A Study on the Transitions and Site of temporary palace(Onyanghaenggung) according to the <Oncheonhaenggungdo>(1795) (<온천행궁도(溫泉行宮圖)>(1795)의 온양행궁지 추정 및 온양행궁 변천 고찰)

  • Onyanghaenggung Palace(temporary palace at Onyang) is an important cultural heritage that can substantially confirm the king's onhaeng(溫行) base on literature records such as <Ongungyeonggoedae(溫宮靈槐臺)>, <Oncheonhaenggungdo(溫泉行宮圖)> of 『Ongungsasil(溫宮事實)』『, Younggoedaegi(靈槐臺記)』and cultural property such as Yeonggoedae(靈槐臺) and Shinjeong Monument(神井碑). As the Onyang Tourist Hotel is located in the presumed site of the Onyanghaenggung Palace, even the identity of the Onyanghaenggung Palace site is being threatened without restoration efforts. The purpose of this study is to estimate the location of Onyanghaenggung Palace based on <Oncheonhaenggungdo> before the damages during the Japanese colonial period. To achieve these purposes, records related to Onhaeng during successive kings' terms in the Joseon Dynasty are first reviewed, before changes in the architecture of Onyanghaenggung Palace that took place in the Joseon Dynasty and damage suffered during the Japanese colonial period are summarized, and finally <Oncheonhaenggungdo>, <Eupji>, <Ancient Maps>, <Jijeokwondo> are reviewed. Based on these processes, the location of Onyanghaenggung Palace is estimated by comparing the current Onyang Tourist Hotel and the surrounding area. The results of this study are as follows. First, if the 1,758 cheok(尺) of 「Onyanggun eupji」 and 「Hoseo eupji」 are converted in Jucheok(周尺), the scope of Onyanghaenggung Palace is close to the inner circumference of the site(垈) in Jijeokwondo(1914). Second, the streamlet leading to Oncheoncheon(溫泉川) from the southern side of Onyanggwan(溫陽館), the hot spring hole in use of <Distribution Map of Hot Spring(溫泉分布見取圖)>(1925, 1928), and considering the relationship of the inner east gate(內東門), Bigak(碑閣), Sinjeong(神井) of <Oncheonhaenggungdo>, the building of Hermann Gustav Theodor Sander and the Copyright Commission's Onyang Hot Springs photograph can be estimated as the Onyanghaenggung Palace Hot-spring, namely Tangsil(湯室). Third, in the process of developing to amusement park, the transfer and relocation of the Yeonggaedae site(a governmentowned property) was requested by Gyeongnam Railway Company, but Chungcheongnam-do denied transfer and relocation of the Yeonggaedae because of the importance in the history of Onyang Hot Springs, so the government-owned Yeonggaedae Monument site were permanently preserved at the current location together with the hoe tree(Sophora japonica L.). Also, Yeonggoedae in <Tourists Attractions around Gyeongnam Railway in Joseon (朝鮮京南鐵道沿線名所交通図絵)> (1929) is shown to exist in its current location, and it can be seen that the Shinjeong Monument Pavilion was moved to the front of Shinjeonggwan (神井館). Based on the circumference of Onyanghaenggung Palace, the location of Onyanghaenggung Palace Hot Spring (Tangsil) and Yeonggaedae Monument Pavilion, changes in roads and lots of land during the Japanese colonial period and the modern period, as well as the location of Onyanghaenggung Palace and other major buildings, can be estimated to extend to the current Shimin-ro and Onyang Hot Spring Market.

A Survey on the Perception of the Counterplans of Medical Accident and Dispute of Dental Hygienist (의료사고 및 의료분쟁에 대한 치위생사의 인식도 조사)

  • In the field of dentistry, there existed relatively few emergency patients or patients who need intensive care and thus had low medical dispute rates. However, these days, there is a general tendency of increased medical disputes. Although many medical disputes are caused by medical accidents of the dentists, because dental assistants are also lawfully involved in practicing dentistry, there is a possibility of medical disputes or medical accidents caused by dental assistants. Therefore, the role of the dental assistants cannot be ignored. This study consists of a survey given to dental hygienists currently working in general hospitals, dental hospitals and private dental clinics. Following is the results of the analysis of 275 respondents' backgrounds, medical disputes rates including patients' complaints, their understanding of medical regulations and their general understanding of overall dental practice and medical disputes. 1. 251 of 274(91.6%) respondents doubted the risk of medical accident and dispute. 2. 81(29.5%) dental hygienist experienced complaint from patients. They have been working in the private dental clinic, the rate of this experience was high. 3. 349 case of 1805(19.3%) the complaints by patients, highest percentage among its category, were those regarding dental fees and poor service. 4. 129 case of 1805(7.1%) patients' complaints, highest percentage among it's subcategory, were those regarding the absence of explanations of precautions or request of agreements before dental treatment. 5. 252 of 267 (94.4%) dental hygienists chart after a scaling treatment. However, only 55(20.7%) dental hygienists chart the fact of explaining the precautions. 6. 6(2.2%) dental hygienists do not inspect patients' medical history, if patients don't mention it. 7. 104 of 274(38.0%) dental hygienists responded to be capable of administering first aid treatment. 8. 115(41.8%) dental hygienists have a first aid kit and equipment. 9. In case of medical dispute, 268(97.8%) dental hygienists respond that, charting plays a big role in resolving the dispute. 10. In case of medical dispute, 272(93.3%) dental hygienists respond that, explanation and agreement before treatment have an important role in settlement of dispute 11. Only 160(58.4%) dental hygienists responded correct answer that the duration of keeping medical records is 10 years. 12. 124(45.3%) respondents thought that it is legal for a dental hygienist to take a panoramic dental X-ray, 71(25.9%) respondents thought that it is legal practice cervical resin treatment by dental hygienist, and 37(13.5%) respondents thought that it is legal extract primary teeth by dental hygienist. 13. 24(18.76%) respondents thought that it doesn't matter to tell patient's state to others 14. 272(99.27%) responded that receiving education for the prevention of medical disputes was needed and of them, 61.0% thought it was urgent. 15. 186(64.2%) has never had classes regarding the prevention of medical disputes while in school and 212(77.4%) has not had the same type of classes after graduating from school. 16. 256(93.4%) responded that there will be even more of an increased number of medical disputes. Among them, 83.3% of respondents though that due to the increased opportunity of acquiring information through the internet and mass media. The study shows that 29.5 percentage of dental hygienists have experienced the medical disputes and complaints and they are lack of recognition of medical regulations and dental hygienist's official duty. So, there is a big potential of the percentage to increase. Therefore, the correct understanding of explaining precautions and requesting agreement before dental treatments and performing them are mandatory. Moreover, classes regarding the prevention and counterplans of medical disputes need to be widely offered.

A Pre-school Teacher's Educational Meaning of an Activity : 'Discourse between Children through Books' in 3-year-old Children's Classroom (유아교사가 생각하는 만 3세 교실에서 '책을 통한 유아들 간의 의견나누기' 활동에 대한 교육적 의미)

  • This study aims to discover how three-year-old children to search for existence and its meaning, while they participate a programme, 'discourse among children through books' as a way of resolving curiosities. Data were collected between April 2, September 10, 2010 and the collected data included teacher's class records, pictures, discussion notes and interview materials. The results showed that 'discourse among children through books' follows steps in a process. The steps include: Deciding a topic that children should search for, choosing an appropriate book to resolve children's curiosities, spending some time for discoursing among children by using pictures in the book, and sharing what they had understood. Through this process, a pre-school teacher met with children who were full of abilities, ideas and motivation to explore with a little assistance. The pre-school teacher was able to overcome preconceived ideas that informational books could be difficult for children, and experienced various lessons, while the children led their own learning. It represents that the activity, 'discourse among children through books' can be a children centered inquiry learning teaching strategy, because the informational books ensure that the children gain knowledge and are actively in cogitation.

Factors affecting the survival of implants: a long-term retrospective study (임플란트의 생존에 영향을 미치는 요인에 대한 장기간의 후향적 연구)

  • Purpose: The aim of the present study was to evaluate the long-term survival of implants retrospectively and determine the risk factors associated with implant failure. Materials and Methods: Of all implants that were placed at the Department of Periodontology of the Dental Hospital of Gangneung-Wonju National University from January 1998 to December 2012, 2265 implants that were followed up until June 2013 were included in this study. Data were collected from clinical and radiographic examinations from previous visits. The information gathered included gender, age, smoking status, implant diameter, implant length, surface of implant, location of implant within the dental arch, surgical techniques and existence of complications. Results: The survival rate before loading was 98.9%. The cumulative survival rate after 5 years of loading was 97.2%, and after 15 years of loading was 95.2%. In a simple logistic regression analysis, gender (P = 0.016), smoking status (P = 0.001), location of implant (P = 0.020) and existence of complications (P = 0.002) were statistically associated with implant failure and included in the multiple regression analysis. As a result of multiple logistic regression analysis, the variables statistically associated with implant failure (P < 0.05) were smoking status (P = 0.049) and existence of complications (P < 0.001). Conclusion: The cumulative survival rate of dental implants after 15 years of loading was 95.2% and that the variables statistically associated with implant failure were smoking status and existence of complications.

THE RELATIONS BETWEEN CRANIOFACIAL MORPHOLOGY AND DENIAL AESTHETIC INDEX(DAI) (치열심미지수(DAI)와 악안면 형태와의 관계)

  • Multiple features of occlusion have been combined by epidemiologists in attempts to objectively to determine malocclusion severity and treatment priority. Jenny et al were developed DAI(Dental Aesthetic Index), which is an orthodontic index based on socially defined aesthetic standards. This study aimed to evaluate association between 10 occlusal components of DAI and individual measures of skeletal morphology by use of univariate and multivariate analysis. In addition the relationship between a Dental Aesthetic Index score and skeletal morphology was evaluated. The data for this study were obtained from cephalometric radiographs and dental casts of 182 Korean patients(79 men, 103 women) with permanent dentition who had been accepted for the orthodontic treatment at Department of Orthodontics, Chonbuk National University. The results are as follows: 1. Several occlusal components of DAI(missing, crowding and spacing in the incisal segment, diastema, largest anterior irregularity on upper and lower arch) were not associated with individual measures of craniofacial morphology. 2. Sagittal occlusal components of DAI were associated with anteroposterior skeletal measures. 3. The relation between vertical occlusal components of DAI and vertical skeletal measures was low magnitude. 4. A Dental Aesthetic Index, which is based on several aspects of occlusion, did not identify craniofacial morphology. We can conclude that combination of certain occlusal components of the DAI may be related with specific skeletal morphology; but, this concept could not be accepted generally.

The Clinical Significances of Bronchial Anthracofibrosis in the Patients with Endobronchial Tuberculosis (기관지 결핵 환자에서 기관지 탄분 섬유화증의 임상적 의의)

  • Background : Bronchial anthracofibrosis is one of the main manifestations of lung disease that is related to woodsmoke inhalation, and it is frequently associated with various pulmonary diseases, such as tuberculosis. The purpose of this study was to evaluate the clinical significance of bronchial anthracofibrosis in patients with endobronchial tuberculosis. Methods : 63 patients, who were diagnosed with endobronchial tuberculosis using bronchoscopy, were included in this study. The patients consisted of 12 males and 51 females, having mean age of 59.5 years. The clinical features, radiologic and bronchoscopic findings between the patients with (37) and without (26) bronchial anthracofibrosis were analyzed retrospectively. Results : When the patients were older, bronchial anthracofibrosis was more frequent. The endobronchial tuberculosis, which was located at the right middle lobal bronchus, was more frequent in the patients with bronchial anthracofibrosis than in the patients without bronchial anthracofibrosis. In the morphologic types of endobronchial tuberculosis, patients with bronchial anthracofibrosis had more edematous-hyperemic and ulcerative types, while patients without bronchial anthracofibrosis had more active caseating. Conclusion : These findings suggest that the presence of bronchial anthracofibrosis can possibly influence the locations and morphologic types of endobronchial tuberculosis.

Study on the Transformation of Ponds and the Account of Reconstruction at Jondeokjeong Area in the Rear Garden of Changdeok Palace (창덕궁 후원 존덕정 일원 지당의 변형과 조영경위에 관한 고찰)

  • This study was carried out to find the changed contents and details of the ponds at Jondeokjeong area in the Changdeok Palace, where was modified during the reign of King Gojong(高宗, 1863~1907), and considered the internal context through the diachronic views about royal garden building in the Joseon Dynasty. The results were as belows. First, this study found that the transformed time of the ponds at Jondeokjeong area was the 21th year of King Gojong(1884) based on the pictures taken in the last half of the 19th century and the records of "Gojongsilrok(高宗實錄: Annals of the King Gojong)". Second, this study also found that the remodeled ponds at Jondeokjeong area by King Gojong were followed the landscape of Geoncheong Palace(乾淸宮) and Hiangwon-pond(香遠池) in Gyeongbok Palace because the remodeled ponds of Jondeokjeong area had the spatial organization similar to Hiangwon-pond. Third, the bridge of remodeled ponds at Jondeokjeong area was utilized as the expedient to expand the function of rear garden, which combines the areas of Yungyungdang(演慶堂) and Jondeokjeong. This was the same method that Konchunggung(乾淸宮) in Gyeongbok Palace had occupied the whole area through the Chui-hiang bridge(醉香橋) which connects to the Hiangwonjeong(香遠亭). Fourth, ponds at Jondeokjeong area and Hiangwonjeong that were created during the year of King Gojong promised the use of Yungyungdang and Konchung Palace, and reflected the statuses of the two building-blocks. Lastly, this study concluded the remodeled Jondeokjeong ponds were not only to create the necessary spaces of the landscape for King Gojong, but also to build the space on the context of Huwonjeongdang(後苑政堂: political structure in rear garden), which has been passed down from their ancestors.

The Designers and the Space-Compositions of the Goon Office Buildings which were newly constructed in the Japanese Colonial Period (일제강점기 신축된 군청사의 설계주체와 평면구성의 특징)

  • The purpose of this study was to discover who designed the common type and the individual type Goon office buildings in the Japanese colonial period, and to determine how the individual type's space-compositions were compared with the common types. The study used the literature research method. The method used an analysis of the digital images of the Goon office buildings kept in the national archive of Korea, the annotation of the images, the official documents between the Japanese government-general of Korea and To, the employee's registers of the Japanese government-general of Korea, and newspapers or journals from the Japanese colonial period. The designers of the common types were the engineers of the building-construction part in the Japanese government-general of Korea and also those in To. The space-composition of the individual types was similar to and also different from the common types at the same time, and the difference was attributed to the designers' ability and their preferences.

Prognosis of Replanted Permanent Incisors after Avulsion Injury: A Retrospective Study (완전탈구 후 재식한 영구 절치의 예후에 대한 후향적 연구)

  • Many studies on the prognosis of avulsed teeth were published but follow-up studies on Korean children and adolescents are insufficient. The aim of this study was to identify clinical factors determining the healing and prognosis of permanent incisors that were replanted after avulsion injury. This study included 184 permanent teeth in 142 patients aged 6-14 years. The clinical and radiographic data of the patients were analyzed from electronic medical records. Immature teeth had a higher probability of pulp revascularization than mature teeth. When extra-alveolar time was longer than 60 minutes, the incidence of inflammatory root resorption (IRR) was higher in immature teeth than mature teeth. However, the incidence of replacement root resorption (RR) was higher in mature teeth than immature teeth under the same condition. The incidence of IRR was increased when pulp was extirpated more than 20 days after replantation. IRR had a more significant influence on the decrease of survival expectation than RR. There was no significant correlation between the root development stage and survival expectation. However, when RR occurred, immature teeth survived shorter than mature teeth. Long term follow-up results from this study are expected to be used as fundamental data for the treatment guidelines and evaluation of the prognosis of replanted teeth.

A cephalometric study in patients with obstructive sleep apnea after use of oral appliance (폐쇄성 수면 무호흡 환자에 있어서 구내장치 반응성에 대한 두부 방사선적 연구)

  • It is well known that oral appliance could improve respiratory difficulty in patients with obstructive sleep apnea (OSA). To evaluate the effect of oral appliance, polysomnography and cephalometry were performed in OSA patients before and after oral appliance was used. Twenty four OSA patients were included in this study. Respiratory difficulty index (RDI) was obtained from polysomnography and the movement of soft palate, tongue, posterior pharygeal wall, hyoid bone, and mandible and the variables of the facial pattern were measured on the cephalogram. The changes of cephalometric parameters were compared to the RDI changes and the correlation was tested. After oral appliance was used, RDI decreased from 46.8 to 13.3. There was significant relationship between RDI improvement and the anterior movement of the mandible and superior movement of the hyoid bone. In 8 patients whose RDI was most improved, RDI improvement rate was correlated with the anterior movement of the tongue and anterior-posterior (AP) diameter of the airway at the levels between the lower portion of the soft palate and epiglottis. These results could conclude that anterior movement of the tongue and superior movement of the hyoid bone would be favorable cephalometric parameters for the improvement of OSA.
