Background: As hypertrophied myocardium predisposes the patient to decreased tolerance to ischemia and increased reperfusion injury, myocardial protection is of utmost importance in patients undergoing aortic valve replacement (AVR) for severe aortic valve stenosis (AS). Methods: Consecutive 314 patients (mean age, $62.5{\pm}10.8$ years; 143 females) with severe AS undergoing isolated AVR were included. Postoperative myocardial injury (PMI) was defined as 1) maximum postoperative creatinine kinase isoenzyme MB or troponin-I levels ${\geq}10$ times of reference, 2) postoperative low cardiac output syndrome or episodes of ventricular arrhythmia, or 3) left ventricular ejection fraction of less than 55% and decrease in left ventricle (LV) ejection fraction of more than 20% of the baseline value. Results: There were 90 patients (28.7%) who developed PMI. There were five cases of early death (1.6%), all of whom had PMI. On multivariable analysis, the use of histidine-tryptophan-ketoglutarate (HTK) solution instead of blood cardioplegia (odds ratio [OR], 3.06; 95% confidence interval [CI], 1.63 to 5.77; p=0.001), greater LV mass (OR, 1.04; 95% CI, 1.01 to 1.07; p=0.007), and increased cardiac ischemic time (OR, 1.13; 95% CI, 1.05 to 1.22; p<0.001) were independent predictors for PMI. Patients who had PMI showed significantly inferior long-term survival than those without PMI (p=0.049). Conclusion: PMI occurred in a considerable proportion of patients undergoing AVR for severe AS and was associated with poor long-term survival. HTK cardioplegia, higher LV mass, and longer cardiac ischemic duration were suggested as predictors of myocardial injury.

The purpose of this study was to the Pre-Post submaximal exercise for analyze the Blood Lactate, LDH(Lactate Dehydrogenase) and Glucose in the runners. The subjects were 12 volunteers 6 sprinters and 6 distance runners who agreed to intention of this study. Subjects peformed until possible all-out trials. Blood samples were taken from a Ante cubital vein Pre-Post exercise and every rest 5min during the all-out trial. The results obtained were summarized as follow; 1) Blood Lactate of Pre-Post exercise in treadmill test showed no significant difference between sprinters and distance runners, but showed significant in sprinters and distance respectively. 2) Serum LDH of Pre-Post exercise in treadmill test showed no significant difference between sprinters and distance runners, and that showed no significant in sprinters and distance respectively. 3) Blood Glucose of Pre-Post exercise in treadmill test showed no significant difference between sprinters and distance runners, and showed no significant in distance, but showed significant in sprinters. These data indicate first that the value of 4mmol/${\ell}$, commonly referred to as OBLA(Onset of Blood Lactate),may often underestimate the upper limit of tolerance to lactate during a maximal endurance peformance test until all-out. second, our date suggested that the sprinters exercise decreases serum LDH activities and the distance increases serum LDH activities. therefore serum LDH concentration changed following a different exercise. Third. for the concentration of the glucose in blood the sprinters was show to be higher. However it needs more studies to find out the relationship between this result and the fitness factor. therefor, since the runners has an intermittent characteristic which includes a frequent momentary change, it needs the fitness training that being consisted of various training. in the training method, it needs the training process which is from the whole fitness to specific fitness to improve general physical ability.
Insulin signaling is coordinated by insulin receptor substrates (IRSs). Many insulin responses, especially for blood glucose metabolism, are mediated primarily through Irs-1 and Irs-2. Irs-1 knockout mice show growth retardation and insulin signaling defects, which can be compensated by other IRSs in vivo; however, the underlying mechanism is not clear. Here, we presented an Irs-1 truncated mutated mouse ($Irs-1^{-/-}$) with growth retardation and subcutaneous adipocyte atrophy. $Irs-1^{-/-}$ mice exhibited mild insulin resistance, as demonstrated by the insulin tolerance test. Phosphatidylinositol 3-kinase (PI3K) activity and phosphorylated Protein Kinase B (PKB/AKT) expression were elevated in liver, skeletal muscle, and subcutaneous adipocytes in Irs-1 deficiency. In addition, the expression of IRS-2 and its phosphorylated version were clearly elevated in liver and skeletal muscle. With miRNA microarray analysis, we found miR-33 was down-regulated in bone marrow stromal cells (BMSCs) of $Irs-1^{-/-}$ mice, while its target gene Irs-2 was up-regulated in vitro studies. In addition, miR-33 was down-regulated in the presence of Irs-1 and which was up-regulated in fasting status. What's more, miR-33 restored its expression in re-feeding status. Meanwhile, miR-33 levels decreased and Irs-2 levels increased in liver, skeletal muscle, and subcutaneous adipocytes of $Irs-1^{-/-}$ mice. In primary cultured liver cells transfected with an miR-33 inhibitor, the expression of IRS-2, PI3K, and phosphorylated-AKT (p-AKT) increased while the opposite results were observed in the presence of an miR-33 mimic. Therefore, decreased miR-33 levels can up-regulate IRS-2 expression, which appears to compensate for the defects of the insulin signaling pathway in Irs-1 deficient mice.

BACKGROUND/OBJECTIVES: Type 2 diabetes (T2D) is more frequently diagnosed and is characterized by hyperglycemia and insulin resistance. $\small{D}$-xylose, a sucrase inhibitor, may be useful as a functional sugar complement to inhibit increases in blood glucose levels. The objective of this study was to investigate the anti-diabetic effects of $\small{D}$-xylose both in vitro and stretpozotocin (STZ)-nicotinamide (NA)-induced models in vivo. MATERIALS/METHODS: Wistar rats were divided into the following groups: (i) normal control; (ii) diabetic control; (iii) diabetic rats supplemented with a diet where 5% of the total sucrose content in the diet was replaced with $\small{D}$-xylose; and (iv) diabetic rats supplemented with a diet where 10% of the total sucrose content in the diet was replaced with $\small{D}$-xylose. These groups were maintained for two weeks. The effects of $\small{D}$-xylose on blood glucose levels were examined using oral glucose tolerance test, insulin secretion assays, histology of liver and pancreas tissues, and analysis of phosphoenolpyruvate carboxylase (PEPCK) expression in liver tissues of a STZ-NA-induced experimental rat model. Levels of glucose uptake and insulin secretion by differentiated C2C12 muscle cells and INS-1 pancreatic ${\beta}$-cells were analyzed. RESULTS: In vivo, $\small{D}$-xylose supplementation significantly reduced fasting serum glucose levels (P < 0.05), it slightly reduced the area under the glucose curve, and increased insulin levels compared to the diabetic controls. $\small{D}$-xylose supplementation enhanced the regeneration of pancreas tissue and improved the arrangement of hepatocytes compared to the diabetic controls. Lower levels of PEPCK were detected in the liver tissues of $\small{D}$-xylose-supplemented rats (P < 0.05). In vitro, both 2-NBDG uptake by C2C12 cells and insulin secretion by INS-1 cells were increased with $\small{D}$-xylose supplementation in a dose-dependent manner compared to treatment with glucose alone. CONCLUSIONS: In this study, $\small{D}$-xylose exerted anti-diabetic effects in vivo by regulating blood glucose levels via regeneration of damaged pancreas and liver tissues and regulation of PEPCK, a key rate-limiting enzyme in the process of gluconeogenesis. In vitro, $\small{D}$-xylose induced the uptake of glucose by muscle cells and the secretion of insulin cells by ${\beta}$-cells. These mechanistic insights will facilitate the development of highly effective strategy for T2D.
The limitation of measurement accuracy and reliability of autostigmatic null lens system are studied for the cases of using inter-distance of null lenses as the adjustment factor of alignment and fixing the distance by mounting. If we investigate the first case, the wavefront aberration of null lens system is compensated by the adjustment process even though the shape of aspherical surface is not properly fabricated. As the result, it brings about the problem of measurement reliability. However, for the fixing process by mounting null lenses, it doesn't cause the reliability problem because the wavefront aberration of null lens system is not compensated. Further, the fixing process shows nearly same result in measurement accuracy to the adjustment process, that is, $0.0316{\lambda}$ vs. $0.0326{\lambda}$. So, we can conclude the setup for autostigmatic null lens system must be constituted by means of the fixing process. Meanwhile, we introduce and define the alignment aperture on aspheircal mirror, which can be approximated as spherical zone for alignment of null lens system, and besides, we calculate the required fabrication accuracy of the zone for the necessary measurement accuracy.
Purpose: Immune suppression is common in patients with advanced breast cancer but the mechanisms underlying this phenomenon have not been sufficiently studied. In this study, we aimed to identify B7 family members that were able to predict the immune status of patients, and which may serve as potential targets for the treatment of breast cancer. We also aimed to identify microRNAs that may regulate the expression of B7 family members. Methods: The Cancer Genome Atlas data from 1,092 patients with breast cancer, including gene expression, microRNA expression and survival data, were used for statistical and survival analyses. Polymerase chain reaction and Western blot were used to measure messenger RNA and protein expression, respectively. Luciferase assay was used to investigate direct microRNA target. Results: Bioinformatic analysis predicted that microRNA (miR)-93, miR-195, miR-497, and miR-340 are potential regulators of the immune evasion of breast cancer cells, and that they exert this function by targeting CD274, PDCD1LG2, and NCR3LG1. We chose CD274 for further investigations. We found that miR-195, miR-497, and CD274 expression levels were inversely correlated in MDA-MB-231 cells, and miR-195 and miR-497 expressions mimic inhibited CD274 expression in vitro. Mechanistic investigations demonstrated that miR-195 and miR-497 directly target CD274 3' untranslated region. Conclusion: Our data indicated that the level of B7 family members can predict the prognosis of breast cancer patients, and miR-195/miR-497 regulate CD274 expression in triple negative breast cancer. This regulation may further influence tumor progression and the immune tolerance mechanism in breast cancer and may be able to predict the effect of immunotherapy on patients.
White jelly fungus (Tremella fuciformis Berk) is effective for hypertension, diabetes, obesity, lung disease, beauty, and has been known as an elixir of life in ancient Chinese dynasty. In this study, we investigated the anti-obesity and diabetic effects of the fermented white jelly fungus extract by the bioconversion process. We fed an obesity-inducing mouse with 5% non-fermented Tremella fuciformis (TF), 2.5% fermented Tremella fuciformis (FTF), and 5% FTF containing High Fats Diet (HFD) and HFD for 8 weeks, respectively. The oral glucose tolerance test (OGTT) was performed analysis after 7 weeks of feeding and the dietary intake, food efficiency ratio, body weight, liver, epididymal fat weight, and serum insulin level were measured after 8 weeks of feeding. Also, HOMA-IR was analyzed. The concentrations of serum total cholesterol, triglycerides test was analyzed. The FTF compare with 5% TF and HFD confirmed that 5% FTF reduced body weight, tissue weight, triglycerides concentration, HOMA-IR, respectively. As a result, we confirmed that the fermented white jelly fungus has the anti-obesity effect. Finally, this study can be used a basic data for obesity treatment using fermented white jelly fungus.

The crops cultivated and consumed in Korea require specific climate conditions corresponding to their own growth characteristics. This study aims to analyze the relationship between climate change and agricultural productivity. According to growing concern about climate change internationally, many agricultural studies are developing technology to prevent damage from climate change. Before developing technology, we should figure out what kind of crop gets huge damage and how much caused by climate change. In the context of agricultural economics, we can define the reduction of agricultural product yield as a decline in productivity. As a result, this study analyzes the effects of climate change on agricultural productivity using Stochastic Frontier Analysis model. There are several kinds of climate change phenomena that increase the inefficiency of production. In other words, there are several kinds of crops that get negative influence by climate change. The result of this study can be used as basic guideline for producers to prepare for changing weather prior to developing disaster tolerance technology coping actively with special weather report.

Objective : The purpose of this study was to investigate smartphone addition and sensory processing character depend on gender for occupational therapy (OT) students, and to identify the correlation between smartphone addiction and sensory processing feature. Methods : Participants of this study were 168 (70 male/90 female) students majoring occupational therapy. Measurements in this study were a questionnaire about general information of subject, smartphone addiction scale, and adult/adolescent sensory profile. Data collection period was from November 2017 to March 2018. Methods for the data analysis included descriptive statistics, independent t-test and Pearson correlation of SPSS 22.0. Results : In term of the purpose of using smartphone, majority response was communication and gaming ranked in next. There were difference between male and female in the total smartphone addition, cyberspace-oriented relationship, withdrawal and tolerance. The result showed that all sensory processing are related with total smartphone addition (r = .236 ~ .603) for man. And for women, total smartphone addition is little related with the taste/smell processing (r= .290), visual processing (r= .324), touch processing (r= .214), low registration (r= .214), sensory sensitivity (r= .243), and sensory avoiding (r= .217). Conclusion : This study found that there is difference between male and female in terms of relation between smartphone addition and sensory processing feature according for occupational therapy students.

Purpose: To report the initial experience of patient-specific quality assurance (pQA) for the wobbling and line-scanning proton therapy at Samsung Medical Center. Materials and Methods: The pQA results of 89 wobbling treatments with 227 fields and 44 line-scanning treatments with 118 fields were analyzed from December 2015 to June 2016. For the wobbling method, proton range and spread-out Bragg peak (SOBP) width were verified. For the line-scanning method, output and two-dimensional dose distribution at multiple depths were verified by gamma analysis with 3%/3 mm criterion. Results: The average range difference was -0.44 mm with a standard deviation (SD) of 1.64 mm and 0.1 mm with an SD of 0.53 mm for the small and middle wobbling radii, respectively. For the line-scanning method, the output difference was within ${\pm}3%$. The gamma passing rates were over 95% with 3%/3 mm criterion for all depths. Conclusions: For the wobbling method, proton range and SOBP width were within the tolerance levels. For the line-scanning method, the output and two-dimensional dose distribution showed excellent agreement with the treatment plans.
