산업간호교육에 관한 실태조사 - 기본 간호교육을 중심으로 -

  • 본 연구는 산업간호사의 역할 확대에 따른 교육과정의 개선 및 보수교육 프로그램의 모색을 위한 기초자료로서 기본간호교육과정에서의 산업 간호교육 실태를 파악하고자 시도되었다. 구체적으로는 첫째, 기본간호교육과정의 일반적 특성을 파악하였으며 둘째, 지역사회간호학의 학습내용을 파악하였고, 세째, 산업 간호교육실태를 분석하였다. 연구방법은 먼저 1991년 2월 18일 부터 2월 25일까지 기존 문헌을 토대로 기본 간호교육과정의 일반적 특성과 지역사회간호학의 학습내용을 파악하였고, 산업 간호교육 실태에 대해서는 2월 26일부터 3월 15일까지 전국에 있는 간호교육기관을 대상으로 작성된 조사표에 의해 전화 또는 서신으로 자료를 수집하였다. 조사 결과는 다음과 같다. 1) 연구의 대상 산업간호교육 실태조사의 대상은 조사에 응답한 간호교육기관 41개교로서 '90년 현재 전국 간호교육기관의 66.1%를 차지하는 것이다. 교육과정별로는 간호대학 14개교 전문대학 27개교였고, 지역별 분포에 있어서는 대학과정 중 6개교(42.9%)가 서울에 있으며 전문대학의 경우 전남 6개교(22.2%), 경북 5개교(18.5%)의 순으로 나타났다. 2) 간호교육과정의 특성 기존의 연구결과를 토대로 교과목 분포 및 주전공 교과목의 내용을 파악하였다. 대학과정의 경우 평균 141.3학점, 전문대학의 경우 평균 120학점이 필수 이수 학점인 것으로 나타났고, 두 경우 모두 전공교육에 관한 학점이 총 이수학점 중 대학 55.5%, 전문대 57.2%를 차지하였다. 주전공 교과목 중에서는 성인간호학 및 실습, 기초간호과학 및 실험, 모아간호 및 실습, 지역사회간호학 및 실습, 정신간호 및 실습의 배당 학점이 특히 많았다. 3) 지역사회간호학의 학습내용 산업간호 교육의 기본 개념틀올 제시하고 있는 지역사회간호학은 체계이론과 Stevens의 간호이론을 토대로 개발된 모형을 중심으로 살펴 보았는데 이 모형에 따르면 산업간호는 지역사회간호 문제의 하나로 다뤄지고 있다. 4) 산업간호 교육 실태 조사 대상 학교 모두 산업간호에 관한 내용을 강의에서 다루고 있었으며, 6개 학교에서는 실습도 병행된 바 있다. 강의는 주로 전문대의 경우 3학년(22개교), 2학년(5개교)에서, 대학의 경우 모두 4학년에서 이루어졌다. 산업간호 강의의 편성형태는 36개교(87.8%)가 지역사회간호의 일부로 다루고 있었으며, 강의시간수는 5~9시간이 19개교(47.5%)로 가장 많았다. 강의시간에 주로 사용되는 교재로는 교수가 직접 작성한 참고자료나 보건학강좌, 산업보건학 등이 사용되었다. 강의내용 또한 매우 다양한 것으로 나타났는데, 특히 전문대학의 경우가 더욱 다양했으며, 전반적으로는 산업간호사업, 산업간호개념, 직업병관리가 가장 많았다. 한편 실습운영에 있어서는 산업체의 비협조 또는 거리상의 문제 등이 어려운 점으로 지적되었다. 실습대상인 업체로는 종이제조업, 제철업, 신발제조업, 전자업체, 봉제업체, 관공서, 유리제조업체가 있었고, 주로 1주일간 실습지침서를 이용한 현지 교수 중심의 지도와 1,2회의 conference에 의해 이루어졌다. 그러나, 1일 견학하는 정도로 운영하는 학교와 선택 학점으로서 3주동안 실습을 하도록 하는 경우도 있었다. 이상의 결과를 바탕으로 다음과 같은 결론을 내릴 수 있겠다. 첫째, 전반적인 전공교육과 특히 지역사회간호학을 통해서 산업간호사의 업무 수행에 필요한 일반적인 능력이 길러지고 있다고 볼 수 있다. 둘째, 산업간호사의 업무 중 보다 전문적인 지식과 기술을 요하는 부분에 대해서는 산업간호라는 교과를 통해 보다 체계적으로 다루어져야 할 것이다. 따라서, 이에 사용될 수 있는 교과서의 개발, 실습현장의 마련, 시간 및 학점의 증가 등이 시급히 이루어져야 하겠다.

The Effects of Music Therapy on Postpartum Blues and Maternal Attachment of Puerperal Women (음악요법이 산모의 산후우울과 모아애착에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effects of music therapy on postpartum blues and maternal attachment of puerperal women. Methods: The research design was a nonequivalent control group non-synchronized design. The participants were puerperal women who agreed to participate in this study and through a convenience sampling, 60 puerperal women were recruited (30 in the experimental group, 30 in the control group). After measuring postpartum blues and maternal attachment, music therapy was provided to the experimental group over 40 min, once a day, and for 8 days. Then, postpartum blues and maternal attachment for the experimental and control group were measured again on the 8th day. The data were analyzed using the SPSS WIN 12.0 Program. Results: The first hypothesis that "the degree of postpartum blues for the experimental group who participated in music therapy would be lower than that of the control group" was accepted (t=4.350, p<.001). The second hypothesis that "the degree of maternal attachment of the experimental group who participated in music therapy would be higher than that of the control group" was accepted (t=4.828, p<.001). Conclusion: These findings indicate that music therapy has positive influences on decreasing postpartum blues and increasing maternal attachment of puerperal women.

Effects of Kangaroo Care on Anxiety, Maternal Role Confidence, and Maternal Infant Attachment of Mothers who Delivered Preterm Infants (캥거루식 접촉이 미숙아 어머니의 불안, 모성역할자신감 및 모아애착에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of Kangaroo Care(KC) on anxiety, maternal role confidence, and maternal infant attachment of mothers who delivered preterm infants. Methods: The research design was a nonequivalent control group pretest-posttest. Data was collected from September 1. 2006 to June 20. 2007. The participants were 22 mothers in the experimental group and 21 in the control group. KC was applied three times per day, for a total of ten times in 4 days to the experimental group. Results: The degree of anxiety was statistically significantly different between the two groups but maternal role confidence and maternal infant attachment was statistically insignificant. Conclusion: This data suggests that KC was effective for mothers anxiety relief but it was not effective for maternal role confidence and maternal infant attachment of mothers. The implications for nursing practice and directions for future research need to be discussed.

Prediction Model on Mother-infant Attachment during the Early Postpartum Period (산욕기 어머니의 모아애착 예측모형)

  • Purpose: The purpose of this study is to identify the influencing factors of mother-infant attachment and construct a descriptive model that explains mother-infant attachment during the early postpartum period. Method: The hypothetical model of this study consisted of 8 variables with 23 constructed paths. The subjects of this study were 152 postpartum women. Data was analyzed to test the hypothetical model using covariance structure analysis. Result: The final model which is modified from the hypothetical model improved to Chi-Square 41.92, GFI .95, AGFI .89, RMSR .02, RMSEA .06, NFI .94, and NNFI .95. Mother-infant attachment during the early postpartum period was proven to be influenced directly by neonatal perception, maternal sensitivity, and maternal-fetal attachment and also indirectly by social support, maternal-fetal attachment and maternal identity. These variables accounted for 32% of the variance of the mother-infant attachment during the early postpartum period. Conclusion: It is necessary that the nurses provide postpartum women with an intervention using social support for improving maternal identity and alleviating maternal role strain. It can be helpful to improve maternal sensitivity and in the end it will facilitate the mother-infant attachment during postpartum period.

A Study of the Effects of Behavior Contact in Early Mother-Infant Attachment (분만 직후 어머니의 신생아 접촉이 모아애착행위에 미치는 영향)

  • Purpose: The main purpose of this study was to establish a nursing intervention data base to improve maternal attachment. Method: The first group of mothers( control group), experienced their first physical contact with their infants after being discharged from the hospital. The second group (experimental group) practiced early initial mother-infant postpartum contact known as the most sensitive period for founding maternal-infancy attachment. The subjects of this study gave birth to normal infants at M hospital from Aug.25 to Sept.30, 2004. During the same time, data was collected through direct observation, with instruments designed by Cropley et al., to assess the behaviors of normal attachment. The statistical methods for data analysis were percentage, mean, standard deviation and t-test with an SPSS program. Result: The group practicing initial mother-infant contact, showed a higher degree of physical and functional bonding than the group experiencing the first contact after discharge from the :10spital (control group)(p<.0l). The group practicing initial mother-infant contact, showed higher degrees of bonding attachment assessments than the group experiencing their first initial contact after their discharge from the hospital(p<.01). Conclusion: The group practicing early mother-infant contact, showed more maternal-infant interaction than the group experiencing their initial contact after their discharge from the hospital. These results show that maternal attachment behavior increases according with an early initial mother-infant contact.

Effects of Sensory Stimulation Program Conducted by Primipara on the Physical Growth and Mother-Infant Feeding Interaction for Full Term Infant (초산모의 감각자극 프로그램이 영아의 성장 및 수유시 모아 상호작용에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effectiveness of sensory stimulation program administered by primipara on the physical growth and mother-infant feeding interaction for the first 6 months of infancy. Method: Data were collected from December 1, 2001 to June 30, 2003. A total of 35 mothers and their infants were assigned to a control(n=17) and an experimental(n=18) group. Mothers in the experimental group received education related to sensory stimulation program and administered to their infants twice a day for 6 months. Both groups were measured the weight, length, head circumference and chest circumference of infants at 6 weeks, 10 weeks, 14 weeks, 18 weeks, and 22 weeks and the mother-infant feeding interaction at 6 weeks, 10 weeks through the home visiting. Result: Compared to the control group, the experimental group had significantly larger increases in head circumference for intervention period. In addition to, the experimental group showed significantly higher score in mother-infant interaction than control group. Conclusion: These data suggested that sensory stimulation program administered by primipara may improve the physical growth of full term infant and mother-infant feeding interaction.

Effects of Early Contact on Maternal Infant Attachment (모아의 조기 접촉이 모성의 애착행위에 미치는 영향)

  • Purpose: The purpose of this study was to evaluate the effects of early contact between mothers and infants on attachment behaviors. Methods: The participants were purposely allocated to an experimental group (n=20) and a control group (n=20). For the experimental group, early contact between mothers and newborns took place for 10 minutes. Mother-infant attachment was assessed in the nursery. Data analysis was done using frequency, percentage, $x^2$ test, mean, standard deviation, and t-test with the SPSS/WIN program. Results: The experimental group practicing early mother-infant contact, showed a higher degree of identifying behaviors (p<.05), modalities of interaction (p<.05), and caretaking behaviors (p<.05) than the control group. Conclusion: The early mother-infant contact at 2 hours post-delivery had positive effects on mother-infant attachment. Therefore an early contact can be recommended for all mothers-to-be as a way of improving mother-infant relationships.

Recognition of Mother-Child Interaction by Preschoolers' Mothers (학령전기 아동 어머니의 모아상호작용에 대한 인식)

  • Purpose: The purpose of this study was to explore recognition of mother-child interaction by preschoolers' mothers. Methods: Data were collected through in-depth interviews with mothers of 12 preschoolers. The data were analyzed following Downe-Wamboldt's content analysis methodology. Results: From the data, 170 significant statements were selected and were classified into 7 categories and three domains (maternal, child, dyadic). First, the maternal domain consisted of maternal sensitivity, acceptable responsiveness, and child developmental encouragement. Second, the child domain consisted of child's sensitivity, responsiveness, and initiative. Third, the dyadic domain consisted of mutuality. Conclusion: These results suggest that preschoolers' mothers recognize the significant qualitative aspects of interaction with their children. Based on these results, instruments for preschooler-mother interaction need to be developed.

Effects of an Integrated Self-Management Program on Self-Management, Glycemic Control, and Maternal Identity in Women with Gestational Diabetes Mellitus (통합 자가관리 프로그램이 임신성 당뇨병 임부의 자가관리, 혈당 및 모성 정체성에 미치는 효과)

  • Purpose: The purpose of the study was to investigate the effects of an integrated self-management program on self-management, glycemic control, and maternal identity in women with gestational diabetes mellitus (GDM). Methods: A non-equivalent control group non-synchronized quasi-experimental design was used. A total of 55 women with GDM were recruited from Cheil General Hospital, Seoul, Korea and were assigned to an experimental (n=28) or control group (n=27). The participants were 24-30 weeks pregnant women who had been diagnosed with GDM as of July 30, 2010. The program was conducted as a 1 hour small group meeting 3 out of 5 times and by telephone-counseling 2 out of 5 times. The integrated self-management program was verified by an expert panel. Results: Although there was no significant reduction in HbA1c (U= -1.17, p=.238), there were statistically significant increases in self-management (U= -3.80, p<.001) and maternal identity (U= -4.48, p<.001), and decreased 2-h postprandial glucose levels (U= -2.43, p<.015) in the experimental group compared to the control group. Conclusion: These findings suggest that an integrated self-management program for women with GDM improves self-management, maternal identity, and glycemic control. Further studies are needed to identify the effects of an integrated self-management program on pregnancy and neonatal outcomes.

Effects on Maternal Attachment, Parenting Stress, and Maternal Confidence of Systematic Information for Mothers of Premature Infants (미숙아 어머니를 위한 체계적인 정보제공이 모아애착, 양육스트레스, 양육자신감에 미치는 효과)

  • Purpose: The aim of this study was to examine the effects of systematic information on maternal attachment, parenting stress and maternal confidence of mothers of premature infants. Methods: Using a non-equivalent control group pre-post quasi-experimental design, 42 participants were assigned to one of two groups, an experimental group (n=20), which received systematic information; and a control (n=22), which received the usual discharge education. Prior to the intervention, baseline data were collected (pretest) and then the systematic information was provided four times for the experimental group. Maternal attachment, parenting stress and maternal confidence were measured. Data were collected three times: Seven to 10 days prior to discharge, day of discharge, and two weeks after discharge. Results: Mothers in the experimental group had significantly higher scores in maternal attachment (F=6.16, p=.005), lower parenting stress scores (F=5.56, p=.004), and higher scores for maternal confidence (F=16.50, p<.001) than mothers in the control group. These tendencies were progressively enhanced even two weeks after discharge from the hospital. Conclusion: The results indicate that provision of systematic information to mothers with premature infants is an effective intervention to enhance mothers' maternal attachment, decrease parenting stress, and enhance maternal confidence.
