Ginger roots are widely used as spices in various foods and herbal medicine due to its characteristic flavor and biological activity. In this study, the protective effect of aged ginger extracts against oxidative stress were investigated using L6 muscle cells. As the results, aged ginger extracts significantly inhibited oxidative stress induced muscle cell damage. The protective effect of aged ginger extracts was higher than non-aged ginger extract. Aged ginger extracts also inhibited the increase in LDH, lactate and GOT in the mouse blood induced by excessive running exercise. Therefore, aged ginger is considered to be effective for protecting excessive exercise stress.
Purpose: To evaluate medications at the end of life care for terminal cancer patients during their last admission. Methods: Medical records of terminal cancer patients during their last admission from July 2003 to April 2008 at a district academic hospital were evaluated. Patient's characteristics, therapeutic drug classification during their last admission and on the patient's day of death, and the administrated route and number of medications on the patient's day of death were analyzed. Results: Total 81 patients were included. The median patient age was 63 years. The median length of admission was 18 days (range: 1~101). 54% of the patients had more than one comorbidities. The most frequently prescribed drugs during the last admission were opioid analgesics (63%), followed by antibiotics (58%) and antacids (53%). On the day of death, common medications were antibiotics (59%), antacids (58%), and opioid analgesics (46%). Intravenous injection was given to 81% of the patients and intramuscular injection was given to 16% of the patients on the day of patient's death. Number of medications prescribed to patients was between 0 and 11 (median: 3) and 12% (10/81) of the patients took over 8 medications including intravenous and oral drugs on the day of death. 6% (5/81) of the patients took potentially futile medications, like multivitamin or statin until the day of death. Conclusion: This study suggests that potentially futile medications and uncomfortable care were given to terminal cancer patients. Multicenter-based studies are necessary to diminish futile medications by essential medication at the end of life care for terminal cancer patients.

We report two cases of accidental overdoses of intramuscular midazolam used for a conscious sedation. A 4-year-old boy with dental caries was scheduled for treatment under conscious sedation. The pedodontist prescribed midazolam ($dormicum^{(R)}$ 5 mg / 5 ml) 2 ml (2 mg) by verbal order to hygienist. The hygienist instead of the pedodontist wrote a prescription for midazolam ($dormicum^{(R)}$ 15 mg / 3 ml) 2 ml (10 mg). The inexperienced nurse gave an injection to his buttock as prescription. The child fell into a deep sedation. A 4-year-old boy with dental caries was scheduled for treatment under conscious sedation. The inexperienced pedodontist gave an injection to his buttock midazolam ($dormicum^{(R)}$ 15 mg / 3 ml) 3 ml (15 mg) instead of midazolam ($dormicum^{(R)}$ 5 mg / 5 ml) 3 ml (3 mg). The child fell into a deep sedation. Both cases had no complications, but the accidents happened as a result of the inexperienced dental staffs. The five times midazoalm instead of the intended doses was inadvertently given intramuscularly, fortunately caused no harm in our cases. However, the situations suggest that we should carefully check the dosage and review the correct procedures, even when using a drug that is considered to be familiar with most practitioners.

The purpose of this study was to compare the effects of either aerobic exercise or polyphenols supplementation on mRNA expression of endoplasmic reticulum stress in skeletal muscle of high fat diet-induced obese mice. In the study, mice were divided into five groups: (1) NC (normal diet for 16 weeks as a control, n=10), (2) HC (high fat diet for 16 weeks as a control, n=10), (3) H-Re (high fat diet with resveratrol 25 mg/kg supplementation for 16 weeks, n=10), (4) H-Ch (high fat diet with chrysin 50 mg/kg supplementation for 16 weeks, n=10), and (5) HE (high fat diet with aerobic exercise for 16 weeks, n=10). Aerobic exercise was performed on a treadmill for 40~60 min/day at 10~14 m/min, 0% grade, four days/week for 16 weeks. Endoplasmic reticulum stress related genes were measured by real-time polymerase chain reaction. ATF6, PERK, $IRE1{\alpha}$, and BIP/GRP78 mRNA were significantly decreased in HE compared with those in HC (p<0.05). Also, ATF6, $IRE1{\alpha}$, and BIP/GRP78 mRNA were significantly decreased in H-Re compared with those in HC (p<0.05). ATF6 mRNA was significantly decreased in H-Ch compared with that in HC (p<0.05). These findings suggest that aerobic exercise, resveratrol, and chrysin supplementation changed ER stress markers. However, aerobic exercise was most effective on ameliorating the high fat diet induced ER stress markers. Thus, it seems that aerobic exercise might have a more positive effect on skeletal muscle endoplasmic reticulum stress compared with polyphenol supplementation in high fat diet-induced obese mice.

A three-year old intact female German shepherd dog weighing 32kg was presented with the history of regurgitation and vomiting. Physical examination revealed a thin body condition, hoarse voice, hypersalivation, and pelvic limb weakness. In radiograph, meagaesophagus was identified. The neostigmine challenge test was positive. The titer of acetylcholine receptor (AChR) antibody was 1.58 nmol/L (reference range, <0.6nmol). Therefore, the dog was diagnosed to be suffering with chronic generalized form of acquired myasthenia gravis and followed by treatment with pyridostigmine bromide, 1mg/kg, q12 hrs. Treatment improved the pelvic limb weakness, however intermittent regurgitation and vomiting persisted. This resulted in aspiration pneumonia f3r which antibiotics were administered However, the dog was euthanized due to poor management and owner's request.
A 7-year-old intact female, Miniature Schnauzer dog was presented with an acute seizure episode and tremors. Fever and panting were noted when presented. Physical examination revealed apparent forelimb muscle rigidity, fasciculations and stiff gait. Characteristic laboratory findings in this dog were severe hypocalcemia with hyperphosphatemia. The low serum concentration of intact parathyroid hormone and ionized calcium levels were consistent with the diagnosis of primary hypoparathyroidism. In addition, there were increased urinary excretion of calcium and decreased urinary excretion of phosphorus in this case. Urgent treatment was initiated with 10% calcium gluconate. The dog remained healthy with vitamin D analogue and oral calcium supplementation. A transient hyperphosphatemia was controlled well with sevelamer hydrochloride. To the author's knowledge, this is the first case report describing the clinical and laboratory characteristic features of canine primary hypoparathyroidism and its clinical outcome in Korea.

Effects of wild plant extract (Lak) based on Korea traditional prescription on maximal exercise performance and endurance were evaluated using calorie-restriction animal model. In acute forced swimming test with 10% body weight attached to tail, dietary Lak supplementation increased exercise performance endurance by increasing concentrations of ATP and insulin-like growth factor-1 (IGF-1) under calorie-restriction condition, and decrement of blood lactic acid concentration and increment of muscle ATP content were observed. These results suggest Lak is very effective for decreasing side-effects of obesity therapy using very low calorie diet.
Equine motor neuron disease (EMND) is a spontaneous neurologic disorder of horses, which results from the degeneration of motor neurons in the spinal cord and brain stem. An 8-year-old Jeju pony gelding presented with weight loss, muscle tremors, frequent recumbence, low head carriage, sweating, and standing with four limbs close together. The gelding has been on the same stable and limited access to pasture for several years. The gelding has been fed with dried hays and commercial concentrated feeds. Laboratory test revealed very low serum vitamin E level (0.14 ${\mu}g/mL$; reference range > 1.5 ${\mu}g/mL$), mildly elevated creatine kinase (402 IU/L; reference range 119-287 IU/L) and aspartate aminotransferase (876 IU/L; reference range 226-336 IU/L). Oral glucose absorption test showed decreased glucose absorption. Histopathologic examination using a biopsy specimen from sacrocaudalis dorsalis medialis muscle revealed atrophic and hypertrophic muscle fiber, centralization of nucleus, degenerating and necrotic muscle fibers. Taken together, the gelding was diagnosed as EMND. After oral vitamin E administrated for 5 weeks, the gelding showed normally improved stance, decreased periods of recumbency, improved head carriage and weight gain except consistent tremors. This is the first report that successfully treated EMND case occurred in a Jeju pony in Korea.

In this study, the anti-diabetic activity of a cold water extract of Korean mistletoe (KME) was investigated in C57BL/6J Lep ob (ob/ob) mice. Oral administration of KME (50 or 100 mg/kg/d) significantly inhibited the level of blood glucose of ob/ob mice after 5 days from the beginning of KME treatment. And the anti-diabetic effect of KME was stabilized 10 days after oral administration, showing a substantial reduction of blood glucose levels by more than 20% as compared with control mice. The results of oral glucose tolerance test (OGTT) revealed that oral administration of KME gave rise to a remarkable improvement in overall glucose response. Oral administration of KME in ob/ob diabetic mice also significantly reduced blood total cholesterol (TCHO) and triglyceride (TG) levels compared with the diabetic control mice. Moreover, in an in vitro experiment using C2C12 myotubes, treatment of KME prominently increased glucose uptake. Interestingly, KME significantly increased the expression of peroxisome proliferator-activated receptor gamma coactivator 1-${\alpha}$ ($PGC-1{\alpha}$), a head regulator of mitochondrial biogenesis and oxidative metabolism, and $PGC-1{\alpha}$-associated genes such as glucose transporter type 4 (GLUT4), estrogen-related receptor-${\alpha}$ ($ERR-{\alpha}$), nuclear respiratory factor-1 (NRF-1), and mitochondrial transcription factor A (TmfA) in C2C12 cells. These results suggest that KME has potential as a novel therapeutic agent for diabetes, and its anti-diabetic activity may be related to the regulation of mitochondrial biogenesis.
Purpose : Obesity is known to be associated with hypertension, dyslipidemia, and fatty liver and is thought to be associated with increased levels of free fatty acids. One of the strategies for decreasing free fatty acid levels is stimulation of hepatic lipid oxidation with L-carnitine. Carnitine is an essential cofactor for transport of long-chain fatty acid into mitochondria for oxidation. This study was designed to evaluate the changes of serum fatty acids and carnitine levels after exogenous injection of L-carnitine. Methods : Sprague Dawley rats were divided into two groups. Group A was control. Group B was given intraperitoneal injection with L-carnitine(200 mg/kg) daily for two weeks. Serum lipid (total cholesterol, triglyceride, HDL-cholesterol, LDL-cholesterol) and fatty acid levels were analyzed on the first day of the first and second weeks after injection of L-carnitine. Total, free, and acyl carnitine levels also were performed by a enzymatic cycling techniques at the same day intervals. Results : There was no significant difference between the two groups in total cholesterol, HDL-cholesterol, LDL-cholesterol levels before and after the administration of L-carnitine. But triglyceride levels were significantly decreased at the first week in group B compared with group A. Among free fatty acids, linoleic acid showed significant decrement(A group : $131.3{\pm}31.3mg/dL$ vs B group : $90.0{\pm}7.0mg/dL$) at the first week. Total, free, and acyl carnitine levels showed significant increments at all days intervals, but only free carnitine showed significant increments according to cumulative doses of carnitine. Conclusion : Plasma linoleic acid, a long-chain fatty acid, showed significant decrement after administration of L-carnitine in the first week. This may suggest that L-carnitine can be used as an antilipidemic agent for obese patients. A prospective study will investigate obese children in the future.
