The Underwriting of Cancer (악성종양의 언더라이팅)

  • 지속적으로 증가 하고 있는 악성 종양 발병률의 증가, 생존률의 향상, 조기발견의 증가 등이 암 기왕자의 보험 수요 상승을 가져오고 있으며, 보험시장에서 더 이상 암 기왕자를 도외시하기 어렵게 만들고 있다. 그러나, 현재 국내에서는 암 기왕자를 인수할 수 있는 상품과 언더라이팅 인수 기법이 부족한 실정이다. 본 논문은 이러한 상황 하에서 기존에 거절체로 인식되었던 암 기왕자의 적극적인 인수를 위해 악성종양에 대한 보험 의학적인 고찰과 사망률, 위험도에 대해 분석하고, 국내에서 암 기왕자의 보험 인수 시 문제점과 그 해결방안을 모색해 보았다. 종양은 종양세포의 성장과 확산의 정도를 나타내는 병기와 조직학적 등급에 의해 그 예후와 경과가 예측 가능하며, 이 병기를 표준화하려는 국제적 시도로 1977년 American Joint Committee on Cancer(AJCC)는 TNM system 을 제시하였다. 병기를 기초로 치료의 결정도 함께 이루어지는데, 주요 치료법을 수술, 방사선치료, 항암치료, 면역요법이 있다. 이러한 치료법들은 후기 부작용을 일으키거나 이차성 암(Secondary malignant neoplasm)의 원인이 되기도 한다. 병기에 따른 암환자의 사망률을 살펴 보면, 암은 일정시기 동안 사망률이 급격히 증가하다가 이후에 점차 사망률이 감소하고, 그 이후에는 사망률에 영향을 미치지 않는다. 이러한 사망률 변화의 특성은 암 기왕자에게 평준식 정액 할증법(Per mille flat extra)의 적용이 적절함을 보여준다. 그러나, 현재 국내에서는 악성 종양 기왕자를 위한 상품이 전무한 상태이며, 암의 병리적 병기와 치료에 관련된 정보를 얻을 수 있는 도구도 부족하고, 명확한 인수 기법이 없다. 또한, 자체의 경험통계의 부족으로 위험에 대한 부담감이 큰 것이 현실이다. 하지만, 병기에 따른 사망률의 변화에서도 알 수 있듯이 평준식 정액 할증법의 도입이나, 악성종양의 병기(Stage)별 정액 할증률을 삭감법으로 전환하는 기준을 개발한다면, 종신보험은 충분히 인수가 가능하다. 또한, CI 보험도 암에 대한 부담보를 하는 선진사의 인수 기법을 도입하거나, 암 기왕자만을 위한 CI 상품을 개발하는 등 새로운 인수 기법을 모색한다면, 암기왕자 인수의 폭은 넓어질 것이다. 이와 같은 노력과 함께, 언더라이팅의 정보 획득을 위한 암 기왕자만의 고지서를 개발하고, 전문 언더라이터의 양성을 위한 노력을 게을리 하지 않는다면, 암기왕자를 적극적으로 인수할 수 있을 것이다. 앞으로 국내 보험사가 위와 같은 노력을 지속적으로 해 나가고, 비교적 예후와 경과가 양호한 0기와 1기 암 기왕자부터 단계적으로 종신보험의 인수를 시도한다면, 비교적 안전하게 경험치를 축적할 수 있을 것이다. 뿐만 아니라 국내 보험업계는 새로운 시장의 확보와 보험에 대한 대외 이미지 개선이라는 효과도 함께 얻게 될 것이다.

Enhancement of Astaxanthin Production of Haematococcus pluvialis by Mutation (돌연변이를 통한 미세조류 Haematococcus pluvialis의 Astaxanthin 생산성의 향상)

  • Haematococcus pluvialis is a great producer of astaxanthin (3,3'-dihydroxy-$\beta$,$\beta$-carotene-4,4'-dione). The activities of astaxanthin include potential cancer prevention, immune response enhancement, antioxidant activity, and so on. Nevertheless, it tried to manipulate by mutation for overcoming low growth rate of wild type and limited production of astaxanthin. Mutated colony that is lager and more reddish one than wild type was selected by attempting to expose strains to UV irradiation and to treat chemical such as EMS and colchicines as mutagen. Selected mutants were further screened using inhibitors of the carotenoid biosynthetic pathway. Inhibitors used were nicotine and diphenylamine and both had decreased the survival rate by 40-50%. Among over 50,000 mutant colonies screened, two strains were selected. One selected mutant strain (U15-5) from UV treatment showed 1.68-fold higher total carotenoid contents per cell than that of the wild type strain. On the other hand, the other selected mutant strains (DS, M4-3) from colchicine treatment showed 20$\sim$30% faster cell growth than the wild type strain.

Experimental study for the irradiation effect of x-ray on the hatching process of fertilized eggs (X선 조사가 닭 유정란 부화과정에 미치는 영향)

  • This study examined the irradiation effect of high energy x-rays on the hatching process of fertilized eggs, particularly with regard to malformation and blood cells change. The experimental groups were five day old fertilized eggs irradiated with x rays at doses of 5, 7.5, 10 Gyusing alinear accelerator. The control group was not irradiated. After three weeks, hatched chicks were sacrificed and examined for blood sampling. The survival rate of the x-ray irradiated groups were significantly lower than that of the control group (46.7vs 80%). The malformation rate of the experimental groups was60%, whereas no congenital malformations were observed in the control group. The experimental groups had a significantly higher malformation rate. The types of malformation were left wing defect, proptosis, microcephaly, cervical spine, and feet anomaly. The incidence of malformation increased with increasing radiation dose. The white blood cell count of control group and eachexperimental groups (5 and 7.5 Gy) were 87.64, 100.76 and 81.42 ($10^3/{\mu}L$), respectively. X-ray irradiation of 5 day old chick embryos increased the rate of death and malformation significantly.The malformations were estimated to have occurred by chromosomal abnormalities. Further genetic studies will be needed to confirm the correlation between high energy X-rays and the cause of malformations.

Production and Characterization of a Recombinant Antibody Neutralizing Botulinum Neurotoxin A (보툴리눔 신경독소 A를 중화하는 재조합 항체의 제조와 특성 분석)

  • Botulinum neurotoxin (BoNT/A) is a neurotoxin that selectively attacks the peripheral cholinergic nerve endings. It is produced by Gram -positive, endospore-forming strict anaerobic bacteria, Clostridium botulinum. Since BoNT/A could be a biothreat agent, as well as a contaminator of food and water supplies, the development of sensitive assays for toxin detection and potent antitoxin for the treatment of intoxication is necessary. In this study, for the purpose of producing monoclonal antibodies (mAbs) that are capable of neutralizing Botulinum neurotoxin type A (BoNT/A), scFv (single-chain variable domain fragment) libraries from the rabbit antisera against BoNT/A was fused to a human IgG. The resulting recombinant scFvIgG antibody protein was expressed in stable cell lines and was purified using a protein A affinity chromatography. The efficacy of scFvIgG mAb was confirmed by ELISA and was evaluated for the neutralization of BoNT/A in vivo. Such an in vivo toxin neutralization assay was performed using mice. Although scFvIgG antibody proteins (10 ug) failed to fully protect the mice challenged with BoNT/A (100,000 $LD_{50}$), it significantly prolonged the survival time. These results suggest that scFvIgG mAb may be capable of neutralizing BoNT/A single-chain variable domain fragment.

Functional Cosmetic Effect of Porcine Placeta (Porcine Placenta의 기능성 화장품소재 특성)

  • Porcine placenta was treated with alkali, acid and enzyme treatment to obtain extracts. Heavy metal contents such as Pb, As, and Hg were low enough to satisfy cosmetic agent standard. As a result of safety test(MTT assay), porcine placenta extracts showed over 80% of cell viability at $50{\mu}g/ml$, and cell toxicity was relatively lower. From antioxidation test using DPPH free radical scavenging assay, antioxidation effect was highest as 63% at $50{\mu}g/ml$ when porcine placenta was treated with alkali in pH 9. From whitening effect test using tyrosinase inhibition assay, tyrosinase inhibition effect was 30% at $50{\mu}g/ml$ concentration in alkali treated procine placenta, however, the efficiency was lower compared with arbutin or vitamin C. In anti-wrinkle effect test from elastase inhibition assay, elastase inhibition effects were 20~30% at $50{\mu}g/ml$ for 5 kinds of porcine placenta treatments, which was superior to standard, and especially, protease treated extracts showed best results. Skin formulation including 1% porcine placenta was made and the formulation was very stable for temperature and storage period. From this research, porcine placenta extract showed high potential for anti-wrinkle functional cosmetic agent.

Primary Non-Hodgkin's Lymphoma in Right Ventricle with Right Atrial Invasion -Report of 1 case- (우심실에 발생하여 우심방을 침범한 원발성 비호지킨씨 림프종 -1예 보고-)

  • Primary cardiac lymphoma is an extranodal malignant lymphoma of any cell type involving only heart and pericardium without dissemination. Patients usually present symptoms of heart failure, arrhythmias, pericardial effusion and cardiac tamponade. Diagnosis of primary cardiac lymphoma can be performed by echocardiogram, CT and MRI and cytologic examination of pericardial effusion or transvenously biopsied cardiac tissue. Prognosis of primary cardiac lymphoma is poor. Because of poor prognosis, early diagnosis and complete surgical excision is essential and postoperative systemic chemotherapy or radiotherapy is useful. In this case extensive tumor infiltration on the right ventricle and atrioventricular groove preclude surgical excision. Tissue biopsy revealed primary cardiac lymphoma. After postoperative chemotherapy and radiotherapy, the size of intracardiac mass is decreased in follow up chest CT scan and echocardiogram and symptoms of patient are relieved. Therefore, we report a case of primary cardiac lymphoma with review of literatures.

Implantation Metastasis of Lung Cancer to Chest Wall after Percutaneous Fine-Needle Aspiration Biopsy (흉부 세침 흡인 생검 후 발생한 폐암의 이식성 체벽 전이 2례)

  • The implantation of malignant cells along the needle tract is an extremely rare complication after a percutaneous fine-needle aspiration biopsy(FNAB). However, it is very serious and may result in a change in the prognosis of lung cancer, especially in the curable early stage(T1-2,N0,M0). Recently, we experienced two cases of such complications. A 43 years old female underwent a fine needle aspiration biopsy and a right middle lobectomy with adjuvant chemotherapy due to an adenocarcinoma(T2N0M0). Two years later, a new tumor developed at the site of the needle aspiraton biopsy. It had the same pathological findings as the previous lung cancer. Therefore, it was concluded to be an implantation metastasis, and she was treated successfully by a right pneumonectomy and a resection of the chest wall mass with adjuvant radiotherapy. In another case, a 62 years old man was diagnosed with squamous cell lung cancer by a fine needle aspiration biopsy and underwent a right upper lobectomy(T2N0M0) with adjuvant chemotherapy. eight months later, a protruding chest wall mass developed at the aspiration site. It showed the same pathological findings as the previous lung cancer. Consequently, a total excision of the mass with adjuvant radiotherapy was done. Two years after the second operation, although the right lung was intact, a metachronous squamous cell lung cancer was found at the left lower lobe. The two patients were still alive 15 and 37months after thenresection of the chest wall mass, respectively.

Biological Characteristics of Scuticociliate, Philasterides dicentrarchi Isolated from Cultured Olive Flounder, Paralichthys olivaceus (양식 넙치에서 분리된 스쿠티카섬모충 Philasterides dicentrarchi의 생물학적 특성)

  • Biological characteristics of Philasteridies dicentrarchi (Ciliophora:Scuticociliate) isolated from the cultured olive flounder Paralichthys olivaceus was determined out by culture, growth conditions, and in vitro relief effect of chemical compounds. The scuticociliate has an active propagation ability by utilizing organic matters obtained from cell strain, bacteria, assorted feed, brain tissue and rotifer tissue. The ciliate achieved population growth activity under the conditions of $5{\sim}45\;ppt$ in salinity and pH 6-9. The ciliate had survived and propagated under the water temperature ranging $10{\sim}30^{\circ}C$, but active growth was observed in the temperature ranges of $10{\sim}25^{\circ}C$. Therapeutic trials were performed with formalin and hydrogen peroxide. The extermination time of the parasites with formalin was in 30 minutes both at 300 and 400 ppm, 60 minutes at 200 ppm, 90 minutes at 100 ppm, and 120 minutes at 50 ppm, respectively. In hydrogen peroxide treatments extermination time was 60 minutes in 300 ppm, 90 minutes in 200 ppm, and 150 minutes at 150 ppm and 100 ppm concentrations.

Comparison of OK-432 and Doxycycline Pleurodesis for Malignant Pleural Effusions Caused by Lung Cancer (폐암에 의한 악성 흉막삼출환자에서 OK-432와 Doxycycline 흉막유착술의 비교)

  • Background : Lung cancer is the leading cause of malignant pleural effusions, which is currently most commonly treated using pleurodesis via bedside thoracostomy. Several agents had been used for the treatment of pleural sclerosis, but with differing efficacies and associated side effects. Our purpose with this study was to compare the efficacy, side effects and disease free survival times of patients being treated with OK-432 and doxycycline sclerotherapy in lung cancer induced malignant pleureal effusions. Materials and Methods : 79 patients who underwent pleurodesis with OK-432 and doxycycline, between Jan.1994 and Aug. 2001, were retrospectively reviewed. Response 30 days following pleurodesis were determined from chest radiographs, with the disease free survival time being evaluated according to the response. Results : The success rates, 30 day followint pleurodesis, with OK-432 and doxycycline 83 and 87%, respectively (p=0.677). With regard to the side effects, fever was more common when OK-432 was used (59%, p=0.001), and pain was more common with doxycycline use (73%, p=0.008). There was no significant difference in disease free survival times between OK-432 (13.6 Months) and doxycycline (11.6 Months) (p=0.532). Conclusion : with the use of OK-432, for pleurodesis, was as effective as doxycycline, can be considered as an alternative treatment for malignant effusion in patients with lung cancer.

Deubiquitinase Otubain 1 as a Cancer Therapeutic Target (암 치료 표적으로써 OTUB1)

  • The ubiquitin system uses ligases and deubiquitinases (DUBs) to regulate ubiquitin position on protein substrates and is involved in many biological processes which determine stability, activity, and interaction of the target substrate. DUBs are classified in six groups according to catalytic domain, namely ubiquitin-specific proteases (USPs); ubiquitin C-terminal hydrolases (UCHs); ovarian tumor proteases (OTUs); Machado Joseph Disease proteases (MJDs); motif interacting with Ub (MIU)-containing novel DUB family (MINDY); and Jab1/MPN/MOV34 metalloenzymes (JAMMs). Otubain 1 (OTUB1) is a DUB in the OTU family which possesses both canonical and non-canonical activity and can regulate multiple cellular signaling pathways. In this review, we describe the function of OTUB1 through regulation of its canonical and non-canonical activities in multiple specifically cancer-associated pathways. The canonical activity of OTUB1 inhibits protein ubiquitination by cleaving Lys48 linkages while its non-canonical activity prevents ubiquitin transfer onto target proteins through binding to E2-conjugating enzymes, resulting in the induction of protein deubiquitination. OTUB1 can therefore canonically and non-canonically promote tumor cell proliferation, invasion, and drug resistance through regulating FOXM1, ERα, KRAS, p53, and mTORC1. Moreover, clinical research has demonstrated that OTUB1 overexpresses with high metastasis in many tumor types including breast, ovarian, esophageal squamous, and glioma. Therefore, OTUB1 has been suggested as a diagnosis marker and potential therapeutic target for oncotherapy.
