  • Objectives: The present study aims to examine associations between job demands and problem drinking, smoking, and practice of regular exercise among middle-aged and older male employees. Methods: Analyses were based on 239 employees aged 50+ and participated the 1st(2006) and the 4th(2012) waves of the Korean Longitudinal Study of Ageing(KLoSA). Panel logit regression analyses were performed to explore longitudinal associations between physical and cognitive job demands and the health behaviors when effects of demographic characteristics and objective job conditions were controlled. Results: Results suggested that first, workers who reported greater cognitive job demands were less likely to engage in problem drinking over the 6-year-period. Second, increased physical demands of the job were associated with greater odds of smoking, while physical demands predicted a reduced likelihood of practicing regular exercise. Conclusions: Results from the present analyses emphasize job demands could lead workers to problem health behaviors and suggest areas for health promotion efforts at the workplace that are sensitive to the needs of aging workers.

  • Korea is one of the fastest aging country in the world. The increase in health expenditure is an inevitable problem in an aging country. We examined trend of benefit expenditure in National Health Insurance (NHI) by age group during 2001 to 2016. The benefit expenditure of NHI was 13.0 trillion won (2001) and 48.7 trillion won (2016); that is 3.76 times to 2001. This rapid increase was mostly due to people aged 65 and over. Proportion of beneficiaries aged 65 and over was 6.9% (2001) and 13.7% (2016), benefit expenditure per capita aged 65 and over was 279 thousand won (2001) and 960 thousand won (2016), and utilization days per capita aged 65 and over 21.44 (2001) and 30.23 (2016). This phenomenon was more pronounced in 75 or 85 aged and over. To contain the health costs for older people, the NHI system should be reformed.

Higher bedtime resistance (aOR=2.2, 95% CI=1.0~4.9) and higher daytime sleepiness (aOR=2.8, 95% CI=1.2~6.3) were statistically significant contributors of external problems. Higher bedtime resistance (aOR=4.3, 95% CI=1.5~12.4) was statistically significant contributors of total problem behavior. Conclusion: School-aged children with sleeping problem are at risk for problem behavior. • The elderly population will increase from 5.8% in 1996 to 12.5% in 2020. The related problems of health will also become a very important issue in the future. Therefore it is important to address the problems of geriatric nursing and geriatric health. 87.6% of the aged were ill with chronic degenerative diseases in 1994. In 1995, hospital admission rates (86.8) for those aged 60 or older were higher than that (56.3) of the total age group. Such high medical utilization will increase national health costs. For the development of geriatric nursing, active nursing intervention in various settings combined with education and research should be developed. Considering the health and welfare of the aged and the present status and views in Korea, I suggest the following: First, the health needs of the elderly in the institution, must be met by a plan that fosters geriatric nurses and programmed service development. Second, health service for the residential elderly must be provided in day care centers, short stays, nursing homes and geriatric hospitals. Geriatric nursing services should be provided in home residential areas, public health centers, public health subcenters as well as having, community health practitioners in primary health care posts and home health nurses. Third, geriatric nursing curriculum must be developed adjust to situations and culture of Korea and be included in the nursing curriculum. And gerontological nurse practitioner or geriatric specialist must be fosteraged to provide the professional care for the aged. Geriatric nursing research should be also achived for geriatric nursing improvements.
