Radiographic examination protocol and patient dose in lateral cephalometric radiography in Korea (국내 의료기관에서 측방두부규격방사선촬영시 임상에서의 촬영조건 및 환자 선량)

  • Purpose : To survey the radiographic examination protocol for lateral cephalometric radiographic examinations and to measure their patient doses in Korea and to compare the dose according to the size of hospital, the type of image receptor system, and the installation duration. Materials and Methods : The radiographic examination protocols (kVp, mA, and exposure time) for lateral cephalometric radiography were surveyed with 61 cephalometric radiographic equipments and their patient dose-area product (DAP) measured with a DAP meter (DIAMENTOR M4-KDK, PTW, Freiburg, Germany) for 51 cephalometric radiographic equipments. The radiographic examination protocols and patient doses were compared according to the size of hospital (university dental hospital, dental hospital, and dental clinic), the type of image receptor system (film-based, DR and CR type) and the installation duration, respectively. SPSS 12.0.1 for Windows (SPSS Inc., Chicago, USA) was used for independent t-test and ANOVA test. Results : The average protocols were 77.0 kVp, 12.7 mA, 6.2 second for cephalometric radiography. The average patient dose (DAP) was $128.0mGy\;cm^2$ and 3rd quartile dose (DAP) $161.1mGy\;cm^2$ for cephalometric radiography for adult male. There was no statistically significant difference at average patient DAP according to the size of hospital, the type of image receptor system, and the installation duration, repectively. Conclusion : The average patient dose was $128.0mGy\;cm^2$ and the third quartile patient dose $161.1mGy\;cm^2$ for lateral cephalometric radiography for adult male in Korea.

  • Mesiodens are supernumerary teeth in the region of the maxillary central incisors, a condition which can lead to disorders of the dentition. Their presence may lead to disorders, such as delay in eruption of permanent teeth, development of dentigerous cysts, resorption of adjacent roots and eruption of a supernumerary tooth into nasal cavity. The optimal time for surgical intervention is controversial. Early diagnosis is important so as to enable good prognosis. Diagnosis primarily depends on x-ray films. Panorama film, occlusal film, periapical film have been used for detection of mesiodens. But, all of them have disadvantages. $SCANORA^{(R)}$ is a multimodal radiology system which utilizes the principles of narrow beam radiology and spiral tomography. Pre-programmed imaging procedure are provided for many dental situations An optional personal computer can be connected into the unit to help design the examination. We report two cases diagnosed by $SCANORA^{(R)}$. When compared with tube shift technique, it is simple and exact method of detecting mesiodens.

A study on the usefulness of a fusion model designed cloak shield to reduce the radiation exposure of the assistant during CT of severely injured patient (중증외상환자 CT 검사 시 검사보조자의 방사선피폭 경감을 위한 융합적 망토 차폐체의 유용성 연구)

  • The purpose of this study is to evaluate the usefulness of a newly fusion model designed cloak shield to reduce the radiation exposure of the assistant during CT(computed tomography) of severely injured patient. Radiation dose was measured in the heart, both axillary and thyroid areas using newly designed cloak shield and existing shield with head phantom and human phantom under the same conditions as brain vascular CT scan. The newly designed cloak shield was measured higher for radiation shielding rate than the existing shields, 61.9 % for heart, 46.2 % for left axillary, 69.8 % for right axillary and 71.1 % for thyroid gland, respectively. a newly developed fusion model of cloak shields are useful for reducing radiation exposure. It is expected to make a significant contribution to reduction of radiation exposure.

  • 앵글씨 분류에 따른 최대개구시 하악의 수직 변위에 대한 차이를 확인하고, 수직 하악운동에 영향을 주는 골격요소를 확인하기 위하여, 측두하악관절 및 저작계 이상에 대한 증상 및 병력이 없으며 발치 및 교정치료의 경험이 없는 광주지역 대학에 재학중인 학생들을 대상으로, 앵글씨 구치부 관계에 근거하여 1급군(남:30명, 여:49명), 2급군(남:18명, 여:24명)과 3급군(남:18명, 여:33명)으로 분류하여 총 172명(연령 범주:20-30세)의 학생을 선택하였다. 전남대학교 병원 구강내과에서 사용하는 계측용자를 이용하여 최대 개구시 상하악 중절치 절단면간의 거리를 측정하였다. 대상자들의 진단모형을 만들어 상하악궁 길이와 폭경를 측정하였다. 대상자들에 대한 두부 규격방사선 사진을 촬영, 작도하고 방사선학적 지표를 계측, 비교분석하였다. 앵글씨 분류 1급군, 2급군 그리고 3급군 절치간 최대개구량은 3급군이 가장 컸으며 모든 군에서 남자가 여자보다 컸다. 구치간 최대개구량은 앵글씨 분류 1급군, 2급군 그리고 3급군에서 각 군간의 유의한 차이는 없었으나, 모든 군에서 남자가 더 컸다. 앵글씨 분류 1급군과 2급군의 하악운동에서 총 하악골 길이, 하악지 길이, 하악 하연부 길이 그리고 상악궁 폭경이 변수로 나타났으며, 상하악궁 길이와는 역상관관계를 나타내었다. 앵글씨 분류 3급군의 하악운동에서 상하악궁 길이와 안면 부길이가 1급군과 2급군과는 다른 중요한 변수로 나타났으며, 상악궁의 폭경과는 역상관관계를 나타내었다. 이상의 결과로 보아 앵글씨 분류 각 군에서 하악개구운동은 안면 골격구조의 영향을 받으며, 각 군간에 영향을 주는 안면 골격 요소들은 차이가 있었다. 따라서 이러한 골격적 요소들은 개구량 개선을 위한 진단과 치료시 고려되어야 할 것으로 사료된다.

Efficient Determination of Iteration Number for Algebraic Reconstruction Technique in CT (CT의 대수적재구성기법에서 효율적인 반복 횟수 결정)

  • The algebraic reconstruction technique is one of the reconstruction methods in CT and shows good image quality against noise-dominant conditions. The number of iteration is one of the key factors determining the execution time for the algebraic reconstruction technique. However, there are some rules for determining the number of iterations that result in more than a few hundred iterations. Thus, the rules are difficult to apply in practice. In this study, we proposed a method to determine the number of iterations for practical applications. The reconstructed image quality shows slow convergence as the number of iterations increases. Image quality 𝜖 < 0.001 was used to determine the optimal number of iteration. The Shepp-Logan head phantom was used to obtain noise-free projection and projections with noise for 360, 720, and 1440 views were obtained using Geant4 Monte Carlo simulation that has the same geometry dimension as a clinic CT system. Images reconstructed by around 10 iterations within the stop condition showed good quality. The method for determining the iteration number is an efficient way of replacing the best image-quality-based method, which brings over a few hundred iterations.

  • Inflammatory myofibroblastic tumors (IMTs) are rare. They are characterized by myofibroblastic spindle cell proliferation with a varying degree of inflammatory cell infiltration. IMT can occur in any anatomic location but has been reported in the lung, mesentery, and omentum, mainly in children or young adults. It rarely occurs in the pancreas and is often difficult to distinguish from other tumors, including some malignant ones. Therefore, it can be challenging to make a radiological diagnosis of IMT. Here, we present a case of IMT that occurred in the pancreas head of a middle-aged female. The patient's ultrasonography, computed tomography, and magnetic resonance imaging findings are presented along with a review of the literature.

Re-conservation of the Iron Sword with Ring Pommel with Three-Pointed Leaf Decoration Excavated from Tomb No. 55 at the Dalseong Ancient Tomb Complex in Daegu and a Study of Its Production Method (대구 달성 55호분 출토 삼엽문이자태도의 재보존처리와 제작 기법 연구)

  • This paper presents the process of re-conservation and the results of research on the production method of the Iron Sword with Ring Pommel with Three-Pointed Leaf Decoration, one of the excavation findings from Tomb No. 55 in the Dalseong Ancient Tomb Complex in Daegu. This iron sword is a double weapon with two large swords housed within a single sheath. Four smaller swords are attached to the surface of the sheath, two on the upper portion and the other two below. It is the only such two-in-one weapon excavated intact thus far from an ancient Korean tomb. The records show that it underwent conservation treatment twice in the past. In this study, it was subjected to conservation treatment again to replace the stiffener in some cracking areas, and its material, composition, and production method were analyzed by CT, XRF analysis, and stereoscopic microscopy. The sword is mainly made of copper, but the golden component contains both gold and mercury, which suggests that the copper was plated in gold using mercury amalgamation. The examination of the production methods indicates that it was intended more to demonstrate the authority of its owner rather than to serve any practical use. The two upper small swords on the sheath were made in the same manner as the main swords, and the two small lower swords were cut from a single metal sheet. The sheath was made by cutting two metal sheets. Supports were used to attach the two small swords to the upper portion of the sheath, while the lower portion of the sheath was slit to allow the other two small swords to be inserted into it. The ring pommels of the main swords have a three-pointed leaf decoration. As for the other designs, the handle of the main sword features a series of semicircles, the decorative bands on the sheath have waves in dots, and the fish tail of the sheath shows diagonal lines of dots.

Evaluation of Radiation Exposure Dose for Examination Purposes other than the Critical Organ from Computed Tomography: A base on the Dose Reference Level (DRL) (전산화단층촬영에서 촬영 목적 부위와 주변 결정장기에 대한 피폭선량 평가: 선량 권고량 중심으로)

  • In this study measured patient exposure dose for purpose exposure area and peripheral critical organs by using optically stimulated luminescence dosimeters (OSLDs) from computed tomography (CT), based on the measurement results, we predicted the radiobiological effects, and would like to advised ways of reduction strategies. In order to experiment, OSLDs received calibration factor were attached at left and right lens, thyroid, field center, and sexual gland in human body standard phantom that is recommended in ICRP, and we simulated exposure dose of patients in same condition that equal exposure condition according to examination area. Average calibration factor of OSLDs were $1.0058{\pm}0.0074$. In case of left and right lens, equivalent dose was measure in 50.49 mGy in skull examination, 0.24 mGy in chest, under standard value in abdomen, lumbar spine and pelvis. In case of thyroid, equivalent dose was measured in 10.89 mGy in skull examination, 7.75 mGy in chest, 0.06 mGy in abdomen, under standard value in lumber spine and pelvis. In case of sexual gland, equivalent dose was measured in 21.98 mGy, 2.37 mGy in lumber spine, 6.29 mGy in abdomen, under standard value in skull examination. Reduction strategies about diagnosis reference level (DRL) in CT examination needed fair interpretation and institutional support recommending international organization. So, we met validity for minimize exposure of patients, systematize influence about exposure dose of patients and minimize unnecessary exposure of tissue.

  • A study was made of a method of rational assessment on the roentgeno-cephalogram in orthodontic practice. Fifty eight measurements were analyzed on the roentgeno-cephalograms of 50 male and 50 female Korean adults with the acceptable profile and the normal occlusion. The means and standard deviations of measurements were studied in male and female groups by various analytic methods, and the sex difference was examined statistically. The obtained data were compared with those of the Japanese and American standards by useing a Japanese adults standard polygon chart reported by lizuka and Ishikawa. Amongs of 58 measurements, 14 have been selected as statistically significant and clinically useful, and used to make a Korean adult standard polygon chart. The results obtained were as follows: 1. Standards for cphalometric analysis in Korean adults were obtained, and the polygon chart and cephalometric profilogram were also made by using the standards in order to apply to a clinical diagnosis. 2. The sex difference was recognized in linear measurements, but not in angular measurments. 3. In comparison with the American standards from Downs and Graber, the Korean showed significant differences in angle of convexity, mandibular plane angle, Y-axis angle, interincisal angle and lower incisor to mandibular plane angle. Thus, the Korean showed a more retruded chin position, convex facial type, than white people. 4. In comparison with the Japanese standards from Iizuka and Ishikawa, the Korean showed a significant similarity with the Japanese, especially in females. 5. Upper incisor to Huxely line angle, established as a new measurement, proved to be paralell to X-Y axis angle.

Formulation of a reference coordinate system of three-dimensional head & neck images: Part II. Reproducibility of the horizontal reference plane and midsagittal plane (3차원 두부영상의 기준좌표계 설정을 위한 연구: II부 수평기준면과 정중시상면의 재현성)

  • This study was performed to investigate the reproducibility of the horizontal and midsagittal planes, and to suggest a stable coordinate system for three-dimensional (3D) cephalometric analysis. Eighteen CT scans were taken and the coordinate system was established using 7 reference points marked by a volume model, with no more than 4 points on the same plane. The 3D landmarks were selected on V works (Cybermed Inc., Seoul, Korea), then exported to V surgery (Cybermed Inc., Seoul, Korea) to calculate the coordinate values. All the landmarks were taken twice with a lapse of 2 weeks. The horizontal and midsagittal planes were constructed and its reproducibility was evaluated. There was no significant difference in the reproducibility of the horizontal reference planes, But, FH planes were more reproducible than other horizontal planes. FH planes showed no difference between the planes constructed with 3 out of 4 points. The angle of intersection made by 2 FH planes, composed of both Po and one Or showed less than $1^{\circ}$ difference. This was identical when 2 FH planes were composed of both Or and one Po. But, the latter cases showed a significantly smaller error. The reproducibility of the midsagittal plane was reliable with an error range of 0.61 to $1.93^{\circ}$ except for 5 establishments (FMS-Nc, Na-Rh, Na-ANS, Rh-ANS, and FR-PNS). The 3D coordinate system may be constructed with 3 planes; the horizontal plane constructed by both Po and right Or; the midsagittal plane perpendicular to the horizontal plane, including the midpoint of the Foramen Spinosum and Nc; and the coronal plane perpendicular to the horizontal and midsagittal planes, including point clinoidale, or sella, or PNS.
