  • The advent of osseointegration and advances in biomaterials and techniques have contributed to increased application of dental implants in the restoration of maxillary partial edentulous patients. Often, in these patients, soft and hard tissue defects result from a variety of causes, such as infection, trauma, and tooth loss. These create an anatomically less favorable foundation for ideal implant placement. Reconstruction of the atrophic maxillary alveolar bone through a variety of regenerative surgical procedures has become predictable; it may be necessary prior to implant placement or simultaneously at the time of implant surgery to provide a restoration with a good long-term prognosis. Regenerative procedures are used for horizontal and vertical ridge augmentation. Many different techniques exist for effective bone augmentation. The approach is largely dependent on the extent of the defect and specific procedures to be performed for the implant reconstruction. It is most appropriate to use an evidenced-based approach when a treatment plan is being developed for bone augmentation cases. The cases presented in this article clinically demonstrate the efficacy of using a autogenous block graft, guided bone regeneration, ridge split, immediated implant placement technique on the atrophic maxillary area.

Full mouth rehabilitation with extremely worn dentition (심한 치아 마모 환자의 전악보철 수복 증례)

  • Pathologic attrition leads to pathologic damage on occlusal plane, functional disorders, occlusal disharmony, esthetic problems, pulpal lesion, temporomandibular joint (TMJ) disorder. In this case, treatment plan should be considered for possibility of vertical dimension loss, occlusal pattern, esthetics, phonetics, amount of vertical dimension increase. This case report was a 71-year-old man who had severely worn dentition. Full mouth rehabilitation was carried out with vertical dimension increase due to limited space for prosthesis. After evaluation of provisional restorations for patient's compliance, final restorations were fabricated and routine clinical assessments were made. This case presents that a satisfactory clinical result was achieved by restoring the worn dentition.

  • The facial cleft and duplicated maxilla are lire congenital anomaly. After Rushton and Walker had reported a unilateral facial cleft with excess tooth and bone formation in 1937, few authors described similar cases. The etiology of this anomaly is not well understood, but considered embryologically as a neurocristopathy. A neurocristopathy is defined as a condition arising from aberrations in early migration, growth and differentiation of neural crest cells. This aberrations result in facial malformation such as facial clefts and loss or duplication of facial structures. We experienced a male newborn baby with bilateral facial cleft and duplicated maxilla. The cleft was surgically corrected when he was 5 months old. The function and appearance of lip are improved. Duplicated maxilla will be surgically removed. We report this case with review of literatures.

A RADIOGRAPHIC STUDY OF DIFFERENTIAL DIAGNOSIS BETWEEN ODONTOGENIC KERATOCYST AND UNICYSTIC AMELOBLASTOMA (치성각화낭과 단방성 법랑모세포종의 감별에 관한 방사선학적 연구)

  • The purpose of this study was to obtain some informations for the radiographic differential diagnosis between odontogenic keratocyst and unicystic ameloblastoma in the mandible. The author compared and analysed the clinico-radiographic features of 48 cases of odontogenic keratocyst and 32 cases of unicystic ameloblastoma. The obtained results were as follows : 1. Odontogenic keratocyst and unicystic ameloblastoma occurred the most frequently in the 2nd and 3rd decades, and both lesions occurred with slight predilection in males. The most frequent lesional site was molar area in odontogenic keratocyst(50.0%) and mandibular angle and ramus area in unicystic amelobla-stoma(71.9%). 2. Cortical thinning and expansion were observed with similar occurrences in odontogenic keratocyst(77.l%) and in unicystic ameloblastoma(72.9%). 3. Typical undulating lesional border was observed more frequently in odontogenic keratocyst(79.2%) than in unicystic ameloblastoma(46.9%). 4. Well-defined lesional outline occurred more frequently in odontogenic keratocyst(97.9%) than in unicystic ameloblastoma(53.1%). 5. Root resorption of adjacent teeth occurred more frequently in unicystic ameloblastoma(65.2%) than in odontogenic keratocyst(18.8%) respectively, but loss of lamina dura was frequently observed in odontogenic keratocyst(79.2%). And tooth displacement occurred more frequently in odontogenic keratocyst(50.0%) than in unicystic ameloblastoma(17.4%). 6. Displacement of mandibular canal occurred more frequently in odontogenic keratocyst(75.0%) than in unicystic ameloblastoma(61.5%). 7. Inhomogeneous lesional radiolucency occurred more frequently in unicystic ameloblastoma(53.l%) than in odontogenic keratocyst(39.6%).

  • The author observed a case of osteogenic sarcoma in a 11-year-old female with complaint of painful swelling on face in right side. The observed results were as follows: 1. Large hematoma was obsered, and patient complanited painfull swelling on c/c site. 2. Predisposing factor of osteogenic sarcoma was not clear, but patient had history of extraction before patient visiting infirmary of our dental collage. 3. Serologic findings were not specific, and serum alkaline level was normal. 4. Radiographic findings were as follows: ① Diffuse faint radiopacity in the lesion ② Bony destruction and increased radiopaciy in right antrum ③ Displacement of multiple teeth on involved area(i. e ;#12, 15. 55, 16, 17, 18), ④, Increased periodontal space in singel tooth(#13) ⑤Destruction of bony crypt on involved teeth(#13, 14, 15, 17, 18) ⑥ Loss of lamina dura of three teeth in involved area(#11, 12, 16) 5. Computed tomographic findings were as follows: ① Large calcific and hetergenous component mass in the Rt. maxillary sinus, and this mass extending to Rt. maxilla, alveolar bone, ethmoid sinus. ②, Soft tissue bulging into Rt. side nasal cavity and oral cavity. ③ Bone destruction of maxillary sinus wall and Rt. alveolar bone.

A Study on the Applications of Herba Menthae Mainly Blended Prescription in Dongeuibogam (동의보감(東醫寶鑑) 중(中) 박하(薄荷)가 주약(主藥)으로 배오(配伍)된 방제(方劑)의 활용(活用)에 대한 고찰(考察))

  • Objectives : This report describes 48 prescriptions related to the use of Herba Menthae main blended from Dongeuibogam. The following conclusions were drawn through investigations on the prescriptions that use Herba Menthae as a key component. Prescriptions that Herba Menthae was taken as a monarch drug are utilized for 15 therapeutic purposes, for example, wind stroke, throat disease, nose disease, head disease and tooth disease. In particular, 12.5% of prescriptions appear in the chapter of each wind stroke, throat disease, nose disease. Methods : Prescriptions that utilize Herba Menthae as the main ingredient are used in the treatment of urticaria, hemoptysis-hematemesis, loss of consciousness due to wind stroke and they are also used for treating 41 different types of disease. Herba Menthae is used for pathogenic factors such as wind and heat. Results : The dosage of Herba Menthae is 0.15pun(about 0.06g) to 4don(about 14g), however 5pun(about 1.88g) has been taken the most for clinical application. Conclusions : Gamgiltang, Piryongbanggamgiltang and Hoichunyanggyeoksan are the most useful base prescriptions which use the Herba Menthae as the main ingredient.

Radiographic Differential Diagnosis between the Fibrous Dysplasia and the Ossifying Fibroma (섬유성이형성증과 골화섬유종의 방사선학적 감별진단)

  • The author observed and compared the radiographic features of 49 cases of the fibrous dysplasia and 14 cases of the ossifying fibroma in the osteoblastic or mature stage radiologically and histopathologically. The obtained results were as follows: 1. Fibrous dysplasia occurred most frequently in the 2nd decade, but ossifying fibroma in the 3rd and 4th decades, and both lesions occurred with slight predilection in females. 2. In most cases, chief complaints were painless facial swelling. And 61.1% of fibrous dysplasia occurred in the maxilla, 92.9% of ossifying fibroma in the mandible, and most of these lesions occurred in the premolar-molar region. 3. In the mandibular lesions, ossifying fibroma was shown more oval and round shape. but fibrous dysplasia was shown fusiform shape. 4. Fibrous dysplasia was shown homogeneously distributed. complete radiopaque shadow at 63%, and ossifying fibroma was shown concentric. mixed appearance of radiolucent and radiopaque shadow at 92.9%. 5. Fibrous dysplasia was entirely shown poorly outlined and blended to normal surrounding bone, but ossifying fibroma was shown well-defined border. 6. Cortical thinning and expansion were observed in these lesions. but degree of cortical expansion was more severe in ossifying fibroma than fibrous dysplasia. 7. Loss of lamina dura. tooth displacement. and displacement of mandibular canal were observed in both lesions. but root resorption was observed in ossifying fibroma only.

Residual bone height measured by panoramic radiography in older edentulous Korean patients

  • PURPOSE. The aim of this study was to obtain statistical data on the residual bone height at different natural tooth positions by panoramic radiography in edentulous Korean patients aged 60-90 years. MATERIALS AND METHODS. The study included the diagnostic panoramic radiographs of 180 randomly selected edentulous patients without systemic diseases affecting bone. The radiographic selection criteria included absence of obvious facial asymmetry, clearly visible anatomic structures, and no surgical and fracture history. The panoramic radiographs of 79 patients met these criteria and were used in the analysis. The same researcher processed all the radiographs by using a standardized method. The height of the residual bone was measured at 18 predetermined sites (7 in the maxilla and 11 in the mandible) on digitized and printed radiographs by using a Digimatic caliper, triangle, and ruler. Gender- and age-related differences were statistically analyzed by using the t-test and rank-sum test (${\alpha}=0.05$). RESULTS. The maxillary residual bone height did not show significant gender-related differences, but male patients had significantly higher residual bone in the mandible(P<.05). No significant height differences at the measured sites were noted among the 60s, 70s, and 80s age groups. CONCLUSION. Dentists should pay greater attention to older female edentulous patients because they are more prone to rapid residual bone resorption. Residual bone resorption may not be affected by age.

The effect of supportive periodontal therapy after periodontal treatment : A 1-year follow up. (치주치료 후 유지치주치료의 효과에 관한 평가 : 1년 관찰)

  • Purpose : Preservation of the periodontal health of the treated patient requires supportive periodontal therapy for the elimination of periodontal disease. After Phase I therapy is completed, patients are placed on a schedule of periodic recall visits for maintenance care to prevent recurrence of the disease. The amount of tooth loss would be the most relevant criterion in an evaluation of the effect of periodontal treatment, but this would require studies with extremely long follow-up periods. Thus the most commonly used outcome criteria in clinical research have been clinical attachment level change, reduction of pocket depth and bleeding frequency. The purpose of this case study is to identify the effect of supportive periodontal therapy after periodontal flap surgery. Materials & Methods : Following routine hygienic phase of treatment, patients with chronic periodontitis received surgical periodontal treatment. Bleeding on probing, pocket depth and clinical attachment level were measured at baseline, pre-operation and 1 year follow up. All procedures were performed by one operator. Results : One year a total of 28 patients (58sites) to recheck remained, when conducted maintenance program after periodontal flap surgery was observed reduction of bleeding frequency, pocket depth and improvement of clinical attachment level. Conclusion : The results from this study indicate that supportive periodontal therapy after periodontal flap surgery is effective for reduction of bleeding frequency, pocket depth and gain of clinical attachment level.

Comprehensive orthodontic treatment of a young girl with an odontogenic keratocyst and impacted teeth in the mandible

  • Odontogenic keratocysts (OKCs) are one of the most aggressive cysts in the oral and maxillofacial area because of their high recurrence rate and infiltrative behavior. In growing patients with OKCs, a radical treatment approach might cause numerous complications, including the disturbance of jaw growth and loss of the involved tooth. This case report describes successful comprehensive orthodontic treatment combined with marsupialization of the cyst in a young girl who exhibited an OKC with impacted teeth. The 10-year-old girl presented with an OKC extending from the mandibular symphysis through the left mandibular body, with ectopic impaction of the mandibular left canine and first premolar, as well as congenitally missing bilateral mandibular second premolars. Interestingly, spontaneous improvement of the positions of the ectopic impacted teeth, along with a reduction in the size of the cyst, occurred during marsupialization. The sequential use of removable and fixed appliances enabled orthodontic traction of the impacted teeth. The treatment outcome was stable at 2.5 years after the end of the treatment. We speculate that comprehensive orthodontic treatment combined with marsupialization can be an effective treatment strategy for patients with OKCs, especially when they are encountered in young, growing patients with impacted teeth.
