Terminal pediatric patient include congenital metabolic abnormalities, chromosome aberrations, congenital anomalies, neuromuscular diseases and other incurable conditions as well as malignant tumors. One third of these diseases are cancers, and two thirds of cancers are lymphadenoma and leukemia. Terminal pediatric patient may feel fear, anger and frustration against treatment process in hospitals, lose control of themselves and stay in helplessness due to restrictions within controlled hospital environment. This study examined the relationship between hospitalized childrens behavior and music using Contextual Support Model of Music Therapya theory stating therapeutic music environments possess three elements of Structure, Autonomy support and Involvement, and increase childrens active engagement. Focused on these three therapeutic elements, this study analyzed music therapy cases of terminally ill children to examine the relationship between childrens behavior and music as environmental supportive medium in the environment. This study is on a single case case subject, a 10-year-old girl with acute lymphocytic leukemia. Nine sessions of music therapy activities were conducted and analyzed using qualitative method. Focusing on three therapeutic elements of Contextual Support Model, analysis of sessions was made on the basis of activities specifically designed for this study. Main music therapy activities included singing while playing musical instruments, singing while listening to music through CD player, making up songs, and searching for song lines. The findings of this study are as follows: a) in terms of Structure, music therapy suggested a direction for effective musical activities for terminally ill children by providing environments where child can sing, play musical instruments and make up songs; b) in terms of Autonomy support, music therapy encouraged childs voluntary participation by giving her chances to make choice while singing or playing musical instruments; c) in terms of Involvement, child could gain positive approval of the therapist and conduct musical activities in cooperation with the therapist. Music therapy serves as supportive medium in the environment, develops spontaneity and self-reliance in emotionally depressed children, and promotes voluntary attitudes in the restricted hospital environment. Based on the positive effects of music therapy, more studies need to be conducted with an approach to more various terminal pediatric patients.

Purpose: The purpose of this study was to compare the periodontal status of end-stage renal disease patients undergoing dialysis and referred for intraoral evaluation prior to renal transplantation surgery with those having normal kidney function. Materials and Methods: Patients who had been undergoing dialysis for end-stage renal disease and been referred to the Dental Clinic Center by the Department of Nephrology at University Hospital for intraoral evaluation prior to kidney transplantation surgery. For comparison of periodontal status, subjects without abnormalities in kidney function were matched with the patients by age and gender and selected as healthy controls. The patients' age, gender, comorbidities, type of dialysis received, and duration of dialysis were investigated by reference to their medical records, and data on their periodontal status were analyzed via the relevant periodontal records. Results: A total of 102 patients, including 51 dialyzed patients and 51 healthy control group subjects, participated in this study. In the patients with end-stage renal disease undergoing dialysis with periodontal probing depth of 5 mm or more, percentage of sites with clinical attachment level of 4 mm or more, percentage of teeth with bleeding on probing, number of missing teeth, and ratio of moderate to severe periodontitis were all significantly greater than in the healthy controls. Conclusion: The periodontal status of end-stage renal disease patients undergoing dialysis and referred for intraoral evaluation prior to kidney transplantation was worse than that of healthy controls.
In this paper, we propose a method of analysing the relation between the patient's face color and his(her) kidney disease using image processing technology. This method is based on the ocular inspection which is one of the most famous diagnosis methods used in the oriental medical system. The way of processing and analysing the face image, which is for visualization and objectification of the color difference, is included. The objects are selected from the patients who suffer the kidney disease and use the hemodialyzer. Their facial images and clinical data are collected. From these data, we propose a hypothesis that the color of the patient's face is changed according to the patient's kidney state. At the same time, we present two algorithms of extracting the specific part of face which can identify the state of the patient's kidney and tracing the history of the color's change. This proposed method is evaluated through the practical experiments and their analysis.
Purpose: This study is to report the operating result of the Hospice Cyber Counselling Program for improving quality of lift for terminal patients and their families. Methods: This study was performed followed by counsellor training, building strategy of public relations, management, and evaluation of the effect in order. Results: 72 subjects visited our cyber homepage and became members for approximately 9 months from Oct. 2004 to July 2005; 17 subjects in their 20s (23.6%), 12 subjects in their 30s (16.7%), 17 subjects in their 40s (23.6%), and 13 subjects above 50s (18%). Purpose: The aim of this descriptive study was to identify factors influencing quality of life among family caregivers of non-cancer patients at the end-of-life stage. Methods: A cross-sectional survey was conducted using a questionnaire. Participants included 172 family caregivers caring for non-cancer patients. Data were collected from April to May 2016 and analyzed with descriptive statistics, an independent t-test, one-way ANOVA, Pearson's correlation coefficient, and a hierarchical regression analysis using the SPSS/WIN 24.0 program. Results: The mean of the participants' quality of life was 51.70±9.98. Factors influencing quality of life among family caregivers were spiritual care (𝛽=-.45, p=.021), coordination among family members or relatives (𝛽=-.27, p=.029), and psychological support (𝛽=-.04, p=.031). The explanatory power of the model was 21.0%. Conclusion: The findings of this study suggest that care needs; spiritual care, coordination among family members or relatives, and psychological support are important factors for family caregivers' quality of life. To improve quality of life among family caregivers who are taking care of non-cancer patients at the end-of-life stage, national systems establishing comprehensive support considering the respective care needs of patients are crucial.
Purpose: This study was conducted to better understand the illness experiences and palliative care needs in community-dwelling persons with cardiometabolic diseases. Methods: This qualitative descriptive study was conducted with 11 patients (and three family members) among 28 patients contacted. Interviews were led by the principal investigator in her office or at participants' home depending on their preference. All interviews were digitally recorded and transcribed by a research assistant. The interviews were analyzed by two independent researchers using a conventional method. Results: Participants' ages ranged from 42 to 82 years (nine men and two women). Three themes were identified: (1) same disease, but different illness experiences; (2) I am in charge of my disease(s); (3) preparation for disease progression. Participants were informed of the name of their disease when they were diagnosed, but not provided with explanation of the diagnosis or meant or how to do self-care to delay the disease progression, which increased the feelings of uncertainty, hopelessness and anxiety. Taking medication was considered to be the primary treatment option and self-care a supplemental one. Advanced care plans were considered when they felt the progression of their disease(s) while refraining from sharing it with their family or health care professionals to save their concerns. All participants were willing to withhold life-sustaining treatment without making any preparation in writing. Conclusion: Education on self-care and advanced care planning should be provided to community-dwelling persons with cardiometabolic diseases. A patient-centered education program needs to be developed for this population.
