Government's Crisis Communication based on the Main Theme, Source, Crisis Responsibility, Frame Analysis and the Network Analysis on the Side Effects of Drug of the Press (정부의 위기 커뮤니케이션 연구: 의약품 부작용 관련 언론 보도에 나타난 주요 주제, 정보원, 위기 책임 귀인, 프레임 분석 및 네트워크 분석을 중심으로)

  • The side effects of drug is closely linked to the people's health and professional. In this case, press report is very important in evaluating the side effects of drug and people's attitude of government's health policy. This study analyzed press report on the side effects of drug based on the main theme, source, responsibility attribution on crisis, frame and network analysis. Theses is a difference of main theme between the press. Daily newspaper focused on the occurrence of crisis and economical newspaper focused on the manufacture company. Professional paper focused on the criticizing of the health policy. The main source of the press on the side effects of drug is the Congressman. There are few articles which mentions about the crisis responsibility. However most articles mentioned that the subject of crisis solutions is the manufacturer. This study found the main meaning by network analysis. This study highlighted the government's crisis management based on the result of content analysis and network analysis.

Development of Mining model through reproducibility assessment in Adverse drug event surveillance system (약물부작용감시시스템에서 재현성 평가를 통한 마이닝 모델 개발)

  • ADESS(Adverse drug event surveillance system) is the system which distinguishes adverse drug events using adverse drug signals. This system shows superior effectiveness in adverse drug surveillance than current methods such as volunteer reporting or char review. In this study, we built clinical data mart(CDM) for the development of ADESS. This CDM could obtain data reliability by applying data quality management and the most suitable clustering number(n=4) was gained through the reproducibility assessment in unsupervised learning techniques of knowledge discovery. As the result of analysis, by applying the clustering number(N=4) K-means, Kohonen, and two-step clustering models were produced and we confirmed that the K-means algorithm makes the most closest clustering to the result of adverse drug events.

  • 서울대병원 약물유해반응 관리센터와 지역약물감시센터가 공동으로 일반인들을 대상으로 '약물유해반응 경험 및 인식도 조사'를 한 결과 우리나라 성인 10명 중 2명은 의약품 부작용을 직접 경험하고도 적극적으로 조치하지 않은 것으로 나타났다. 또 약물유해반응으로 인해 조취를 취한 경우는 20.8%에 그쳐 의약품 부작용에 관한 홍보가 절실한 것으로 드러난다.

  • 우리나라는 최근 의료기술의 발달로 고령화 사회에 진입하여 만성 질환을 가진 노인의 수가 증가하고 있다. 이에 건강보조식품과 만성질환의약품을 복합하여 복용하는 노인들의 약물부작용이 다수 발생하고 있다. 이러한 부작용을 해결하기위해 노인만성질환자들이 복용하는 의약품과 건강보조식품의 상호작용 양상을 알아보고 해결방안을 모색해야할 필요성이 있다. 본 연구는 현재 만성질환의약품과 건강보조식품의 부작용이 일어나고 있는 5가지를 지정하여 노인들이 쉽게 알아볼 수 있도록 표시해주는 그림문자를 개발하였다. 건강보조식품과 만성질환의약품간의 상호작용에 대한 적극적인 복약지도와 그림문자를 통해 만성질환 노인환자의 부작용 발생을 예방함으로써 보다 안전하고 효과적인 의약품 사용이 가능해 질 것으로 기대된다.

A Study on Bi-LSTM-Based Drug Side Effects Post Detection Model in Social Network Service Data (소셜 네트워크 서비스 데이터에서 Bi-LSTM 기반 약물 부작용 게시물 탐지 모델 연구)

  • 본 연구에서는 소셜 네트워크 서비스(Social Network Service, SNS) 데이터로부터 약물 부작용 게시글을 추출하기 위한 순환 신경망(Recurrent Neural Network, RNN) 기반 분류 모델을 제안한다. 먼저, 처방 빈도가 높으며 게시글을 많이 확보할 수 있는 케토프로펜 약물에 대하여 국내 최대 소셜 네트워크 플랫폼인 네이버 블로그와 카페의 게시글(2005 년~2020 년)을 확보하고 최종 3,828 건을 분석하였다. 결과적으로 케토프로펜에 대한 3 종(약물, 부작용, 불용어)의 렉시콘을 정의하였으며 이를 기반으로 Bi-LSTM 분류모델 기준 87%의 정확도를 얻었다. 본 연구에서 제안하는 모델은 SNS 데이터가 약물 부작용 정보 획득을 위한 기존 (전자의무기록, 자발적 약물 부작용 보고 시스템 등) 자료원에 대한 보완적 정보원이 되며, 개발된 Bi-LSTM 분류모델을 통해 약물 부작용 게시글 추출의 편리성을 제공할 것으로 기대된다.

The Effectiveness and Side Effects of Conscious Sedation using Chloral hydrate, Hydroxyzine, and Nitrous oxide (Chloral hydrate, hydroxyzine 및 N2O/O2 의식하 진정의 효과와 부작용)

  • This retrospective study assessed the effectiveness and side effects of conscious sedation using chloral hydrate, hydroxyzine, and N2O/O2 in the sedation of 149 pediatric patients over 188 sedation sessions, and to identify associated variables. The effectiveness of the sedation was evaluated using the Houpt scale, and was considered effective for scale categories of excellent or good. Effectiveness and side effects were assessed every 15 minutes. The effectiveness decreased and side effects increased over time. The effectiveness of sedation during 60 minutes was 57.4%, and one or more side effects occurred in 18.1% of sessions. Effectiveness of sedation increased with body mass index (BMI). When patients were sedated at the beginning of the procedure, the effectiveness was greater. Side effects increased with patient age. When sedation was divided into two sessions, the number of sedation did not affect the effectiveness or side effects. It can be suggested that sedation should be performed over two separate sessions, as a single prolonged session may lead to reduced effectiveness and increased side effects. To maximize effectiveness and minimize side effects, several variables such as BMI, whether to sedate at the beginning of the procedure and age should be considered thoroughly before sedation.

Consumption Status and Experience of Adverse Effects of High-caffeine Energy Drink among High School Students (청소년의 고카페인 에너지음료 섭취 실태 및 부작용 경험)

  • This study was conducted in order to identify consumption and experience of adverse effects of high-caffeine energy drink among high school students. This study was a descriptive survey including 245 students in D city. Data were analyzed using descriptive statistics and ${\chi}^2$ test. In the results, 79.5% of the students had previous experience of energy drink consumption. 50.6% of students were experienced adverse effects, mainly palpitation. Among the general characteristics, there was a significant difference according to the experience of high-caffeine energy drink consumption depending on monthly allowance(p=.019) and drinking(p=.033), and a significant difference from adverse effects depending on gender(p=.001) and health condition(p=.006). Also, there was a significant difference according to the experience of adverse effects depending on drinking reason(p=.040) and drinking time(p=.005). After, policy measures are needed to limit the sale of high-caffeine energy drinks and continuous health education is needed to foster the ability of students to control themselves.

The Impact of Bladder Volume on Acute Urinary Toxicity during Radiation Therapy for Prostate Cancer (전립선암의 방사선치료시 방광 부피가 비뇨기계 부작용에 미치는 영향)

  • Purpose: Three-dimensional conformal radiation therapy (3DCRT) and intensity-modulated radiation therapy (IMRT) were found to reduce the incidence of acute and late rectal toxicity compared with conventional radiation therapy (RT), although acute and late urinary toxicities were not reduced significantly. Acute urinary toxicity, even at a low-grade, not only has an impact on a patient's quality of life, but also can be used as a predictor for chronic urinary toxicity. With bladder filling, part of the bladder moves away from the radiation field, resulting in a small irradiated bladder volume; hence, urinary toxicity can be decreased. The purpose of this study is to evaluate the impact of bladder volume on acute urinary toxicity during RT in patients with prostate cancer. Materials and Methods: Forty two patients diagnosed with prostate cancer were treated by 3DCRT and of these, 21 patients made up a control group treated without any instruction to control the bladder volume. The remaining 21 patients in the experimental group were treated with a full bladder after drinking 450 mL of water an hour before treatment. We measured the bladder volume by CT and ultrasound at simulation to validate the accuracy of ultrasound. During the treatment period, we measured bladder volume weekly by ultrasound, for the experimental group, to evaluate the variation of the bladder volume. Results: A significant correlation between the bladder volume measured by CT and ultrasound was observed. The bladder volume in the experimental group varied with each patient despite drinking the same amount of water. Although weekly variations of the bladder volume were very high, larger initial CT volumes were associated with larger mean weekly bladder volumes. The mean bladder volume was $299{\pm}155\;mL$ in the experimental group, as opposed to $187{\pm}155\;mL$ in the control group. Patients in experimental group experienced less acute urinary toxicities than in control group, but the difference was not statistically significant. A trend of reduced toxicity was observed with the increase of CT bladder volume. In patients with bladder volumes greater than 150 mL at simulation, toxicity rates of all grades were significantly lower than in patients with bladder volume less than 150 mL. Also, patients with a mean bladder volume larger than 100 mL during treatment showed a slightly reduced Grade 1 urinary toxicity rate compared to patients with a mean bladder volume smaller than 100 mL. Conclusion: Despite the large variability in bladder volume during the treatment period, treating patients with a full bladder reduced acute urinary toxicities in patients with prostate cancer. We recommend that patients with prostate cancer undergo treatment with a full bladder.

  • Recent advances in techniques and strategies use to detect cancer in its early stages and to treat it effectively has the survival rate of cancer patients and the number of long-term cancer survivors continually increasing. Unfortunately, many cancer survivors are at risk for various late and long-term effects of cancer treatments including the radiotherapy. Long-term cancer survivors can be also seen for a hospice and palliative care because of cancer recurrence and they are at risk of delayed reactions to radiotherapy. So, the understanding and knowledge of radiation reactions is required for the proper medical diagnosis, management, and coordination of the potential reactions that may occur in these care setting. In effort to increase the survival rate in cancer patients and to decrease the adverse effects of cancer treatment, many clinical studies have been and continue to be conducted. The efforts of these studies have thus resulted in the advancement of cancer treatments. Regrettably, the overall interest in how to manage adverse effects of cancer treatment such as radiotherapy appears seemingly low in clinical practice and its advanced studies as a whole are delayed and deficient. It is imperative that the medical community show an enthusiastic interest in the aftercare of cancer patients and cancer survivors in order to create a complementary integrative approach that will eliminate radiotherapy related pain/discomfort or illness in hospice and palliative care settings.

  • 전문의약품은 의약품안전사용서비스(DUR)를 이용하여 병원에서 처방받은 약에서 부작용이 나타났을 때 기록하고 다음에 약을 처방받을 때 부작용이 나타난 약과 비슷한 계열의 약은 처방받지 않는다. 하지만 일반의약품은 약 구매 기록조차 남지 않아 어떤 약을 언제 처방받았는지 모르고 부작용을 관리할 수 없어 불편함을 겪는다. 이 연구를 통해 제안하는 어플은 처방내역과 복약관리, 약 추천, 약국 찾기로 구성된다. 일반의약품을 처방받은 날짜와 시간, 증상, 효과, 부작용에 대하여 기록하며, 기록을 분석하여 증상에 대한 약을 추천함으로써 치료 효과를 높일 수 있다. 환자가 스스로 투약에 관심을 가지고 기록을 관리함으로써 환자가 주체가 되어 질병을 개선할 수 있다.
