Toxins and Antibiotic Resistance of Methicillin-Resistant Staphylococcus aureus Isolated from Clinical Specimens (임상검체로부터 분리된 methicillin 내성 Staphylococcus aureus의 독소 및 항생제 내성)

  • Seventy five methicillin- resistant Staphylococcus aureus (MRSA) strains and 24 methicillin- susceptible S. aureus (MSSA) were isolated from clinical specimens obtained from a hospital in Suncheon, Jeonnam province, Korea, from July to December, 2009. Antibiotic resistance was determined using the disc diffusion method. Genes encoding enterotoxin (SE), toxic shock syndrome toxin-1 (TSST-1), exfoliative toxin (ET) and Panton-Valentine leukocidin (PVL) were detected by multiplex PCR-mediated amplification using specific primers. Sixty (80%) MRSA isolates possessed either one or more toxin genes and the most common pattern that coexisted in MRSA was seb, sec, seg, sei and tst (22.7%) followed by coexistence of sec, seg, sei and tst genes (18.7%). Gene pvl encoding leukocidin was not found. Significant correlation between the production of sec, seg, sei and tst genes was found. MRSAs were resistant to erythromycin (89% of the isolates), gentamicin (70.7%), ciprofloxacin (69.3%), clindamycin (61.3%) and tetracycline (58.7%), while MSSAs were susceptible to the antibiotics with the exception of erythromycin. Toxin genes seb, sec and tst were related to the tetracycline resistance of MRSA.

Survey of Mycotoxin Contamination in Grains and Grain Products (곡류 및 곡류가공품 중 곰팡이독소 오염도 조사 연구)

  • This study surveyed mycotoxin contamination in grains and grain products, which were purchased from supermarkets and traditional markets from October 2017 to September 2018 in Gwangju (Metropolitan City). A total of 127 samples including adlay, sorghum, millet, rice, oats, barley, buckwheat, corn as grains, and rice flour, buckwheat flour, roasted barley and corn, as grain products were surveyed. The tested mycotoxins were aflatoxin ($AFB_1$, $AFB_2$, $AFG_1$, $AFG_2$), fumonisin ($FUB_1$, $FUB_2$), ochratoxin A (OTA), and zearalenone (ZON). Mycotoxins were analyzed simultaneously with a UPLC-tandem mass spectrometry method. Fumonisin ($B_1+B_2$) was detected at the range of $4.8{\sim}738.5{\mu}g/kg$ in 35 samples and zearalenone at $8.4{\sim}507.6{\mu}g/kg$ in 20 samples, respectively. No other mycotoxins were detected. Risk assessment was evaluated by using estimated daily intake (EDI) and provisional maximum tolerable daily intake (PMTDI) in accordance with the Joint FAO/WHO Expert Committee on Food Additives (JECFA). When the hazard index (HI) was expressed as $(EDI/PMTDI){\times}100$, the HI (%) showed in the range of 0.0019~1.9526%. Based on these results, mycotoxin concentrations in the grains and grain products were within safe levels.

The Studies on Real Condition of Mycotoxin Contamination in the Fields before Harvest and by the Storage of Rice Straw using as Roughage in Korea (국내 조사료용 볏짚의 수확 전 작물포 및 수확 후 저장에 따른 곰팡이독소 오염 실태 연구)

  • The studies were conducted to investigate real condition of mycotoxin contamination in the fields before harvest and by the storages of rice straw used as roughage in Korea. It was found mycotoxin contamination before harvest of rice straw that the rice plants were detected deoxynivalenol at the initial growth, ochratoxin A and deoxynivalenol at the middle growth, and deoxynivalenol and zearalenone at the harvest periods in the fields. Also, the rice plants were contaminated by various fungi such as Fusarium sp., Fusarium proliferatum, Penicillium sp., Gibberella sp., Gibberella zeae, Mucor circinelloides and Aspergillus oryzae. The levels of fungal contamination were $10^{3-4}$ cfu/g at the initial growth, and $10^{4-5}$ cfu/g at the middle growth and harvest periods. All storage types of rice straw were contaminated with zearalenone, deoxynivalenol and ochratoxins A. The samples of rice straw contaminating mycotoxins were account for 3% in bundle rice straw, and 38% in both types of square rice straw and rice round bale silage, respectively. When 105 samples of rice bale silage were analyzed for mycotoxins depending on the regional area, mycotoxin contamination was found in 46% of total samples in Korea. Regional contaminations of mycotoxins were respectively 48, 33, 40, 50 and 57% of samples in Gyeonggi, Gangwon, Chungcheng, Yeongnam and Honam area. Rice round bale silage was contaminated by three kinds of mycotoxins (zearalenone, deoxynivalenol and ochratoxinsA) in the all of area without Chungcheong area where was contaminated zearalenone and deoxynivalenol. Ochratoxins A, deoxynivalenol and zearalenone were respectively determinated with the average levels of 2.6, 413 and $338{\mu}g/kg$ in rice round bale silage for the overall area, even if it was some difference depending on each regional area. Therefore, the above results clearly show that the rice straws were exposed to the contamination by mycotoxin and mycotoxigenic fungi before harvest in the fields, and mycotoxin contamination was not dependent on the regional area or the storage types such as bundle rice straw, square rice straw and rice round bale silage.

  • Toxin-producing Bacillus cereus is the causative agent of two different types of food poisoning: the emetic and the diarrheal types. This study was conducted to investigate the presence of enterotoxin and emetic toxin genes in 263 B. cereus isolated from 619 different ready-to-eat food items. Hemolytic enterotoxins hblA, hblC, and hblD were detected in 85.6, 41.1, and 76.8%, respectively, of the B. cereus isolates. About 67.0% (175/263) of the isolates presented all of three genes. Non-hemolytic enterotoxins nheA, nheB, and nheC were detected in 100, 97.0, and 68.4% of the isolates, respectively. Approximately 90.0% (236/263) of the isolates presented all of these three non-hemolytic enterotoxin genes. Emetic toxin gene, CER, was detected in 132 of 263 (50.2%) isolates. Computer-assisted cluster analysis of Rep-PCR profiles showed a high genetic diversity among the isolates. All B. cereus isolates from food samples tested in this study carried at least 6 of 10 toxin genes.

A Study on the Safety of Mycotoxins in Grains and Commonly Consumed Foods (곡류 등 다소비 식품 중 곰팡이독소 안전성 조사 연구)

  • The purpose of this study was to investigate and evaluate the safety of the grains, nut products, beans and oilseeds being sold in Gyeonggi province by analyzing mycotoxins. A multi-mycotoxins analysis method based on LC-MS/MS was validated and applied for the determination of eight mycotoxins, including aflatoxins ($B_1$, $B_2$, $G_1$ and $G_2$), fumonisins ($B_1$, $B_2$), zearalenone and ochratoxcin A in 134 samples. The limit of detection (LOD) and limit of quantitation (LOQ) for the eight mycotoxins ranged from 0.14 to $8.25{\mu}g/kg$ and from 1.08 to $7.21{\mu}g/kg$, respectively. Recovery rates of mycotoxins were determined in the range of 61.1 to 97.5% with RSD of 1.0~14.5% (n=3). Fumonisin $B_1$, $B_2$, zearalenone, and ochratoxin A were detected in 22 samples, indicating that 27% of grains, 12.5% of beans and 11.8% of oilseeds were contaminated. Fumonisin and zearalenone were detected simultaneously in 2 adlays and 3 sorghums. Fumonisin $B_1$ and $B_2$ were detected simultaneously in most samples whereas fumonisin $B_1$ was detected in 1 adlay, 1 millet and 1 sesame sample. The average detected amount of fumonisin was $49.3{\mu}g/kg$ and $10.1{\mu}g/kg$ for grains and oilseeds, respectively. The average detected amount of zearalenone was $1.9{\mu}g/kg$ and $1.5{\mu}g/kg$ for grains and beans, respectively. In addition, the average amount of ochratoxin A was $0.08{\mu}g/kg$ for grains. The calculated exposure amounts of fumonisin, zeralenone and ochratoxin A for grains, beans and oilseeds were below the PMTDI/PTWI.

Survey of Fungal Infection and Fusarium Mycotoxins Contamination of Maize during Storage in Korea in 2015 (2015년 국내산 저장 옥수수에서의 후자리움 독소 오염 및 감염 곰팡이 조사)

  • Maize is one of the most cultivated cereals as a staple food in the world. The harvested maize is mainly stored after drying, but its quality and nutrition could be debased by fungal spoilage and mycotoxin contamination. In this study, we surveyed mycotoxin contamination fungal infection of maize kernels that were stored for almost one year after harvest in 2015. The amount of deoxynivalenol and zearalenone detected were higher than the other mycotoxin, such as aflatoxin, ochratoxin, fumonisin and T-2 toxin. In particular, level of deoxynivalenol was detected as $1200{\pm}610{\mu}g/kg$ in small size kernels, which was four to six times higher than the large and the medium size kernels. Moreover, the amount of deoxynivalenol, zearalenone, and fumonisin were increased with discolored kernels. 10 species including Fusarium spp., Aspergillus spp. and Penicillium spp. were isolated from the maize kernels. F. graminearum was predominant in the discolored kernels with detection rates of 60% (red) and 40% (brown). Our study shows that the mycotoxin contents of stored maize can be increased by discolored maize kernels mixed. Therefore elimination of the contaminated maize kernels will help prevent fungal infection and mycotoxin contamination in stored maize.

Streptococcal Toxic Shock Syndrome Occurred during Postoperative Radiotherapy in a Cancer Patient with Preexisting Lymphedema and Chronic Illness -Case Report- (수술 후 림프부종과 만성질환을 동반한 종양 환자에서 방사선치료 기간 동안 발생한 연쇄구균독소충격증후군 예)

  • A case is reported of a man with malignant fibrous histiocytoma (MFH) in right thigh who developed streptococcal toxic shock syndrome (STSS) during postoperative radiotherapy. Before radiotherapy, a patient complained wax and wane lymphedema following wide excision of tumor mass which was confirmed as MFH. He took some nonsteroidal antiinflammatory drug (NSAID) for about one month. He suffered preexisting hepatitis C virus (HCV) infection, diabetes and well-controlled hypertension. The patient received conventional radiotherapy to right thigh with a total dose of 32.4 Gy at 1.8 Gy per day. At last radiotherapy fraction, cutaneous erythematous inflammation was suddenly developed at his affected thigh. At that time, he also complained of oliguria, fever and chills. The patient was consulted to internal medicine for adequate evaluation and management. The patient was diagnosed as suggested septic shock and admitted without delay. At admission, he showed hypotension, oliguria, constipation, abnormal renal and liver function. As a result of blood culture, Streptococcus pyogenes was detected. The patient was diagnosed to STSS. He was treated with adequate intravenous antibiotics and fluid support. STSS is one of oncologic emergencies and requires immediate medical intervention to prevent loss of life. In this patient, underlying HCV infection, postoperative lymphedema, prolonged NSAID medication, and radiotherapy may have been multiple precipitating factors of STSS.

Profiles of Toxin Genes and Antibiotic Susceptibility of Bacillus cereus Isolated from Perilla Leaf and Cultivation Areas (들깻잎과 생산환경에서 분리한 Bacillus cereus의 독소 유전자와 항생제 감수성 분석)

  • Two-hundred Bacillus cereus isolated from perilla leaf cultivation areas in Miryang, Korea were investigated for toxin genes and antibiotic susceptibility. Toxigenic patterns of isolates were identified to be 11 groups through toxin gene profiles. 21% of strains isolated from the perilla leaves had both enterotoxin and emetic toxin. Toxin genes entFM (100%), nheA (100%) and hblA, C, D (65.5%) were frequently found in the perilla leaves, whereas EM (21.0%) was less common. Most isolates were susceptible to 10 antibiotics, but they were highly resistant to penicillin (100%), ampicillin (100%), oxacillin (94.9%), amoxicillin-clavulanic acid (95.6%), cefazolin (78.2%), and rifampicin (58.0%). These results indicate that food-borne outbreak caused by B. cereus might lead to diarrhea and emetic syndromes.
