STATEMENT OF PROBLEM. Proper proximal contact is important for maintaining and stabilizing the dental arch. However, the proximal contact strength (PCS) is not a constant value and can be affected by a variety of factors. PURPOSE. This study examined the influences of postural changes on the posterior PCS. MATERIAL AND METHODS. Twelve adults with a normal occlusion and had not undergone prosthetic treatment or proximal restoration were participated in this study. A metal strip was inserted into the proximal surface and removed at a constant velocity. The contact strength was measured in every contact point between canine to second molar in both arches. The PCSs were obtained initially in the upright position, secondly in the supine position and finally in the upright position again. All measurements were repeated after a 2 hour period. Statistical analysis was carried out using the Friedman test (P < .05). RESULTS. Generally, a decrease in PCS occurred when the posture was changed from the initial upright to supine position, while it increased when the posture was changed from the supine to upright position. A significant change was observed in all areas except for between the canine-first premolar in the maxilla and between the first molar-second molar in the mandible areas. CONCLUSION. The posterior PCS, which dentists generally believe to be a static feature of occlusion, is affected significantly by posture.
Standing up from a supine and prone position is very important for physical independence. All kinds of patients have a bed rest or lying on floor without special care. Even though the patient had complete from illness. He must train the functional activities before discharge. There are many method for the functional activity training. Likewise, sing of reflex, voluntary movement for muscle strength increasing, but clinically ideal method is approaching with motor developmental stage. Supine and prone progression of proprioceptive neuromuscular facilitation have a ideal reason. That is reflex integratation, development of muscle tone, develop of motor control, dynamic and static, motor behavior and cognition.
The understanding the nature of occlusal tooth contacts of natural dentition is important for correct diagnosis and treatment of diseases developed in stomatognatic system. Several investigator have studied the distribution of tooth contacts in maximum intercuspation and have repored contact locations with respect to the tooth position. However, there are few report the variation of the occlusal contact point with change in each head position. This study analysed the number of occlusal contact point with change in each head position. 30 subject(male 17, female 13), who ahad natural occlusion and no symptoms of temporomandibular disorder, were selected. The numbers and patterns of tooth contact were recorded by silicone bite registration on stone model at four different head positions with head anguration gauge(from the supine to the upright position). The results obtained were as follows : 1. The numbers of total occlusal contact point on teeth increased to average 25, 29, 35, 42 points as head angulation was changed from the supine to the upright position against the ala-tragus line, and there was significant difference(P<0.05). 2. In the 19 subject(65%)of total 30 subject, the perforated point of the silicone bite indicated that the locus for the prime contact point moved mesially as the head angulation was changed from the supine to the upright position. 3. On the basis of the fact that the anterior occlusal contact point increase as head angulation changed from the supine to the upright position, we could find that the mandibular position is moved anteriorly.
It is important to find the effective position for cough and sputum clearance in respiratory physical therapy. The purpose of this study was to compare the changes in peak expiratory flow (PEF), forced expiratory volume in 1 second ($FEV_1$), and peak cough flow (PCF) related to functional level and measurement position in patients with Duchenne muscular dystrophy. Twenty one subjects were classified into three functional levels, and measurements was undertaken in three different measurement positions (upright sitting, $45^{\circ}$ reclining and supine). Vitalograph PEF/FEV DIARY was used to measure PEF and $FEV_1$, and Ferraris Pocket Peak was used to measure PCF. Mixed two-way analysis of variance and Bonferroni post-hoc test were used for statistical analysis. The results of the study were as follows: 1) Significant main effects for measurement position were found. 2) PEF was the highest in upright sitting, followed by $45^{\circ}$ reclining, and supine in order. 3) $FEV_1$ in upright sitting and $45^{\circ}$ reclining were significantly greater compared with that in supine. 4) PCF in upright sitting and $45^{\circ}$ reclining were significantly greater compared with that in supine. 5) No significant main effects for functional level were found in PEF, $FEV_1$, and PCF. 6) No significant functional level by measurement position interactions were found in PEF, $FEV_1$, and PCF. Therefore, it is concluded that upright sitting and $45^{\circ}$ degree reclining positions are recommended for effective cough and sputum clearance.
This study was performed to design a modified Y-View as an imaging method for the Y-View in supine position for patients who requires Y-View imaging for the diagnosis of shoulder impingement syndrome but having trouble for the positioning of patients complaining of shoulder pain. On the result of comparative analysis of the images obtained by changing the lateral-medio degree of X-ray tube into $35^{\circ}$, $40^{\circ}$, and $45^{\circ}$ while patient is in supine position, $40^{\circ}$ of X-ray tube in lateral-medio direction produced the most valuable image for the diagnosis by best describing the shapes of acromion, clavicle space, and coracoacromial arch. Therefore, patients who have difficulty in Y-View position to obtain Y-View image, modified Y-View can be applied as a useful alternative method. By this study, various applications not only in shoulder impingement syndrome but also in diverse omarthralgia diseases are expected.
To evaluate the diagnostic significance of the blood disappearance rate of $^{198}Au$-colloid and to evaluate the change of hepatic blood flow during position change from supine to erect, we measured the half time of blood disappearance rate of $^{198}Au$-colloid using external counting method in 94 normal persons and in 77 patients with various hepatic diseases. The results obtained were as follows: 1. In normal control, the mean blood disappearance half time of $^{198}Au$-colloid in supine position was $2.7{\pm}0.4$ minutes. There was no significant difference of hepatic blood flow in age and sex. 2. In acute hepatitis, chronic hepatitis and hepatic cirrhosis, the mean blood disappearance half times in supine position were $3.0{\pm}0.45,\;3.5{\pm}0.74,\;7.2{\pm}3.6$ minutes respectively. The hepatic blood flow of the patients with chronic hepatitis and hepatic cirrhosis were significantly decreased than that of normal control. 3. In the normal control and acute hepatitis, the decreaces of the hepatic blood flow during the position change from supine to erect were 27.7% and 22.6% respectively.
Purpose: This study examined how performing chin tuck exercises in supine, sitting, and standing positions affects changes in the muscle activity of the neck and shoulder. Methods: Seventeen men and three women with forward head posture participated in the study. The subjects performed the chin tuck exercise using a pressure biofeedback unit. After determining the ideal order for performing the chin tuck exercise in supine, sitting, and standing positions through randomized controlled trials, muscle activity was measured in the dominant-side sternocleidomastoid, scalenus anterior, upper trapezius, and levator scapulae. Muscle activity was measured three times in each muscle for each position, and the average of the three measurements was used for analysis. Results: In this study, the chin tuck exercise performed in a supine position reduced muscle activity of the sternocleidomastoid, scalenus anterior, upper trapezius, and levator scapulae significantly more than performing the exercise in either sitting or standing positions (p < 0.05). No significant difference in muscle activity was observed when the exercise was performed in a sitting position versus a standing position (p < 0.05). Conclusion: Performing a chin tuck exercise in a supine position is more effective for stabilizing the neck and shoulder than performing it in a sitting or standing position, as it reduces excessive tension and fatigue in the neck and shoulder.
Objective: This study was to identify the effect of pressure biofeedback applied in various postures that allow the application of abdominal drawing-in. Design: A cross sectional study. Methods: The study intended to compare changes in the thickness of abdominal muscles between different postures when abdominal drawing-in was performed using a pressure biofeedback unit in five postures and to compare differences in terms of measures such as the transverse abdominis's preferential activation ratio(PAR). Data measured from 30 healthy individuals were used for data analysis. A paired t-test and repeated measures analysis of variance was performed to compare the thickness of each abdominal muscle. Results: The transverse abdominis's and internal oblique's thickness showed statistically significant differences in all postures when abdominal drawing-in (p<0.05). In the comparison between the postures, statistically significant differences were observed between the positions of hook-lying and wall support standing and between supine and wall support standing and between hook-lying and sitting (p<0.05). In terms of the transverse abdominis's PAR in each posture, statistically significant differences were observed between hook-lying and quadruped, hook-lying and sitting, hook-lying and wall support standing, quadruped and supine, sitting and supine, as well as wall support standing and supine (p<0.05). Conclusions: When abdominal drawing-in using pressure biofeedback unit is performed for stabilization exercises, selecting and applying specific postures according to targeted muscles and the subject's functional ability will help provide a more efficient and accurate intervention.
PURPOSE. The aim of this study was to evaluate the reliability of implant stability measuring devices depending on the location of the implant and the position of the patient. MATERIALS AND METHODS. Six implants were installed in different dentate sextants of six artificial bone models. Implant stability was measured in three conditions of the bone model (without mounting on a phantom head, mounted on a phantom head in supine position, and mounted on a phantom head in upright position). A resonance frequency analysis device (Osstell) and two damping capacity analysis devices (Periotest and Anycheck) were used to measure implant stability. The values measured outside the phantom head were treated as controls, and the values inside the phantom head were compared using an independent t-test. RESULTS. Osstell showed different results in two of the six divisions in both the supine and upright positions compared to outside of the mouth (P < .05). Periotest showed different results in all six parts in the supine position and in five parts in the upright position compared to outside of the mouth (P < .05). While Anycheck showed different results in five areas in the supine position compared to outside of the mouth, it showed different results in only one area in the upright position (P < .05). CONCLUSION. In the difficult implant position for the operator to access, the implant stability measuring devices show less reliability. The accessibility of implant is greatly affected in the order of Osstell, Anycheck, and Periotest.
