Genetics of Hereditary Peripheral Neuropathies (유전성 말초신경병의 유전학)

  • Hereditary peripheral neuropathies can be categorized as hereditary motor and sensory neuropathies (HMSN), hereditary motor neuropathies (HMN), and hereditary sensory neuropathies (HSN). HMSN, HMN, and HSN are further subdivided into several subtypes. Here, we review the most recent findings in the molecular diagnosis and therapeutic strategy for hereditary peripheral neuropathies. The products of genes associated with hereditary peripheral neuropathy phenotypes are important for neuronal structure maintenance, axonal transport, nerve signal transduction, and functions related to the cellular integrity. Identifying the molecular basis of hereditary peripheral neuropathy and studying the relevant genes and their functions is important to understand the pathophysiological mechanisms of these neurodegenerative disorders, as well as the processes involved in the normal development and function of the peripheral nervous system. These advances and the better understanding of the pathogenesis of peripheral neuropathies represent a challenge for the diagnoses and managements of hereditary peripheral neuropathy patients in developing future supportive and curative therapies.

Lately, embodied this to aid a little in disease treatment that follow that there is no invasion that there is no stimulation by medicine development. Go amplitude treatment pulse (traditional magneto-therapy of greatly great that strong) does curative effect greatest at short time and becomes thought that demand is magnified greatly at the future in this research.

  • Fatigue is a significant problem in health care. The relationship between chronic fatigue and psychiatric disorders has generated significant interest in recent years. Increased rates of psychiatric disorder among individuals with chronic fatigue have been reported. Fatigue may be defined as physical and/or mental weariness resulting from exertion. The cause of fatigue is complex, influenced by events occurring in both the periphery and the central nervous system. Because fatigue may result from a number of causes, it should be treated comprehensively. The treatment is largely symptomatic and rehabilitative.

The Influences of Depression, Anxiety, Social Support and Knowledge of Anticancer Drugs on the Chemotherapy-induced Peripheral Neuropathy among Colorectal Cancer Patients Receiving Oxaliplatin (Oxaliplatin을 투여 받는 대장암 환자의 우울, 불안, 사회적지지, 항암제 지식수준이 말초신경병증에 미치는 영향)

  • Purpose: The purpose of this study was to identify factors influencing chemotherapy-induced peripheral neuropathy among colorectal cancer patients receiving oxaliplatin. Methods: A total of 132 patients hospitalized for chemotherapy were surveyed at K University Hospital in Seoul, Korea. This study was a descriptive causal relationship study using a self-report questionnaire survey method. Correlation and multiple regression analysis between the factors were performed using SPSS 18.0. Results: The regression model was significant (F=31.64, p<.001), which meaned that the experience of chemotherapy-induced peripheral neuropathy among the participants was statistically significant. The factors influencing the chemotherapy-induced peripheral neuropathy were depression (${\beta}=.34$, p<.001), followed by anxiety (${\beta}=.32$, p<.001), medical staff support (${\beta}=-.17$, p=.037) and the level of knowledge of anticancer drugs (${\beta}=-.16$, p=.045). The explanatory power of these factors on the chemotherapy-induced peripheral neuropathy of colorectal cancer patients was 69%. Conclusion: The factors influencing the chemotherapy-induced peripheral neuropathy of colorectal cancer patients receiving oxaliplatin were identified as depression, anxiety, level of knowledge of anticancer drugs and medical staff support.

Effectiveness of Non-Pharmacologic Interventions in Chemotherapy Induced Peripheral Neuropathy: A Systematic Review and Meta-Analysis (항암화학요법 관련 말초신경병증에 적용한 비약물적 중재의 효과: 체계적 문헌고찰 및 메타분석)

  • Purpose: This study was conducted to evaluate the effectiveness of non-pharmacologic interventions in chemotherapy-induced peripheral neuropathy (CIPN). Methods: PubMed, Cochrane Library CENTRAL, EMBASE, CINAHL, and several Korean databases (Until August 2017) were searched. The main search strategy combined terms for peripheral neuropathy and presence of neoplasms. The risk of bias was assessed using the Cochrane's Risk of Bias tool for randomized studies and the Risk of Bias Assessment tool for non-randomized studies. To estimate the effect size, a meta-analysis of the studies was performed using the Rev Man 5.3 program of the Cochrane Library random-effects models were used in the analyses. Results: Twenty-two studies with a total of 954 participants met the inclusion criteria. Of the 22 studies, 12 were used to estimate the effect size of the non-pharmacologic interventions. The non-pharmacologic interventions used in patients with CIPN were exercise, acupuncture, massage, and foot bath. The acupuncture significantly reduced CIPN symptoms and signs (d=-0.71) and CIPN pain (d=-0.73) (p<.001). Massage and foot bath were also effective in reducing CIPN symptoms (d=-0.68; 95% CI=-1.05, -0.30; p<.001; $I^2=19%$).Exercises were effective in improving muscle strength and endurance(d=-0.55) and quality of life (d=-2.96), but they were not significantly effective in improving CIPN. Conclusion: Although these results provide little evidence of the effectiveness of acupuncture, massage, and foot bath in the treatment of CIPN, they suggest that these interventions can reduce CIPN symptoms in patients with cancer. However, the findings of this study should be interpreted with caution as there is a relative lack of data in this field, and additional well-designed studies are needed.   • Burning mouth syndrome(BMS) is a chronic oral pain and a symptom complex disorder usually unaccomplished by mucosal lesion or other clinical and laboratory signs of organic disease. BMS is characterized by a spontaneous burning sensation that mainly affects middle-aged and postmenopausal women. The etiology of BMS is poorly understood even though evidence for a possible neuropathic pathogenesis. BMS cause from various local or systemic factor, including nutritional deficiencies, hormonal change, local infection, dental procedure, dry mouth, medication and systemic disease including diabetes mellitus. Many studies suggest peripheral alteration in sensory of trigeminal nerve system. BMS patients with supertaster indicates pathologies of central and peripheral nerve system induced by an alteration in the taste system at the level of chorda tympani and glossopharyngeal nerve. The author discuss our current understanding of etiology and pathogenesis of BMS that refered chronic oral pain.

  • According to International Associating for the Study of Pain (IASP) definition, neuropathic pain is a disorder characterized by dysfunction of the nervous system that, under normal conditions, mediates virulent information to the central nervous system (CNS). This pain can be divided into a disease with provable lesions in the peripheral or central nervous system and states with an incorporeal lesion of any nerves. Both conditions undergo long-term and chronic processes of change, which can eventually develop into chronic pain syndrome, that is, nervous system is inappropriately adapted and difficult to heal. However, the treatment of neuropathic pain itself is incurable from diagnosis to treatment process, and there is still a lack of notable solutions. Recently, several studies have observed the responses of CNS to harmful stimuli using image analysis technologies, such as functional magnetic resonance imaging (fMRI), positron emission tomography (PET), and optical imaging. These techniques have confirmed that the change in synaptic-plasticity was generated in brain regions which perceive and handle pain information. Furthermore, these techniques helped in understanding the interaction of learning mechanisms and chronic pain, including neuropathic pain. The study aims to describe recent findings that revealed the mechanisms of pathological pain and the structural and functional changes in the brain. Reflecting on the definition of chronic pain and inspecting the latest reports will help develop approaches to alleviate pain.

  • Neurogenic tumors are the most common posterior mediastinal tumors and accounting for $19{\sim}39%$ of all mediastinal tumors and 75% of all posterior mediastinal tumors. Neurofibromatosis is an autosomal dominant disorder with variable expression of tumors, including neurologic tumors of the peripheral nerves, nrve roots, and plexi. A posterior mediastinal neufibroma in neurofibromatosis patients is rare. We report here a case of posterior mediastinal neurofibroma in a patient with type 1 neurofibromatosis.
