2008년 우리나라 휴대폰 가입자는 4500만 명을 넘어서 1인 1휴대전화 시대에 돌입했다. 그래서 휴대폰을 이용하여 건강을 체크한다면 현대인들의 식습관, 만성질병, 생활습관이 개선될 전망이다. 본 논문에서 제안하는 건강관리 서버는 사람들의 건강을 손쉽게 체크하여 미리 병을 예방하고 건강관리를 할 수 있도록 설계 되었으며, 휴대폰에 적용된 가속도 센서를 이용하여 걸음 수, 이동속도, 이동 거리 등의 사용자 운동량 정보를 파악 할 수 있다. 휴대폰에서 수집된 운동량 정보를 바탕으로 건강정보를 관리하므로, 사용자가 별도의 건강기기를 소지하지 않아도 손쉽게 웰빙을 추구할 수 있다.

병원 이외의 환경, 특히 댁내 기반의 헬스 모니터링 관련 연구는 최근 헬스케어 시스템 관련 연구자 및 개발자들의 많은 관심을 받고 있다. 심장병이나 당뇨병 같은 만성질환을 앓고 있는 경우에는 생리학적 요소들을 일상 생활 중 지속적으로 모니터링 하는 과정이 특히 중요하다. 본 논문에서는 댁내 환경에서 서비스되고 있는 헬스케어 시스템과 병원에서 운용되고 있는 헬스케어 시스템과의 연동을 지원하고, 클라우드 환경을 활용한 생리학적 데이터 분석을 지원하는 통합 헬스케어 시스템의 구조를 제안한다. 또한, 제안된 통합 헬스케어 시스템의 구조를 활용한 질병진단 시나리오도 제시한다.

Background: The main factors associated with weight loss in patients with COPD are not well known. Since chronic inflammation and oxidative stress play a major pathogenic role in COPD, these factors may be responsible for the patients' weight loss. Therefore, this study measured the body mass index (BMI) in COPD patients and evaluated the variables, such as systemic inflammatory marker, oxidative stress and lung function, that correlate with the BMI. Method: The stable COPD patients (M:F=49:4, mean age=$68.25{\pm}6.32$) were divided into the lower (<18.5), normal (18.5-25) and higher (>25) BMI group. The severity of the airway obstruction was evaluated by measuring the $FEV_1$. The serum IL-6 and TNF-$\alpha$ levels were measured to determine the degree of systemic inflammation, and the carbonyl protein and 8-iso-prostaglandin $F_2{\alpha}$ level was measured to determine the level of oxidative stress. Each value in the COPD patients and normal control was compared with the BMI. Results: 1) Serum 8-iso-prostaglandin $F_2{\alpha}$ in COPD patients was significantly higher ($456.08{\pm}574.12pg/ml$) than that in normal control ($264.74{\pm}143.15pg/ml$) (p<0.05). However, there were no significant differences in the serum IL-6, TNF-$\alpha$, carbonyl protein between the COPD patients and normal controls. 2). In the COPD patients, the $FEV_1$ of the lower BMI group was significantly lower ($0.93{\pm}0.25L$) than that of the normal BMI ($1.34{\pm}0.52L$) and higher BMI groups ($1.72{\pm}0.41L$) (p<0.05). The lower $FEV_1$ was significantly associated with a lower BMI in COPD patients (p=0.002, r=0.42). The BMI of very severe COPD patients was significantly lower ($19.8{\pm}2.57$) than that of the patients with moderate COPD ($22.6{\pm}3.14$) (p<0.05). 3). There were no significant differences in the serum IL-6, TNF-$\alpha$, carbonyl protein and 8-iso-prostaglandin $F_2{\alpha}$ according to the BMI in the COPD patients. Conclusion: The severity of the airway obstruction, not the systemic inflammatory markers and oxidative stress, might be associated with the BMI in stable COPD patients. Further study will be needed to determine the factors associated with the decrease in the BMI of COPD patients.
Objectives : The Illness Intrusiveness Rating Scale (IIRS) is a well-validated self-report instrument for assessing negative impact of chronic illness and/or adverse effects of its treatment on everyday life domains. Although extensive literature probed its psychometric properties in medical illness, little attention was paid for its validity for psychiatric population. This study aimed to test factorial structure of the Korean Version of the IIRS (IIRS-K) in a consecutive sample of psychiatric outpatients. Methods : Data set of 307 first-visit patients of psychiatric clinic at Guri Hanyang univ. Hospital were used. Exploratory and confirmatory factor analysis, internal consistency were tested in IIRS-K. We also checked Spearman's correlation analysis between IIRS-K, Zung's self-report anxiety scale and Zung's self-report depression scale. Results : 76.9% of the patients were with anxiety disorder and depressive disorder. The principal component factor analysis of the IIRS-K extracted three-factor structure accounted for 63.2% of total variance that was contextually similar to the original English version. This three-factor solution showed the best fit when tested confirmatory factor analysis compared to the original IIRS, two-factor model of IIRS-K suggested from medical outpatients, and one-factor solution. The IIRS-K also showed good internal consistency (Cronbach's α=0.90) and good convergent validity with anxiety and depression scales. Conclusions : The IIRS-K showed the three-factor structure that was similar but not identical to original version. Overall, this study proved factorial validity of the IIRS-K and it can be used for Korean clinical population.

Objective: Farmers are known to have high prevalence of asthma and chronic obstructive pulmonary disease(COPD). This study aims to investigate the current status of asthma and COPD in domestic and foreign farmers with the ultimate goal of raising awareness of asthma and COPD among farmers and contribute to supporting evidence for the prevention of respiratory diseases in farmers. Methods: The study utilized data from the "Farmer's Occupational Disease Survey," conducted by the Rural Development Administration, to determine the prevalence of respiratory diseases in domestic farmers. The prevalence of asthma and COPD in overseas farmers was evaluated by researching other foreign studies and articles. Results: The prevalence of asthma and COPD in domestic farmers was less than of foreign farmers'. The prevalence of asthma in domestic farmers was high as upto 8.4% and COPD upto 5.5%. Studies from Europe and America showed prevalence of asthma in farmers high as upto 14.8% and COPD upto 17.1%. Conclusions: This study conducted an investigation using the 'Survey on Occupational Diseases and Injuries in Farmers to understand the current status of asthma and COPD in domestic farmers. Due to a lack of relevant domestic research, we examined and compared with the research results on asthma and COPD among overseas farmers. Further research is necessary and preventive measures for respiratory diseases need to be developed.
This study was conducted to survey isoflavone intake among adult women in menopause with diseases such as metabolic syndrome and osteoporosis and to analyze the relationship between each of these chronic diseases followed by isoflavone intake and the related health risk index. The average age of the subjects was 49.97 years old, while that of the pre-menopausal subjects was 45.14 years, and the post-menopausal subjects was 55.99 years. The average body mass index (BMI), waist-hip circumference, body fat percentage, blood pressure, blood sugar and blood lipid content of the post-menopausal subjects were higher in significant difference than those of the pre-menopausal subjects. The bone density of the hip and spine in post-menopausal subjects was lower in significant difference than that of the pre-menopausal subjects. After menopause, the subjects had a lower ratio of individuals at risk of anemia when compared with the subjects before menopause, but had higher health risk ratio related to each type of chronic disease, including obesity, hypertension, high cholesterol and osteoporosis than the subjects before menopause. The intake frequency of each soybean food was similar among subjects before/after menopause. The most common soybean based foods consumed by the subjects were soybean, soybean curd and soybean paste. The average daily intake level of isoflavone among subjects before menopause was 25.48 mg, while that of subjects after menopause was 32.25 mg. Evaluation of the distribution of the isoflavone level revealed that the pre-menopausal subjects consumed 3.29~78.36 mg and the post-menopausal subjects consumed 3.18~116.59 mg. The intake level by each individual varied greatly. The pre-menopausal subjects had a low BMI index and systolic blood pressure as much as their isoflavone intake level was high. Additionally, the post-menopausal subjects had a low menarche age and high menopause age when their isoflavone intake level was high, the BMI index and waist-hip circumference ratio was highest among individuals with lowest isoflavone intake level. This study showed that there was a possible relationship between soybean isoflavone intake and health problems such as obesity, high cholesterol, and osteoporosis in women after menopause with diseases such as metabolic syndrome and osteoporosis, even if this relationship was not great.
While traditionally threatening human infectious diseases are decreasing, chronic diseases such as cancer and diabetes, including cardiovascular diseases, are spreading rapidly. Reflecting the characteristics of chronic diseases that are difficult to treat, the management of risk factors and the preparation of health promotion policies corresponding to them have emerged as important concerns around the world. Providing so-called multi-sector approach such as health promotion and disease prevention policy and solution for community response, The value of primary care is once again highlighted in the changed medical environment. As the existing medical delivery system has become difficult to cope with the people's desire for rapid aging, disease paradigm change, and quality of life improvement, European Union countries have made various efforts to improve the quality of their primary medical system, which provides continuous, com- prehensive and coordinated management. This paper examines the current status and problems of the healthcare delivery system in Korea, draws implications from the European Union examples related to the strengthening of primary care, and discusses the plans for establishing a patient-centered future medical delivery system.

Based on data obtained from the Korea National Health and Nutrition Examination Survey (KNHANES) conducted by Korea Centers for Disease Control & Prevention (KCDC), this study analyzed the relationship between existence of the spouse, number of family members, and disease afflictions of the Korean elderly, and factors affecting their quality of life. The collected data were analyzed by Chi-square test, t-test and One-way Anova, using the SPSS / WIN 25. Our comparative analysis on the health status and quality of life revealed that individuals in the singles group had significantly more diseases than the married group. The singles group considered their health was subjectively poor, whereas the quality of life of the married group was higher. Analyzing the characteristics of chronic disease and activity restriction showed significantly higher prevalence of several diseases in the singles group. Besides, the singles group revealed significantly higher limitations of activity. Taken together, our results reveal a difference in the health status, quality of life, and the presence of chronic diseases in accordance to existence of the spouse, indicating that having a spouse increases the level of health and quality of life of the elderly.

This study is a descriptive survey study to investigate the quality of sleep and to compare the factors relating quality of sleep in the young-old people and the old-old people. The subjects of the study were 200 elderly people who live in the G-region applying convenient expression method. Using a structured questionnaire, sociodemographic characteristics, lifestyle and health-related characteristics, depression, and quality of sleep were examined. We analyzed ${\chi}^2-test$, t-test, ANOVA(scheffe's test), Pearson's correlation coefficient and multiple regression using IBM SPSS WIN/21.0. The results of the study were as follows. The quality of sleep was significantly worse in the old-old people than in the young-old people. In young-old people, only depression (r=-.22, p=.038) showed a significant negative correlation with sleep quality. Old-old people was a significant negative correlation between sleep quality and depression (r=-.19, p=.045) and subjective health status (r=-.29, p=.002). In the young-old people, job, chronic illness, and economic level were significant influencing factors in the quality of sleep. In the old-old people, chronic disease, subjective health status, and smoking were significant influencing factors. The results of this study can be used as basic data for establishing tailored strategy for ages that can improve health of the elderly.
