Cytology of the Uterine Cervico-vaginal Smear of Clear Cell Adenocarcinoma in Uterine Cervix - Report of a Case - (자궁경부 투명세포 샘암종의 자궁경부질 세포학적 소견 - 1예 보고 -)

  • Primary clear cell adenocarcinoma of uterine cervix is rare and cytomorphology in the vaginal smear have not been previously described in Korean literatures. The cytologic characteristics of clear cell adenocarcinoma of the uterine cervix include : malignant cells with abundant, finely vacuolated cytoplasm ; hobnail appearance, and distinctive basement membrane-like hyaline materials within cellular aggregates. A 36-year-old woman presented with vaginal bleeding. Cytologic examination of vaginal smear and histopathologic examination of a radical hysterectomy specimen allowed the diagnosis of hemorrhagic tumor in the uterine cervix as a clear cell adenocarcinoma. Cytologic findings were very characteristic. The tumor cells had abundant, pale, finely vacuolated cytoplasm with indistinct cytoplasmic membrane. The nuclei were round to oval with finely dispersed chromatin. Extracellular basement membrane-like hyaline substance, which stained a light green color in Papanicolaou's preparation, was frequently observed within the cancer cell clusters.

  • We report a case of placental site trophoblastic tumor with cytologic features of cervico-vaginal smear. The smear revealed several loose clusters of atypicai cells in slightly hemorrhagic and inflammatory background. Tumor cells were large in size and polyhedral to round with abundant cytoplasm Nuclei revealed consider-able variation in size, shape, and degree of chromatin clumping. Cytologically, it was difficult to differentiate from squamous cell carcinoma of uterine cervix. Curettage and hysterectomy specimen revealed typical histologic features of placental site trophoblastic tumor composed of intermediate type trophoblasts.

Studies on Vaginal Cytology after Induction of Estrus in the Bitch (인공적(人工的) 발정유기견(發情誘起犬)의 질세포상(膣細胞像)에 관(關)한 연구(硏究))

  • The present study was carried out to investigate the estrous behavior and vaginal smear after induction of estrus with exogenous hormones in the premature, metestrous and anestrous bitches. A total of 21 bitches(Mixed breed: 16, Jindo breed: 5), from 10 months to 5 years of age and weighing 8 to 15 kg was studied the change of vaginal smear and the estrous behavior before and after induction of estrus. The results obtained are as follows: 1. In the treatment A(They were given the $PGF_2{\alpha}$, estrone, estradiol-$17{\beta}$, PMSG and HCG) proestrus commenced in $10.16{\pm}1.44$($Mean{\pm}SEM$) days after treatment, The mean duration of proestrus, and estrus was $7.50{\pm}1.44$ and $13.50{\pm}3.44$ days, respectively. In the treatment B(They were given the PMSG and HCG) proestrus commenced in $5.53{\pm}0.59$ days after treatment. The mean duration of proestrus and estrus was $6.60{\pm}0.71$ and $14.60{\pm}1.14$ days, respectively. 2. All of the 6 bitches in the treatment A showed vulval swelling and vaginal discharge, 14 of the 15 bitches in the treatment B showed vulval swelling and vaginal discharge. However, all of the treatment A and B showed male acceptance. 3. The main change of vaginal smear in proestrus and estrus after induction of estrus was a increase in the proportion of anuclear and superficial cells associated with a decrease in small intermediate and parabasal cells. By the estrous behavior and vaginal smear the estrus was induced in all the premature, metestrous and anestrous bitches.

Vaginal Endoscopy for Detecting Estrual Cycles and developing Artificial Insemination Technique in the Bitch (개에서 내시경을 이용한 발정주기 검사 및 인공수정 기법의 실험조사)

  • The vaginal endoscopy was performed in two Labrador Retrievers from onset of vaginal bleeding until metestrus accompanying with vaginal smear and blood progesterone concentration to investigate estral changes, and the intrauterine infusion of radiopaque fluid using vaginal endoscope (rigid 25-degree endoscope, Karl Storz) was detected by radiogrphy to determine if theis technique is feasible for future artificial insemination in the dogs. During the proestrus vaginal mucous membrane folds were edematous and a large amount of clear red discharge was present. In the beginning of estrus, the mucous membrane folds was present and the amount of the red discharge was decreased. In the estrus there were obbvious shrinkage, angulation of the membrane folds and decreasing of the discharge. In metestrus the vaginal folds started to round out and the surface appeared moist and sticky. The endoscope used for endoscopic monitoring of the vaginal mucosa was advanced until the external os of the cervix could be visualized. A plastic catheter, 8Fr guage was manupulated into the cervical os and then advanced through into the uterus. The radiopaque fluid was injected into the uterus through the catheter and radiographs taken to detect the site of insemination using the endoscopic technique revealed the exact infusion of the fluid in the uterus, consequentlyto prove that this technique is usable for more successful artificial insemintion tn the bitch.

  • A female mixed miniature pinscher dog, about 5 years old, 4 kg, was referred to veterinary teaching hospital of Chungnam National University with chief complaint of excessive hemorrhagic vaginal discharge for a month, partial alopecia and hyperpigmentation. There are no specific change with CBC, blood chemistry and X-ray findings but we figured out high estrogen concentration with hormone analysis and detected a majority of the superficial epithelial cells with pyknotic nuclei by cytologic examination of vaginal smear preparation and multiple ovarian cysts on both ovaries by ultrasonography. Based on the clinical signs, hormone analysis, vaginal smear preparation and ultrasonography, the authors diagnosed this case as hyperestrogenism with ovarian cysts.

  • Curschmann's spirals morphologically similar to those seen in sputum were found in cervico-vaginal smears of six patients ranged from 28 to 40 years of age, during 18 months from January. 1985 to June 1986. The prevalence was 1 in 2147 smears in that period. All of them had gynecologic disorders without systemic effect such as chronic cervictis in three, leiomyoma in one, pelvic inflammatory disorder in one, and primary infertility in one. The Curshmann's spirals in the smear showed varying degrees of maturation from wavy incipient ones to highly celled mature ones, admixed with thick mucinous back-ground, suggesting of their production in the uterine cervix itself. Also the recent history of undergone cryocautery, electrocautery or parturition suggest its production in the endocervical gland due to mechanical obstruction and/or change in biochemical composition of mucus.

Pap Smear Screening Results for Turkish Pregnant Women

  • Objective: Cervix cancer is one of the most common gynecological cancer types that cause cancer deaths among women. This study was planned based on a descriptive method in order to evaluate the results of PAP smear screening during pregnancy for prevention of cancer. Materials and Methods: The research involved 110 pregnant women registered at the Obstetrics and Gynecology Polyclinic of Bagcilar Training and Research Hospital and 86 non-pregnant women of the same ages as a control group. As criteria for acceptance were conditions such as not being in coitus within the last 48 hours, not using vaginal ovule, and not performing vaginal lavage. A survey consisting of 33 questions was conducted and the results were processed using Bethesda. Results: The average ages were $27.1{\pm}4.70$ for the pregnant women and $28.8{\pm}4.24$ for the control group. 60.7% of cases had previously heard of a PAP smear test, 49% were aware of why PAP smear tests were conducted, 26.4% of pregnant participants and 27.3% of non-pregnant participants had previously undergone a smear test. In this study, smear results of all cases were 95.4% sufficient. 18.2% of pregnant cases had an infection, 54.5% had reactive cellular change, and 0.9% had atypical squamous cells of undetermined significance (ASC-US). 16.3% of non-pregnant cases had an infection, 58.1% had reactive cellular change, 3.5% had atypical squamous cells of undetermined significance (ASC-US), and 1.2% had low-grade squamous intraepithelial lesions (LGSIL). Conclusion: PAP smear test is a good opportunity to identify pre-invasive lesions in early phases of pregnancy.

Changes of Estradiol, Progesterone and Vaginal Epithelial Cells of a Pseudopregnant Captive Coyote (Canis latrans) during Breeding Season

  • Most captive canids and felids at Zoos in advanced countries have been examined enough to apply artificial reproductive techniques to them. We investigated reproductive hormones and vaginal epithelial cells of a 6-year-old, female coyote, hoping these data could eventually be extended to artificial insemination with frozen-thawed conspecific semen at Seoul Zoo. As a relative of pet dogs, coyote exhibited a similar appearance with only minor differences. In vaginal smear, an increase in the number of superficial cells suggests that the bitch has reached a state close to estrus. A sudden decrease of estradiol and increase of progesterone is considered as a preovulatory event. Vaginal epithelial cells and hormones might be useful for determining the optimal time of artificial insemination in coyotes' breeding.

  • Endometrial papillary serous carcinoma (EPSC) is a distinct variant of endometrial adenocarcinoma that histologically resembles ovarian serous papillary adenocarcinoma and has an aggressive clinical course. Usually, the tumor is diagnosed at the advanced stage. The tumor has well confused with metastatic ovarian tumor of identical histology. Dignosis of EPSC should be considered when the cervico-vaginal smear reveals nomerous papillary clusters of tumor cells with macronucleoli and psammoma bodies. Recently, we have experienced two cases of EPSC diagnosed on cervico-vaginal smears, which revealed characteristic cytologic features including numerous papillary clusters of tumor cells with macronucleoli. The cytologic diagnoses were confirmed on histologic sections.

The Clinical Trial of Quicure Vaginal Irrigation on Cenvico-Vaginitis (Quicure Vaginal Irrigation에 의한 자궁경부염 및 질염치료에 관한 임상적 연구(예보))

  • This study was intended to investigate the effectiveness of Quicure vaginal irrigaticn for treatment of cervico-vaginitis. 53 patients who visited to severance hospital during July to December 1982 were included in this study. Antiseptics used for irrigation were 1% KMnO4, 10% Betadine and Albright solution, each solution was prepared for warm ($37^{\circ}C$) and cold ($15^{\circ}C$) form respectively, Acridine Orange immunofluorescent stainiry of cervico-vaginal smear was used for assessment of effectivenese. The results were as followed. 1. On unitial examination 62%s of patients had trichomonas infection, 19% had moniliasis infection, 19% had non specific infection. 2. Smear findings of patients~who had trichomonas infection or moniliasis infection, treated with cold 1% KMnO4 or Albright solution, became>negative except case of on each group. But the findings of all patients treated with warm solution became negative. Changes of findings of patients who had non specific infection showed similar result. This results indicated that warm solution would be better than cold solution. 3. The findings of all patients treated with Betadine solution became negative except, patient who had non specific infection, showed that Betadine solution would be better than 1% KMnO4 or Albright salution. 4. Patients' feeling of using warm saline was more satisfactory than that of cold solution
