The Effect of Alternative Medicine on Quality-of-Life Outcomes for Dementia Patients: A Systematic Review and Meta-Analysis

  • Objectives: As effective treatments for dementia are lacking in Western medicine, complementary and alternative medicine (CAM) is considered a useful option. While the quality of life (QoL) is a vital outcome for patients with dementia, the QoL of patients receiving CAM for dementia remains ambiguous. This study aimed to determine the effect of CAM on QoL outcomes in dementia patients. Methods: A search was performed using the keywords "dementia," "Alzheimer's," "cognitive impairment," "Chinese," "Korean," "oriental," "herbal," "acupuncture," and "quality of life". All quantitative data were synthesized using R version 4.1.1. Results: Twenty-five randomized controlled trials (RCTs), 16 pre-post trials, and two cohort studies were selected for the systematic review. QoL in Alzheimer's disease (QOL-AD) (n=11, 25.6%) and geriatric QoL in dementia (GQOL-D, n=9, 20.9%) were the most utilized QoL instruments. Significant benefits in QoL were observed after receiving mind, body, combined mind and body, nursing, oriental medicine, and acupuncture therapies. In the meta-analysis, the combined effect was shown to significantly increase QOL-AD compared to before CAM interventions (standardized mean difference, SMD: 0.507; 95% confidence interval (CI), 0.191~0.824; p<0.01). The overall synthesized estimates in the GQOL-D showed a significantly improved QoL (SMD: 0.537, 95% CI: 0.238~0.837 p<0.01; one group; SMD: 1.465, 95% CI: 0.934~1.996, p<0.01). The seven studies assessing the cost-effectiveness of CAM reported uncertain outcomes. Conclusions: This study showed that CAM interventions benefited patients with dementia by improving their QoL. While additional standardized research is required, CAMs are suggested as effective clinical management for patients with dementia. They are also suggested as complementing therapies for these patients.

  • We establish a weighted norm inequality for a class of rough parametric Marcinkiewicz integral operators $\mathcal{M}^{\rho}_{\Omega}$. As an application of this inequality, we obtain weighted $L^p$ inequalities for a class of parametric Marcinkiewicz integral operators $\mathcal{M}^{*,\rho}_{\Omega,\lambda}\;and\;\mathcal{M}^{\rho}_{\Omega,S}$ related to the Littlewood-Paley $g^*_{\lambda}-function$ and the area integral S, respectively.

GLOBAL AXISYMMETRIC SOLUTIONS TO THE 3D NAVIER-STOKES-POISSON-NERNST-PLANCK SYSTEM IN THE EXTERIOR OF A CYLINDER

  • In this paper we prove global existence and uniqueness of axisymmetric strong solutions for the three dimensional electro-hydrodynamic model based on the coupled Navier-Stokes-Poisson-Nernst-Planck system in the exterior of a cylinder. The key ingredient is that we use the axisymmetry of functions to derive the Lp interpolation inequalities, which allows us to establish all kinds of a priori estimates for the velocity field and charged particles via several cancellation laws.

  • This paper is devoted to establishing certain Lp bounds for the generalized parametric Marcinkiewicz integral operators associated to surfaces generated by polynomial compound mappings with rough kernels given by h ∈ ∆γ(ℝ+) and Ω ∈ Wℱβ(Sn-1) for some γ, β ∈ (1, ∞]. As applications, the corresponding results for the generalized parametric Marcinkiewicz integral operators related to the Littlewood-Paley g*λ functions and area integrals are also presented.

  • In this paper, the weighted Lp boundedness of multilinear commutators and multilinear iterated commutators generated by the multilinear singular integral operators with generalized kernels and BMO functions is established, where the weight is multiple weight. Our results are generalizations of the corresponding results for multilinear singular integral operators with standard kernels and Dini kernels under certain conditions.

Multi-component kinetics for the growth of the cyanobacterium Synechocystis sp. PCC6803

  • The growth kinetics of phototrophic microorganisms can be controlled by the light irradiance, the concentration of an inorganic nutrient, or both. A multi-component kinetic model is proposed and tested in novel batch experiments that allow the kinetic parameters for each factor to be estimated independently. For the cyanobacterium Synechocystis sp. PCC6803, the estimated parameters are maximum specific growth rate $({\mu}_{max})=2.8/d$, half-maximum-rate light irradiance $(K_L)=11W/m^2$, half-inhibition-rate light irradiance $(K_{L,I})=39W/m^2$, and half-maximum-rate concentration for inorganic carbon $(K_{S,Ci})=0.5mgC/L$, half-maximum-rate concentration for inorganic nitrogen $(K_{S,Ni})=1.4mgN/L$, and half-maximum-rate concentration for inorganic phosphorus $(K_{S,Pi})=0.06mgP/L$. Compared to other phototrophs having ${\mu}max$ estimates, PCC6803 is a fast-growing r-strategist relying on reaction rate. Its half-maximum-rate and half-inhibition rate values identify the ranges of light irradiance and nutrient concentrations that PCC6803 needs to achieve a high specific growth rate to be a sustainable bioenergy source. To gain the advantages of its high maximum specific growth rate, PCC6803 needs to have moderate light illumination ($7-62W/m^2$ for ${\mu}_{syn}{\geq}1/d$) and relatively high nutrient concentrations: $N_i{\geq}2.3 mgN/L$, $P_i{\geq}0.1mgP/L$, and $C_i{\geq}1.0mgC/L$.

  • Let $T^{\alpha}_{\lambda}$(p, A, B) denote the class of functions $$f(z)=z^p-{\sum\limits^{\infty}_{k=1}}{\mid}a_{p+k}{\mid}z^{p+k}$$ which are regular and p valent in the unit disc U = {z: |z| <1} and satisfying the condition $\left|{\frac{{e^{ia}}\{{\frac{f^{\prime}(z)}{z^{p-1}}-p}\}}{(A-B){\lambda}p{\cos}{\alpha}-Be^{i{\alpha}}\{\frac{f^{\prime}(z)}{z^{p-1}}-p\}}}\right|$<1, $z{\in}U$, where 0<${\lambda}{\leq}1$, $-\frac{\pi}{2}$<${\alpha}$<$\frac{\pi}{2}$, $-1{\leq}A$<$B{\leq}1$, 0<$B{\leq}1$ and $p{\in}N=\{1,2,3,{\cdots}\}$. In this paper, we obtain sharp results concerning coefficient estimates, distortion theorem and radius of convexity for the class $T^{\alpha}_{\lambda}$(p, A, B). It is further shown that the class $T^{\alpha}_{\lambda}$(p, A, B) is closed under "arithmetic mean" and "convex linear combinations". We also obtain class preserving integral operators of the form $F(z)=\frac{p+c}{z^c}{\int^z_0t^{c-1}}f(t)dt$, c>-p, for the class $T^{\alpha}_{\lambda}$(p, A, B). Conversely when $F(z){\in}T^{\alpha}_{\lambda}$(p, A, B), radius of p valence of f(z) has also determined.

건강한 한국인에서 미다졸람 집단약동학 분석: CYP3A 매개 약물상호작용 평가 (Population Pharmacokinetics of Midazolam in Healthy Koreans: Effect of Cytochrome P450 3A-mediated Drug-drug Interaction)

  • Objective: Midazolam is mainly metabolized by cytochrome P450 (CYP) 3A. Inhibition or induction of CYP3A can affect the pharmacological activity of midazolam. The aims of this study were to develop a population pharmacokinetic (PK) model and evaluate the effect of CYP3A-mediated interactions among ketoconazole, rifampicin, and midazolam. Methods: Three-treatment, three-period, crossover study was conducted in 24 healthy male subjects. Each subject received 1 mg midazolam (control), 1 mg midazolam after pretreatment with 400 mg ketoconazole once daily for 4 days (CYP3A inhibition phase), and 2.5 mg midazolam after pretreatment with 600 mg rifampicin once daily for 10 days (CYP3A induction phase). The population PK analysis was performed using a nonlinear mixed effect model ($NONMEM^{(R)}$ 7.2) based on plasma midazolam concentrations. The PK model was developed, and the first-order conditional estimation with interaction was applied for the model run. A three-compartment model with first-order elimination described the PK. The influence of ketoconazole and rifampicin, CYP3A5 genotype, and demographic characteristics on PK parameters was examined. Goodness-of-fit (GOF) diagnostics and visual predictive checks, as well as bootstrap were used to evaluate the adequacy of the model fit and predictions. Results: Twenty-four subjects contributed to 900 midazolam concentrations. The final parameter estimates (% relative standard error, RSE) were as follows; clearance (CL), 31.8 L/h (6.0%); inter-compartmental clearance (Q) 2, 36.4 L/h (9.7%); Q3, 7.37 L/h (12.0%), volume of distribution (V) 1, 70.7 L (3.6%), V2, 32.9 L (8.8%); and V3, 44.4 L (6.7%). The midazolam CL decreased and increased to 32.5 and 199.9% in the inhibition and induction phases, respectively, compared to that in control phase. Conclusion: A PK model for midazolam co-treatment with ketoconazole and rifampicin was developed using data of healthy volunteers, and the subject's CYP3A status influenced the midazolam PK parameters. Therefore, a population PK model with enzyme-mediated drug interactions may be useful for quantitatively predicting PK alterations.

  • In the present paper, we give some characterization of the L2 maximal estimate for the operator Pta,γf(Γ(x, t)) along curve with complex time, which is defined by $$P^t_{a,{\gamma}}f({\Gamma}(x,t))={\displaystyle\smashmargin{2}{\int\nolimits_{\mathbb{R}}}}\;e^{i{\Gamma}(x,t){\xi}}e^{it{\mid}{\xi}{\mid}^a}e^{-t^{\gamma}{\mid}{\xi}{\mid}^a}{\hat{f}}({\xi})d{\xi}$$, where t, γ > 0 and a ≥ 2, curve Γ is a function such that Γ : ℝ×[0, 1] → ℝ, and satisfies Hölder's condition of order σ and bilipschitz conditions. The authors extend the results of the Schrödinger type with complex time of Bailey [1] and Cho, Lee and Vargas [3] to Schrödinger operators along the curves.

The effect of small embankments on wind speeds

  • Full-scale measurements have been made to determine the increase in wind speed over two exposed embankments, one of $23^{\circ}$ slope and 4.7 m in height, the other of $24^{\circ}$ slope and 7.3 m in height. Measurements were made at heights of 5, 10 and 15 m above the upper edge of each embankment and at the same heights approximately 100 m upwind in the lower-level approach fetch. Despite the modest sizes of the embankments, the maximum recorded increase in mean wind speed was 28% and the minimum was 13%; these increase relate to increases in wind loads on structures erected at the top of the embankments of 64% and 28% respectively. The associated increases in gust speeds are estimated at 33% and 18%, which imply increases in gust loading of 77% and 39% respectively. These experimental results are compared with predictions obtained from a computational fluid dynamics (CFD) analysis, using three high Reynolds number eddy-viscosity models and estimates from the UK wind loading code, BS 6399: Part 2. The CFD results are generally in agreement with the experimental data, although near-ground effects on the embankment crest are poorly reproduced.
