A Review of HLA Genes in Pharmacogenetics: Risk Assessment of Adverse Drug Reactions

  • Adverse drug reactions (ADRs) is a hypersensitivity reactions to specific medications, and remain a common and major problem in healthcare. ADRs suchc as drug-induced liver injury and life-threatening severe cutaneous adverse drug reactions including Stevens-Johnson syndrome, toxic epidermal necrolysis, and drug rash with eosinophilia and systemic symptoms can be occurred by uncontrolled expansion of oligoclonal T cells according to genetically predisposing HLA. In this review, I summarized the alleles of HLA genes which have been proposed to have association with ADRs caused by different drugs.

Mimicking Odontogenic Pain Caused by Burkitt's Lymphoma: A Case Report

  • Burkitt's lymphoma is a malignant monoclonal proliferation of early B-lymphocyte. Since Burkitt's lymphoma is a highly aggressive disease, early detection is a crucial. This disease often involves jaw and mandibular mass or swelling may also be seen, but in the early phase of Burkitt's lymphoma these symptoms cannot be observed. A rare case of Burkitt's lymphoma without any mandibular mass and the general symptoms was present. The excruciating toothache led the patient to visit the dental clinic and misdiagnosis of chronic periodontal abscess was made initially. Dentists should consider the oral manifestations of systemic disease when the multiple periodontal ligament space widening is observed and the dental treatment for mimicking odontogenic pain has no effect.

A Case Report of Neuro-Behcet's Disease with Paraparesis (하지부전마비를 동반한 Neuro-Behcet 병 1례 보고)

  • Behcet's disease is a systemic disease affecting multiple organs including the central nervous system. Neuro-Behcet's disease was regarded as relatively rare, but thanks to the development of diagnostic tools, more and more cases are being reported. We are reporting a case of neuro-Behcet's disease in which the patient displayed paraparesis, dysarthria and involuntary tremor as neurologic symptoms. The patient's brain MRI showed cerebellar atrophy, and a spinal cord MRI failed to reveal any significant lesions. The patient experienced a couple of fever attacks during hospitalization, which were managed adequately by herbal medicines. Her main neurological symptoms such as paraparesis were, however, grossly unchanged at discharge.

Efficacy of Stellate Ganglion Block in Cholinergic Urticaria with Acquired Generalized Hypohidrosis

  • Cholinergic urticaria with acquired generalized hypohidrosis, and its pathophysiology is not well known. Autoimmunity to sweat glands or to acetylcholine receptors on sweat glands has been mentioned as one of the possible etiologies. Systemic steroid therapy, antihistamines, anticholinergics, and avoidance of the stimulatory situations are recommended for treatment. We experienced a case of cholinergic urticaria with acquired generalized hypohidrosis in a patient who had no other associated disease, and the symptoms eased after repeated bilateral stellate ganglion block. Stellate ganglion block normalized the elevated sympathetic tone and may relieve symptoms in patients with this condition.

Whole Lung Irradiation for Metastatic Lung Malignancy (전이성 폐암의 전폐 방사선치료)

  • The author reviewed four cases of multiple lung metastases who were treated by whole lung irradiation alone for palliation. All patients tolerated the treatment well without complication. Two patients who had symptoms from pulmonary metastases had subjective relief of symptoms and all four patients had objective regression of lung metastases on repeat chest X-rays. Present report and review of literature support that some selected patients with multiple lung metastases can be benefited by whole lung Irradiation. However, systemic chemotherapy should be main modality of treatment for this kind of disseminated disease whenever possible.

Clinical Usefulness of Aripiprazole Treatment in a Girl with Prader-Willi Syndrome and Psychosis

  • Prader-Willi syndrome (PWS) is a quite rare multi-systemic genetic disorder strongly associated with psychiatric illness in adults, especially psychosis. This report presents a 16-year-old female with PWS and symptoms of brief psychotic disorder with a complete resolution of symptoms under aripiprazole medication. However, an exacerbation occurred after aripiprazole reduction. Apart from a weight gain of about 2 kg over the course of two years, no adverse effects could be found. This first report on the use of aripiprazole in a subject with PWS and psychosis suggests that aripiprazole might be a promising treatment approach in this distinct group of patients.

  • Langerhans cell histiocytosis (LCH) is a rare disorder characterized by the proliferation of dendritic cells resulting in local or systemic symptoms. The clinical symptoms of patients with Langerhans cell histiocytosis depend on the site and the degree of involvement. This article describes two case histories of unifocal bony Langerhans cell histiocytosis with mandibular involvement and further discusses the appropriate management of such via a review of the literature.

  • Pollen-food allergy syndrome (PFAS) is an immunoglobulin E-mediated immediate allergic reaction caused by cross-reactivity between pollen and the antigens of foods-such as fruits, vegetables, or nuts-in patients with pollen allergy. A 42.7% prevalence of PFAS in Korean pediatric patients with pollinosis was recently reported. PFAS is often called oral allergy syndrome because of mild symptoms such as itching, urticaria, and edema mainly in the lips, mouth, and pharynx that appear after food ingestion. However, reports of systemic reactions such as anaphylaxis have been increasing recently. This diversity in the degree of symptoms is related to the types of trigger foods and the characteristics of allergens, such as heat stability. When pediatric patients with pollen allergy are treated, attention should be paid to PFAS and an active effort should be made to diagnose it.

A Clinical Study of the Pediatric Patients Who Visited in Emergency Room of Oriental Medical Hospital (모 한방병원 응급실에 내원한 소아 환자에 대한 임상적 연구)

  • Objectives The purpose of this study was to revitalize pediatric emergency care in oriental medical hospital. Methods The study was composed of 281 pediatric patients who visited emergency room of the ${\bigcirc}{\bigcirc}$ university oriental hospital, from March 2006 to February 2007. Results The age distribution showed that 1 to 3 year's patients(40.6%) are the most New pediatric patients(59.1%) were more than who received medical treatment in this place. The seasonal distribution of between month's section and the 24 solar terms's section made no differences and pediatric patients who visited in emergency room was most in spring, least in autumn. Moreover the changes of season's patients were more than others. Pediatric patients who visited during the weekends and holidays(50.9%) were more than during the weekdays(49.1%). Systemic division of the major problems were the digestive symptoms(44.5%) and nervous symptoms(30.6%), respiratory symptoms(12.5%), musculoskeletal symptoms(5.3%), dermatological symptoms(1.8%), cardiovascular symptoms(0.7%) and others(4.6%) followed. The time interval between arrival and onset : within 6 hours were the most(54.5%). Pediatric patients who had digestive, nervous, musculoskeletal diseases within 6 hours were the most, but in the case of patients who had respiratory diseases, most of them were visited within 48 hours. Acting and herb-med treatment(77.9%) were the most in medical treatments. Pediatric patients(69.0%) who didn't revisit in this hospital after treatment in emergency room were more than who revisited(29.9%). Conclusions Pediatric patients who visited in emergency room of oriental medical hospital were most not acute form of a serious diseases, patients most visited in disease prefer to oriental medicine; such as dyspepsia, convulsions, crying, ankle sprain. Further studies will be needed for the actual circumstance's reflecion from this hospital and other oriental hospitals nearby or other western emergency rooms for the accurate studies.

A Study on Health Effects of VDT Syndrome in Hospital Workers (병원종사자의 VDT증후군 자각증상과 건강영향에 관한 연구)

  • Purpose : This study aims to examine hospital employees' subjective symptoms of VDT syndrome (Video display terminal syndrome) and figure out their effects on health conditions and correlations. Materials and Methods : This study used a structured self-administering questionnaire and gained data from 125 subjects. The questionnaire consists of total 62 questions, and they went through real number, percentage, ${\chi}^2$-terst, t-test, one-way ANOVA, and logistic regression analysis. Results : Regarding the difference in the types of subjective symptoms of VDT syndrome by jobs, there was statistically significant difference in ophthalmic symptoms, systemic symptoms, and musculoskeletal symptoms. About the correlation between the subjective symptom points of VDT syndrome by the types of symptoms, there was statistically significant correlation all in skin trouble, backache, upper limb muscle pain, lower limb muscle pain, fatigue in the head and eyes, and depression or anxiety disorders. There was significant correlation between the subjective symptom points of VDT syndrome and subjective health conditions. Conclusion : Hospital employees' subjective health conditions had significant correlation with subjective symptom points of VDT syndrome. This result shows that as radiologists' and also other hospital employees' working conditions change into VDT environment, VDT syndrome gradually appears more and more. This implies that it is needed to designate not only radiologists but also all the other hospital employees as jobs exposed to the working environment of VDT syndrome and provide preventive measures, education, and publicity for it afterwards.
