Predictors of Intra-Aortic Balloon Pump Insertion in Coronary Surgery and Mid-Term Results

  • Background: We aimed to investigate the preoperative, operative, and postoperative factors affecting intra-aortic balloon pump (IABP) insertion in patients undergoing isolated on-pump coronary artery bypass grafting (CABG). We also investigated factors affecting morbidity, mortality, and survival in patients with IABP support. Methods: Between January 2002 and December 2009, 1,657 patients underwent isolated CABG in Izmir Katip Celebi University Ataturk Training and Research Hospital. The number of patients requiring support with IABP was 134 (8.1%). Results: In a multivariate logistic regression analysis, prolonged cardiopulmonary bypass time and prolonged operation time were independent predictive factors of IABP insertion. The postoperative mortality rate was 35.8% and 1% in patients with and without IABP support, respectively (p=0.000). Postoperative renal insufficiency, prolonged ventilatory support, and postoperative atrial fibrillation were independent predictive factors of postoperative mortality in patients with IABP support. The mean follow-up time was $38.55{\pm}22.70$ months and $48.78{\pm}25.20$ months in patients with and without IABP support, respectively. The follow-up mortality rate was 3% (n=4) and 5.3% (n=78) in patients with and without IABP support, respectively. Conclusion: The patients with IABP support had a higher postoperative mortality rate and a longer length of intensive care unit and hospital stay. The mid-term survival was good for patients surviving the early postoperative period.

Semiparametric support vector machine for accelerated failure time model

  • For the accelerated failure time (AFT) model a lot of effort has been devoted to develop effective estimation methods. AFT model assumes a linear relationship between the logarithm of event time and covariates. In this paper we propose a semiparametric support vector machine to consider situations where the functional form of the effect of one or more covariates is unknown. The proposed estimating equation can be computed by a quadratic programming and a linear equation. We study the effect of several covariates on a censored response variable with an unknown probability distribution. We also provide a generalized approximate cross-validation method for choosing the hyper-parameters which affect the performance of the proposed approach. The proposed method is evaluated through simulations using the artificial example.

  • The purpose of this paper is to develop a knowledge-based real-time decision support system to support decision makers for efficient berth operation of Inchon Port. In these days, the efficient berth operation has been many studied. The berth operation rules differ from port to port and the problem is highly dependent on natural, geographical and operational environment of port. In Inchon Port, the ship's entrance into port and departure from port is extremely affected by the status of dock and berth because of capacity restriction. First, we analyzed the specific characteristics of Inchon Port such as dock based on the data of 1997. And then, we construct the database of experts knowledge for berth utilization. Finally, we build the real-time decision support system for the efficient berth operation of Inchon Port to make the better berth allocation in case of not only regular scheduling but also dynamic scheduling such as delay in berth operation and exchange of ship between berths. The DSS is developed with graphic user interface(GUI) concept to help the user determining user interactive updating of the port status. Then this DSS will be provide decision maker with an efficient and fast way to berth allocation, and reduce wastes of time, space, and manpower in Inchon Port operation.

청년농업인의 자립의지와 영농지속성에서 가족지지의 매개효과 분석 (Analysis of the Mediating Effect of Family Support in Young Farmers' Self-reliance and Farming Continuation)

  • In the meantime, the government has implemented various policies for young farmers at a time when the number of people in charge of agriculture and rural areas is decreasing, but it is difficult to secure a stable agricultural workforce. This study analyzed the mediating effect of family support in the will to self-reliance and farming continuation for young farmers at a time when agricultural and rural farmers are decreasing and aging. According to the analysis, young farmers are young, highly educated, have short farming experience, and mainly livestock and gardening were high. In addition, it was found that the will of young farmers to stand on their own feet influenced the sustainability of young farmers by partially selling their family support. The main implications are the need for support or support for internal growth of young farmers and the need for continuous family support. It was also understood that young farmers should have the will to stand on their own feet in order to have continuous and satisfactory farming results.

한국의 초산모들의 산후 사회적 지원 경험: 해석학적 현상학 연구 (Postnatal social support experiences in primiparous women in Korea: a hermeneutic phenomenological study)

  • Purpose: Social support is essential for postpartum well-being, but little is known about the postpnatal social support preferred by primiparous women. This study aimed to comprehensively understand and describe the meaning of postnatal social support experience in primiparous women. Methods: The participants were seven primiparous women who were within 1 year after childbirth, recruited through purposive and snowball sampling from an online parenting community. The data were collected through in-depth interviews from November 14 to 28, 2022. Participants were interviewed face-to-face or via phone or online platform, according to their choice. Colaizzi's phenomenological qualitative research method was applied to analyze the meaning of the participants' experience. Results: Five theme clusters and fourteen themes were identified from the data. The five theme clusters are as follows: (1) Shortcomings of the childbirth and postpartum care system I learned through my experience; (2) Government policies focusing on childbirth and child-rearing rather than postpartum recovery; (3) Driving force of postpartum recovery: Shared childbirth process; (4) Childcare on my own; and (5) Conflicted between being a stay-at-home mom and a working mom under inadequate maternity protection policies. Conclusion: Despite postpartum support from the government that was perceived as inadequate, first-time mothers regained confidence and motivation for parenting with the help of family, peers, and social networks. First-time mothers need support from professionals and reliable online communities for postpartum recovery and parenting.

다지점 가진을 고려한 장경간 사장교의 비선형시간이력해석 (Nonlinear Time History Analysis of Long Span Cable-Stayed Bridge Considering Multi-Support Excitation)

  • 본 논문에서는 다지점 가진 시 장대교량에 대한 지진응답해석을 수행하고 설계상의 다양한 요구에 유연하게 대처하기 위해서 다지점 가진 해석에 필요한 비선형시간이력해석 알고리즘(영향계수법)을 제안하고, 이를 신뢰성있는 비선형 유한요소해석 프로그램(RCAHEST)에 추가하였다. 동일한 유한요소모델에 대해 범용 유한요소해석 프로그램 SAP2000의 Multi-support Excitation 기능을 이용하여 연구에서의 결과에 대한 비교 검증을 수행하였다. 이 연구결과를 바탕으로 인천대교에 대해서 유한요소모델링을 실시하고 다지점 가진을 고려한 비선형시간이력해석을 수행하였다. 수평변위응답의 분석 결과 시간지연이 늘어날수록 최대 수평변위가 줄어드는 것을 확인할 수 있었다. 또한 입력지진파의 최대가속도를 단계적으로 증가시키며 극한해석을 수행하여 대상 교량의 사용성을 평가하였다.

스테레오 영상에서의 적응적 영역 가중치 매칭의 효율적 근사화 방법 (An Efficient Approximation method of Adaptive Support-Weight Matching in Stereo Images)

  • 영역기반 스테레오 매칭 분야에서 최근 인간의 시각체계(Human Visual System)에 기반하여 영역내의 밝기값과 거리값에 따라 적응적으로 가중치를 부여하는 적응적 영역 가중치(Adaptive Support-Weight, ASW) 방법이 좋은 매칭 결과를 보이고 있다. 그러나 이 방법은 좋은 매칭 결과에 비해서 많은 연산비용을 필요로 하게 되고, 매칭의 실시간 시스템화에 큰 장애요소로 작용한다. 이에 Bilateral filter 수식으로 근사화 후 Integral Histogram 기법을 적용하여 영역 윈도우의 크기에 상관없이 상수 시간 O(1) 내에 매칭을 수행하는 연구가 진행되었다. 하지만 이 방법은 근사화 과정에서의 원 ASW 수식을 왜곡하기 때문에 매칭 정확도의 손실을 가져오게 된다. 본 논문에서는 적응적 영역 가중치 알고리즘의 매칭 정확도를 유지하면서 적응적 영역 가중치 알고리즘의 계산 비용을 줄이는 알고리즘을 제안한다. 이를 위해 영역내의 픽셀을 그룹화하여 근사화된 매칭을 수행하는 Sub-Block 방법과 영상의 에지 정보에 따라 적응적으로 시차 탐색 범위를 조정하는 방법을 제안한다. 결과적으로 제안된 기법은 기존 방식보다 좋은 매칭 정확도를 유지하면서도 효율적으로 계산 수행 시간을 줄이게 된다.

청소년의 인터넷 사용시간에 영향을 미치는 요인: 융복합적 함의를 중심으로 (Factors associated with internet use time among adolescents: focused on convergent implications)

  • 본 연구의 목적은 청소년의 인터넷 사용시간에 영향을 미치는 요인을 인구사회학적 관련 요인, 가구유형 관련 요인, 사회적지지 관련 요인, 건강 관련 요인을 통해 탐색하는 것이다. 청소년의 인터넷 사용시간에 영향을 미치는 요인을 살펴보기 위해 다중회귀분석을 실시하였으며 자료는 질병관리본부가 실시한 2014년 제10차 청소년건강행태 온라인 조사를 활용하였다. 연구분석결과, 인구사회학적 관련 요인에서는 남학생인 경우에, 가계의 수준이 낮은 경우에, 중학생인 경우에, 학업성적이 낮을수록, 가구유형 관련 요인에서는 소년소녀 가구, 한부모 가구, 조손가구, 다문화 가족, 북한이탈주민 가족의 경우에, 사회적지지 관련 요인에서는 가족지지, 친구지지, 선생님지지가 없는 경우에, 건강 관련 요인에서는 운동시간이 적을수록, 스트레스 수준이 높을수록, 주관적 행복감이 낮을수록 청소년의 인터넷 사용시간은 증가되는 것으로 나타났다. 이를 토대로 결론에서 청소년의 적절한 인터넷 사용을 위한 효과적인 개입전략을 모색할 때 고려해야 할 융복합적 함의를 논의하였다.

  • In this paper, We proposed an development method of application framework for using the precision time protocol(PTP) based on physical layer devices to synchronize clocks across a network with IEEE1588 capable devices. The algorithm was not designed as a complete solution across all conditions, but is intended to show the feasibility of such a for the PTP(Precision Time Protocol) based on time synchronization of heterogeneous network between devices that support in IEEE 1588 Standard application framework. With synchronization messages per second, the system was able to accurately synchronize across a single heavily loaded switch. we describes a method of synchronization that provides much more accurate synchronization in systems with larger networks. In this paper, using the IEEE 1588 PTP support for object-oriented modeling techniques through the 'application framework development Development(AFDM)' is proposed. The method described attempts to detect minimum delays, or precision packet probe and packet metrics. The method also takes advantage of the Tablet PC(Primary to Secondary) clock control mechanism to separately control clock rate and time corrections, minimizing overshoot or wild swings in the accuracy of the clock. We verifying the performance of PTP Systems through experiments that proposed method.

  • This paper considers the analysis of QoS(Quality of Service) routing mechanisms to support the NGIs(Next Generation Internets). NGIs are constructing high-speed IP layer networks to support all data services. To support real time multimedia services on NGIs, it is important to satisfy the required QoS parameters on networks. To support QoS requirements for NGI networks, new QoS routing methods are essential. In this paper, serveral new QoS routing algorithms are explained. Some problems for the high speed QoS routing will be explained and possible solutions are suggested.
