The Relationship between HbA1c Control and Diabetes Self-care Knowledge, Competence, Behavior and Quality of Life on Diabetes elderly (노인 당뇨환자에서 당뇨 자가관리 지식, 자신감, 행위 및 삶의 질과 당화혈색소 조절의 관련성)

  • This study was conducted to identify the diabetes self-care (DSM) knowledge, DSM competence, DSM behavior, and quality of life according to HbA1c control in diabetic elderly. The participants were 205 diabetes elderly who visited the citizen health promotion center located in D city, Korea. Data were collected from April, 4 to August 31 2015, and were analyzed by the t-test and chi-squared test using IBM SPSS 23.0. The mean scores of DSM knowledge, DSM competence, DSM behavior and quality of life were $50.61{\pm}16.39$, $71.27{\pm}10.21$, $62.78{\pm}1.29$ and $0.86{\pm}0.11$, respectively. Additionally, DSM behavior (t=2.17, p=0.031), education level (t=11.80, p=0.0019) l, BMI (t=0.001, p=0.012), and number of visits to citizen health center (t=16.497, p=0.001) differed significantly between the controlled HbA1c group and uncontrolled HbA1C group. However, ANCOVA revealed that the level of HbA1c did not affect the DSM behaviors. Therefore, it is necessary to develop and apply a DSM education program that reflects the characteristics knowledge level of the elderly. In addition, healthcare institutions with high accessibility in terms of distance and cost to guide and manage desirable diabetic self-care behaviors should be provided everywhere.

Diabetes Self-Management Experience of Patients with Diabetes: Focused on the Visually Impaired (당뇨 환자의 당뇨 자가관리 경험: 시각장애인을 중심으로)

  • Purpose: This study aimed to understand and describe the diabetes self-management experience of visually impaired people with diabetes. Methods: Ten participants were recruited through a website used by the visually impaired from February to March 2020. Data were collected through two focus group interviews conducted in June 2020; each group consisted of five participants. All interviews were recorded with the consent of the participants and transcribed verbatim. The transcribed data were analyzed using qualitative content analysis. Results: Seven categories were as follows; a two-faced, lifelong companion, an unprepared encounter, struggle to live, love-hate relationship with family, strategies to adapt, lessening attention to self-management, the desire to learn properly. Conclusion: It can be concluded that the visually impaired have fewer opportunities for receiving diabetes self-management education than general diabetic patients. Consequently, plans to improve the education available to such patients are required. Additionally, psychological counseling and diabetes education for patients' families are necessary, and improving the perception of medical workers regarding the visually impaired will be prove useful.

The Analysis of Self-care Knowledge, Competence, Behavior, and Quality of Life in Community Diabetes Elderly (지역사회 당뇨노인의 당뇨 자가관리 지식, 자신감, 행위 및 삶의 질 분석)

  • This study was conducted to identify the level of diabetes self-care knowledge, confidence, behavior and quality of life in elderly people with diabetes. The participants were 205 elderly with diabetes who visited a citizen health center located in D city. Korea. The data were collected from April, 20th to August 31th, 2015, and were analyzed by the t-test and ANOVA. The results were as follows; 1) The mean scores for diabetes self-care knowledge, confidence, behavior, and quality of life were 6.99 (2.17), 71.27 (10.21), 62.78 (1.29) and 0.86, respectively. 2) There were significant differences in the self-care behavior according to the education level, duration of diabetes mellitus and subjective health, and the quality of life differeddepending on the level of subjective health(F=35.253, p=0.000). 3) Knowledge of diabetes diet was the lowest among the various aspects of diabetes self-care knowledge. Confidence for exercise was lowest, while confidence for medication and blood sugar check was highest. However, the participants exhibited good medication behavior, whereasthey did not carry out blood glucose checks in their self-care management behavior. The most uncomfortable aspect of the quality of life was found to be pain and discomfort. Based on the results of this study, it is necessary to develop guidelines and methods for dietary education reflecting the dietary habits of Koreans, and to investigatethe reasonsfor thelow performance of theblood glucose check. In addition, customized nursing interventionstaking into consideration the characteristics of elderly with diabetes need to be developed and verified.

  • The purpose of this study was to test the effect of a diabetic education program using the self-efficacy theory on the self-care behavior and glucose metabolism. The subjects of the study consisted of 25 NIDDM patients who had participated with a diabetic education program from June 23 to July 14. 1999. The research design was one-group pretest-posttest design. The pretest included measuring. self-care behavior and 2PPBS. the diabetic education program was conducted to the group for 4weeks. After the diabetic education program. the posttest included remeasuring of self-care behavior and 2PPBS. Wilcoxon signed rank test was used for the significence of the differences between values before and after the diabetic education program. The results are as follows. The diabetic education program was effective in increasing the score of the self-care behavior in exercise but was not effective in increasing the score of the self-care behavior in diet, test and medication. The diabetic education program was not effective in decreasing the levels of glucose metabolism.

  • This study was carried out to investigate the effects of contents for the serious game development of the customized diet education in Type II Diabetes. Because the development of the existing serious game focuses on Type I Diabetes, this study was designed to carry out customized education through the interaction between Type II Diabetes and health manager. The period of data collection was from July 11 to December 26 in Y2015, the subjects were selected from a total of 26 people suitable for Type II diabetes who visited the public health center to apply for a diabetic diet education and completed the 8 continued education. After receiving customized diet prototype education, the self-care behaviors and HbA1C of the subjects were statistically significantly increased(p<.05)

  • This study identifies the moderating effects of cognition on the relationship between self-care confidence and self-care behavior in the diabetic elderly. The participants were 205 elderly subjects with diabetes, who visited a citizen health center located in D city, Korea. Data were collected from 20th April to 31st August 2015, and were analyzed by t-test and multiple regression. Our results reveal no significant differences between the depressive and non-depressive groups, when considering diabetes self-management confidence, knowledge, behavior and cognitive function. Cognitive function had a moderating effect in the relationship between self-care confidence and self-care behavior only in the depressed group. Our results indicate that it is therefore necessary to apply individual nursing intervention based on cognitive function and depression level, and to develop various programs for improving the cognitive function and depression amongst the diabetic elderly.

Comparison of blood glucose concentrations from capillaries and veins in SMBG (자가혈당측정시 채혈방법에 따른 혈당치의 비교)

  • 당뇨병환자의 자가혈당조절에 있어서의 기준이 되는 혈당측정에 있어서 하나의 지침을 제공하기 위해 본 연구는 말초혈관과 상완정맥에서의 혈액을 채취하여 이들 검사물들의 혈당치를 직접적으로 비교했다. 30명의 건강한 한국인 성인 남자(연령 : 평균=30세)로부터, 공복시의 혈액을 세가지의 서로 다른 방법들(우선 손가락 끝에 말초혈관을 울혈이 되도록 한 뒤에, 이후 같은 편의 팔의 상완정맥에서, 마지막으로 다시 처음 검채를 시행했던 바로 그 말초의 천자부위에서 혈액을 짜내는 방법으로.)로 각각의 대상에서 검채하였다. 검사물들의 혈당치는 간편측정용 혈당측정기(Glucocard, KDK Corp., Japan. 1995)로 측정한 결과 상완정맥에서 채취한 혈액의 혈당치(107.58$\pm$21.03mg/dl)가 말초에서 채취한 혈액(울혈 후 : 100.21$\pm$24.03mg/dl, .짜낸 경우 : 97.84$\pm$22.45mg/dl)들 보다 높았다(T=2.86. p=.0081 : T=3.084. p=.0055). 그러나 이 결과는 기타의 연구들에서의 결과와는 일치하지 않았다. 이러한 불일치는 자가혈당측정기가 인슐린 의존형 당뇨환자는 물론이고 비의존성 당뇨환자들의 자가관리에 결정적인 지침이 된다는 점을 고려할 때, 좀 더 통제된 상황에서 더욱 많은 사람을 대상으로 한 임상시험을 통해 자가 혈당측정치에 있어서 기계요인에 대한분석이 이루워질 필요를 시사하고 있다. 또한 동일한 부위에서 서로 다른 방법들(혈관을 울혈을 시킨 천자하여 얻은 경우와 천자 후 그 부위를 짜서 검사물을 얻는 방법)에 의해서 얻어진 혈액의 헐당치 측정 결과 간에 차이가 없는 것으로 나타나서(t=0.72, p=.4791), 일반적으로 혈당치의 측정 시 검채한 양이 부족할 때, 새로이 천자하여 검사를 시행하는 것을 원칙으로 인식하고 있으나 본 연구결과를 볼 때, 이러한 채혈의 방식에 의한 차이는 환자의 안녕의 차원에서 고려하여 필요하다면 간과될 수도 있음을 의미하고 있다.

Compliance and Barriers to Self-care Behaviors in Patients with Type 2 Diabetes (제2형 당뇨병환자의 자가간호행위 이행과 장애수준)

  • Purpose: The purpose of the study was to examine the level and correlation of compliance and barriers to self-care behavior in patients with type 2 diabetes. Methods: The participants for this study were 122 patients with type 2 diabetes enrolled in the out-patient clinic of C national general hospital. Data collection was done by self-report questionnaires. Results: The mean score of the patients with type 2 diabetes for self-care behavior was higher than average level. Level of barriers to exercise was also higher than average, and higher than other barriers of diet and medication; There were low but significant correlations between self-care behavior and barriers to diet, exercise and medication; Variance in barriers to exercise, using alcohol, regular hospital visits, medical treatment and good foot care explained 29.6% of the variance in self-care behavior. Conclusion: According to the study, the self-care behavior of medication and barriers to exercise were the highest. Barriers to exercise highly influenced self-care behavior. Barriers to diet and exercise were significantly correlated the self-care behavior. Further study or interventions are necessary to decrease barriers in patients with type 2 diabetes.

A Survey on Knowledge of Diabetes and Self-care Behavior of Rural Patients with Diabetes Mellitus (농촌지역 당뇨병 환자의 당뇨지식 및 자가관리에 대한 연구)

  • Purpose: The purpose of this study was to examine the knowledge of diabetes and self-care behavior of patients with diabetes mellitus living in rural area. Methods: Participants in this study were 94 patients with type 2 diabetes mellitus. Data were collected from July 13 to September 14, 2012. The instruments were the knowledge of diabetes measurement developed by Park Oh Jang and the measurement of self-care behavior for patients with diabetes developed by Kim Young Ok. Data were analyzed using the SPSSWIN 18.0 program for t-test, one-way ANOVA and Scheffe test. Results: There were significant differences in knowledge of diabetes according to education level (p=.011), alcohol use (p=.010), regular exercise (p=.046), and duration of illness (p=.045). There were significant differences in self-care behavior of patients with diabetes according to satisfaction with income (p=.031), regular exercise (p<.001), experience with diabetes education (p=.001), number of diabetes education sessions (p<0.001), and complications (p=.001). Conclusion: Based on the result of this study, there is a need to develop and confirm the efficacy of education programs by examining their fit to characteristics of patients with diabetes mellitus living in rural areas.

Mental Health and Self-care Activities according to Perceived Stress Level in Type 2 Diabetic Patients with Metabolic Syndrome (대사증후군이 있는 당뇨병 환자의 지각된 스트레스에 따른 정신건강과 자가당뇨관리)

  • Purpose: This study was to identify the mental health and self-care activities according to perceived stress level in type 2 diabetic patients with metabolic syndrome. Methods: The descriptive survey design was conducted using a convenient sample. One hundred and fifty-two diabetic participants with metabolic syndrome were recruited in a university hospital. The data were analyzed using descriptive statistics, Students' t-test, and ANOVA with SPSS/WIN 12.0. Results: The high, moderate, and low stress groups according to perceived stress score consisted of 30.9%, 38.2%, and 30.9%, respectively. Differences of mental health and total self-care activities among the three groups were statistically significant. Depression (F=46.73, p<.001), trait anger (F=5.75, p<.01), and anger expression-in (F=4.60, p<.01) of participants in the high stress group were higher than other groups, whereas the total of self-care activities (F=2.85, p<.05) and exercise (F=3.49, p<.05) were lower than other groups. Conclusion: This study concluded that type 2 diabetic patients with metabolic syndrome with a high stress level were low in self-care activities along with elevated depression and anger.
