with high breast density. Mammographic dense breast has been reported both as a cause of false-negative findings on mammography and as an indicator of increased breast cancer risk. The purpose of this study was to evaluate the association between breast density and body mass index. Furthermore, we considered proper screening method of breast cancer in Korean women. The study was performed on 496 women who underwent health checkup in a university hospital. Age and body mass index were negatively associated with breast density respectively. In postmenopausal women, age and body mass index showed statistically significant association with breast density. Therefore, we should consider sensitive additional method for breast cancer screening especially in younger age and underweight women.

Digital Breast Tomosynthesis as a Breast Cancer Screening Tool for Women with Gynecologic Cancer (부인암을 가진 여성에서 유방암의 선별검사로서의 디지털 유방단층 촬영술)

  • Purpose The purpose of our study was to evaluate digital breast tomosynthesis as a breast cancer screening modality for women with gynecologic cancer. Materials and Methods This retrospective study included patients with underlying gynecologic malignancies who underwent screening digital breast tomosynthesis for breast cancer. The cancer detection rate, recall rate, sensitivity, specificity, and positive predictive value (PPV) were calculated. PPV1 was defined as the percentage of all positive screening exams that have a tissue diagnosis of cancer within a year. PPV2 was defined as the percentage of all diagnostic exams (and Breast Imaging Reporting and Data System category 4, 5 from screening setting) with a recommendation for tissue diagnosis that have cancer within a year. PPV3 was defined as the percentage of all known biopsies actually performed that resulted in a tissue diagnosis of cancer within the year. For each case of screen-detected cancer, we analyzed the age, type of underlying gynecologic malignancy, breast density, imaging features, final Breast Imaging Reporting and Data System assessment, histologic type, T and N stages, molecular subtype, and Ki-67 index. Results Among 508 patients, 7 with breast cancer were identified after a positive result. The cancer detection rate was 13.8 per 1000 screening exams, and the recall rate was 17.9%. The sensitivity was 100%, and the specificity was 83.2%. The false negative rate was 0 per 1000 exams. The PPV1, PPV2, and PPV3 were 7.7, 31.8, and 31.8, respectively. Conclusion Digital breast tomosynthesis may be a promising breast cancer screening modality for women with gynecologic cancer, based on the high cancer detection rate, high sensitivity, high PPV, and high detection rate of early-stage cancer observed in our study.

A Study on Availability about Positron Emission Mammography of Breast Cancer (유방암 환자의 Positron Emission Mammography에서의 유용성에 관한 연구)

  • Purpose: Until now the general study for breast cancer patient has been mammography, breast sonography for anatomic diagnostics and $^{18}F$-FDG whole body PET for functional one. But the PEM (Positron Emission Mammography) was developed to increase sensitivity, specificity and accuracy to improve the disadvantage of each study. Therefore this present study aims to describe availability of PEM for improving diagnosis of breast cancer. Materials and Methods: During 3 months from January in 2010, PEM was performed on 100 patients who had history of breast cancer. Using Naviscan's PEM Flex Solo II scanner, PEM images of breast were acquired. And then we evaluated sensitivity, specificity and accuracy of PEM data by comparing results of PEM images with postoperative pathologic finding. Results: On the 100 patients, it could reveal 89 true positive, 9 false positive, 7 false negative and 87 true negative. Thus the results of sensitivity, specificity and accuracy for PEM was evaluated 92.7%, 90.6% and 91.7%. Conclusion: The sensitivity, specificity and accuracy have been reported 83.7%, 68.5% and 77.1% in mammography, and that was 89.1%, 79.1% and 83.4% in breast sonography. However the results of PEM were more increased than others. Therefore the results of this study will be available that PEM used diagnosis with breast cancer, used preoperative operation plan and helps to discovery of a part recurrence.

  • The purpose of this study is to evaluate the breast tool to improve the diagnostic value of the image in the breast examination. Breast tool was made of using FRP. And then it was compared by radioactivity counting rate and image. In the evaluation of the Breast tool, the left and right counts per $1{\mu}Ci$ are 185 counts and 189 counts, respectively. The image obtained in the prone position was close to the circle. To increase diagnostic value of image, it is considered to use Breast-tool in the breast examination.

  • Upper gastrointestinal series is an examination that uses X-rays. It is important to defend against exposure to radiation during upper gastrointestinal examination because the organs, such as thyroid gland, lens, breasts, and gonads, with relatively high biological sensitivity to radiation are distributed on the examination area. We have made a whole body phantom that can change the depth of organs. radiation dose of eye, thyroid gland, breast and gonads were measured by the same procedure as the actual upper gastrointestinal examination. When performed only fluoroscopy the mean dose reduction of lens, thyroid gland, breast and gonads was 62.2%. The mean dose reduction of lens, thyroid gland, breast and gonads was 59.0% when both fluoroscopy and spot shoot were performed. Therefore, when performed upper gastrointestinal examination it was confirmed that shielding of the lens, thyroid gland, breast and gonads was effective in decreasing the exposure dose. The manufactured human phantom can be used in measuring radiation dose for deep organ because it can adjust the height corresponding to the organs located in the human body.
