도시지역 여성의 요실금, 하부요로증상과 일상 및 성생활과의 관계 (A Study on the Urinary Incontinence, Lower Urinary Tract Symptoms, Lifestyle and Sexual Matters of Women in an Urban Area)

  • Purpose: The purpose of this study was to identify the frequency of urinary incontinence and lower urinary symptoms, to identify lifestyle and sexual matters for UI of adult women in an urban area. Method: The sample consisted of 364 educated women from a women's health education program in a public health center. For data collection, the Bristol Female Lower Urinary Tract Symptoms Questionnaire developed by Jackson, et. al.(1996) and the demographic questionnaire were used. For data analysis, SAS 8.2 program was used. Result: The results were as follows: 1. the prevalence rate of UI was 73.63%. 45.88% of them were stress incontinence and 27.75% of them were urge incontinence. 2. Remarkable differences were found between the type of UI and LUTS in the case of urgency($x^2=42.0585$, P<0.001), unexplained incontinence($x^2=59.0585$, P<0.001), noctural incontinence($x^2=18.8080$, P<0.001) and catetherization($x^2=10.7207$, P= 0.004), burning sense($x^2=27.7400$, P<0.001). 3. Remarkable differences were found between the type of UI and lifestyle matters in the cases as follows fluid intake restriction($x^2=31.0532$, P=0.008), interference in physical activity($x^2=36.7481$, P=0.001), interference in relation with others($x^2=22.2729$, P=0.034). 4. Remarkable differences were found between the type of UI and sexual matters with difficulty of sexual intercourse($x^2=16.1898$, P=0.002), and urine leakage during sexual intercourse($x^2=17.9752$, P=0.001). Conclusion: Based on the results, developing and carrying out an adequate UI care and education program is needed.

오령산(五苓散)으로 호전된 여성 하부 요로감염증 환자 치험 4례 (Four Cases Report of the Female Lower Urinary Tract Infection with Oryeong-san)

  • Purpose: This paper aims to report the effects of Oryeong-san on lower urinary tract infection. Methods: Four women were admitted to Dunsan Oriental Hospital with chief complaint that was not supposed to be a symptom of urinary tract infection. During the admission, urine test showed pyuria with white blood cell and their chief complain also accompanied. We prescribed Oryeong-san three times a day, and then took follow-up 4 days later. Results: After treatment, symptoms of lower urinary tract infection were decreased and the results of urine test improved also. Conclusions: These cases indicate that Oryeong-san is effective in treatment of the lower urinary tract infection.

Sacral Acupuncture for Lower Urinary Tract Symptoms: A Systematic Review of Randomized Controlled Trials

  • Lower urinary tract symptoms (LUTS) associated with storage, voiding, and post-micturition reduce quality of life and cause mental health problems. In traditional medicine, Baliao points have been empirically used to treat urinary system diseases. In this review, randomized controlled trials (RCTs) using sacral acupuncture on Baliao points with sham treatment, other remedies, or other acupoints were retrieved from 8 electronic databases up to June 2021. Sixteen RCTs met the inclusion criteria. The quality of the included studies was assessed using a risk-of-bias (ROB) tool. Most of the evaluation indicators used in the included RCTs showed that sacral acupuncture had a significant therapeutic effect compared with the sham control intervention groups, and other remedies. However, all studies using acupoints (other than the Baliao points) as a control intervention had a "high" ROB and only reported secondary processed information, making it difficult to evaluate the efficacy of sacral acupuncture treatment for LUTS. No serious adverse effects were reported for sacral acupuncture, and only a low number of minor side effects were observed. These results suggest that sacral acupuncture could be considered as an alternative to existing treatments, with the added benefit of low cost. Large-scale, long-term RCTs are required in the future.

Differentiation between Viral and Urinary Tract Infections Using the Modified Rochester Criteria In Febrile Infants Younger than three Months

  • Purpose: The symptoms and signs of urinary tract infection (UTI) in early infancy are non-specific. Prompt diagnosis of UTI is important, as untreated UTI results in renal damage. Especially, febrile UTI in young infants coexist with other serious bacterial infections. The purpose this study was to propose modified Rochester criteria to differentiate viral infection from urinary tract infection. Methods: We carried out a retrospective investigation of 168 infants less than three months old with a tympanic temperature $>38^{\circ}C$ who were admitted to Chung-Ang University Hospital between 2011 and 2014. We compared the symptoms, physical examination results, and laboratory data between viral infection and UTI groups. A modified Rochester criterion was composed of statistically significant factors. Results: A total of 76 and 92 infants with UTI and a viral infection, respectively, were included. Statistically significant differences in gender, previous admission history, neutrophil ratio, and urine WBC count were found between the two study groups. Using a cut off value of 3 points, the sensitivity and specificity of the modified Rochester criteria were 71.28% and 78.57%, respectively. Conclusion: The modified Rochester criteria may give an outline for identifying young infants with UTI.

소아에서 Shigella dysenteri에 의한 신우신염 1례 (An Unusual Case of Acute Pyelonephritis Caused by Shigella dysenteri in a Child)

  • Shigella infection usually produces gastrointestinal symptoms but rarely causes urinary tract infection. A 7-year-old girl was admitted for fever, chills, right flank pain, and dysuria. She had no vomiting or diarrhea. There was mild tenderness in her right lower abdomen, and right CVA tenderness was also noted. Acute pyelonephritis was diagnosed by abdominal CT. She showed improvement with intravenous administration of antibiotics. The first urine culture grew $1{\times}10^5$ CFU/mL Shigella dysenteri. Although urinary tract infections due to Shigella species are extremely rare, Shigella species should be considered as a possible cause of pediatric urinary tract infection. We report the first case of urinary tract infection caused by S. dysenteri, which presented as acute pyelonephritis without gastrointestinal symptoms in a child.

전립선암 노인 환자의 하부요로증상 (Lower Urinary Tract Symptoms of Elderly Prostate Cancer Patients)

  • Purpose: The purpose of this study was to investigate the degree of LUTS (Lower Urinary Tract Symptoms) and factors affecting LUTS in patients with prostate cancer. Methods: Fifty-four patients with prostate cancer participated and they completed a structured questionnaire at a hospital in Seoul. The LUTS level was measured by using the International prostate symptom score (1992) and the data were analyzed by Wilcoxon rank sum test and Kurskal-Wallis test. Results: Most subjects obtained a higher than moderate LUTS score. The participants who had a lower perceived health status and did not drink alcohol were more likely to had a lower LUTS score than other patients who had higher perceived health status and drink alcohol. In addition, eye diseases were significantly associated with the LUTS score. In LUTS, all the patients received higher IPSS-QOL (International prostate symptom score quality of life) and frequency, nocturia. Conclusion: For proper nursing care related to LUTS, nurses should focus on the factors identified when performing nursing assessments and should develop nursing intervention programs for symptoms management.

노년기 요실금 여성의 하부요로증상, 신체활동, 우울이 수면의 질에 미치는 영향 (Influence of Lower Urinary Tract Symptoms, Physical Activity, and Depression on the Quality of Sleep in Elderly Women with Urinary Incontinence)

  • Purpose: This study was to identify factors influencing the quality of sleep in elderly women with urinary incontinence and examine the relationship among the factors. Methods: The study investigated 113 elderly women with urinary incontinence who were subject to the visiting care program at health centers in D-gu, D-si, South Korea. The collected data were analyzed using IBM SPSS version 19.0 along with ${\chi}^2-test$, t-test, ANOVA, Pearson's correlation, and multiple regression. Results: The total Pittsburgh Sleep Quality Index (PSQI) of the participants was found to be $11.41{\pm}4.34$. As a result of the correlation analysis, PSQI was found to have significant correlations with Female Lower Urinary Tract Symptoms (BFLUTS) (r= .32, p< .001). The Korean version of Physical Activity Scale for Elderly (K-PASE) was (r= -.34, p< .001) and the Geriatric Depression Scale Short Form (GDS-SF) was (r= .23, p= .012). Multiple regression analysis found sleeping pill use (${\beta}=-.35$, p< .001) had the strongest influence on PSQI, followed by physical activity (${\beta}=-.20$, p< .001), depression (${\beta}=.20$, p< .001), and lower urinary tract symptoms (${\beta}=.19$, p< .001). The total model explanatory power was 28.3% (F= 12.07, p< .001). Conclusion: The study findings are expected to be useful for enhanced understanding of the quality of sleep in elderly women with urinary incontinence.

  • In this paper, We used the microphone unlike existing urodynamic inspection which diagnoses the LUTS(lower urinary tract symptoms) and proposed sound measurement the method which the urination happens and apply at a blockade diagnosis. To analyze the blockade degree, We made the experiment set to modeling the Lower Urinry System of the human body and made the microphone of the stethoscope form for the measurement of the urinary sound which the urination happens. We used PC for the analysis of the urinary sound to be measured and implemented the analysis program to use the MATLAB. Hereafter, An analysis result usage, The Practical use will be possible as one of new inspection methods's.

중년남성 근로자의 수면의 질에 영향을 미치는 요인 (Factors Influencing Sleep Quality among Middle-aged Male Workers)

  • Purpose: The purpose of this study was to identify quality of sleep and its influencing factor among middle-aged male workers. Methods: The subjects of this study were 411 middle-aged male workers living in Seoul and Gyeonggi-do. The data were based from self-reported using structured questionnaires asking about low urinary tract symptoms, circadian rhythm, quality of life and sleep quality including general characteristics. The data were collected from September 11 to October 31, 2013 and analyzed by t-test, one-way ANOVA, Pearson correlation coefficient and hierarchical regression. Results: Mean score of the sleep quality was $5.03{\pm}2.57$ (range: 0~21) and reported as bad sleep quality in 55.7%. Low urinary tract symptoms (${\beta}$=.30, p<.001), circadian rhythm (${\beta}$=-.17, p<.001), quality of life (${\beta}$=-.14, p<.001) were shown as significant association of sleep quality. Conclusion: Strategies to improve sleep quality in middle-aged male workers are necessary and nursing intervention should be developed to improve education program for prevention and management of low urinary tract symptoms. It is also necessary to legalize the institutional devices to upgrade work environment and to place regulations on overtime at work places in order for these workers to obtain sufficient sleep time for recovery of biological rhythms and improving sleep quality.

제2형 당뇨병 환자의 하부요로증상 관련요인 (Factors Associated with Lower Urinary Tract Symptoms in Patients with Type 2 Diabetes Mellitus)

  • Purpose: This study was performed to identify lower urinary tract symptoms (LUTS), and to evaluate the factors affecting LUTS in patients with type 2 diabetes mellitus (T2DM). Methods: The cross sectional study was used with a structured questionnaire to collect data through interviews with 181 T2DM patients and their clinical data from a university hospital diabetes clinic from October 2010 to April 2012. LUTS were measured using the International Prostate Symptom Score (IPSS), depression using the Center for Epidemiologic Studies Depression Scale (CES-D), and glycosylated hemoglobin (HbA1c) from the clinical data. Results: Of all patients with T2DM, the mean IPSS of LUTS was $9.34{\pm}6.86$. Concerning the reported severity of LUTS, 53.6% of the subjects were in the moderate and severe group. In each symptom score of LUTS (range 0-5), nocturia was the highest 2.04, weak stream 1.62, and frequency 1.45. LUTS was significantly predicted by HbA1c and depression, and 14.3% of the variance in LUTS was explained. Conclusion: HbA1c and depression were found to be very important factors associated with LUTS in T2DM patients.
