Background: Minimally invasive surgery of pectus excavatum by Dr. Nuss is a new technique that allows the repair of this deformity without any cartilage resection or sternal osteotomy We describe the early experiences with Nuss procedure. Material and Method: From December 1999 to January 2001, twenty patients with pectus excavatum underwent repair by Nuss procedure There were 14 males and 6 females whose mean age was 10.1$\pm$7.7 years, ranging from 1 to 33 years. Most patients(N=19) were below 20 years, except 33 years old female patient(N=1). Result: The severity of depression was assessed by computed topography(CT). CT index was mean 4.9$\pm$5.7(ranged from 3.3 to 8). The average operating time was 85.8$\pm$23.7 minutes. The used metal bars were ranged in length from 8 inches to 16 inches(average 11.8$\pm$14.4 inches). Early postoperative complications were pneumothorax in three patients, paralytic ileus in one, and postoperative chest pain requiring analgesics in all patients. Epidural analgesia was used in one adult patient for control of postoperative pain. In our experiences, there were no serious complications posteoperatively. Conclusion: There were good early results with the Nuss procedure that we performed for repairing of pectus excavatum. However, we believe the procedure needs to be observed for the long term results for it to be broadly accepted.
Purpose: The purpose of this study was to evaluate the result of anterior cruciate ligament (ACL) reconstruction using a fourstrand single semitendinous tendon to decrease the donor site morbidity due to harvest both semitendinosus and gracilis tendon. Materials and Methods: Thirty seven consecutive patients who had underwent ACL reconstruction using four-strand single semitendinosus tendon were evaluated. Mean age was 28.6 years old. Male was 34, female 3 patients. Time from injury to surgery was 5.4 months. Combined injuries were 10 meniscus injuries, 3 medial collateral ligament injuries and 1 osteochondral injury. Mean follow-up period was 16 months(12~18 months). Clinical evaluation was done using range of motion, Lachman test, pivot-shift test, Lysholm score & KT-2000 arthrometer. Results: All patients showed the normal range of motion of mean 150..at follow-up. Lachman test and pivot-shift test was negative in 35 cases. Lysholm score was improve from 84 to 92. Two cases had residual laxity due to poor compliance. Mean anterior translation compared to contralateral side by KT-2000 arthrometer improved from 6.7 mm preoperatively to 2.1 mm at follow-up. Conclusion: Reconstruction of the anterior cruciate ligament with use of a four-strand single semitendinosus tendon autograft showed good clinical results.

Purpose: The aim of this study was to evaluate the clinical characteristics of children diagnosed as cryopyrin-associated periodic syndrome (CAPS) in Korea. Methods: Diagnosis was made based on clinical features and confirmed by a mutation in the cold-induced autoinflammatory syndrome 1 (CIAS1) gene. Especially, osteocartilaginous overgrowth in the patella or distal femur was so characteristic that its presence warranted a diagnosis of chronic infantile neurologic cutaneous and articular/NOMID. Results: We observed the clinical features of 9 Korean CAPS patients. All the patients suffered from an urticarial rash with recurrent fever. Among the 9 patients, 6 presented with rash and 4 with fever on the 1st or 2nd days of birth. Eight patients showed myalgia, and 7 patients showed arthralgia in the joints, and 6 patients showed radiologic findings of arthropathy including cupping of the metaphysis, excessive growth of the epiphysis, osteopenia or overgrowth of the cartilage. Four patients showed brain atrophy, enlarged ventricles or leptomeningeal enhancement on magnetic resonance imaging. Intellectual disability was observed in 1 patient. Five patients had eye involvement as conjunctivitis, uveitis, chorioretinitis, avascular area or papillary edema, and 3 patients showed progressive hearing loss. All 9 patients showed increased C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR). Conclusions: All the patients carried a mutation on exon 3 of the CIAS1 gene. After the anakinra (interleukin-1 receptor antagonist) therapy, the fever and rash immediately disappeared, and CRP and ESR were improved.

Background: Astragali radix (A) and Lithospermi radix (L) have long been used as traditional medicines due to their known anti-inflammatory effects. This study aimed at evaluating, their optimal mixing ratio and their functional compounds by investigating the inhibitory effects of mixed extracts of A and L and their active compounds on matrix metalloproteinases (MMPs). Methods and Results: A and L extracts were obtained by extraction at $80^{\circ}C$ using 50% and 70% fermented alcohol, respectively, and then mixed at a ratio of 5 : 5, 6 : 4, 7 : 3 and 8 : 2 (w/w). The activities of MMP-1, MMP-3, and MMP-13 were evaluated in interleukin-1beta ($IL-1{\beta}$)-induced SW1353 cells. The extract mixtures showed synergistic inhibitory effects on MMP-3 and MMP-13, higher than the effects of the individual A and L extracts. The 7 : 3 mixture (ALM16) showed the most effective MMPs inhibitory activity, while among the active ingredients, calycosin-7-O-${\beta}$-D-glucoside and lithospermic acid exhibited excellent MMPs inhibitory activity. Additionally, an HPLC method was established for simultaneous quantification of the effective components of the extract mixtures, and validated by measuring the linearity, precision and accuracy of the limit of detection (LOD) and limit of quantification (LOQ). Conclusions: ALM16 showed the most effective MMPs inhibitory activity. Calycosin-O-${\beta}$-D-glucoside, calycosin and lithospermic acid were identified as useful candidates, as they were the major functional compounds in the MMP inhibitory activity. Summarily, ALM16 might be a highly effective in osteoarthritis management, owing to its because it exhibits a protective effect on cartilage via excellent inhibition of MMPs.

Objectives : The purpose of this study was to evaluate the effects of Daeganghwal-tang on knee cartilage in monosodium iodoacetate(MIA)-induced osteoarthritis rats. Methods : Forty SD rats were randomly divided into five groups(n=8/group): normal group was SD rats group injected with normal saline at left knee joint and administrated orally distilled water, control group was MIA-induced osteoarthritis SD rats group administrated orally distilled water, Indomethacin group was MIA-induced osteoarthritis SD rats group administrated orally indomethacin 2 mg/kg, DGHT(L) group was MIA-induced osteoarthritis SD rats group administrated orally 1280 mg/kg of Daeganghwal-tang, and DGHT(H) group was MIA-induced osteoarthritis SD rats group administrated orally 2560 mg/kg of Daeganghwal-tang. After orally administration of drugs for 4 weeks, gross appearance and histological analysis were used to evaluate the degree of knee cartilage damage. In addition, pro-inflammatory cytokines, bone degrade factor and bone defence factors were analyzed to investigate the anti-inflammatory and cartilage protection effects of Daeganghwal-tang. Also, hematological test, biochemical test, and liver and kidney tissue were analyzed to determine the safety of Daeganghwal-tang. Results : Daeganghwal-tang inhibited the damage of the knee cartilage, and significantly prevented the reduction in cartilage thickness. In addition, the pro-inflammatory cytokines and the bone degrade factor significantly decreased, and the bone defence factors significantly increased. In the safety assessment of Daeganghwal-tang, there were no significant differences among the experimental groups and no abnormal findings were observed. Conclusions : Daeganghwal-tang has anti-inflammatory effect, inhibits cartilage damage, and protects cartilage in MIA-induced osteoarthritis rats.
This study analyzed the impressional characteristics and commonality of the seven people who reached the final final in the 2020 TV Chosun Mister Trot contest, which is the result of the success of the entertainer through the impression. The analysis criteria were set by referring to the classic Ma-uisangbeob and the academic papers on Impressionism, and the faces of seven subjects were collected from the Internet and media. The results of the analysis showed that the following common points were found: First, hair was developed on both sides of the forehead rather than the development of the forehead, which is suitable for arts and physical education rather than studying. Second, most of them had eyebrow bones [the brains of the brain] and cartilage inside their ears was protruding, which is a type of success through effort, deciding on their own life. Third, the mouth was large, the lips were straight, and the neck was thick. This also showed the temperament and talent of artistic ability. Fourth, it developed greatly in the corresponding part by age. In conclusion, while the inherently innate features of facial features cannot prove all that is fatalistic, the characteristic elements of certain talents were clearly manifested. And what they were able to gain and be loved in the entertainment industry was the result of their own efforts beyond it. These results have implications that can be used to determine their talents or career paths in simple aspects.
Wrist PA-grip view is used to diagnose triangular fibrocartilage complex (TFCC) tear because it can easily diagnose damage to the surrounding wrist ligaments. However, despite advances in radiology equipment, distortion of images due to geometric elements still has many limitations. In this paper, we propose a method that can minimize the distortions of images by analyzing the distortions occurring in the wrist PA-grip view. A source of image distance (SID) were set at 130 cm and 150 cm for comparison with 110 cm. Depending on the SID, the phantom of wrist was moved at 0, 2, 4, 6, 8, and 10 cm in the X-axis and Y-axis directions, respectively. For quantitative evaluation, the difference of distance between the radius and ulna was measured in picture archiving and communication system (PACS) system. As a qualitative evaluation, survey was conducted among 20 radiologic technologists who examined the Wrist PA-grip view. The Kruskal Wallis test was performed to compare the distortion according to the phantom movement in the X-axis and Y-axis directions based on the SID, and the Tukey test was performed as a post-test. In the quantitative evaluation results, the measured values obtained in the X-axis was not significantly different in all groups (p>0.05). The measured values obtained in the Y-axis was significantly different in the most groups (p<0.05). Therefore, to reduce distortion while maintaining image quality, we recommend what examine the SID at 150 cm than 110 cm.

To restore a tooth with a fracture line extending below the marginal bone level, a surgical crown lengthening procedure accompanied by ostectomy could be considered to expose the fracture line and reestablish the biologic width. However, this procedure could lead to esthetic failure, especially in the anterior teeth. Therefore, orthodontic extrusion, which elevates the fracture line from within the alveolar socket without sacrificing the supporting bone and gingiva, is recommended. This technique allows for the proper placement of the crown on a sound tooth structure, with the reestablishment of the biologic width. Alternatively, surgical extrusion is an one-step procedure that is simpler and less time-consuming than orthodontic extrusion; placing and adjusting the orthodontic appliance does not require multiple visits. This study presents successful restoration in 2 cases with a crown-tooth root fracture of the maxillary central incisor treated using a multidisciplinary approach through orthodontic extrusion or surgical extrusion followed by successful restoration.

Objectives : 3-Acetyl-11-keto-𝛽-boswellic acid (AKBA) is a major active compound in Boswellia serrata. We investigated the arthritic changes following AKBA administration in monosodium iodoacetate (MIA)-induced osteoarthritis rats. Methods : All rats were randomly divided into five groups: Normal, Control, INDO (indomethacin 2 mg/kg treated), AKBA30 (AKBA 30 mg/kg treated), and AKBA60 (AKBA 60 mg/kg treated); drugs were given 2 weeks before MIA injection. For all groups except the normal group, 50 µL of sterile saline with MIA (80 mg/mL) was injected into the right knee joint 2 weeks after drug administration. The drug administration was continued for 4 weeks from 1 week after osteoarthritis induction. The histomorphological changes of knee joint cartilage were observed by H&E staining. Also, the levels of glycosaminoglycan (GAG), cartilage oligomeric matrix protein (COMP), 5-lipoxygenase (5-LOX), 5-LOX-activating protein (FLAP), and leukotriene B4 (LTB4) in the knee joint were determined by the ELISA kits. The expressions of mitogen-activated protein kinases (MAPKs), inflammatory cytokines, and matrix metalloproteinases (MMPs) in knee joint were detected by Western blot. Results : Data show that levels of 5-LOX, FLAP, LTB4, and COMP were downregulated significantly in the AKBA treated groups when compared to those in the Control group. On the other hand, GAG levels were significantly elevated. As a result of Western blot, the AKBA-treated groups significantly inhibited phosphorylation of MAPKs. In addition, significant downregulation of the expression of inflammatory cytokines and MMPs was found in the AKBA-treated groups. Conclusion : Our findings suggest that administration of AKBA could exert better chondroprotective and anti-inflammatory effects for MIA-induced osteoarthritis rats.

The family Pristiophoridae belonging to the order Pristiophoriformes consists of about 8 species and 2 genera in the world. Among them, only one species (Pristiophorus japonicus) is distributed in northwestern Pacific. The family is characterized by having an elongated and flattened rostrum with laterally projecting a low of sharp teeth. In order to clarify variability of local populations of P. japonicus, we investigated 16 individuals collected from four locations (Korea, n=6; Miyagi, n=1; Kochi Pref., n=1; Okinawa, n=8) for morphological and molecular analyses. Morphological analysis showed that there may exist three morphotypes, of which A morphotype (Korea, Miyagi, Kochi Pref.) has a short snout (26.8% of TL), being most similar to syntypes of P. japonicus. On the other hand, B morphotype (Okinawa) has a long (31.7% of TL) and wider snout (snout width at nostrils 5.2 times pre-oral length) and, C morphotype (Okinawa) has a long (31.7% of TL) and narrower snout (6.3 times). Molecular analysis using mitochondrial DNA cytochrome b DNA sequences (386 base pairs) showed that the K2P distance between A and C morphotypes were 2.1~2.7%, but those within A morphotype were 0.0~0.5%. We identified A morphotype as P. japonicus, and designated B and C morphotypes as Pristiophorus sp. 1 and Pristiophorus sp. 2, respectively. Further studies are required to elucidate the taxonomic status of two morphotypes of the sawshark in Okinawa.
