A Study on the Chronic Change of the Space Allocation and the Development Trend in General Hospitals in Korea - Focused on the 1970's, 80's and 90's - (국내 종합병원의 연대별 면적변화와 발전과정에 관한 조사 연구 -1970, 80, 90년대를 중심으로-)

  • The Korean hospitals have been changed a lot from 1970's to 1990's, not only in their space allocation but also in their functions. These changes were affected by the medical development, social atmosphere, increasing population, needs of medical demands, and so on. In this study, we compared net area allocation involve in these affect of hospitals which were built in 1970's, 80's, and 90's, and through this comparison, we analyzed developing tendencies of hospitals. In doing so, we tried to provide basic research data for planning of new hospital buildings.

  • Patients use the senses all the time, mostly simultaneously. Senses are never turned off even patients in serious condition use some of the senses. Space is the inexhaustible source of stimuli. So far we have thought that it is impossible to come up with a set of rules and guidelines that cover all the possible situations involving senses and when there have been usually no conscious coordination between different sensorial stimuli and the comprehensive perception of space always results from the engagement of combination of senses and other inputs based on knowledge and experience. This study concerns about patients' demand analysis through senses. The reflection of patients' demand to the ward planning can develope the healing environment.

  • This thesis aims to establish a criterion to determine the scale of emergency beds and emergency wards equipped in general hospitals in the suburbs of Seoul. A new, large hospital (over 1000 beds) located in the southeast area of Seoul was selected and investigated for the case of this study, and throuh the P.O.E. a few mistakes in the method of determining the scale of the emergency department there came to light. Joining together, the effectiveness of the scale determination method devised by us (Lee's formula) was verified, and finally the optimum scale of the emergency department for this general hospital was proposed.

  • Recently, hospital architecture has acknowledged remedial value of environment and atmosphere, which consider patient's statement of physical, spiritual, psychological and social focus, escape from the past focus of functional and rational space composition. The many research of hospital architecture just analyze elements of architectural planing which focus how to make a healing environment for patients. The purpose of this study is to abstract the design principle of architectural space composition for healing environment significance, through case study which follow up space composition. To abstract healing environment significance by space composition, each case study hospitals should be analyzed and estimated on the spatial composition elements. It is to provide the basic data for effective architectural planning of the hospital.

Analysis of Pain Records Using Electronic Nursing Records of Hospitalized Patients in Medical Units at a University Hospital (일개 대학병원 내과 병동 입원환자의 전자의무기록에 사용된 통증간호 기록 분석)

  • Purpose: This study was done to analyse nursing records to identify the nature of pain and actual conditions of pain management in patients hospitalized in one university hospital. Methods: The participants in this study were 783 patients with a length of stay of 3 to 30 days who were discharged from medical wards between June 1 and June 30, 2009. Data on nursing records related to pain management from these patients were reviewed using the Electronic Nursing Records (ENRs) system. Results: Over 30 percent of 10,702 nursing records related to pain assessment had no record on region, severity, nature or frequency of pain. About 30 percent of 13,638 nursing records related to pain intervention showed non-drug pain management techniques. Conclusion: Accurate and complete records on pain assessment including region, severity, nature and frequency of pain are essential to effectively manage patients' pain. Improvement in ENRs system for better assessment and management of pain is required as well as education programs on a standardized measuring tool for both nurses and patients.

The Effect of Child's Mother and Nurse Partnership on the Anxiety and Perceived Quality of Nursing Care of Hospitalized Child's Mother (일 종합병원 아동병동의 환아 어머니와 간호사의 파트너십이 환아 어머니의 불안과 간호의 질 인식에 미치는 영향)

  • This study was conducted to investigate child's mother and nurse partnership, anxiety, and quality of pediatric nursing care, and to identify factors influencing anxiety and quality of pediatric nursing care of the hospitalized child's mother. A descriptive survey was conducted from September 21 to October 4, 2015. Data were collected using a self-reporting questionnaire of 140 the mothers of a hospitalized child in a general hospital. Completed data were analyzed using descriptive statistics, t-test, ANOVA, and multiple regression analysis by IBM SPSS 18.0. The results showed that the frequency of hospitalization, genito-urinary disease, other diseases, and child's mother and nurse partnership significantly influenced anxiety. Additionally, the frequency of hospitalization, genito-urinary disease, other diseases, and child's mother and nurse partnership significantly influenced the quality of pediatric nursing care. Based on these results, child's mother and nurse partnership had a significant predictor of the anxiety and the quality of pediatric nursing care. These finding showed that there was a need to develop educational and supportive strategies to enhance and improve the partnership between hospitalized child's mother and nurse to ensure qualified pediatric nursing care.

Determinants of Patients Satisfaction and Intent to Revisit Oriental Medical Hospitals (한방병원 환자 만족도 및 재이용 의사 결정요인)

  • This study aimed to investigate the determinants of patients satisfaction and intent to revisit oriental medical hospitals. The level of overall patient satisfaction was used as an intervening variable and the level of intent to revisit was used as a dependent variable. The sample used in this study consisted of 578 patients from 3 oriental medical hospitals located in Chungnam Province. Data were collected from October 2014 to December 2014 with a structured and self-administrated questionnaire and analysed using path analysis. The results of the study indicate that oriental medical hospitals should make an effort to improve the overall satisfaction of patients, in the case of outpatient, especially focusing on the doctors' service and medical procedure, and in the case of inpatient, especially focusing on the facilities convenience, medical price and hospital ward life which will lead to high level of intent to revisit of patients.

Development and Validation of a Education Nurse Specialist-Led Education Satisfaction Scale for Nurses Working at Comprehensive Nursing Care Service Wards (교육전담간호사가 주도하는 교육 프로그램에 대한 간호·간병통합서비스 병동 간호사의 교육 만족도 측정 도구 개발 및 타당도 검정)

  • Purpose: The purpose of this study was to develop and validate a education nurse specialist-led educational satisfaction scale for nurses working at comprehensive nursing care service wards. Methods: A methodological study was conducted. A total of 237 nurses working at comprehensive nursing care service wards in a general hospital participated in this study. The scale was developed through literature reviews, in-depth interview, development of preliminary items, verification of content validity, development of secondary items, evaluation of construct validity, and extraction of final items. Analysis included exploratory factor analysis, pearson's analysis, and reliability analysis using cronbach's α. Results: The education nurse specialist-led educational satisfaction scale for nurses working at comprehensive nursing care service ward consisted of 21 items. Two factors (critical thinking disposition and clinical competency) were identified which explained 63.5% of the total variance. Cronbach's α of each factors were >.95. Conclusion: The results suggested that the education nurse specialist-led educational satisfaction scale for nurses working at comprehensive nursing care service wards demonstrated acceptable validity and reliability. Items of the instrument can assess the level of satisfaction with regards to education led by education nurse specialist among nurses working at comprehensive nursing care service wards.

Application of Sociodrama for Reducing Burnout of Nurses in General Hospital (종합병원 간호사의 소진개선을 위한 소시오드라마의 활용)

  • Objectives : The aim of this study was to evaluate the effect of sociodrama for reducing burnout among health care workers and to investigate relationship between the job satisfaction, conflict with other professions, and burnout. Methods:15 nurses in general hospital participated in sociodrama directed by psychiatrist. One week after acting sociodrama, they were requested to complete ward organizational feature scale(WOFS) and maslach burnout inventory(MBI). The other twenty nurses in same ward requested to complete questionnaire without participating in sociodrama at the same time. Results : Sociodrama group(N=15) showed significantly lower score in burnout when compared to control group(N=17). Nurse/physician relationship was significantly correlated with job satisfaction and burnout. After controlling for interaction between variables by multivariate regression analysis, only job satisfaction was significant predictive factor to burnout. Conclusion : Our result suggests that the sociodrama can be effective in reducing burnout of health care workers in general hospital. These finding also underline the importance of relationship with physicians in job satisfaction and burnout of healthcare workers in general hospital.

The Effect of Organizational Efficacy, Self-efficacy, Nursing Professionalism on Organizational Commitment in General (종합병원간호사의 조직효능감, 자기효능감, 간호전문직이 조직몰입에 미치는 영향)

  • The purpose of this study was designed to help the operation of the hospital organization through analysis of factors influencing organizational commitment of general hospital nurses. 129 nurses in general hospitals were targeted in G City. This study was conducted to analyze the effects of group efficacy, self-efficacy, and nursing professionals on organizational commitment. T-test, ANOVA, Pearson's correlation coefficient, Descriptive statistics and multiple regression were used. Stepwise regression analysis found that organizational commitment was affected by group efficacy(β=0.240, p =.003), nursing professionalism(β=0.229, p =.004) and daytime work(β=0.249, p =.003), The F statistic was 10.478 (p <.001). Therefore, in order to improve organizational commitment, it will be necessary to prepare communication programs and cooperation programs per unit to improve collective efficacy. In addition, it is necessary to improve the system, such as a system dedicated to night workers, so that the working hours of nurses in general hospitals can be maintained constant. In addition, it is necessary to develop a support program so that nursing professionals do not decrease due to disappointment in reality in the nursing field.
