The effect of horizontal microgap location on the bone loss around 2-piece implants (2-piece 임플란트에서 미세 간극의 수평적 위치 변화에 따른 골 소실 비교 연구)

  • 2-piece 임플란트에서는 초기 치유 기간 동안 0.9-1.6mm의 골 소실이 일어나는데 2-piece 임플란트의 미세 간극과 그에 따른 생물학적 폭경의 형성이 중요한 원인이다. 최근 수직적으로 미세 간극의 위치를 변화시킴으로 골 소실의 양을 줄일 수 있다는 보고가 있다. 이번 실험의 목적은 미세 간극의 수평적 위치 변화에 따른 골 소실의 양을 비교하는데 있다. 하악에서 인접하여 최소 2개의 임플란트를 식립할 수 있는 7 무치악 부위에 총 15개의 Osseotitie XP 4/5를 식립하였다. 이때 임플란트의 상연이 주위 치조골과 일치하게 식립하였고 무작위로 선택하여 한 그룹(W군)에서는 wide diameter healing abutment를 연결하였고 다른 한 그룹(S 군)에서는 standard diameter healing abutment를 연결하였다. 3개월의 치유 기간후 보철 과정을 시작하였으며 이 때 healing abutment와 같은 크기의 prosthetic component를 이용하였다. 임플란트 식립 직후, 3개월의 치유 기간이 지난 보철 직전(Interval I)에, 보철 과정 직후(Interval II)에 각각 치근단 방사선 사진을 찍어 각 단계에서의 골 소실 양을 비교하였다. W 군의 경우 골 소실의 양이 Interval I에서 $1.60{\pm}0.78$, Interval I+II에서 $2.36{\pm}0.29$이었고 S 군에서는 Interval I에서 $1.5810{\pm}0.3030$, Interval I+II에서는 $1.7346{\pm}0.4199$이었다. W군에서는 Interval I와 I+II에서의 골 소실 양이 통계학적으로 유의할 만한 차이를 보였으며, Interval I+II에서의 W 군과 S 군에서의 골 소실 양도 통계학적으로 유의할 만한 차이를 나타내었다. Interval I에서는 두 그룹에서 골 소실의 차이가 없었는데 이는 1 stage surgery시 healing abutment 주위로 mucoperiosteal flap 접합의 어려움 때문으로 생각된다. 한편 Interval II에서는 abutment manipulation 등의 과정이 추가적인 골 소실을 야기한 것으로 생각된다. Interval I+II에서 W 군과 S 군 사이의 골 소실 양 차이는 미세 간극의 수평적 위치 변화의 양과 유사한 결과를 나타내었는데 이로 미루어 미세 간극의 수평적 이동은 임플란트 주위의 골 소실 양에 영향을 미칠 수 있다고 생각된다.

  • Patients with cleft tip and palate present severe maxillary hypoplasia due to scar of lip and palate, often accompanied by compromised mastication, speech abnormalities. Sometimes maxillary hypoplasia persist even though active orthodontic treatment was done. In theses cases, patients born with cleft lip and palate will be potential candidates for maxillary advancement with bone grafting after growth to correct the functional deformities and improve aesthetic facial proportions. But, maxillary advancement using standard surgical approaches has several limitations : increased relapse tendency after maxillary advancement, necessity of additional bone graft and mandibular setback surgery. Distraction osteogenesis is current treatment modality to overcome these limitations, thus has become popular for treatment of maxillary hypoplasia associated cleft lip and palate, craniosyntosis. Especially, rigid external distraction, contrary to internal device, has advantages : better vector control of osteotomized segment, effective traction of the bony segments, the ease of the application and removal the distraction device. This study showed that relatively successful result could be generated by using rigid external distraction osteogenesis(RED) in the case of cleft lip and palate with severe maxillary hypoplasia, 6 years 7 months old.

Effect of Cortical Bone on Acoustic Properties of Trabecular Bone in Bovine Femur In Vitro (생체 외 조건의 소 대퇴골에서 해면질골의 음향특성에 대한 피질골의 효과)

  • The purpose of the present study is to investigate the effect of cortical bone on acoustic properties of trabecular bone, such as speed of sound (SOS) and normalized broadband ultrasound attenuation (nBUA), in bovine femur in vitro. Twelve trabecular bone samples and three cortical bone plates with thicknesses of 1.00, 1.47, and 2.00 mm were extracted from the proximal end of two bovine femurs. The correlations between acoustic properties and trabecular apparent bone density were also examined before and after attaching a cortical bone plate to the trabecular bone samples. SOS increased linearly with increasing thickness of the cortical plate attached to one side of ultrasonic incidence of the trabecular bone samples, whereas nBUA showed a nonlinear dependence on the thickness of the cortical plate. All the SOS (r = 0.95-0.97) and nBUA (r = 0.53-0.73) measurements with and without the cortical bone plate with various thicknesses were found to exhibit high correlations with the trabecular apparent bone density. These results imply that the acoustic properties measured in the femur with lateral cortical layers in vitro can be useful indices for the prediction of trabecular bone mineral density.

Arthroscopic Technique of Bone Defect in Anterior Shoulder Instability (골 결손이 동반된 전방 견관절 불안정성에서 관절경적인 수술 술기)

  • Purpose: The bone defects that are associated with shoulder anterior instability may be the causes of failure of arthroscopic surgery. For the treatment of traumatic shoulder instability, we tried to determine the arthroscopic techniques that can be used for the bone defect of the glenoid and the humeral head. The purpose of this study is to assess the surgical techniques for the arthroscopic reconstruction of the shoulder with anterior instability and bone defects. Materials and Methods: We analyzed the articles that have been recently published on anterior shoulder instability and we assessed the arthroscopic surgical techniques. We compared the articles and the methods of arthroscopic surgical techniques for treating bone defects of the anteroinferior glenoid and the posterolateral humeral head, which were considered as the causes of recurrence of shoulder instability. Results: There are the anteroinferior bone defects of the glenoid and Hill-Sachs lesions in the bone defects that appear in patients with anterior shoulder instability. These bone defects are currently the causes of failure of arthroscopic surgery. Conclusion: Open shoulder surgery may be the treatment of the choice for a shoulder with instability and significant bone defects of the glenoid and the humeral head. But efforts are being made to overcome the weaknesses of open surgery by the use of arthroscopy.

Ultrasonic methods for measuring the cortical bone thickness in bovine tibia in vitro (생체 외 조건의 소 경골에서 초음파를 이용하여 피질골 두께를 측정하기 위한 방법)

  • The cortical bone thickness of the tibia is related to fracture risk and overall bone status. The present study aims to investigate the feasibility of two different ultrasonic methods for measuring the cortical bone thickness in bovine tibia in vitro. In the reflection technique, the tibial cortical thickness was determined from ultrasonic reflections from the periosteum and the endosteum producing specific peaks in the signal envelope. In the axial transmission technique, the tibial cortical thickness was determined from ultrasonic guided wave velocities measured along the axial direction of the tibia. The cortical bone thickness determined by using the reflection technique correlated significantly with that measured by using a caliper, with a Pearson's correlation coefficient of r = 0.97 (p < 0.0001). In contrast, the correlation coefficients for the axial transmission technique were r = 0.92 (p < 0.0001) for the first arriving signal method and r = 0.89 (p < 0.0001) for the slow guided wave method. Clinical feasibility should be demonstrated with an in vivo application to address the question whether the ultrasonic methods presented here could be useful as a screening tool for osteoporosis and potentially could be applied to other skeletal sites such as the femur and the radius.

Availability of Bone Scan in Chest Trauma Patients (흉곽부위 골절에 대한 골스캔의 유용성 고찰)

  • Background: In trauma patients, bony thorax are exposured to the trauma in many cases. With simple x-ray, we can not detect all definitive bony abnormalities, especially in less severe cases. Bone scan is very sensitive diagnostic method in such cases. Materials and methods: We experienced 680 cases and results were as follows. Results: 1. Diagnostic sensitivity was 97.4% and false negative rate was 2.6%. 2. In sensitivity study, the time factor(when we perform bone scan) was the most important thing according to trauma pattern. In rib fracture, sensitive test time was after 1 week. In sternal fracture, sensitive test time was after 1 week, too. In costochondral junction fracture and combined cases, it was after 3 days. Conclusions: We recommend timely using of bone scan as definitive diagnostic method in bony thorax trauma patients.

Analysis of Bone-Remodeling Process Using Quasi-molecular Dynamics (요추 추체의 골 -재형성에 대한 준분자 동력학적 접근 방법)

  • A new method for analyzing the bone-remodeling process using quasi-molecular dynamics was proposed in this study. The effect of pressure due to bone marrow, which could not be considered in previous methods, was also considered in this method. Bone-remodeling response of the 2D vertebral body of lumbar spine to a uniaxial compressive displacement of 1.8564mm. corresponding to approximately 2kN of compressive load, was studied. Converged shape change of the cortical shell and rearrangement of cancellous bone structure matched well with a normal shape of the vertebral body. The calculated responses in the spinal elements also shows closed results compared with experimental results.

  • 앞 턱 부위가 짧고 후방으로 위치해 있을 때 턱의 높이를 증가시키고 전방으로 돌출시키는 축조 이부성형술의 방법으로는, 매식체 삽입술, 골 절단술, 골 절제술 또는 이들을 동시에 시행하는 혼합술등이 행해져 왔다. 그러나 매식체 삽입술에서는 감염 또는 매식체의 유동성 등의 합병증의 발병이 빈발하고 매식체의 밑 부위에 골이 흡수된다든지 연조직의 변화를 예측하기 어렵다든지 하는 복잡한 문제점들이 종종 보고되고 있다. 따라서 매식체를 이용하지 않는 골 절단술이나 골 절제술만을 이용한 다양한 방법의 이부성형술이 많이 시행되고 있다. 하악골이 전후방 관계로 발육부전인 환자에게 양측 하악지 시상분할술을 시행하여 전방 이동시킨 후 앞 턱의 위치를 좀더 개선하기 위하여 $Michel{\'{e}}t$ 테크닉의 변형법을 이용한 이부성형술을 추가로 시행하였다. 이 방법의 이부성형술은 턱의 수직적, 수평적 길이를 증가시키기 위하여 하악결합면 부위에 장부와 장붓구멍 모양을 한 골 절단술을 시행하고 광범위한 연조직경을 보존하여 골편으로의 혈류 차단을 최소화하였으며 나사를 이용한 견고한 고정을 용이하게 하였다. 이 술식은 턱을 수직적으로 증가시킬 때 두 골편 사이의 공간에 골 이식을 시행할 필요가 없으며, 금속판을 이용하지 않아도 정확한 위치에 골편을 고정하기가 용이하고, 또한 고정용 나사가 향후 골형성 부위에 위치하게 된다는 장점을 경험하게 되어 이에 한 증례를 보고하는 바이다.

  • We report a case of bizarre parosteal osteochondromatous proliferation of the right femur in an 18-year-old man. Roentgenograms showed a calcific mass attached to the underlying cortex with a broad base. Histologically, the lesion showed hypercellular cartilaginous tissue with maturation into trabecular bone, which contained spindle cells and lymphocytes in the intertrabecular spaces. Bizarre parosteal osteochondromatous proliferations are a form of heterotopic ossification and should not be mistaken for osteosarcoma or chondrosarcoma.

Radiographic evaluation of marginal bone level alteration around narrow implants placed in narrow alveolar ridge using guided flapless surgery (폭이 좁은 치조골에서 컴퓨터가이드를 이용한 무절개 임플란트 수술로 식립한 narrow implant의 방사선학적 평가)

  • Purpose: The aim of this study was to compare radiographic outcomes around narrow-diameter implants placed using guided flapless surgery at longer than 6 month post-placement. Materials and methods: A total of 12 implants were assessed in 12 patients for the sites where 0.5 - 1.5 mm labial bone was covering the implants, using CBCT. Results: A statistically significant preservation of crestal bone was observed in the narrow-diameter implants. Conclusion: Guided flapless implant surgery may be important in preventing bone loss around the narrow-diameter implants that are placed in narrow alveolar ridges.
