The Clinical Significance of Cathepsin D and p53 Expression in Locally Advanced Rectal Cancer (국소진행된 직장암에서 Cathepsin D와 p53 발현의 임상적 의의)

  • Purpose: Cathepsin D(CD) is a lysosomal acid proteinase that is related to malignant progression, invasion, and a poor prognosis in several tumors. The aim of this study was to evaluate the prognostic clinical significance of CD and p53 expression in pretreatment biopsy specimens from patients with locally advanced rectal cancer who were treated with preoperative chemoradiation. Materials and Methods: Eighty-nine patients with locally advanced rectal cancer(cT3/T4 or N+) were included in this study. Preoperative chemoradiation consisted of a dose of 50.4 Gy of pelvic radiation and two concurrent cycles of administration of 5-fluorouracil and leucovorin. Surgery was performed six weeks after chemoradiation. CD and p53 expression in pretreatment formalin-fixed paraffin-embedded tumor biopsy specimens were assessed by immunohistochemical staining using a CD and p53 monoclonal antibodies. The threshold value for a positive stain in tumor tissue and stromal cells was 1+ intensity in 10% of the tumors or stromal cells, respectively. Results: Positive CD expression was found in 57(64%) of the tumors and 32(35%) of the stromal cell specimens. There was no association with CD expression of the tumor or stromal cells and patient characteristics. There was a correlation between tumor CD expression with stromal cell CD expression(p=0.01). Overexpression of p53 was not a significant prognostic factor. The 5-year overall survival(OS) and disease-free survival(DFS) rates were not different between tumor CD-negative and positive patient biopsy samples(69% vs. 65%, 60% vs. 61%, respectively). The 5-year OS rates in the tumor-negative/stromal cell-negative, tumor-negative/stromal cell-positive, tumor-positive/stromal cell-negative and tumor-positive/stromal cell-positive biopsy samples were 75%, 28%, 62%, and 73%, respectively. Stromal cell staining only without positive tumor staining demonstrated the worst overall survival prognosis for patients(p=0.013). Conclusion: Overexpression of p53 in rectal biopy tissue was not associated with prognostic significance. In the pretreatment biopsy specimens, an exclusive increase in CD expression in stromal cells without tumor expression was related to poor overall survival in patients with locally advanced rectal cancer treated with preoperative chemoradiation.

  • Phyllodes tumor of the breast, especially malignant, is a rare tumor which has characteristic cytologic features. We recently experienced a case of fine needle aspiration cytology of malignant phyllodes tumor of the breast metastatic to the lung. The aspirate showed cellular smear with biphasic population of epithelial and stromal cells. Many individual large cells with round hyperchromatic nuclei were found in the necrotic background and stromal cell atypia was also present. Chondrosarcomatous fragments were also seen in cytologic specimens.

Malignant Solitary Fibrous Tumor of the Pleura A case report (흉막에 발생한 악성 고립성 섬유성 종양 1예 보고)

  • Solitary fibrous tumor of the pleura has been classified as a type of mesothelioma. This tumor have been recently described and distinguished from the mesothelioma, immunohistochemically and elect r om ic ro scop ical 1 y Thls 65 years female was admitted with right chest pain, coughing and dysnea. Chest X-ray and C-T scan showed a huge mass in the right thorax. Right thoracotomy was done and a 12$\times$12$\times$6cm(400gm) sized mass was excised, and the tumor had metastasized to the diaphragm, parietal pleura and lung parenchyma. Microscopically, the tumor Is composed of oval-round and plump spindle cells with diffuse pattern. There are occasional mitoses (311 OHPFS) and invasion to lung parenchyma. The immunohistochemical and electromicroscopical findings are consistent with malignant solitary fibrous tumor of the pleura. We experienced a case of pleural malignant solitary fibrous tumor and report this case with the review of literature.

  • Warthin's tumor is the second most common tumor of the parotid gland after pleomorphic adenoma. It is well known to occur as bilateral and multiple patterns. The incidence of extraparotid Warthin's tumor (EPWT) is about 2.7% to 12%, peri-parotid and upper cervical area are the most common sites. Warthin's tumor with synchronous intraparotid and extraparotid area is extremely rare, only a few cases have been reported. We report a-71-year old man with unilateral swelling of the parotid area and upper neck, pathologically confirmed Warthin's tumor with literature review.

Quantitative Analysis of Thallium-201 Scintigraphy in Bone Tumor (골종양에서 탈륨 스캔의 정량적 분석)

  • Purpose: This study was designed to know the ability of thallium-201 scintigraphy to discriminate malignant bone tumor from benign by analysing the quantitative thallium uptake ratio. Materials and Methods: We took thallium-201 scintigraphy prospectively with other imaging studies in 82 bone tumor suspecting patients. The results of scintigraphy were read qualitatively and calculated quantitatively, and retention indexes were estimated. For the statistical analysis the patients were divided as four group; high grade malignant bone tumor, benign bone tumor, giant cell tumor and low grade malignant bone tumor. Results: The mean thallium uptake ratio was 4.14 in early phase and 2.26 in delayed phase in high grade malignant bone tumor group, 1.16 and 1.09 in benign bone tumor, 3.15 and 1.94 in giant cell tumor, and 1.41 and 1.31 in low grade malignant bone tumor. Retention indexes were 0.62, 0.97, 0.66, 0.93 in same order. The thallium uptake ratio and retention indexes were statistically correlated in high grade malignant bone tumor and benign bone tumor group(p<0.001). Conclusion: Thallium-201 scintigraphy proved as useful imaging study to discriminate malignant bone tumor from benign, but had exception in giant cell tumor and low grade malignant bone tumors.

Expression of Tumor Markers and its Clinical Impacts in Resectable Gastric Cancer (절제 가능한 위암에서 종양표지자의 발현과 임상적 의의)

  • Purpose: This study was performed to evaluate the effectiveness of the serum tumor markers CEA, CA 19-9, and CA 72-4 in monitoring the recurrence of gastric cancer and in its preoperative assessment. Materials and Methods: Two hundred fifty-five patients who underwent potentially curative surgery during period from January 1995 to December 2000 at the Department of Surgery were assessed. Serum samples were obtained preoperatively, 2 weeks after the surgery, and at 6-month intervals. The cut-off levels were established as 5 ng/ml for CEA, 36 U/ml for CA 19-9, and 4 U/ml for CA 72-4. The tumor stage was described according to the 5th edition of the Union Internationale Contra la Cancer (UICC) TNM classification in 1997. Results: The preoperative positivities were $10.5\%$ for CEA, $9.7\%$ for CA 19-9, and $12.4\%$ for CA 72-4. The serum levels of the three tumor markers decreased after curative surgery. The preoperative serum levels of the three tumor markers were significantly related to the depth of invasion, the tumor size, lymph-node metastasis, the pathologic stage, and recurrence, except that CEA was not associated with tumor size. The marker sensitivities in recurrent cases were $43.3\%$ for CEA, $\%41.8$ for CA 19-9, and $50.0\%$ for CA 72-4, and the marker specificities were $85.1\%$ for CEA, $96.8\%$ for CA 19-9, and $87.8\%$ for CA 72-4. Conclusion: The preoperative serum levels of CEA, CA 19-9, and CA 72-4 are not useful for the initial diagnosis of gastric cancer because of their low positivity. However, we should consider their relationship with depth of invasion, lymph-node metastasis, tumor size, pathologic stage, and recurrence. Also, the follow-up levels of the three markers have a statistical relationship with recurrence of gastric cancer even though their sensitivities are low.

Treatment of Malignant Peripheral Nerve Sheath Tumor Using Surgery and Metronomic Chemotherapy in a Dog (개에서 발생한 악성 말초 신경집 종양의 외과적 절제와 메트로놈 화학요법을 이용한 치료 증례)

  • A 6-year-old female Shih-tzu dog was referred with left elbow joint mass associated with weight bearing lameness. Ultrasonography demonstrated an encapsulated hyperechoic mass at the left elbow joint. Radiography was performed on elbow joint, chest, and abdomen, but there was no evidence of metastasis. Clinicopathologic examination revealed the existence of neoplastic cells with anisocytosis, pleomorphism and increased nuclear-cytoplasmic ratio. The left forelimb was amputated for cure. Histopathological examination diagnosed the mass as a malignant peripheral nerve sheath tumor. During the 4 weeks follow-up, the patient showed full remission and adopted to walk with remaining three legs. Thereafter, the tumor recurred after 5 months of first surgery at the operation site and other two cutaneuos regions. Cytology test revealed the mesenchymal originated tumor cells with malignancy. These tumors were surgically removed and histopathological examination of the resected tissue revealed the recurrence of primary tumor and metastasis. Metronomic therapy with cyclophophamide (10 mg/$m^2$, PO, sid) and piroxicam (0.3 mg/kg, PO, sid) had been adopted for 5 months. At 26 months of follow up after the first surgery, the dog alive with satisfactory quality of life. Aggressive surgical resection with metronomic chemotherapy should be the most effective treatment for malignant peripheral nerve sheath tumor.

Pre-operative Evaluation of Consistency in Intra-axial Brain Tumor with Diffusion-weighted Images (DWI) and Conventional MR Images (확산강조영상과 고식적 자기공명영상을 이용한 수술 전 축내 뇌종양의 경도 평가)

  • Purpose : To retrospectively evaluate the usefulness of diffusion-weighted images, ADC maps and conventional MR images for determination of brain tumor consistency. Materials and Methods : Twenty-three patients with brain tumor underwent MR examinations with T1, T2 and diffusion-weighted images. Regions of interest (ROIs) were drawn in the tumors, and the measured signal intensities (SI) were normalized with the contralateral side. We evaluated the correlation between SI ratios from various images and tumor consistency assessed at surgery. In three patients with both cystic and solid components, each component was evaluated independently. Qualitatively observed SIs were also correlated with tumor consistency. Results : Statistical analysis revealed significant correlation between tumor consistency and ADC ratio (r = -0.586, p = 0.002), SI ratios on T2-weighted images (r = -0.497, p = 0.010), and observed SIs on T2-weighted images (r = -0.461, p = 0.018). The relative ratio of ADC value correlated with tumor consistency most strongly. Conclusion : The measured ratio of ADC, SI ratio and observed SI grade on T2-weighted images can provide valuable information about the consistency of brain tumor.

Classification of Microsatellite Alterations Detected in Endoscopic Biopsy Specimens of Gastric Cancers (단순반복염기서열의 변이 형태에 따른 위암 내시경 조직의 유전자형 분류)

  • Purpose: Individual gastric cancers demonstrate complicated genetic alterations. The PCR-based analysis of polymorphic microsatellite sequences on cancer-related chromosomes has been used to detect chromosomal loss and microsatellite instability. For the purpose of preoperative usage, we analyzed the correspondance rate of the microsatellite genotype between endoscopic biopsy and surgical specimens. Materials and Methods: Seventy-three pairs of biopsy and surgical specimens were examined for loss of heterozygosity and microsatellite instability by using 40 microsatellite markers on eight chromosomes. Microsatellite alterations in tumor DNAs were classified into a high-risk group (baselinelevel loss of heterozygosity: 1 chromosomal loss in diffuse type and high-level loss of heterozygosity: 4 or more chromosomal losses) and a low-risk group (microsatellite instability and low-level loss of heterozygosity: 2 or 3 chromosomal losses in diffuse type or $1\∼3$ chromosomal losses in intestinal type) based on the extent of chromosomal loss and microsatellite instability. Results: The chromosomal losses of the biopsy and the surgical specimens were found to be different in 21 of the 73 cases, 19 cases of which were categorized into a genotype group of similar extent. In 100 surgical specimens, the high-risk genotype group showed a high incidence of nodal involvement (19 of 23 cases: $\leq$5 cm; 23 of 24 cases: >5 cm) irrespective of tumor size while the incidence of nodal involvement for the low-risk genotype group depended on tumor size (5 of 26 cases: $\leq$5 cm; 18 of 27 cases: >5 cm). Extraserosal invasion was more frequent in large-sized tumor in both the high-risk genotype group ($\leq$5 cm: 12 of 23 cases; >5 cm: 23 of 24 cases) and the low-risk genotype group ($\leq$5 cm: 7 of 26 cases; >5 cm: 16 of 27 cases). The preoperative prediction of tumor invasion and nodal involvement based on tumor size and genotype corresponded closely to the pathologic tumor stage (ROC area >0.7). Conclusion: An endoscopic biopsy specimen of gastric cancer can be used to make a preoperative genetic diagnosis that accurately reflect the genotype of the corresponding surgical specimen.

Ultrastructures of Canine Transmissible Venereal Turner Cells at Stages of Maturation and Regression (개 전이성 생식후 종양 세포의 성숙기 및 퇴축기에 따른 미세구조)

  • Naturally occurring canine transmissible venereal tumors of genital organs in mature and regressive stages from 6 dogs were examined by transmission electron microscope. The tumor cells at the stage of maturation were comprised of large round and ovoid cells with prominent nuclei and nucleoli, a few spindle-shaped cells, and irregularly shaped cells. The mature round cells were characterized by the presence of a central ovoid to irregularly round nucleus with a large eccentric nucleolus, vesicular endoplasmic reticulum, round to oval swollen mitochondria with few cristae, Golgi's apparatus, and plasma membranes with numerous microvilli. As the tumor degenerated, the tumor cells were increased in the number of spindle-shaped, fibroblast-like and irregularly shaped cells, collagen bundles, and mainly lymphocytes, in contrast to those of the stage of maturation. Regressing tumor cells were characterized by the swelling and vacuolation of mitochondria and endoplasmic reticulum, membrane-bound granules, lamellar complex, tubular structures, and dense bundles of collagen. It was suggested that transformation might occur in the course of tumor growth causing morphological change from the round to the fibroblast-like cells, and that there was the evidence of cell-mediated tumor cell lysis by lymphocyte infiltration.
