Aster Yomena (AY) has been used in traditional medicine to treat diseases such as obesity, hyperlipidemia, atherosclerosis, diabetes and osteoarthritis. However, protective effect of AY on acute pancreatitis (AP) has not been reported. The present study examined the anti-inflammatory effects of an ethanol extract of AY on cerulein-induced AP. AP was induced in mice by intraperitoneally injecting cerulein ($50{\mu}g/kg$) hourly for 6 times. 70% ethanol extract of AY (0.1, 0.2, and 0.5 g/kg) was orally administered for 1 week before acute pancreatitis induction. The mouse was killed at 6 hours after the final cerulein injection. The pancreas and lung were rapidly removed for histological examination and myeloperoxidase (MPO) assay. Blood samples were taken to determine serum amylase and lipase activity. In addition real-time reverse transcription-polymerase chain reaction (RT-PCR) was also performed to investigate mRNA expression of proinflammatory cytokines such as $TNF-{\alpha}$. $IL-1{\beta}$, and IL-6. Administration of AY significantly ameliorated pancreatic weight to body weight ratio, histological damages and MPO activity during AP. In addition, AY inhibited the serum amylase and lipase activity during AP. Also, mRNA expression of $TNF-{\alpha}$, $IL-1{\beta}$ and IL-6 were inhibited by AY against AP. Our results revealed that pre-treatment of AY reduces the severity of cerulein-induced AP. Therefore, AY may have a protective effect drug against AP.
Objective: The purpose of this study was to investigate the effect of electroacupuncture on acute pancreatitis (AP). Method: The search engines that were used included PubMed, CNKI, OASIS, NDSL, the Korean Traditional Knowledge Portal, and J-STAGE. The search period was from the creation of the search engine to May 6, 2019; there were no language limitations. Extractions and selections from the literation were made by two authors. The study included randomized controlled trials with electroacupuncture for patients with acute pancreatitis. Cochrane's risk of bias (RoB) was used as the methodological quality assessment scale. Results: Nine studies were finally selected. It was observed that using electroacupuncture with general Western medicine treatment for AP was more effective than using only general Western medicine treatment. Although most of the studies showed a high risk of bias, there were significant differences among the indicators. Conclusion: The electroacupuncture treatment with Western medicine may be more effective in reducing symptoms of AP than Western medicine alone. However, due to the limitations of the research design, it was not enough to obtain reliable information. As a result, high quality research will be additionally required.

Water extract of CM (0.1, 0.2, or 0.5 g/kg) was administrated intraperitoneally 1 h prior to the first injection of cerulein. The mice were killed at 6 h after the final cerulein injection. Pancreas was rapidly removed for morphologic and histochemical examination, myeloperoxidase (MPO) assay. Blood samples were taken to determine serum amylase and lipase activities. Results : Administration of CM significantly inhibited pancreatic weight/body weight ratio, pancreas histological injury. And CM administration inhibited the serum digestive enzyme elevation such as amylase and lipase on cerulein-induced pancreatitis. In addition, Pancreas MPO activity which indicates neutrophil infiltration was inhibited by CM extract on cerulein-induced pancreatitis. Conclusions : In conclusion, our results could suggest that pre-treatment of CM reduces the severity of cerulein-induced AP. Therefore, CM could be used as a protective agent against AP. Also, this study could give a clinical basis that CM could be a drug or agent to prevent AP.
Octreotide는 somatostatin 동족체로서 8개의 aminoacid로 구성된 합성 polypeptide이다. 이는 외분비선과 내분비선, 특히 위의 분비기능과 췌장분비 기능을 억제하는 작용이 강하며 somatostatin에 비하여 체내 반감기가 길고 장기에 대한 선택성이 높기 때문에 소화성 궤양에 의해 유발되는 출혈, 급성췌장염, 그리고 소화기관의 분비선에 나타나는 종양의 치료에 대한 적용이 시도되고 있다. 저자등은 somatostatin이 말초 교감신경에 존재하며, 신경전달인자 또는 신경조정인자로서 작용할것이라는 보고와 임상적용시 혈압과 심박동수를 감소시킨다는 보고에 흥미를 가지고 그 유도체인 octreotide가 말초 교감신경에 미치는 작용을 관찰하기 위하여 다음과 같은 실험을 하였다. 흰쥐 (Sprague-Dawley)의 전립선 부위 정관 절편을 적출 근편 실험조에 현수하여 등척성 장력을 측정하였다. 흰쥐 적출정관은 전기장 자극(duration : 1 mSec, voltage: 50 V, frequency: 5 Hz or 30 Hz, train: 10 Sec)에 대하여 first phasic component (FPC)와 second tonic component (STC)로 구성된 수축 반응을 나타내었으며, 이러한 수축반응은 tetrodotoxin($10^{-6}M$) 전처치로 소실되었다. Octreotide는 전기장 자극에 의해 유발되는 수축 반응을 FPC, STC 모두 농도의존적으로 억제하였다. 그리고 24 시간 전에 reserpine(1 mg/kg, IP)을 전처치하여 norepinephrine을 고갈시킨 흰쥐에서 적출한 정관 절편과 beta-gamma-methylene ATP를 전처치하여 퓨린성 수용체를 탈감작시킴으로써 ATP의 작용을 배재한 정관절편의 전기장 유발 수축을 공히 억제하였다. 이러한 octreotide의 모든 수축 억제 작용들은 5 Hz의 전기장 자극을 가하였을 때, 30 Hz의 전기장 자극을 가하였을 때 보다 더 강하게 나타났다. 그러나 octreotide는 정관 절편의 기본장력에는 영향을 미치지 않았으며, 외부에서 첨가한 norepinephrine과 ATP에 의해 유발되는 수축반응에는 영향을 미치지 않았다. 이상의 결과로 미루어 보아 octreotide는 흰쥐적출 정관의 교감신경성 신경원으로부터 ATP와 norepinephrine의 유리를 억제함으로써 그 수축성을 억제하는 것으로 사료된다.
우리 나라 고유의 인체 기생 장흡충 Gwmopholloides seoi Lee, Chai and Hong, 1993은 급성 췌장염 환자에서 처음 발견되었고. 신안군 도서지방에 큰 유행지가 있음이 밝혀져 의학적 중요성이 점차 높아지고 있으나 지금까지 인체 감염원이 되는 제2중간숙주가 알려져 있지 않았다. 이번 연구에서는 자연산 참굴(Crossoskrea gigas)이 제2중간숙주의 하나일 것으로 추정하고 신안군 유행지에서 참굴을 채집한 후 gniulophallid metacercariae에 감염되어 있는지 조사하였다. 조사한 참굴 50마리 모두에서 glrnnophallid metacercariae가 발견되어 100%의 감염률을 보였고. 참굴 1마리 당 평균 610개(개체당 2-4.792개)의 감염량을 보였다 충체의 종은 추후 마우스 감염 실험을 통하여 G. seoi로 동정할 수 있었다. 유충(metacercariae)은 낭을 가지고 있지 않았고, 구흡반으로 참굴의 외투막(mantle surface)을 흡착하고 있었으며, 참굴의 절편 표본에서 참굴큰입흡충 속의 특징 중 하나인 복측흡(ventral pit)이 관찰되며. 복흡반 전방에 근육질이 아닌 생식공이 있고 저정낭으로 연결되어 있었다. 저정낭은 대부녁의 충체에서 단낭성(mono-sac)이어서 유일한 다른 종인 G. toiensis가 이낭성(bipartite) 저정낭을 가진 것과는 차이를 보였다. 이 연구를 통하여 참굴이 G. seoi의 제2중간숙주이며 동시에 가장 중요한 인체 감염원일 것으로 판단하였고, 이 흡충의 우리말 이름을 참굴큰입흡충으로 제안한다.
For the study of the hemodynamic changes of the cranial pancreaticoduodenal arterial flow(cPDAF) in the dog with acute pancreatitis, acute pancreatitis was experimentally induced in 10 dogs by the injection of oleic acid into the accessory pancreatic duct. The parameters of cPDAF were measured by transcutaneous pulsed-wave Doppler ultrasonography. The hemodynamic changes included resistive indexe(RI), pulsatility index(PI) and maximum velocity (Vmax). Ultrasonographic scans were performed before the induction of pancreatitis and once daily for five days after the induction. The RI, PI and Vmax were increased with day as follows; the RI prior to induction was 0.625$\pm$0.096 (mean$\pm$SD), the PI was 1.117$\pm$0.289 and the Vmax was 0.349$\pm$0.094 m/s. After five days, the RI was 0.727$\pm$0.051 (p<0.0l), the PI was 1.480$\pm$0.284 (p<0.0l) and the Vmax was 0.585:$\pm$0.114 m/s (p<0.00l). These results show that there is some relation between the increase of the RI, PI and Vmax of cPDAF and the progress of acute pancreatitis in dogs. Therefore, the measurements of the hemodynamic changes of cPDAF may be a valuable technique for the evaluation of acute pancreatitis in dogs.
Acute pancreatitis(AP) in children is not common but can be associated with severe morbidity rates and its diagnosis is often delayed. Thus, reported mortality rates range from 0 to 78%. We have treated 26 patients with AP from 5 to 17 years of age over the past 17 years. We are intended to assess the relevance of the prognostic criteria used to assess severity of adult AP and to review the etiology, clilical presentation, diagnosis, and management of AP in children. The authors retrospectively reviewed 26 children with AP managed in Kyung Hee University Hospital from 1978 to 1995. Among 26 patients with AP, male were 12, and female were 14. And the mean age of patients was 11.8 years. In 9(34.6%), no definitive cause was identified. Common causes of AP were trauma(23.1%) and biliary tract disease(23.1%). Other etiologies were viral infection(15.4%) and post ERCP(3.8%). The presenting features were abdominal pain(92.3%), vomiting(61.5%), fever(19.2%), submandibular pain(11.5%), and abdominal mass(7.6%). Back pain was rare(3.8%). Abdominal ultrasonographic findings were abnormal in 10 of 16 patients(62.5%) and abdominal CT findings were abnormal for 9 of 9 patients(100%). Seventeen patients(65.3%) were managed conservatively, and nine patients(34.6%) required surgical treatment. There was no mortality. To evaluate the severity of disease, we used the Imrie prognostic criteria used to assess the severity in adult AP. The number of positive criteria was correlated to the duration of hospitalization(r2=0.91) but statistically insignificant(p>0.05). But, the number of positive criteria was correlated to the operative incidence(r2=0.93) and statistically significant(p<0.05). The common causes of AP in children were unknown origin(34.6%), trauma(23.1%), and biliary tract disease(23.1%). Ultrasonography and computed tomography were useful imaging tools of AP in children. The Imrie criteria used to evaluate the severity in adult AP were suspected to be valuable to assess the severity of AP in children.
Objectives: The aim of this study is to investigate the effect of a Dachaihu decoction for hyperlipidemic acute pancreatitis (HLAP) by systematic review and meta-analysis of Chinese clinical studies. Methods: China National Knowledge Infrastructure (CNKI) was utilized as the major search engine. The date of the literature search was March 7, 2020. Randomized controlled trials (RCTs) about using a Dachaihu decoction for HLAP were included in this study. Meta-analysis was performed by synthesizing outcome data, including total effective rate, abdomen pain relief time, first bowel movement time, blood amylase recovery time, and triglyceride (TG) levels (mmol/L). The selected literature was assessed using Cochrane's risk of bias (RoB). Results: Twelve of 44 RCTs met the inclusion criteria. Most studies were evaluated with RoB as having unclear risk. The total effective rate of herbal medicine treatment based on the Dachaihu decoction was significantly higher than that of symptomatic supportive treatment in 10 articles (risk ratio=1.15, 95% CI: 1.08 to 1.21, p<0.00001, I2=0%). Herbal medicine treatment based on a Dachaihu decoction was significantly more effective than symptomatic supportive treatment in terms of reducing abdomen pain relief time (in all articles; mean difference=-1.70, 95% CI: -1.91 to -1.41, p<0.00001, I2=45%), first bowel movement time (in 7 articles; mean difference=-1.46, 95% CI: -1.86 to -1.05, p<0.00001, I2=73%), blood amylase recovery time (in 8 articles; mean difference=-1.48, 95% CI: -2.04 to -0.92, p<0.00001, I2=90%), and TG levels (in 8 articles; mean difference=-1.59, 95% CI: -2.28to -0.91, p<0.00001, I2=90%). Only one article reported side effects of treatment among the intervention group and control group, citing pancreatic ulcer and pancreatic pseudocyst formation. Conclusions: This study suggests that herbal medicine treatment based on a Dachaihu decoction could yield higher efficacy for HLAP than symptomatic supportive treatment alone. However, the results might be somewhat biased because of the poor quality and small sample size of the included RCTs. Well-qualified clinical studies are needed to prove the effectiveness of Dachaihu decoction therapy for HLAP.

Water extracts of PO (100, 300, or 500 mg/kg) was administrated intra-peritoneally 1 h prior to the first injection of cerulein. The mice were killed at 6 h after the final cerulein injection. Pancreas and lung were rapidly removed for morphologic and histochemical examination, myeloperoxidase (MPO) assay. Blood samples were taken to determine serum amylase and lipase activities. Results : Administration of PO significantly inhibited pancreatic weight/body weight ratio, pancreas and lung histological injury. And MPO activity which indicates neutrophil infiltration was inhibited by PO extracts on cerulein-induced pancreatitis. In addition, PO administration inhibited digestive enzymes such as serum amylase and lipase activity on cerulein-induced pancreatitis. Conclusion : Our results could suggest that pre-treatment of PO reduces the severity of cerulein-induced AP, thereby, PO could be used as a protective agent against AP. However, the protective effect of DRC water extract on acute pancreatitis (AP) has not been deeply reported. Therefore, we aimed to evaluate the protective effects of DRC water extract on cerulein-induced AP. Methods : AP was induced via intraperitoneal injection of supramaximal concentrations of stable cholecystokinin analogue cerulein ($50{\mu}g/kg$) every hour for 6 times. DRC water extract (0.05, 0.1, or 0.2 g/kg) or saline was administrated intraperitoneally 1 h before to the first injection of cerulein. The mice were sacrificed at 6 h after the final cerulein injection. Pancreas was rapidly removed for histochemical examination and myeloperoxidase (MPO) assay. In addition, polymerase chain reaction (PCR) was performed to examine mRNA levels of proinflammatory cytokines such as Interleukin $(IL)-1{\beta}$, IL-6 and Tumor necrosis factor $(TNF)-{\alpha}$. Results : Administration of DRC water extract significantly inhibited the pancreatic weight to body weight ratio, pancreas histological damages and increase of pancreatic MPO activity during cerulein-induced AP. In addition, increased pancreatic mRNA levels of $IL-1{\beta}$, IL-6 but not $TNF-{\alpha}$ were significantly inhibited by treatment of DRC water extract against cerulein-induced AP. Conclusions : In conclusion, we have revealed that pre-treatment of DRC water extract reduces the severity of cerulein-induced AP. Accordingly, our results could give a clinical basis that DRC could be used as a drug or agent to prevent AP.
