Characteristics and Death Risk Factors of Patients in Long-Term Care Hospital Connected to Special Hospital (일개 특수 요양병원 환자들의 특성 및 사망 위험요인)

  • The objective of this study is to provide high-quality medical service, by understanding the characteristics and death risk factors of patients at V long-term care hospital, that has established and operated the cooperative treatment system in connection with a public institution, the S veterans hospital. A follow-up survey targeting a total of 850 discharged patients was conducted for the year 2017. Average age of patients was 79.2 years, with 86.4% belonging to the 70 years or over group. The men of national merit were 75.4%, which was more than three times higher than general patients. Hospitalization for 180 days or more was 46.4%, which was higher than general long-term care hospitals (37%). The major death risk factors were age and the number of hospitalized days. Our data indicate that transfer to S veterans hospital results in providing quicker treatment and higher effectiveness of the numerous recovery therapies. We therefore propose that the cooperative treatment system should be expansively operated to increase the quality of happy life, by alleviating the health of patients with chronic diseases in a long-term care hospital.

A Drug-Induced Liver Injury by Western Medication (양약으로 유발된 약인성 간손상 환자 임상보고)

  • Objectives : To investigate the clinical features of drug-induced liver injury (DILI) and traditional Korean medicine (TKM) -based management. Methods : A female patient diagnosed with DILI caused by Western drugs had been treated with Oriental therapies, then the subjective clinical outcome and biochemical parameters were monitored. Results : A 73-year-old female had taken Western drugs (nonsteroidal anti-inflammatory and skeletal muscle relaxants) for about 3 months, and complained of severe abdominal discomfort and tiredness. Her RUCAM score was 9, which met the criteria for DILI (AST 90 IU/L, ALT 100 IU/L, ALP 191 IU/L, and GGT 614 IU/L). She was treated with herbal drugs, moxibustion, and acupuncture, and her symptoms completely resolved, with normalized hepatic enzymes within two weeks. Conclusions : This case report provides a clinical characteristic for a typical DILI caused by Western medicine, and shows an example of a TKM-based application.

  • In the oral cavity, the teeth undergo wear and corrosion throughout their lives. Progressive and constant tooth wear is a natural phenomenon of aging, but wear and corrosion due to specific factors are pathological factors. It can cause pathological damage of the occlusal surface, aesthetic problems, dimensional loss and jaw joint disorders. This case is a 26-year-old female patient with general tooth abrasion and erosion on the entire dentition. Diagnostic wax-up was fabricated based on the information including digital facial analysis, physiological stabilization, and evaluation of anterior crown length. Through the digital analysis, the necessary guides for crown lengthening were prepared and the mastication function and esthetics were evaluated by using temporary crowns. Definitive prosthesis was fabricated with the zirconia restorations. The results were satisfactory when they were observed 3 months of follow-up.

Mandibular full-arch rehabilitation with increased vertical dimension of a patient with severe tooth wear (심한 치아 마모 환자의 수직 교합고경 증가를 동반한 하악 편악 수복 증례)

  • In the case of patients with pathological tooth wear, it is necessary to evaluate the loss of the vertical dimension. Setting the appropriate vertical dimension is important to rehabilitate the patient's stable intermaxillary relationship. A 77-year-old female patient's chief complaint was the pain of the mandibular incisors and the lack of molars. At the first visit, pathologic tooth wear of the mandibular incisors were observed. After diagnosis and evaluation, loss of vertical dimension was not observed, but insufficient intermaxillary space for prosthetic restoration was confirmed. Mandibular rehabilitation was performed with vertical dimension increase and this showed satisfactory results both functionally and aesthetically.

  • Polyacrylamide gel (PAAG) has been used for augmentation mammoplasty in certain countries, such as China or Eastern European countries. However, owing to various complications, its use is currently banned in most countries. Nevertheless, because of the migratory stream of female from these countries, we also encounter patients with PAAG mammoplasty in Korea. Owing to the various imaging features of PAAG mammoplasty, it may interfere with the identification of malignancy or other pathologic conditions. Therefore, it is important to identify the imaging findings related to PAAG mammoplasty. However, there is limited literature on the radiologic findings of complications related to PAAG mammoplasty in Korea. Herein we report four cases of PAAG mammoplasty, the complications faced, and depict various radiologic features through multimodality imaging.

  • Intraluminal duodenal diverticulum (IDD) is a rare congenital abnormality, consisting of a saclike mucosal lesion in the duodenum. Cases of IDD can present with gastrointestinal bleeding, duodenal obstruction, or pancreatitis. Here, we report a rare case of a 25-year-old female presenting with IDD complicated by duodeno-duodenal intussusception and recurrent pancreatitis. The diagnosis was based on findings from radiologic examinations (CT and MRI), upper gastrointestinal series (barium swallow), and gastroduodenofiberscopy. Laparoscopic excision of the presumed duodenal duplication was performed. The subsequent histopathologic evaluation of the excised sac revealed normal mucosa on both sides, but the absence of a proper muscle layer confirmed the diagnosis of IDD. Radiologic detection of a saccular structure in the second portion of the duodenum can indicate IDD with duodeno-duodenal intussusception as the lead point.

Transvaginal Direct Puncture and Ethanol Sclerotherapy for Cervicovaginal Venous Malformations: A Case Report and Literature Review (자궁경부 및 질에 발생한 정맥기형에 대한 질 경유 직접 천자 및 에탄올 경화요법: 증례 보고 및 문헌 고찰)

  • Cervicovaginal venous malformations are extremely rare. Sclerotherapy is proven to be effective for superficial venous malformations but not for venous malformations in the lower genital tract of female. A 52-year-old female presented with intermittent vaginal bleeding. The amount of vaginal bleeding gradually increased over 3 months. Contrast-enhanced pelvis CT showed several phleboliths and dilated vessels, but pelvic angiography showed no early draining veins, nidus, or feeding artery. We performed transvaginal direct puncture and ethanol sclerotherapy rather than surgical treatment because she wanted to preserve the uterus. After four sessions of sclerotherapy, she had significantly decreased vaginal bleeding without complications. Here, we report the first case of cervicovaginal venous malformations successfully treated with transvaginal direct puncture and ethanol sclerotherapy.

Relationship between Serum Uric Acid Levels, Insulin Resistance and Components of Metabolic Syndrome in Korean Adults (한국 성인의 혈청 요산 수치와 인슐린저항성 및 대사증후군 구성요소간의 연관성)

  • The purpose of this study was to investigate the relationship between serum uric acid levels, insulin resistance and components of metabolic syndrome. It was conducted on 4,428 adults over the age of 20 who had undergone health checkups at a general hospital in Gyeonggi-do from June 2018 to May 2020. As a result of the study, uric acid levels were higher in the metabolic syndrome subjects than normal subjects in both men and women. and the incidence of metabolic syndrome and its components was higher in the hyperuricemia group than in the normal group. Hyperuricemia was found to increase the risk of elevated blood pressure(p=0.006) and hypertriglyceridemia(p<0.001) in men and metabolic syndrome(p=0.012) and low HDL-cholesterol(p<0.001) in women. Thus, in both men and women, hyperuricemia was associated with metabolic syndrome and its components, and it was confirmed that it was an independent predictor of the onset of metabolic syndrome in women.

Assessment of Obesity Indices in Consideration of Gender and Age (성별과 연령을 고려한 비만 지표 평가)

  • In order to increase the accuracy and efficiency in measuring obesity conducted outside the hospitals, we examined not only the Body Mass Index and cutoff values of waist circumference of 3,281 adults but also considered their gender, age and various body characteristics. This study included 1,505 males (45.9%) and 1,776 females (54.1%) who visited the comprehensive health checkup center. The average body fat content of male participants was 22.2%, and their Body Mass Index was 24.4kg/$m^2$. The average Body Mass Index of female participants was 22.9kg/$m^2$. The average of waist circumference of male subjects was 86.2cm while that of female subjects was 76.9cm. The conventional method to measure obesity using BMI and WC only does not reflect the actual body fat which may change according to one's gender and age. Therefore, for those who visit individual's place to measure obesity outside the hospitals only based on BMI and WC, we need a new standard to measure obesity more accurately.

A Survey of the Pain and Quality of Life of the Middle-Aged Women with the Subjective Symptom of Urinary Incontinence (요실금 자각 증상이 있는 중년여성들의 통증 및 삶의 질 정도에 대한 조사)

  • The objective of this survey was to understand the effect of the subjective degree and pain of urinary incontinence on the quality of life of the middle-aged women with the subjective symptom of urinary incontinence. A questionnaire was carried out to investigate the quality of life in relation to urinary incontinence for 114 patients of chronic musculoskeletal system from Feb. to May in 2010. Overall, the subjects responded that their quality of life was medium(($61.36{\pm}13.2$score) and the pain related to urinary incontinence was low back pain. The degree of pain was between the moderate and a little severe in the VAS (Visual Analogue Scale) range of 4 to 7. It was also shown that the action of the subjects was affected by urinary incontinence in forms of avoidance limitation and psycosocial effect. Since the survey showed that the urinary incontinence and pain substantially affect the quality of life of the middle-aged women, the result may provide basic information in developing a practical intervention program.
