사회인지론에 근거한 나트륨 섭취 줄이기 소비자 영양교육 프로그램의 효과 평가 (Evaluation of Consumer Nutrition Education Program to Reduce Sodium Intake Based on Social Cognitive Theory)

  • Objectives: This study was performed to evaluate the consumer education program for reducing sodium intake based on social cognitive theory (SCT) and investigate consumer perceptions of environmental, cognitive and behavioral factors. Methods: Consumers (n=4,439) were recruited nationwide in Korea to participate in a nutrition education program for reducing sodium intake which was targeted on senior housewives (SH), parents (P), and office workers (OW). Questions regarding main factors of SCT were asked both before and after the education program. Results: SH and P recognized external social efforts and information to reduce sodium including nutrition labeling more than OW. The main barriers to practice reducing sodium intake were limited choice of low sodium food and menu, interference with social relationship when dining with others, and limited information, knowledge and skills. SH had lower barriers to practice reducing sodium intake and OW perceived 'preference to soup or stew' and 'preference to Kimchi, salted fish and fermented sauces' as barriers more than other groups at the baseline. Less than 50% of participants knew the relationship between sodium and salt, sodium in nutrition labeling, and recommended sodium intake. In addition, OW had little knowledge for capability to reduce sodium intake and lower self-efficacy to practice compared with SH and P. After education, positive outcome expectations such as lowering blood pressure, prevention of cardiovascular disease and osteoporosis were increased and barriers to practice reducing sodium intake were decreased in all groups (p < 0.05). The knowledge for behavioral capability and self-efficacy to reduce sodium intake were also improved but OW had still lower scores compared with other groups. Conclusions: These results suggested that nutrition education programs could be an effective tool to impact general population by facilitating awareness and increased capability to reduce sodium intake.

직영 및 위탁 사업체 급식소 영양사 직무 명세 특성 분석 (Analysis on job specification characteristics of dietitians in self - operated vs. contracted employee foodservice)

  • The purpose of this study were to investigate the job analysis questionnaire was mailed to 250 dietitians who are members of The Korean dietetic association practice group in self-operated foodservices(hereafter group A)and 250 dietitians who are employed in contracted foodservice companies(hereafter group B). Completed questionnaires were received from 285 dietitians(121 in self-operated, 164 in contracted) ; a response rate of 57%. Statistical data analysis was completed using the SAS/win packages for descriptive analysis, t-test, $x^2$-test, The results of this study can be summarized as follows. 1. The dietetic training or internship experiences were significantly different between the two sample group(p<.001), but the perceptions of needs for training or internship were not different between and they thought one to six month training would be needed for the job. 2. The question about the time needed in order to perform an expert job was answered significantly differently(p<.001) 3. Group A usually took part in obligatory continuing education and short-term training courses, and group B usually took part in obligatory continuing education and on-the-job trainings, respectively. However both group perceived all three types of educations would be needed for performing the job better. 4. As for the physical demands of the job, a certain degree of accuracy and dexterity for physical tasks and equipment operations was needed for the job. Concerning the mental demands, they answered that their job needs considerable creativity and judgement for planning and performing their tasks was needed. 5. As for responsibilities, group B's responsibilities were heavier than group A’s(p<.001) and need for confidentiality was also greater for group B than for group A(p<.001). 6. As for the supervising role differences, group A rarely received supervision from others and they were given directions only in essential situations, whereas group B was usually supervised when planning and performing their job and tasks, showing significant difference in positions between the two sample groups(p<.001). But each group supervised co-workers and employees in their department. 7. With regard to personal contact, both groups frequently contacted people in their department and other departments at the company and sometimes people outside the company. 8. Concerning the work environment, there were some risks and discomforts in the physical environment and there was no significant difference between the two sample groups.

융합적 요소를 고려한 의료서비스 질 인식도와 의료이용의 관련성: 서울시 보건소를 이용하는 노인을 대상으로 (Patients' Perception of Quality of Health Service and Utilization considering convergence factors: A Survey of Elderly Patients of Public Health Centers, Seoul, Republic of Korea)

  • 오늘날 의료가 단지 치료라는 개념을 넘어 서비스라는 인식이 확대되면서 의료서비스 소비 주체인 이용자들이 인식하는 의료서비스 질의 중요성은 날로 강조되고 있으며, 일차의료라고 해서 예외는 아니다. 이 연구는 65세이상 노인들이 보건소에서 제공하는 의료서비스의 질을 어떻게 인식하고 있는지 파악하고, 어떠한 요인들이 의료서비스 이용량과 관련성이 있는지를 탐색하고자 하였다. 이 연구는 서울시 보건소를 이용하는 노인들을 대상으로 연구원이 면접조사를 통하여 자료를 수집하였고, 총 307명의 설문자료를 분석하였다. 의료서비스 질은 융합적 요소를 고려하여 접근하였으며, 분석 결과, 이들의 구성항목 중에서 이용시설 및 환경, 환자중심진료, 의사의 전문적 지식 및 기술수준, 그리고 직원 및 간호사의 정서적 지지에 대하여 노인들이 긍정적으로 인식할수록 보건소의 의료이용이 증가하였다. 따라서 보건소가 쾌적하고 편리한 시설환경을 갖추고, 직원 및 간호사들은 이용자들과 원활한 의사소통을 위해 노력하며, 동시에 의사가 환자중심의 전문적인 의료서비스를 제공한다면 보건소의 의료서비스 질과 가치 향상에 도움이 될 것이다.

재활병원 작업치료사의 근골격계 증상과 직무스트레스와의 관련성 (The relations between the Musculoskeletal Symptoms and Job Stress of Occupational Therapists at Rehabilitation Hospitals)

  • 본 연구는 재활병원 작업치료사들의 근골격계 증상과 직무스트레스와의 관련성을 알아보기 위해 서울 및 경기도에 위치한 재활병원 작업치료사 총 210명을 대상으로 2014년 6월 1일부터 7월 30일 사이에 설문조사를 실시하였다. 연구에 참여한 대상자들의 평균 연령은 $25.4{\pm}3.4$세이었고, 평균 근무기간은 $2.3{\pm}2.1$년이었다. 참여자 중 여자가 151명(71.9%)이었고 정규직이 171명(81.4%)이었다. 대상자들 중 114명(54.3%)이 근골격계 증상을 경험하였고, 주된 손상부위는 손/손가락(21%)과 다리(18.6%)였다. 여성이 직무스트레스의 고위험군이 남성에 비해 더 많았고 직무스트레스에 대한 위험도가 높을수록 근골격계 발생 빈도가 증가하였다. 남성은 '직무요구', '보상부적절'과 '직장문화'에서 스트레스가 높을수록 근골격계 발생 빈도가 증가하였고, 여성은 '관계갈등', '보상부적절' 에 대한 스트레스가 높을수록 근골격계 발생 빈도가 증가하는 것을 확인할 수 있었다(p<0.05).

경관계획에서 제시된 경관 미래상의 현황 분석 (An Analysis on the Vision of Visual Landscape Planning in Korea)

  • 본 연구는 경관계획 내용 중 경관 미래상의 작성 현황을 분석하여 문제점을 도출하고 이에 대한 향후 개선방안을 제안함으로써 향후 지자체 경관향상을 위한 실용적인 경관계획을 위한 기초자료를 제공하는 것을 목적으로 하였다. 이를 위해 본 연구는 경관 미래상을 작성하고 있는 실무자 대상 인터뷰를 통하여 미래상 작성 방법을 조사하였고, 수도권 32개 지자체에 수립된 경관계획을 조사하여 경관 미래상의 작성 형식과 내용을 분석하였다. 그리고 경관 미래상과 유사한 목적과 형태로 작성되고 있는 도시계획 상의 도시 미래상과 지자체 비전을 담고 있는 도시 슬로건을 함께 조사하여 경관 미래상과 작성 경향 및 내용을 비교 분석함으로써 경관 미래상의 작성 방향 및 시사점을 도출하고자 하였다. 연구결과를 요약하면 다음과 같다. 경관 미래상은 주로 경관계획 수립을 위해 기본구상 작성에 앞서 조사한 경관자원현황과 관련 상위 계획의 미래상 및 목표를 참조하는 것으로 나타났다. 이는 경관계획의 세부 내용 간 일치성과 관련 계획 간의 정합성을 높이는 방법이 될 수 있으므로 이러한 방법을 경관계획 수립 지침에 구체적으로 언급되는 것이 바람직하다. 또한 현재 경관 미래상은 도시 슬로건과 유사하게 구호 형태로 작성되고 있으나, 경관 미래상은 홍보의 수단이나 응용되어 활용되지 않으므로 일괄적으로 작성되고 있는 구호형 경관 미래상에 대한 검토가 필요하다. 그리고 경관 미래상과 도시 미래상, 도시 슬로건의 내용적 연관성을 분석한 결과 경관 미래상은 도시 슬로건에 비해 도시 미래상과 내용적 일치성이 높은 것으로 나타났으며, 이는 상위계획과의 정합성을 고려하여 작성하고 있다는 측면에서 긍정적 분석 결과라 판단된다. 다만 향후 이러한 정합성이 유지 및 증진될 수 있도록 경관계획수립지침에서 경관 미래상 작성과 관련한 구체적인 방법을 제시할 필요가 있다.

중량물 취급 보행 시 하지의 역학적 정렬에 따른 생체역학적 변화 분석 (Analysis of Biomechanical Changes According to Mechanical Alignment of the Lower Limbs when Gait with a Material Handling)

  • Objective : Walking with a Material handling is an activity frequently undertaken by agricultural workers in Korea, due to the nature of their work. This study aimed to investigate differences in biomechanical variables according to the mechanical alignment of the lower limbs when walking with a heavy load, and to use this as basic data in the design of various working environments to reduce the skeletomuscular burden on the knee joint. Method : The study subjects comprised of 22 right-foot dominant adult men and women aged between 20 and 23 years. The subjects were divided into a varus or valgus group according to the mechanical alignment of the lower limb by using radiographic findings. The subjects walked without any load and with a load of 10%, 20%, or 30% of their body weight held in front of them. The Kwon3d XP program was used to calculate biomechanical variables. Results : The flexion/extension moment of the knee joint showed a decreasing trend with increased load, irrespective of the mechanical alignment of the lower limb, while the varus group did not show normal compensatory action when supported by one leg at the point of maximum vertical ground reaction force. In addition, in terms of the time taken, subjects showed no difficulties in one-foot support time up to 20%/BW, but at 30%/BW, despite individual differences, there was an increase in single limb. The increased load resulted in a decrease in the ratio of standing phase to ensure physical stability. The valgus group showed a trend of increasing the stability of their center of mass with increasing load, through higher braking power in the early standing phase. Conclusion : In conclusion, although there was no statistical difference in biomechanical variables according to the mechanical alignment of the lower limbs, the varus group showed a more irregular walking pattern with a Material handling than the valgus group, partially proving the association between lower limb alignment and walking with a Material handling.

치과환자 및 보호자가 인지한 치과위생사 이미지 (A Study on the Image of Dental Hygienists in Dental Patients and Caregivers)

  • The purpose of this study was to examine the image of dental hygienists. The subjects in the study were 396 patients and their caregivers who visited 14 dental hospitals and dentists' offices in and around Seoul. After a survey was conducted, the following findings were given: 1. The people investigated gave a mean of 4.0 to the image of dental hygienists. Dental hygienists rated highest in terms of looking neat and tidy(4.33), and were given the lowest marks in arbitrary job performance(3.46). 2. There appeared four different types of dental hygienist images, which made a 59.824 prediction of the image of dental hygienists in general. A typical image(4.11) was most dominant, followed by professional one(4.07), personal one(4.01) and social one(3.73). 3. The relations between the general characteristics of the respondents and their image of dental hygienists could be described as below: 1) By age, those who were in their 60s(4.12) had the best image of dental hygienists, and the teenagers(3.90) had the worst image of them. 2) By gender, the men(4.05) looked at dental hygienists more positively than the women(3.96). They had a significantly different opinion on the social image of dental hygienists according to gender(pE0.05). 3) By marital status, the married people(4.00) had a better image of dental hygienists than the unmarried ones(3.95). 4) As for the influence of the type of dental institutions, they had a more favorable image of dental hygienists in dentists' offices(4.13) than in dental hospitals(3.88). There was a broad significant difference in the way they looked at the professional and personal images of dental hygienists according to the type of dental institutions(pE0.001). 5) As to the impact of educational level, those who received community-college or higher education(4.01) had the best image of dental hygienists, and those who received middle-school or lower education(3.91) had the worst image of them. 6) By occupation, the government workers and students(4.07) had the best image of dental hygienists, and the company employees(3.90) had the worst image of them. They took a significantly different view of the social image of dental hygienists according to occupation(pE0.05). 7) Concerning the impact of the frequency of receiving dental treatment, the respondents who had received it seven times or more(4.16) had the best image of dental hygienists, and those who hadn't(3.79) the worst image of them. There was a wide significant gap according to that frequency. The way they looked at the typical, social and professional images of dental hygienists was quite different significantly according to that frequency as well(pE0.001). 8) As for the influence of whether they were accompanied by caregivers or not, the people who weren't accompanied by caregivers(3.99) had a better image of dental hygienists than those who were(3.97).

과학기술인력의 성별 임금격차에 관한 연구 (Study on Gender Pay Gap of Scienceand Engineering Labor Force)

  • 창조경제를 견인하기 위해 다양성과 창의성이 중시되는 환경에서 여성과학기술인력의 활용은 무엇보다 중요해지고 있으며, 경제적 지위를 측정하는 기준으로 노동의 시장가격인 임금에 대한 분석을 통해 성차별을 인식하고 개선하기 위한 노력이 필요하다. 이에 본 연구에서는 Oaxaca-Ransom 임금분해 방법(1994)을 활용하여 과학기술인력을 대상으로 남녀 임금에 미치는 영향 요인 및 성별 임금 분해를 실시하였다. 연구결과는 다음과 같다. 첫째, 과학기술인력의 특성을 살펴보면 여성의 평균 임금 수준은 남성의 65%에 불과하며, 학력 등 개인적 특성에서 남성이 우세한 것으로 나타났다. 둘째, 임금에 영향을 미치는 변수를 살펴보면 여성과 남성 모두 학력 연령이 높을수록, 근무기간 및 주당근로시간이 많을수록, 관리자 직종에 근무할수록 임금 프리미엄이 존재하는 것으로 나타났다. 셋째, 남성과 여성의 임금분해 결과를 살펴보면, 과학기술인력의 경우 개인적 특성에 의한 생산성 차이가 약 58%, 노동시장에서의 특성에 기인한 성별에 의한 차별은 41%로 나타났다. 비과학기술인력에 비교하였을 시 과학기술인력이 생산성으로 인한 임금격차가 크며 성별에 의한 차별은 덜 한 것으로 나타났다. 그러나, 전공별 및 학력별로 분석한 결과 성에 의한 여성차별은 전체 인력 중 여성의 비율이 낮을수록 노동시장에서 임시근로자 비율이 높을수록 심한 것으로 나타났다. 이를 개선하기 위해서는 여성과학기술인력의 노동시장에서의 여성 불균형 현상이 무엇보다 필요하며, 여성스스로는 개인의 역량을 지속적으로 강화하는 노력이 수반되어야 할 것이다.

사업장(事業場) 근로여자(勤勞女子)의 영양실태(營養實態)에 관(關)한 연구(硏究) (Nutritional Survey for Working Females at a Factory)

  • 여성(女性)의 노동(勞動)과 영양(營養)의 문제가 근로자의 건강과 생산성(生産性)과의 중요성이 있을 뿐만 아니라 다음 세대(世代)의 건강에까지 미치는 영향이 크다는 관점에서 1969년 8월 13일부터 8월 20일까지 1주일간(週日間) 서을 시내 소재 한 가발공장 여자 기숙사 근로자 949명(名)에 대하여 식품및 영양 섭취상태를 조사하였고 아울러 대상자의 신장및 체중을 조사하였다. 그리고 그 중(中) 임의유출법(任意油出法)에 의하여 선정된 44명(名)에 대하여 임상조사 및 생화학적 검사를 실시하였다. 그 결과(結果)를 요약하면 조사대상자는 체위(體位)가 표준치(標準値)에 미달(未達)이었고 16세(歲)${\sim}$20세(歲)가 전인원(全人員)의 82%이상을 차지하고 있는 여자 근로자로서, 노동정도는 중등노동(中等勞動)에 속하였다. 급식(給食)상황에 있어서는 기초식품의 배합이 고르지 못하여 누락된 식품군이 매(每)식단마다 발견할 수 있었다. 완전영양을 위한 균형식사가 여기에 강조되어야 하겠다. 식단(食單)이 너무 단순한 탓으로 저(低)칼로리와 저(低)영양상태를 초래하고 있으며 열량, 단백질의량에 있어서도 권장량보다 부족되며 특히 동물성 단백질량이 부족된다. 그러나 단백질의 질적(質的)문제에 있어서는 비록 그 양은 적으나 75의 단백가(蛋白價)로서 비교적 양질(良質)이다. 여기서 식물성단백질에서 섭취되는 좋은 현상을 발견할수 있다. 그리고 지방질의 섭취량이 평균 1일(日) 1인당(人當) 8.8g으로 매우 적고 무기질 및 비타민의 섭취량도 모두 우리나라 권장량 보다 저치(低値)를 나타내고 있다. 임상조사(臨床調査) 및 생화학적(生化學的) 검사결과(檢査結果)에 있어서도 비타민 결핍증과 빈혈(貧血) 특히 철(鐵)결핍성 빈혈을 초래하고 있는 사실을 발견할 수 있다. 이러한 급식은 장구히 계속 된다면 근로자의 체위저하(體位低下)는 물론 작업능력 및 건강에도 악(惡)영향을 미칠 것으로 본다. 이와 같은 특수환경에 있는 대상에 대해서 앞으로 영양관리와 건강관리에 대한 채선이 시급히 요망된다고 사료되는 바이다.

농촌보건사업지역(農村保健事業地域)의 아동영양(兒童營養) 실태조사(實態調査) (Child Nutrition Survey in Rural Health Project Areas)

  • The main purposes of the nutrition and clinical surveys were to provide baseline information on the nutritional status of pre-school children in rural health demonstration project areas of the Korea Health Development Institute (KHDI) for nutrition guidance services for the MCH target group. The survey covered a total of 222 pre-school children and 135 mothers in Okgu Gun, Cholla Pukto Province from August 10 to August 17, 1979. The survey results are summarized as follows: 1) Family Environment Seventy percent of the households had more than three children, and the mean family sire was 6. Sixty-nine percent of the mothers and 47% of the fathers of the surveyed households were educated at or below the primary school level. The majority, 70% of the mothers, were aged between 20 years and 35 years. 2) Anthropometric Measurements and Hemoglobin Value 4.5% of the children were lower than 80% weight for age of the Korean standard, and 5.4% were lower than 85% arm circumference for age of the Jelliffe's standard resectively, and those were suffering from protein-energy malnutrition. Angular stomatitis were observed on 66.2% of the subjects. Mean hemoglobin value was 11.1g/100m1, and 44.2% of the subjects were categorized as anaemia. 3) Food and Nutrient Intake of animal foods was very low, ranging from 2.9 to 17%. Consumption of eggs was less than 2% of total food intake, and intake of legumes was also very scanty, between 0.8 to 3.7%. These data present evidence of very poor protein intake, quality as well as quantity. Energy intake of children was 60.0 to 64.4% of the recommended allowance, and mean protein intake only met 47.4% of the recommendation. Low intake of vitamins except thiamin were also found. 4) Mother's Nutrition Knowledge Eighty-five percent of the mothers were entirely ignorant regarding the 'five basic food group' which is most important fact on food and nutrition guidance. Mean knowledge score from 14 basic questions about food and nutrition was as low as 5.1. There was a significant positive correlation between mother's educational level and nutrition knowledge score. 5) Family Planning Variable There were significant correlation among maternal, family planning variables, and some of the nutritional and physical measurements. The study revealed that the mother's educational level and nutrition knowledge score are more crucial factors than the family planning variables on effecting food intakes on children. Recommendation : According to the results of the surveys, there were high incident rates of nutritional anaemia and angular stomatitis among pre-school children, and most of rural women had very limited knowledge about food and nutrition. As a main part of the health education activities, the community health workers should provide nutrition education to the village mothers to improve the nutrional status of young children in rural areas. Nutrional promotion at the primary health care level should be mainly based on appropriate nutrition education.
