본 연구는 산약(Dioscorea Rhizoma)이 급성 위궤양을 유발한 흰쥐의 위장 장애에 미치는 효과를 조사하였다. 산약 분말사료를 섭이한 실험군과 산약 분말액을 투여한 실험군의 궤양성 출혈은 1 일 이상 진행되었으나 시간이 경과함에 따라 서서히 호전되었으며, 특히 산약즙을 처리한 실험군의 경우 급성 위궤양 유발직후에 국소적인 위점막 울혈만이 관찰되었다. 또한 산약분말액과 산약즙을 투여한 실험군에서는 급성 위괘양 유발 후 증가되는 gastrin과 histamine의 농도가 감소되어 산약이 위산의 과다 분비를 억제하는 것으로 조사되었으며, 산약을 투여한 모든 실험군에서 과도한 지질과산화물의 생성이 감소되었다. 이와 더불어 산약의 투여는 급성 위궤양에 의해 급격하게 증가된 SOD와 GPX의 활성을 감소시키는 반면, CAT의 활성은 높여주어 조직 내 항산화효소 활성을 정상 수준으로 회복시켜주는 것으로 나타났다. 따라서 산약은 급성 위궤양에 의한 위 조직의 손상을 감소시키고 혈장 성분과 항산화효소의 활성을 정상수준으로 회복시켜 위궤양으로부터 위장기능을 개선하는데 높은 효과가 있을 것으로 사료된다.
To evaluate the effects of GP on contact dermatitis, we examined the composition of immune cells from drain lymph node in DNCB-induced contact dermatitis murine model NC/Nga mice. And the amount of pathologic cytokines of spleen and antioxidant activity were investigated. The results were summarized as followers; 1. GP did not show cytotoxic effect on mLFC in vitro. 2. GP did not have hepatotoxicity in vivo in the level of ALT, AST. 3. GP decreased the production of DPPH and in a dose-dependent. 4. GP significantly decreased total cell number of DLN in DNCB-induced NC/Nga mice compared to the untreated control group. 5. GP significantly decreased the number of CD3+, CD19+, CD4+, CD8+, CD3+/CD69+ and CD4+/CD45+ in DLN of DNCB-induced NC/Nga mice compared to the untreated control group. 6. GP significantly reduced the level of IL-4 and IFN-$\gamma$ in splenocytes of DNCB-induced NC/Nga mice compared to the untreated control group. Taken together above results, GP have therapeutic effects on contact dermatitis by regulating T cell activation. This study warranted further investigations of molecular mechanisms of GP on contact dermatitis.
This study was undertaken to investigate the effect of the acidosis on the gastric acid secretion in the isolated whole stomach of the rat and the effect of prostaglandin $E_1$ on the gastric acid secretion influenced by the acidosis. Twenty-two male albino rats(Sprague-Dawley strain) were used. The isolated whole stomach from each rat was introduced into the Kreb's solution which was continuously gassed with $95%O_2-5%CO_2$ for 1 hour, after irrigation of the lumen with cold physiological saline$(4^{\circ}C)$. Thereafter, each stomach was irrigated again with 5% dextrose solution (pH 7.4, $37^{\circ}C$), and filled with the dextrose solution. All the stomachs with the dextrose solution were divided into 4 groups according to the Kreb's solutions in which each stomach was incubated for 30 min: 1) control group, in the pH 7.4 solution, 2) $PGE_1$ group, in the pH 7.4 solution containing $5\;{\mu}g/ml$ of $PGE_1$, 3) acid group, in the pH 7.0 solution, and 4) $acid+PGE_1$ group, in the pH 7.0 solution containing $5\;{\mu}g/ml$ of $PGE_1$. After incubatory period, the contents of each stomach were collected and centrifuged(1,500 rpm, room temperature) for 15 min. The acid output in the supernatant was determined with 0.012 N NaOH by means of autotitrator(Dosimat, Metrohm Herisau Co.) at pH 7.4. Results obtained were as follows: 1) The acid output of the acid group increased significantly in comparison with the control value. 2) The acid output of the $acid+PGE_1$ group decreased significantly in comparison with the acid group. It is inferred from the above results that the acidosis facilitates the gastric acid secretion and $PGE_1$ inhibits the gastric acid secretion induced by the acidosis.
Objectives: Pathogenesis treats cause of disease. Pyungweesan is pathogenic prescription of indigestion. This study researched for developing questionnaire of prescribing pyungweesan. Methods: Korean medical doctor classified the patients into two group, pyungweesan symptom group and non pyungweesan symptom group. After that, each group responsed to the questionnaire. We tested for both the reliability and validity of the questionnaire. For validity test, we compared the sum, average and standard deviation of 20 items of pyungweesan symptom group with those of non-pyungweesan symptom group through independent sample t-test, For reliability test, we computerized crohnbach's a value of the items. Results: The developed questionnaire showed remarkable reliability. (Cronbach's ${\alpha}$ =0.854) There were a significant differences on the sum of the 20 items and each 19 item's average and standard deviation, except 1 item; Do you have an irregular discharge from the womb? Conclusions: Based on the results of the above analysis, the korean medical doctor will make an effort to apply the pyungweesan questionnaire clinically. In the future, the additional research is needed.
Objective : To investigate the effects of Pyeongwi-san-gamibang on atopic dermatitis, this study measured TEWL(transepidermal water loss), observed scratching behaviors and checked levels of Total IgE, IL-4, IFN-${\gamma}$ and conducted skin biopsy on NC/Nga mice with atopic dermatitis. Methods : NC/Nga mice were challenged with DNCB during 5 weeks to develop atopic dermatitis-like skin lesions. The NC/Nga mice with atopic dermatitis were divided into three groups of control group, PW-d, PW-e group. Once a day for 22 days, Pyeongwi-san-GamiBang extract with water was administered for the PW-d group and the extract with 80% ethanol was administered for PW-e group compared with saline solution for control group. During drug administration, sensitization by DNCB had lasted for three times per week. Results : 1. TEWL had no statistical difference among 3 groups. 2. The scratching behaviors had no statistical difference among 3 groups. 3. The levels of Total IgE in PW-d, PW-e group had a statistically significantly higher than that of the control group although difference between the control group and the PW-d, PW-e group were similar. 4. The level of IL-4 had no statistical difference among 3 groups. 5. The level of IFN-${\gamma}$ had no statistical difference among 3 groups. 6. As the observation of toluidine blue stained lesion, both PW-d and PW-e group had lower level of histamine releasement compared with the control group. Conclusion : Result based on these experiments, Pyeongwi-san-GamiBang on atopic dermatitis-like skin inflammation in NC/Nga mice is not effective. But, as the study showed significantly individual differece, we need to repeat these study after supplementing the object number and modified indicator of clinical severity.
본 결과를 통해, 왕우렁이 추출물의 역류성 식도염 모델에서 위 내용물의 역류로 인한 조직손상에 대한 보호효과를 확인하였다. 식도 점막의 육안 소견과 조직병리학적 분석 결과 점막의 괴사, 염증세포의 침윤 등의 증상이 완화됨을 확인하였으며, 상피조직의 tight junction을 보호하여 위산과 펩신으로부터 점막을 보호하는 것을 확인하였다. 이러한 왕우렁이 추출물의 식도 보호효과는 NF-κB 기전의 조절을 통해 점막의 염증반응을 억제하여 나타나는 것으로 생각된다. 이상의 결과를 종합하였을 때, 왕우렁이 추출물이 역류성 식도염 완화를 위한 기능성 소재로서의 가능성을 가지는 것으로 사료된다.
Objectives : Decoction, in Korean Medicine, is a pharmacological method of extraction, by boiling, of dissolved chemicals, or herbal prescriptions, which may include stems, roots, bark and rhizomes. Decoctions differ from most teas, infusions, in that they are usually boiled. This study was performed to compare the difference of water decoctions extracted by different decoction extractor and extraction time and to analyze the reason of decoctions extracted by each decoction extractor have different taste. Methods : With water decoction samples by Pressure extractor, Non-pressure extractor and Ultrasonic waves merge extractor for 1 hr, 2 hr and 3 hr were investigated the yield and the concentration of hesperidin and glycyrrhizin by HPLC/DAD system in Pyungwi-san decoction. Results : The samples of each extractor were gradually increased the yield and the concentration of hesperidin and glycyrrhizin. The HPLC pattern of samples is similar. The yield and the concentration of hesperidin and glycyrrhizin of Ultrasonic waves merge extractor was most highest of the three. The rate of increase of the yield and the hesperidin concentration of between 1 hr and 2 hr in Pressure extractor was the most highest of the three. But the concentration of glycyrrhizin in Pressure extractor was relatively similar to Non-pressure extractor. Conclusions : The yield and the concentration of reference compounds in Pyungwi-san water decoction was influenced by extracting method and extracting time. For scientific movement and standardization of extracting medicinal herbs method need to study of extractor validation and to study in vitro and in vivo.
Objectives : Pyeongwi-san is widely used in Korean medicine for acute indigestion or gastrodynia. As a therapeutic agent for digestive diseases of modern people, in order to confirm the mechanism of Pyeongwi-san on digestive tract disease and the difference of therapeutic efficacy between its formulation, a comparative efficacy test was conducted on digestive tract disease animal model. Methods : For LPS enteritis animal model, male SD rats were intraorally treated with different formulation types of Pyeongwi-san, and then intraperitoneally administered LPS one hour later to induce enteritis. After 5 hours, blood was collected and TNFα, IL-1β, IL-6 and PGE2 were confirmed by ELISA. For acute gastritis animal model, male SD rats were intraorally treated with different formulation types of Pyeongwi-san according to the prescribed concentration, and then intraorally administered 60% ethanol and 150 mM HCl one hour later to induce acute gastritis. After 5 hours, blood was collected and TNFα,IL-6 were confirmed by ELISA. Results : In the LPS-administered enteritis animal model, Pyeongwi-san decreased TNFα, IL-1β, PGE2 and especially IL-6. Pyeongwi-san also decreased IL-6 in acute gastritis animal model. In addition, there was no significant difference in efficacy between the two formulations when compared with inflammatory markers. Conclusions : The efficacy of Pyeongwi-san was confirmed in the inflammatory markers related to digestive inflammatory diseases, and the efficay between two formulations of Pyeongwi-san was relatively similar. Further studies are needed to investigate the new applicability of Pyeongwi-san on different inflammatory diseases that have similar inflammation markers identified in this experiment.
Background and objective : Rabeprazole is a new generation proton pump inhibitor, which has a rapid onset after first dose, predictable efficacy in all patients regardless of CYP2C19 genotype status, and less nocturnal acid breakthrough. The aim of the study is to investigate clinical efficacy and safety of rabeprazole sodium (Pariet 10mg qd)when administered once daily to patients with laryngopharyngeal reflux(LPR) disease. Methods : Among the patients who had visited the Department of Otolaryngology, those with LPR symptoms, had undergone laryngoscopy. Symptoms and endoscopic laryngeal sings were recorded initially, at 1 month, 2 months, 3 months, and more than 3 months, All patients were evaluated for clinical efficacy on the basis of symptom scores, reflux finding score(RFS), and side effects. Results : In general, most symptom scores and RFS improved over the time. Efficacy of the Pariet on LPR-related symptoms were $63.2\%,\;77.5\%,\;78.7\%,\;and\;90.9\%$ before 4 weeks, 4 to 8 weeks, 8 to 12 weeks, and after 12 weeks respectively. Efficacy on the RFS were $61.8\%,\;78.4\%,\;82.9\%,\;and\;85.5\%$ before 4 weeks, 4 to 8 weeks, 8 to 12 weeks, and after 12 weeks respectively. Pariet was well tolerated and was associated with few drug-related side effects. Conclusion Because of its efficacy and safety, Pariet may prove to be an alternative to currently available proton pump inhibitors.
$^{99m}Tc-Pertechnetate\;(TcO_4^-)$ is concentrated by the stomach after intravenous injection, allowing the detection of ectopic gastric mucosa. It has been used to develop a noninvasive test of gastric secretion. However the cellular site of concentration is still controversial, that is whether mucin-secreting epithelial cell or acid-secreting parietal cell. This study is planned to investigate the effects of cimetidine and gastric acidity on the retention of $TcO_4^-$ in the gastric wall of the rat. Also we further attempted to clarify the uptake and secreting cell of $TcO_4^-$ in the gastric mucosa. One hundred rats were divided into two groups, preliminary (40 rats) and main examination group (60 rats). Preliminary examination group was composed of fasting group (20 rats) for the detection of the time for reaching stable $TcO_4^-$ retention ratio in gastric wall and post-prandial group (20 rats) for the detection of the time for reaching the maximal gastric acidity. Main examination group was composed of fasting group (30 rats), which was subdivided into control group (10 rats), cimetidine group (10rats), $Mylanta^{(R)}$ group (10 rats) and post?prandial group (30 rats), which was subaivided into 90 min group (10 rats), 90 min cimetidine group (10 rats), and 120 min group (10 rats). Retention ratio (%) of $TcO_4$ in the gastric wall and the pH of the gastric contents were measured in the extracted stomach of the six groups. Gastric wall retention ratio of $TcO_4^-$ was calculated by the gastric wall radioactivity (cpm) divided by total gastric radioactivity (cpm) at 30 mins after intravenous injection of 0.4 mCi of $TcO_4^-$. The results were as follows: 1) The time required for reaching stable $TcO_4$ retention ratio and the lowest gastric PH were 30 min and 90 min, respectively. 2) In the fasting group, the gastric wall retention ratio of $TcO_4^-$ was significantly increased in the cimetidine group, compared with the control group (P < 0.01). However there was no significant difference between the control and $Mylanta^{(R)}$ group 3) The $TcO_4^-$ retention ratios of 90 min and 120 min groups were lower than that of the fasting control group (p < 0.05), either. After administration of cimetidine, the retention ratio was significantly increased in 90 min group (p < 0.01). 4) While $TcO_4^-$ retention ratio and gastric pH were well correlated in the post-prandial 120 min group (r=0.7112, p<0.05), in the post-prandial 90 min and 90 min cimetidine groups correlated poorly. However, there was no correlation in the three fasting groups at all. Referring the above results, we infer that $TcO_4^-$ is secreted into the gastric lumen by both parietal and non-parietal cells, with dominant non-parietal $TcO_4^-$ secretion in the fasting state and dominant parietal $TcO_4^-$ secretion in the stimulated state.
