Research on Factors Affecting Intention to Re-Use Hospitals for Patients Who Have Used Integrated Nursing and Care Services (간호·간병통합서비스를 이용한 입원환자의 병원재이용의도에 영향을 미치는 요인)

  • The purpose of this research was to investigate the effects of the communication ability, communication type and satisfaction of nursing service on the re-use intention of nursing and care integration services of the hospital. The research subjects were 291 patients who were hospitalized for more than 3 days in the integrated nursing and care service center. Data were analyzed using SPSS 21.0. As a result of the study, the variables that significantly affect the reuse of integrated nursing services in hospitals were satisfaction with nursing services and integrated services. The explanatory power of these variables was 63.4%. Therefore, to increase the intention to re-use integrated nursing and care services of the hospitals, efforts are needed to increase the satisfaction of nursing services. And research on other variables that affect the guarantee is highly recommended.

  • The purpose of this study was to identify a survey on total care services, to verify the perception, performance and importance of nursing practices between nursing students and nurses. The sample of the study were 93 nursing students and 38 nurses, A structured questionnaire used in this study composed of 235 items. The collected data were analyzed using the SPSS WIN 20.0 to calculate frequencies, percentage, mean, standard deviation and t-test. The Major findings of this study were as follows: The amount and responsibility of nursing practices in total service showed that 'very much increase'was the most. There were statistically significant differences in performance of total care service by nutritional care and elimination care. There were statistically significant differences in importance of total care service by respiratory Care.

  • The aim of this study is to develop strategies activating long-term care visiting nursing. The research design was a descriptive survey study, and the data were collected from the visiting nursing center managers, customers, and long-term care insurance staffs. The major results were as follows. To activate the long-term care visiting nursing, first, the basic nursing care for ensuring sustainable health management has to be included. Second, the visiting nursing must be designated as mandatory use in standard guideline for using long-term care services. Third, the insurance pricing of visiting nursing must be based on the cost of visiting nursing. And, last, using a visiting nursing must be possible without a doctor's order sheet, when it is required for the assessment of patient's health status.

The Effect Nursing Organizational Culture on the Quality of Nursing Service: Mediating Effect of Work Engagement (간호사가 지각하는 간호조직문화가 간호서비스의 질에 미치는 영향: 직무열의의 매개효과를 중심으로)

  • This study is a descriptive research study to confirm the mediating effect of job engagement on the relationship between the nursing organizational culture and the quality of nursing service perceived by nurses. The research subjects and data were collected from June 1 to June 7, 2022, and 215 nurses working at hospital-level or higher medical institutions recruited through the online community for nurses. As a result of the study, Relation orientated culture(Z=3.88, p<.001) and Task orientated culture(Z=3.16, p=.001) affected the quality of nursing service, and it was found that job engagement fully mediating effect. Hierarchical orientated culture (Z=2.39, p=.017) affected the quality of nursing service, and it was found that job engagement had a partial mediating effect. Therefore, in order to improve the quality of nursing services, it is necessary and to promote work engagement by forming a culture to establish a trust relationship among members based on the order and procedure for safety in nursing sites that require rapid and accurate response in emergency situations and performing tasks efficiently to achieve organizational goals.

Performance and Requirements of Visiting Nursing Care in Long-Term Care Insurance Using the OMAHA System (노인장기요양보험 방문간호서비스 수행도와 필요도 : 오마하시스템 문제분류체계를 이용하여)

  • Purpose: The aim of this study was to compare between performance and requirements of visiting nursing care in long-term care insurance using the OMAHA system. Methods: The subjects were 72 nurses who had worked in a visiting nursing care center in long-term care insurance. Data were collected from December 5, 2016 to January 31, 2017 using self-recorded questionnaires. The collected data were analyzed using descriptive statistics and paired t-tests. Results: Four dimensions of the OMAHA system showed statistically significant differences between performance and requirements of visiting nursing care in long-term care insurance. The requirements of visiting nursing care were higher than was performance on all 40 items of the OMAHA system. The greatest difference was in environmental domain and then the psychosocial domain. Conclusion: Based on the results, we found that the environmental and psychosocial domains were the largest gap areas. Therefore, with the reality of elderly people living alone and the increase in elderly couples, active intervention connected with the community is needed in residential areas. Further, we suggest that the OMAHA system can be utilized as an integrated conceptual framework for developing and enhancing visiting nursing care in long-term care insurance.

Predictors of Nursing Service Need for Nursing Homes Residents (장기요양시설 노인의 기능상태에 따른 간호서비스 필요도 예측)

  • Purpose: The purposes of this study were to explore the functional status of elderly residents and to analyze time use, and finally identify factors to predict nursing care needs in relation to functional status and health related variables. Methods: In this study a descriptive-correlational design was used. Functional status of participants was obtained through interviews, and nursing care time was examined using a 1 min time-motion study with a standardized instrument developed by Korea Long-Term Care Planning Committee (2005). Results: The mean total functional score was 65 (range 28-125) and mean total nursing care time was 144.15 min per day. There were significant positive relationships between total nursing care time, marital status, back pain, dementia, and vision impairment. Multiple regression analyses showed that a liner combination of number of illnesses, types of primary disease, ADL, IADL, cognitive function, nursing demand, and rehabilitation demand explained 42.8% of variance of total nursing time. ADL (${\beta}$=-.533) was the most significant predictor of nursing service need. Conclusion: Identifying factors that result in variations of service need has implications for adequate nursing service, estimation of optimum nurse to patient ratio, quality of care and patient safety.

ASSESSMENT OF THE NEEDS OF SELF-CARING RETIREMENT VILLAGE RESIDENTS FOR COMMUNITY SERVICES (RETIREMENT VILLAGE에 거주하는 자가 돌봄이 가능한 노인들의 지역사회 서비스에 대한 욕구 사정)

  • 지역 사회 서비스는 노인들이 질적으로 보다나은 삶을 영위할 수 있도록 돕는 기능을 해야 한다. 적절한 서비스의 공급을 위해서는 지역사회 서비스 욕구 사정의 과정이 우선적으로 요구되어지며, 이에 본 연구는 호주 NSW주 Illawarra지역내의 retirement village에 거주하는 자가 돌봄이 가능한 노인들의 지역사회 서비스에 대한 욕구를 평가하는데 그 목적을 두었다. Illawarra지역내의 여덟개 retirement village에 거주하는 98명의 자가 돌봄이 가능한 노인들을 대상으로 그들의 1) functional status; 2) 지역 사회 서비스에 대한 지식정도; 3) 지역 사회 서비스 사용정도; 4) 지역 사회 서비스 만족도; 5) 잠재적인 지역 사회 서비스 욕구 등에 대한 자가 보고형 설문조사가 이루어졌다. 수집된 자료들은 JMP(for Mackitosh) program으로 통계 처리되었고, computed frequencies가 본 연구의 목표 달성을 위해 주로 사용되었다. 연구 결과를 요약하면 아래와 같다. 1. 평균 연령 76세인 대상자들의 80%이상이 그들의 일상 생활 운영에 다른 사람의 도움을 필요로 하지 않으며 건강상태 또한 양호하다고 자각했다. 2. 90%이상의 대상자들이 지역 사회 서비스를 이용하지 않았다. 3. 70%이상의 대상자가 지역 사회 서비스의 존재에 대해 지각하고 있었다. 4. 60%이상의 대상자가 필요시에는 지역 사회 서비스에 관한 정보를 어렵지 않게 얻을 수 있다고 생각했다. 5. 40%의 대상자가 기존의 지역 사회 서비스에 대한 양과 질에 만족을 나타냈고, 그 외의 대상자들 (60%)은 그 지역 사회 서비스에 대한구체적인 지식이 없다고 보고했다. 6. 25%의 대상자들이 여러 지역 사회 서비스에 관한 관심을 보였다. 결론적으로 대다수의 대상자들은 대부분의 지역 사회 서비스들을 필요로 하지 않았고, 이러한 결과는 현재 자가간호가 가능한 retirement village 거주 노인들에게 당연한 것으로 보여진다. 그러나, 자존감이 강한 노인들에게 서비스 욕구는 자가 돌봄이 건강에 대한 포기정도로 왜곡되어 자각될 가능성이 있는 만큼, 본 연구를 통해 보고된 대상자들의 지역 사회 서비스에 대한 관심도는 고려해 볼만한 가치가 있다고 여겨지며, 이에 노인들의 자존감이 서비스 욕구에 미치는 영향에 대한 연구를 제언하는 바이다.

Comparison of satisfaction and need on nursing service perceived by the patients and nurses (입원환자와 간호사의 간호요구도 및 간호서비스 만족도 비교연구)

  • This study evaluates the need of nursing (NN) and satisfaction on nursing service (SNS) in patients and nurses. Questionnaires were completed by 105 patients and 105 nurses in one hospital. The mean score of NN was $3.73{\pm}0.76$ for patients and $3.88{\pm}0.60$ for nurses (t=9.23, p<.001). The top score came from cure territory, while the lowest from physical territory in both patients and nurses. The mean score of SNS was $3.35{\pm}0.74$ for patients and $3.31{\pm}0.44$ for nurses (t=3.88, p<.001). The top score came from cure territory, while the lowest from physical territory in both patients and nurses. When NN and SNS are compared, the score of NN was higher than that of SNS in both patients (t=3.77, p<.001) and nurses (t=9.23, p<.001). As a result, they provided unsatisfactory nursing services, although nurses worked hard to improve them. Nurse administrators should develop strategies and apply them.

The effects of Emotional Labor, Resilience, and Nursing Work Environment on Turnover Intentions of Comprehensive Nursing Care Medical Service Nurses (간호간병통합서비스병동 간호사의 감정노동, 회복탄력성, 간호근무환경이 이직의도에 미치는 영향)

  • The purpose of this study was to examine the effect of emotional labor, resilience, nursing work environment on turnover intentions of comprehensive nursing care medical service ward nurses. The data were collected from 186 subjects using a structured self-report questionnaires and analyzed using SPSS WIN Version 22.0. The mean score of emotional labor, resilience, nursing work environment, and turnover intentions were 3.66, 3.52, 2.62, and 3.82. Turnover intentions had significant correlation with emotional labor, resilience and nursing work environment. The influencing factors on turnover intentions were sex, emotional labor, resilience and nursing work environment of comprehensive nursing care medical service ward nurses. These result indicates that in order to reduce the turnover intentions of comprehensive nursing care medical service ward nurses, it is necessary to develop and apply an intervention programs considering sex, emotional labor, resilience, and nursing work environment.

  • This is a descriptive study to investigate influencing factors of job satisfaction among nurses of comprehensive nursing care ward. The participants were 114 nurses working in comprehensive nursing care ward. Collected data were analyzed by using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficients, and multiple linear regression analysis. The results showed that professional self-concept and teamwork were influencing factors of job satisfaction. Therefore, in order to improve job satisfaction of nurses in comprehensive nursing care ward, education and policy support to improve professional self-concept and teamwork. are needed.
