A Bibliographic Studies on the Bufonis Venenum for Clinical Treatment: Important to Toxicity and Processing (섬수의 임상적 활용을 위한 문헌적 고찰 -독성 및 수치를 중심으로-)

  • Objectives : The purpose of this study is to find out the clinical possibility of Bufonis Venenum used without toxicity and side effect. Methods : We investigated the pharmacological effects, toxicity and processing of Bufonis Venenum through the literatures and studies. Results : Bufonis Venenum is made by parotid gland of dermato gland of Bufo bufo gargarizns or B. melanostictus Schneider, and it is dried for using. The medical ation of Bufonis Venenum are cardiotonic, respiration stimulation, anticancer, topical anesthesia. The toxic symptoms of Bufonis Venenum are relative with digestive, circulatory, nervous system similared with digitalis toxicity. It is important to take 0.015-0.03g by mouth, external use about 1-4% 0.5-3ml and 2-8ml injections by 20ml mix to 5% dextrose fluid. Bufonis Venenum is processed to prevent toxicity and evaluate efficacy by alcohol and milk. There are 68 prescription consisted by Bufonis Venenum in KTKP(Korean Traditional Knowledge Portal). They usually use for antiabcess, anticancer with Moschus moschiferus(麝香), Cinnabar(朱砂). Conclusions : The results from above literary studies show that internal, external medicine and Aqua-acupuncture of Bufonis Venenum could be clinically used to sedative, antiinflammatory, anticancer and topical ataralgesia without toxity through optimum dose and processing.

Effects of amygdalin on the functional recovery and c-Fos expression in the ventrolateral periaqueductal gray region after sciatic crushed nerve injury in rats

  • Peripheral nerve injuries are a commonly encountered clinical problem and often result in a chronic pain and severe functional deficits. The expression of c-Fos is sometimes used as a marker of increased neuronal activity. We have prepared the aqueous extract of amygdalin from Armeniacae semen for pain control. In the present study, we investigated the effects of amygdalin on the recovery rate of the locomotor function and on the expression of c-Fos in the ventrolateral periaqueductal gray (vlPAG) region following sciatic crushed nerve injury in rats. Walking track analysis for the evaluation of functional recovery and immunohistochemistry for the c-Fos expression were used in this study. In the present results, characteristic gait change with dropping of the sciatic function index (SFI) was observed and c-Fos expression in the vlPAG was suppressed following sciatic crushed nerve injury in rats. Amygdalin enhanced SFI value and restored c-Fos expression in the vlPAG to the control value. The present our study indicated that amygdalin activates neurons in the vlPAG, and it facilitates functional recovery following peripheral nerve injury.

Studies on the Mutagenicity and Hepatotoxicity of Antaeum (안태음의 변이원성 및 간독성에 관한 연구)

  • Antaeum(ATE) has been used as a prescription for threatened abortion, associated with pregnancy in traditional medicine. Because gravida could be administered ATE for a long period, its administration might cause a harmful effect on fetus and gravida during the pregnancy. This study aimed to determine whether exposure to ATE caused mutagenicity or hepatotoxicity during the pregnant period. For mutagenicity test of ATE, Salmonella typhimurium and Bacillus subtilis were used as indications for DNA damage. In the Ames test, Samonella typhimurium TA98 and TA100 were used for mutagenicity testing, and the number of histidine revertants was measured. In Rec-assay, Bacillus subtilis H $17(Rec^+)$ and $M-45(Rec^-)$ strains were used to clarify the DNA damage property. In the SOS umu test, Salmonella typhimurium TA15335 containing plasmid pSK1002 was used as a tester strain, and we monitored the levels of umu operon expression by measuring the ${\beta}-galactosidase$ activity. From the tested results, ATE did not show DNA damage and mutagenicity. On the other hand, hepatotoxicity of ATE to female ICR mice was monitored by the measurements of s-GOT, s-GPT and LDH activities after oral feeding for 15 days. ATE did not show significant change of s-GOT, s-GPT and LDH activities in mice sera.

A Literature Review about Labor theory and practice - Focused on Bulsusan(佛手散) - (임산(臨産) 의론(醫論)과 의방(醫方)에 대한 문헌고찰(文獻考察) - 불수산(佛手散)을 중심으로 -)

  • Object : From the ancient times, the importance of childbirth has been well recognized by Korean Traditional Medicine. Numeral methods have been developed to ease the labor process and keep the mother and child healthy from conception to labor. Bulsusan(佛手散) is one of the main remedies to healthy labor in KTM, both widely known and applied as well. Method : This paper examines the labor theory and practice of KTM focused on Bulsusan which is composed of Angelica gigas Nakai(當歸) and Cnidium officinale MAKINO(川芎). Result : 1. From the ancient times until the Q$\bar{i}$ng period, much attention was placed to the handling of the placenta, as it was conceived as bearing much relation to the health of the mother and her fate, and thus included in the labor process. 2. There was a recognition of the 'birth pulse[離經脈]', an intense change in the pulse that presents itself prior to somatic signs of labor. 3. There were numerous prescriptions that were administered beforehand to ease the process. They are mostly constituted with medicinals that nurture Gi(氣) and stimulate its flow, which in turn makes the fetus firm and reduces the volume, easing the labor process. 4. The medical practice of labor-induction was called 'Choesaeng(催生)'. The prescriptions which functioned as such were mostly constituted with blood medicinals such as Angelica gigas Nakai and Cnidium officinale MAKINO, those which nurture both Gi(氣) and blood, and medicinals that physically lubricate the labor pathway such as honey, oil and Talcum(滑石). Conclusion : Bulsusan can be used in most problems concerning pregnancy and labor, and cases of emergency blood loss due to injury. The term 'bulsu(佛手)' infers to the medical ability of the great doctor who takes care of major blood-loss situations resulting from discharge of dead fetus, cesarean delivery, etc. The prescription name takes after this meaning, as it deals with similar conditions in its effect.

Screening of Insulin-like Substances from Traditional Herbs of Diabetes Prescription in Donguibogam (동의보감 당뇨 처방에 사용되는 한약재에서 인슐린성 물질(Insulin-like substances)의 탐색)

  • In order to search for insulin-like substances from the constituted herbs of Sogal prescriptions, we selected 19 traditional herbs, based on a review of the Donguibogam. The effects of the hot-water extract from the selected herbs on the proliferation and the differentiation of 3T3-L1 fibroblasts were tested. The various water-extracts from Pinellia ternata, Magnolia obovata, Rheum palmatum, Acanthopanax sessiliflorun, Atractylodes japonica and Strychnos ignatii inhibited the proliferation of 3T3-L1 fibroblasts, did not influence entirely in differentiation of 3T3-L1 fibroblasts. Treatment of 3T3-L1 fibroblasts with the extract from Ephedra sinica, Trichosanthes kirilowii, Scrophularia buergeriana and Sophora flavescens significantly increased the differentiation of the cells. In conclusion, these may contain such compounds that play a role of insulin-like action.

Studies on Gene Expression of Yukmijihwang-tang using High-throughput Gene Expression Analysis Techniques (대규모 유전자 분석 기법을 이용한 육미지황원의 유전자 발현 연구)

  • Yukmijihwang-tang(YM) is a noted herbal prescription in Chinese and Korean traditional medicines, and it has been known to reinforce the vital essence and has been widely used for a variety of disease such as stroke, osteoporosis, anti-tumor, and hypothyrodism. Regarding its traditional use, YM has been known to reinforce the Yin (vital essence) of liver and kidney. Also it has been known to reinforce nutrition and biological function in brain. Recently, studies suggested that YM increase antioxidant activities and exert the protective effect against oxidant-induced liver cell injury. We investigated the high-throughput gene expression analysis on the Yukmijihwang-tang administrated in SD rats. Microarray data were validated on a limited number of genes by semiquantitative RT-PCR and Western blot analyses. The recent availability of microarrays provides an attractive strategy for elaborating an unbiased molecular profile of large number of genes in drug discovery This experimental approach offers the potential to identify molecules or cellular pathways not previously associated with herbal medicine. Total RNA from normal control brain and Yukmijihwang-tang administrated brain were hybridized to microarrays containing 10,000 rat genes. The 52 genes were found to be up-regulated(twice or more) excluding EST gene. The nine genes were found to be down-regulated(twice or more) excluding EST gene. Gene array technology was used to identify for the first time many genes expression pathway analysis that arecell cycle pathway, apoptosis pathway, electron transport chain pathway, cytoplasmic ribosomal protein pathway, fatty acid degradation pathway, and TGF-beta signaling pathway. These differentially expressed genes pathway analysis have not previously been iavestigated in the context of herbal medicine efficacy and represent novel factors for further study of the mechanism of herbal medicine efficacy.

Introduction of evidence-based practical medicine through safety classification for herbal medicine(1) (한약의 안전성 등급화를 통한 근거중심실용의학적 연구(1) - Aristolochic acid 함유 한약재를 중심으로 -)

  • Objectives: Evidence-based medicine(EBM) advocates the use of up-to-date "best" scientific evidence from health care research as the basis for making medical decisions. EBM also has been applied to traditional Korean medicine(TKM), especially in the field of safety. Recently, the standard prescription for TKM by Korea Institute of Oriental Medicine was published based on toxic index from various toxicity tests. However, there are some limitations when the results from the study based on EBM are applied in clinics. To overcome these imitations, the term "evidence-based practical medicine" was developed and defined as clinically applicable results from the study based on EBM. And safety classification for TKM was suggested as an example of evidence-based practical medicine. Methods: For safety classification for TKM, the data for $LD_{50}$(50% lethal dose), which was transformed to theoretical $LD_1$(1% lethal dose), was analyzed as one of tools for EMB study and divided by maximum dose used in clinics. Results and Conclusions: As a result, human equivalent dose(HED)-based MOS(margin of safety) for korean traditional medicine was calculated and used for safety classification with 5 categories. These categories would be helpful for oriental medicine clinicians to decide the increase and decrease of dosage according to various factors such as patient's sensitivity, potential toxicity of herbal medicines, clinician's experience for better cure. Thus, this safety classification provides some evidences enough that evidence-based practical medicine should be not the same with EBM and defined differently from EBM.

Cardiorespiratory Responses of Qi-training: A Pilot Study

  • Six volunteers (mean $age=25.7{\pm}1.7$, $height=173{\pm}1.9$ and $weight=63.4{\pm}2.3{\;}kg$) participated in a graded exercise test and one hour of basic form of ChunDoSunBup (CDSB) Qi-training to investigate the cardiorespiratory responses and exercise intensity of Qi-training, a Korean traditional psychosomatic training. In the maximal exercise, the trainee showed $96.2{\pm}8.89{\;}l/min$ in ventilation (VE), $46.0{\pm}4.4$ in breath frequency (BF), $1.31{\pm}0.05$ in respiratory exchange ratio (RER), $180.7{\pm}3.0$ in heart rate (HR), and $2.6{\pm}1.1{\;}l/min$ or $40.7{\pm}2.3{\;}ml/kg/ml$ in oxygen consumption $(VO_2)$. Qi-training induced significant changes in BF, RER, HR, and $VO_2$. The exercise intensity of Qi-training were 42.3%, 46.9% and 38.7% of $HR_{max}$ during the sound exercise, slow motion (haeng-gong) and meditation respectively and the average was 46.2% of $HR_{max}$. We conclude that Qi-training is an aerobic exercise of a light (mild) intensity exercise, and it leads to decrease the metabolic rate in the trainee by breathing efficiently and relaxing them. In addition, Qi-training may affect cardiorespiratory function of BF, RER, HR and $VO_{2max}$ in trainees.

The Radioprotective Effect of Sam-Ryung-Baek-Chul-San (San-Ling-Bai-Shu-San) as a Prescriptions of Traditional Chinese Medicine in Irradiated Mice (방사선 피폭 마우스에서 삼령백출산 및 구성단미의 방호효과)

  • We performed this study to determine the radioprotective effect of Sam Ryung Baek Chul San (San Ling Bai Shu San), as a prescription of traditional Oriental medicine, and its major ingredients. Jejunal crypt survival, endogenous spleen colony formation, and apoptosis in jejunal crypt cells were investigated in irradiated mice with high and low dose of rays. Sam Ryung Baek Chul San administration before irradiation increased the formation of endogenous spleen colony(p<0.05) and reduced the frequency of radiation induced apoptosis(p<0.05). In the experiment on the effect of ingredients of Sam Ryung Baek Chul San, the result indicated that the extracts of Panax ginseng, Poria cocos and Coix lacryma jabi might have the major radioprotective effects. Although the mechanisms of these inhibitory effects remain to be elucidated, these results indicated that Sam Ryung Baek Chul San might be a useful radioprotector, especially since it is a relatively nontoxic natural product.

  • Objectives : Jakyakgamcho decoction is a traditional prescription known to be an effective pain control medication and muscle relaxant. For more localized treatment outcomes achieved in a shorter period of time, Jakyakgamcho decoction was reprocessed into a form of pharmacopuncture. An analysis of Jakyakgamcho decoction pharmacopuncture showed that there was a significant loss of paeoniflorin(Jakyak's index component). This study was designed to investigate ways to minimize this loss. Methods : After making changes to the processing methods of Jakyakgamcho decoction pharmacopuncture, we measured the quantity of paeoniflorin using high performance liquid chromatography(HPLC) for a before-and-after analysis Results : Paeoniflorin loss was observed 15 minutes after sterilization with $Na_2HPO_4$ at $121^{\circ}C$ Conclusions : It was found that paeoniflorin loss did not occur when pH was not controlled for during processing.
