Restoration of severly damaged teeth after endodontic treatment had been an interest to many dentists, and it is a fact that there have been lots of studies about it. In these days, although we have used Para-Post, pins, threaded steel post, cast gold post and core, and so on, as a method of restoration frequently, it has been in controversy with the influence of them on the teeth and surrounding periodontal tissue. In this study, we assume that the crown of the upper incisor have severly damaged, so, after the root canal therapy, 4 types of restoration had been carried out; 1) coronal-radicular amalgam restoration, 2) after setting up the Para-Post, restore with composite resin core only, 3) after setting up the Para-Post; restore with amalgam core, then cover with the PPM crown 4) after setting up the Para-Post, restore with composite core, then cover with the PPM crown. After restoration, in order to observe the concentration of stress at internal portion of the teeth and the sourrounding periodontal tissue, developing a 2-dimensional finite element model of labiopalatal section, then loading forces from 3 direction - direction of 45 degrees from lingual side near the incisal edge, horizontal direction from labial height of contour, vertical direction at the incisal edge-were applied. The analyzed results were as follows: 1. Stress of the normal central incisor was concentrated on the dentin aroundpulp chamber, labiocervical portion of a tooth and root apex, but with the alveolar bone, in the case of load from the direction of 45 degrees from lingual side near the incisal edge showed remarkable concentration of stress: 2. Coronal-radicular amalgam technique -showed less concentration of stress on the root and surrounding periodontal tissue than the restoration with the Para-Post. 3. The von Mises equivalent stress on the Para-Post showed maximum value at root-core junction rather than both ends and model with PPM restoration with amalgam core showed the least concentration of stress. Only the force from horizontal direction showed large shear stress on internal portion of the root, root apex and alveolar bone. 4. PPM crown with composite core rarely showed the concentration of stress on root and periodontal tissue. 5. As for alveolar bone, remarkable shear stress was concentrated on labial and palatal side by horizontal load.
어린이는 8-10세 경에 외상을 자주 받으며 구강 내에서는 상악 절치부가 호발하는 부위이다. 외상으로 인하여 상악 절치가 상실되면 어린이에게 합병증을 야기할 수 있으므로 가능한 한 상악 절치를 유지해야 하지만 조기 상실되는 경우가 있다. 상악절치의 조기 상실로 인한 합병증으로는 심미적 문제나 치조골의 수직적 높이와 수평적 너비의 감소, 인접치의 경사, 악궁 장경 감소 등이 있다. 그 중에서 치조골이 소실되면 보철적 수복 시에 기능성과 안정성, 심미적인 면에 영향을 준다. 이 증례는 외상으로 인하여 상악 중절치가 조기 상실된 9세 여아와 6세 남아에서 각각 5년 5개월, 3년 7개월 후의 치조골소실 정도를 cone beam computed tomography와 진단 모형 상에서 관찰하였기에 보고하는 바이다.
치열궁의 크기와 폭경 변화에 대한 연구가 사용된 장치 형태, 사용시기에따라, 그리고 사용 목적에 따라 다양한 연구가 활발히 이루어져 왔다. 또한 Quad-helix도 선학들에게 관심의 대상이었으며 Quad-helix의 안정성과 장치 사용 전, 후에 대한 효과도 연구가 이루어졌다. 그러나 장치 사용에 대한 장기적인 효과에 대한 연구가 미흡하여 본 논문은 이 장치를 사용하여 치료한 환자의 치료전,후 및 보정후의 치열궁 변화에 대한 연구를 시행하여 Quad-helix사용 전, 후 구치부의 적응 양상을 연구함으로써 임상에서 Quad-helix를 통한 치료를 시행할 때 이 장치사용에 따른 효과 및 예후에 대하여 효과를 이해함으로써 교정치료에 도움을 주고자 연구를 시행하여 다음과 같은 결과를 얻었다. 1. Quadhelix를 사용한 교정치료시 상악 제 1 소구치 와 제 1 대구치는 치료전과 치료후 및 보정후에서 교두간 폭경의 증가를 보였고 제 2 소구치에서는 치료후와 보정후간에 유의성있는 증가를 보였다. 2. 상악 제 1 소구치와 제 1 대구치의 치축은 치료전과 치료후 그리고 보정후에서 협측 경사도가 유의하게 증가를 보였으며, 상악 제 2 소구치는 치료전과 치료후 및 치료전과 보정후 비교에서 설측 경사도가 유의하게 증가를 보였다. 3. 최대 풍융부 항목에서 상악 제 1, 2소구치는 치료전과 치료후의 비교에서 유의한 증가를, 제 1 대구치에서는 치료전,후, 및 보정후에 유의한 증가를 보였다. 4. 상악 제 1 대구치의 설면구 폭경은 치료전,후 및 보정기간을 통하여 유의한 증가를 보였고,구개 높이는 치료전과 치료후 비교시 유의한 증가를 보였다. 5. 확장시킨 상악 치열궁은 회귀현상을 보이지 않고 안정적으로 유지되었다.
임플란트 식립시기가 심미적인 부분에 영향을 미치는 것에 대한 연관성을 평가하기 위해 전치부 영역에 식립된 34개의 임플란트 (27명 환자)를 후향적으로 조사했다. 즉시, 조기 및 지연식립으로 시기별로 그룹을 나누고 모든 환자를 보철완료 1년후에 연조직 평가를 시행했다. 통계적으로 그룹별 유의한 의미는 없었다. 식립 시기에 상관없이 만족할 만한 최종결과를 위해 다양한 골이식술과 연조직 이식술이 이루어졌다. 즉, 식립시기로 그 결과를 결정지을 수 없다는 점은 다각적인 면에서 고려사항이 요구된다는 점을 시사한다.
The purpose of this study was to evaluate the removable partial denture prescriptions including surveyed crowns and design of component parts sent to the laboratory technician. A total of 351 casess with prescription forms and master cast in maxillary and mandibular semi-edentulous situations collected from dental laboratory by random sampling were selected for this study. The evaluation and study observed here involved the classification of edentulous situations, status of abutment splinting, form of rest seats and guiding plane of surveyed crows, location of maxillary major connectors and tripodig marks on the master casts. Removable partial denture prescriptions contained (1) general request (upper and lower cast framework), (2) types of metal, (3) location of retainer(retention, lingual bracing, rest area, guiding plane surface), (4) location and type of major connector, (5) relief area and amount, (6) and other specific instructions. The following informations based on the classified groups such as Group I was those cases sent with no real prescriptions. They say 'make a partial.' No prescriptions, no thought beforehand, Group II was those cases sent with a minimal prescriptions. They say 'make a partial with clasps on May be some preparations, usually inadequate. Group III was those cases sent with a moderately good prescription. Adequate but could be much better. No tripoding but it tell what clasps go where. Still not good prescriptions. Group IV was good cases, tripoded with adequate prescriptions and a prescription which exactly describes what is expected from the laboratory. The analyzed results were as follows: 1. The normal form of rest seats and guiding plane of surveyed crowns in Class. I and Class. II edentulous situations on the maxillary cast were observed 31.9% and 27.89%, respectively. The abutment teeth and retainer without occlusal rests of Class. I and Class. II were showed 11.58% and 8.86%, respectively. In mandibular cases, the normal form of rest seats and guiding plane of surveyed crowns showed 27.54% and 8.82% in Class. I and Class. II situation. The abutment teeth and retainer without rest seats were showed 15.19%, respectively. 2. The splinted surveyed crowns of Class. I and Class. II maxillary edentulous situations in distal extension cases were showed 34.51% and 28.85%, but 28.52% and 10.29%, respectively. 3. The location and type of maxillary major connector delineated on the master cast were 66 cases (44.89%). 4. The results of 351 cases were classified as Group I 146(41.59%), Group II 115 (32.76%), Group III 57 (16.23%), and Group IV 33 (9.48%). 5. The delineation of abutment tooth for clasping were 176 cases (50.14%) among total of 351 cases. 6. The delineation of height of contour line were showed 45 cases (12.8%) in Group II, 14 cases (3.98%) in Group III and 33 cases (9.40%) in Group IV with total 92 cases (26.21%). 7. In surveying procedure, the delineation of tripoding marks and reference line were showed 17 cases (4.84%).
목적: 치은 형성 후 인공치아의 이동에 대하여 평가를 시행하였다. 연구 재료 및 방법: 10벌의 동일한 치아배열과 치은의 외형을 가진 납의치를 이용하여 실험하였다. 이 납의치 상에서 치은 형성을 시행하였고, 10분 간격으로 120분 간 모델 스캔을 시행하여 치아 간 거리를 측정하였다. SPSS Ver. 22. 0를 이용하여 통계학적 분석을 시행하였다. 결과: 치은 형성 후 평균적으로 치아 사이의 거리가 0.0999 mm에서 0.1787 mm로 나타났다. 인공치아 이동의 변화율에 대해서는 40 - 50분과 50 - 60분 사이와 50 - 60분과 60 - 70분 사이에서 통계학적으로 유의한 차이가 관찰되었고, 그 이후에는 변화율의 유의한 차이가 없었다. 결론: 120분 간 측정한 결과 인공치아의 이동이 관찰되었고, 60 - 70분 이후에는 시간에 따른 평균 이동량의 변화가 관찰되지 않았다. 이를 종합하면, 치은 형성 후 60분 이상 경과된 시점에서 교합에 대한 재평가를 시행한 후에 최종 의치를 제작하는 것이 필요하다.
Objectives: The purpose of this study was to investigate the ratio of 4 root canals and the incidence of Radix Entomolaris in mandibular first molars and find out anatomical difference according to number of roots by analysis of cone-beam CT images in a Korean population. Materials and Methods: Total 142 images containing mandibular first molars were selected from CBCT images taken from 2013 to 2017 at Gangneung-Wonju National University Dental Hospital. After reconstructing the image with reference to the Cemento-enamel junction, the root canals were detected at the bottom of the pulpal floor and the number of roots and root canals were analyzed. Various lengths and MLO-DLO-DBO angle were measured between each canal orifices and the external contour line of the tooth, and the distolingual canal wall thickness was measured. Student t-test was used for statistical significance. Results: Among the total 142 teeth, 4 canals were 42.2% and Radix Entomolaris was 25.3%. As the results of measuring various lengths and the angle, the distolingual canal orifice in Group 2(with Radix Entomolaris) tends to deviate to the lingual side than the mesiolingual canal orifice and to the mesial side than the distobuccal canal orifice. Besides, thickness of the distolingual canal wall in Group 2(with Radix Entomolaris) was significantly thinner than that of Group 1 at every level except pulpal floor level. Conclusion: It is necessary to consider the difference according to the presence of Radix Entomolaris in endodontic treatment.
연구 목적: 본 연구의 목적은 전통적인 금속 도재관과 열-가압을 이용한 금속-도재관의 변연 적합도를 비교하는 것이다. 연구 재료 및 방법: 상악 제1소구치 레진 치아를 교합면 2.0 mm, 협, 설면과 인접면을 1.0 mm 두께로 삭제하였으며, 변연 형태는 chamfer margin으로 그리고 축면 경사는 6도 taper로 삭제하였다. 삭제한 레진치를 복제하여 금속 다이 20개를 제작하였고, 이 금속 다이를 인상 채득하여 작업을 위한 석고 다이를 제작하였다. 금속 도재관과 $PoM^{(R)}$ 시스템 (Ivoclar vivadent., Liechtenstein)의 두 그룹으로 나누어 금속 코핑을 각각 10개씩 총 20개를 제작하였으며, 광학 현미경을 이용하여 변연 간격을 측정하였다. 도재 축성 후, 위와 동일한 방법으로 변연 간격을 측정하였다. 통계 처리는 Wilcoxon Signed Ranks test와 Mann Whitney U test를 이용하여 분석하였다. 결과: 본 연구의 결과는 다음과 같다. 1. 금속 도재관은 코핑 상태 ($64.93{\pm}12.48\;{\mu}m$)와 도재 축성 후 ($63.43{\pm}12.86\;{\mu}m$)의 변연 간격에 유의성 있는 차이를 보이지 않았다. 2. $PoM^{(R)}$ 시스템은 코핑 상태 ($50.00{\pm}12.28\;{\mu}m$)와도재축성후($56.72{\pm}13.80\;{\mu}m$)의 변연 간격에 유의성 있는 차이를 보이지 않았다. 3. 금속 도재관과 $PoM^{(R)}$ 시스템 변연 간격은 유의성 있는 차이를 보이지 않았다. 결론: $PoM^{(R)}$시스템은 왁스 소결법을 이용해 변화가 적은 기술적인 이점이 있으며, 세밀한 기술을 요구하는 축성 방법과는 대조적으로 왁스 성형으로 도재의 완전한 형태를 만들 수 있는 편리함을 제공하는 장점을 가진다. 본 연구 결과와 이런 장점을 토대로 앞으로 임상적 사용을 고려해 볼 수 있을 것으로 사료된다.
