Research for the antiinflamation and antioxidation effect on the Lycoris squamigera Maxim (소염 및 항산화제 개발을 위한 상사화의 효능 연구)

  • Objectives : The Lycoris squamigera Maxim has been used traditionally for treatment of various diseases. However, the studies on the effect of Lycoris squamigera Maxim have not been carried out. In the present study, extract of Lycoris squamigera Maxim were tested for their anti-inflammatory and anti-oxidation effect. Methods : The anti-inflammatory effect of the various solvent extract was studied in lipopolysaccharide (lps)-treated mouse macrophage cells. RAW 264.7 cells were pre-incubated with Lycoris squamigera Maxim extracts for 4h and treated with $1\;{\mu}g/m{\ell}$ lps for 18h, and then the anti-inflammatory effects of extracts were determined. The anti-oxidation effect of extracts measured by DPPH method, reductive potential test, total phenolics test. Results : Extracted root's ethyl acetate layer showed a significant decrease in nitric oxide. And that layer (root's ethyl acetate extract) was showed decrease in TNF-${\alpha}$ concentration dependently. Separated from Root's ethyl acetate extract was fraction 1 has $0.1{\sim}5\;{\mu}M$ range, fraction 2 has $0.1{\sim}10\;{\mu}M$ range did not showed cytotoxicity. Anti-oxidation result as DPPH test showed the best was root ethyl acetate extract. Redusing power was made a comparison between fractions and standard. They were showed similar value. Fraction's total phenol containing result was better then standard. Conclusions : These results suggest that these extracts can be used as anti-inflammatory, anti-axidation materials.

A Case Report of Polyarticular Type Juvenile Idiopathic Arthritis(JIA) Patient Ineffective Response to DMARDs and NSAIDs (항류마티스제(DMARDs)와 비스테로이드성 소염진통제(NSAIDs)에 반응하지 않는 다수관절형 소아기 류마티스 관절염 환아 증례 보고)

  • Objectives This study is to examine case that has an significant result on treating Polyarticular Type Juvenile Idiopathic Arthritis(JIA) the patient who did not response to DMARDs and NSAIDs with oriental medicine. JIA is the most common form of persistent arthritis in children before age 16. The symptoms of JIA is pain and edema of joints, fever, lethargy, reduced physical activity, and poor appetite. But the symptoms vary. Methods The patient felt pain and had limitation of mobility on his joints, but the DMARDs and NSAIDs were not effective. Therefore, he was treated with oriental medicine and withdrew from DMARDs and NSAIDs. The patient took DMARDs and NSAIDs from May 2007 to November 2008(He did not took DMARDs and NSAIDs from December 2007 to June 2008), and took oriental medicine from November 2008 until now. Results The patient's pain and limited mobility on his joints, edema, morning stiffness, fatigue was gone, and could maintain the condition. He doesn't have DMARDs or any other western medicine anymore. Conclusions This study shows that a Polyarticular Type Juvenile Idiopathic Arthritis patient who had ineffective response to DMARDs and NSAIDs can reach to the clinical remission by oriental medicine treatment. His morning stiffness, fatigue, pain of joints, edema was gone and ESR was in normal range with Oriental medicine treatment.

Effects of Herbal Acupuncture(Soyeum) on Cancer Patients Accompanied by Pain (소염약침이 암성통증을 수반한 각종 암환자에 미치는 영향)

  • Objectives : This study was designed to evaluate effects of 'Soyeum' on cancer patients accompanied by pain. Materials and Methods : We retrospectively analyzed the medical records of 9 patients accompanied by pain who had been injected with 'Soyeum' for over 14 days continuously in East-West Cancer Center of Oriental Hospital of Daejeon University from June 2002 through august 2002. Results : The statistical significance between the pre-treatment and post-treatment results (Changes of Cytokine Level, QOL, BPQ and Pain relief after pain management) were analyzed. Analysis of cytokines (IL-12, IFN- ) level showed that the percentage of increase of lL-12 is $60\%$, IFN- is $60\%$. Analysis of QOL showed that the percentage of maintenance and improvement is $77.8\%$. $55.5\%$ of the patients reported a 'worst pain' intensity score of 3 or greater, $44.5\%$ reported a 'least pain' intensity score of 2 or greater, and $66.7\%$ reported 'average pain' intensity score of 2 or greater. $33.3\%$ of the patients were in pain at the time of interview and $22.2\%$ had a current intensity score of 2 or greater. Analysis of pain relief after pain management showed that the percentage of pain relief score of 2 and 3 is $55.6\%$. The data was expressed as Mean SE by using descriptive statistics. Statistical significance examined by using paired t-test. Conclusions : It is suggested that 'Soyeum' has effects on pain of cancer patients, also expected that 'Soyeum' is useful to improve immunoactivity and for cancer patients

Anti-inflammatory Effect of Cinnamomi Ramulus on Collagen Induced Arthritis;a Model for Rheumatoid Arthritis in DBA/1J Mice and Cytokine Production in Raw264.7 Cells (계지(桂枝)의 collagen 유발 관절염에 대한 소염 효과;DBA/1J mouse에서의 병태 관찰 및 RAW264.7에서의 cytokine 분비측정)

  • In order to examine anti-inflammatory effect of Cinnamomi Ramulus (CR) alcohol extract on rheumatoid arthritis, the present study investigated the viability and TNF-${\alpha}$ production in Raw264.7 cells treated with CR and collagen induced arthritis in DBA/1J mice which were orally administered with CR prior to immunization. The results are as follows: CR extract at $20{\mu}g/ml$ inhibited the viability of Raw264.7 by 50%. CR at 15 and $20{\mu}g/ml$ showed a significant decrease in TNF-${\alpha}$ levels from Raw264.7 cells treated with LPS. CR administration decreased arthritic index in DBA/1J mice immunized with bovine collagen type II but it did not reach statistical significance. CR administration significantly decreased spleen weights obtained from mice in 6 weeks after immunization. CR administration significantly decreased serum anti-type II collagen antibody levels compared with control group. CR administration decreased serum IL-6 levels compared with control group but it did not reach statistical significance.

Effects of HP228 on Analgesia Alone or in Combination with Morphine (새로운 해열, 진통, 소염제인 HP228의 단독 또는 Morphine과의 병용투여가 제통효과에 미치는 영향)

  • Background: The new drug HP228 is a cytokine restraining agent with a broad spectrum of anti-inflammatory, analgesic, and antipyretic activity. Six healthy, adult, male volunteers were studied to determine the independent and interactive effects of HP228 and morphine on pain perception. Methods: Two groups of stimuli were applied to each volunteers before drug administration as control, 20 min after morphine and HP228 administration, and 20 min after combined administration of these two drugs. Two adhesive electrically-conducting pads were applied on opposite sides of the arm approximately 8 cm apart. The electrode were connected to an electrical impulse generator and 50 Hz 1 msec pulses of incrementally increasing intensity were delivered at 1 sec intervals. The analgesic endpoints were the current intensity (mA) at which the subject first detected the stimulus (THRESH), the intensity at which the stimulus was first idenfied as being painful (PAIN), and the intensity at which the subject requested that the stimulus be terminated due to discomfort (LIMIT). A second series of stimuli were applied immediately thereafter using 1-sec duration 50 Hz tetanus pulses with increasing intensities at 2~5 sec intervals. Results: There were significant differences between drug treatments (Morphine, HP228, HP228/Morphine) and control (No drugs) in any of the measurements (PAIN, LIMIT) except THRESH with the twitch and tetanus test. Conclusions: The data suggests that HP228 is an analgesic, but it does not appear to interact with morphine in an additive manner.

In vitro Screening of Natural Product Inhibitors on the UVB Phototoxicity (In vitro 실험법에 의한 천연물 중의 UVB 광독성 억제제 검색)

  • The phototoxicity inhibitory activity of 15 natural products having antiinflammatory effect was screened by three in vitro methods : yeast growth inhibition test with Candida albicans, RBC photohemolysis and MTT assay. We induced phototoxic reaction by irradiating UVB (312 nm) on chlorpromazine (CPZ) that has been widely documented as phototoxic agent in clinical and experimental studies and then observed the effects of the natural products after treating them with CPZ. In yeast growth Inhibition test, P. persica and E. officinalis showed the inhibitory effect on the UVB phototoxicity and E. officinalis, yeast, P. suffruticosa showed phototoxicity inhibitory effect in that their % hemolysis compared with control were 45.76 $\pm$ 0.91, 34.42$\pm$1.01, 35.30 $\pm$4.76 on UVB. In MTT assay, all tested natural products increased cell viability compared with the contort.

Effect of Other Medications on the Stability of Omeprazole in Aqueous Solution for the Peptic Ulcer Disease (소화성궤양시 병용약물이 수용액 중의 오메프라졸 안정성에 미치는 영향)

  • The stability of omeprazole in the aqueous solutions containing loxoprofen or Sodium bicarbonate was examined at room temperature. Loxoprofen or Sodium bicarbonate (60 mg) was added to omeprazole (600 ${\mu}g$/ml) solution to check the stability profile. Then, the solution was kept at room temperature for 80 hours. The concentration was assayed at each concentration by stability-indicating High performance liquid chromatography (HPLC) method. Aliquots of the solution were withdrawn at specified time intervals and assayed by chromatographic analysis for intact omeprazole. The relation between omeprazole concentration and peak area was linear from 5 to 160 ${\mu}g$/ml. The analysis method was precise with relative standard deviation (% RSD) no greater than 3.05 %. The remaining percentage-time curves revealed that omeprazole was degraded rapidly as functions of time and temperature following pseudo first-order kinetics. In conclusion, the stability of omeprazole was significantly affected by liquid solutions mixed with alkalizer (Sodium carbonate) or the NSAIDs (loxoprofen).

Separation Characteristics of Ibuprofen in Kromasil HPLC Column (Kromasil HPLC 칼럼을 이용한 Ibuprofen의 분리특성 연구)

  • Chiral separation of racemic ibuprofen was achieved on a Kromasil KR100-5CHI-TBB column. Some chromatographic parameters (resolution, number of theoretical plates, HETP, capacity factor) are calculated under different separation conditions such as change of mobile phase compositions (hexane / t-BME : 85 / 15, 75 / 15, 65 / 35, 55 /45) as well as acetic acid concentrations for adjusting pH (0.1 to 1 $v/v\%$). Flow rate versus number of theoretical plates and HETP were compared to evaluate column efficiency. To determine the adsorption isotherms, PIM (Pulsed Input Method) was carried out. At concentrations of racemic ibuprofen between 0.1 and 0.3 mg/ml, S- and R-ibuprofen have the same retention time of 4.48 and 5.81 min. Ibuprofen isotherms show a linear form under concentrations of 0.3 mg/ml with eluent (hexane / t-BME = 55 / 45).

  • Antiinflammatory and antioxidative effects of mulberry fruit were investigated by using bioassay screening system. The extract of muberry fruits showed inhibitory effect in phospholipase $A_2$ activity and cyclooxygenase II activity. Among nine varieties of mulberry tree, the antiinflammatory activities of the Shingwangppong, Ficus, Cheongilppong and Keomseolppong were higher than that of the other varieties. Also, antioxidative activity of mulberry fruit was examined by DPPH free radical scavenging method. The radical scavenging activity of the mulberry fruit decreased as following order : Shingwangppong > Keomseolppong > tetraploid Ficus> diploid Ficus.

Compression Neuropathy (압박성 신경병증)

  • Nerve compression is caused by external force or internal pathology, which symptom develops along nerve distribution. There are median, ulnar and radial nerve compression neuropathies below elbow. Carpal tunnel syndrome at the flexor retinaculum is most common among all the entrapment neuropathies. Other causes of median nerve neuropathy include Struther's ligament, biceps aponeurosis, pronator teres, FDS aponeurosis and aberrant muscles, which induce pronator syndrome or anterior interosseous nerve syndrome. Ulnar nerve can be compressed at the elbow by arcade of Struther, medial epicondylar groove, FCU two heads, which develops cubital tunnel syndrome, at the wrist by ganglion, fracture of hamate hook and vascular problem, which develops Guyon's canal syndrome. Radial tunnel syndrome is caused by supinator muscle, which compresses its deep branch. Treatment is conservative at initial stage like NSAID, night splint or steroid injection. If symptom persists, operative treatment should be considered after electrodiagnostic or imaging studies.
