Surgical Treatment of Tracheal Stenosis -Report of 3 Cases- (기관협착증 치험 3례)

  • Increasing success in the management of patients with severe respiratory failure by mechanical respirators has produced iatrogenic tracheal stenosis. And the surgical management of these lesions have provided a major field for tracheal reconstructive surgery. Recently we have experienced three cases of postintubation tracheal stenosis between December, 1985 and October, 1987 and successfully performed circumferential resection and end to end anastomosis of the trachea. The lesion of the first case which was located in the subcricoid level was resected about 2cm length with cervical incision. And the lesion of the second case located at the cuff site was also resected about 2.5cm length with cervical and median sternotomy incision. Also the lesion of the third case located at the stoma site was resected about 1.8cm length with cervical incision. The postoperative courses were uneventful but there was extubation difficulty in the third case because of stupor mentality and problem of secretion excretion. So we have observed the postoperative course after T-tube insertion.

Agenesis of the vulva in a Poodle dog

  • Vulvar agenesis in an eight-month-old toy poodle dog is described. Urinary incontinence and dysuria were the main clinical signs. The morphology of the urogenital system was assessed with contrast radiography. The constructers of lower urinary tract and reproductive system except for the vulva were normal features. Through episiostomy, a perineal stoma, resembling a vulva, was created, resulting in complete resolution of the clinical signs.

Clinical Analysis of Cases of Laryngotracheal Reconstruction Treated with End to End Anastomasis (단단문합술에 의한 기관재건의 임상적 고찰)

  • From 1988 to 1995, 59 patients with laryngotracheal stenosis were operated with resection of stenotic segment and end to end anastomosis in Seoul national university. Among these patients, 36 were tracheal stenosis, 17 were combined lesion of subglottis and trachea and 5 were subglottic stenosis. The success of procedure was determined by stoma closure or decannulation and an overall 89.8% success rate was achieved following 165 procedures. The success rate showed correlation with severity of stenosis and the number of procedures done was related to the site of stenosis. There was no mortality and granulation formation, unilateral vocal cord palsy were the common complication of end to end procedures.

Effect of Retinoic Acid on Fgf-8 Expression in Regenerating Urodele Amphibian limbs

  • In our previous study, we have shown that Fgf-8 is expressed in the basal layer of the apical epithelial cap (AEC) and in the underlying thin layer of mesenchymal tissue of the regenerating limbs of Mexican axolotl, Amby-stoma mexicanum. Our present RT-PCR data also demonstrate that Fgf-8 transcript is localized both in the mesenchymal and epidermal tissues. To understand the effect of retinoic acid (RA) on the expression of Fgf-8 in the regenerating axolotl limbs, RA was injected intraperitoneally at the dediffer-entiation stage of limb regeneration. The RA treatment caused 8 change in the Fgf-8 expression profile of the regenerating limbs. In RA-treated limbs, duration of Fgf-8 expression was prolonged and a high level of expression was maintained during dedifferentiation and blastema formation stages. These results suggest that Fgf-8 is an important molecule in the process of pattern duplication of regenerating salamander limbs evoked by RA treatment.

Plastic Reconstruction of Tuberculous Bronchostenosis with Rib Cartilage (결핵성 기관지협착 환자에 있어서 늑연골을 이용한 기관지성형술 치험 1례 보고-)

  • We experienced plastic reconstruction of tuberculous bronchostenosis with patient`s rib cartilage. He suffered from coughing and sputum for 2months, and was treated for pulmonary tuberculosis 10 years ago. In preoperative bronchoscopy, left main bronchus was fibrotic obstructive and LUL bronchus was severely destructed. After thoracotomy, we harvested the rib cartilage at the 6th rib, and designed semicircular and tubular graft. And then onlayed the graft over the longitudinal bronchotomy site by simple interrupted sutures with 4-0 Vicryl Postoperative course was good, coughing and sputum disappeared. In postoperative bronchoscopy, the patch graft was good in that position, and the internal diameter was sufficient, but the granuloma was found in the stoma of LUL bronchus. He was discharged without any other event.

  • A comparative study on 'Dang-gui' and 'Kang-whal' the two species of Angelica growing in Korea was carried out about the morphology and cytology. The somatic chromosome number of Cham dang-gui, Ill dang-gui, Badi-namul was found to be 2N=22. The somatic chromosome number of Kang-whal was found to be 2N=110, that of Chili kang-whal to be 2N=44, that of Mul kang-whal to be 2N=22. Stoma of species of Dang-gui were same in size. Those of Chili kang-whal and Kang-whal were strikingly larger than the other. Chili kang-whal and Kang-whal were verified to be polyploid in the genus Angelica.

  • Admirandus multicavus Belogurov and Belogurova, 1979, a new record of free-living marine nematode species belonging to family Oncholaimidae is reported based on the specimens collected from the intertidal sediments of Korea. Admirandus multicavus is characterized by the following unique characteristics: cephalic setae (1-2 ㎛ long), spicules (71-126 ㎛ long) and gubernaculum (15-31 ㎛ long), midventral preanal supplementary organ, three glandular duct openings of the Demanian system, and stoma length measuring twice the width. A comparative analysis of the biogeographical and ecological characteristics of Admirandus species is presented. DIC (Differential interference contrast) photomicrographs of the species are also provided herein. This report represents the first taxonomic analysis of the genus Admirandus Belogurov and Belogurova, 1979 discovered from the Korean seawaters.

Analysis of Ruminal Dry Matter and Crude Protein Digestibility on Major Roughage, Wormwood and Green Tea (주요 조사료원과 쑥, 녹차의 반추위 건물 및 조단백질 소화율에 대한 분석)

  • The comparative in vitro and in situ analysis were conducted to evaluate the rumen degradability and physical structure of domestic roughage as rice straw, timothy, alfalfa, wormwood and green tea. The feedstuffs incubated with rumen fluid and was used to determine gas production, microbial growth rate and pH changes in an in vitro experiment. The gas production was increased during incubation times and was significantly(p<0.05) lower in green tea than other feedstuffs. The microbial growth rate in the feedstuffs was increased during incubation times. However, microbial growth rate was significantly(p<0.05) lower in wormwood and green tea than other feedstuffs. Ruminal pH was decreased during incubation times, and timothy was the lowest, and rice straw was the highest among feedstuffs. The disappearance rate of dry matter(DM) and crude protein(CP) in all feedstuffs were increased during incubation times and green tea was the highest(p<0.05) compared with other feedstuffs. In effective degradability, when rumen out-flow rate was assigned to 4%, wormwood showed the highest in DM, and alfalfa was the highest in CP. Whereas, green tea was the highest in both in situ DM and CP degradability. Many cilia on the surface and stoma of wormwood and stoma in green tea were observed by scanning electron microscopy. Microbes breaked down the cilia at the beginning and then degraded the surface in wormwood. In case of green tea, microbes attached to stoma. Therefore, wormwood and green tea have a potential value as ruminal feed stuffs.

Effects of Artificial Asian Dust on Photosynthesis, Respiration, Growth and Stoma in Five Tree Species (인공황사 처리가 몇 수목의 광합성, 호흡, 생장 및 기공에 미치는 영향)

  • Artificial Asian dust was treated in this study to compare and determine its effect on growth, photosynthesis, respiration and stomata of 5 species trees. Sophora japonica's growth and respiration using artificial Asian dust treatment which is 10 times of average contents of Asian dust: $5,000{\mu}g{\cdot}m^{-2}$ decreases after 3 weeks while no significant difference in other species. The maximum photosynthesis of Sophora japonica and Betula pendula decreased significantly after 3 weeks and 6 weeks, respectively, using artificial Asian dust treatment which is average contents of Asian dust: $500{\mu}g{\cdot}m^{-2}$. At $5,000{\mu}g{\cdot}m^{-2}$ treatment, photosynthesis of both species decreased just after treatment of artificial Asian dust. Quercus acutissima, Quercus mongolica and Acer triflorum have no significant differences. Dust was found inside the Sophora japonica's stomatas under heavy dust treatment when viewed under the light electron microscope. By scanning electron microscopy, the higher rate of stomatal opening of Sophora japonica and Betula pendula was recognized and the size of both stomatas was bigger than other species. This result means that Asian dust has an influence in decreasing the photosynthesis, respiration and growth of some species especially if the stomatas are large.

Effects on Quality of Life in Patients with Neurogenic Bladder treated with Clean Intermittent Catheterization: Change from Multiple Use Catheter to Single Use Catheter

  • Purpose: To evaluate changes in quality of life (QoL) in patients with neurogenic bladder treated with clean intermittent catheterization (CIC), who changed from a multiple use catheter (MUC) to single use catheter (SUC). Methods: The Modified Intermittent Self-Catheterization Questionnaire (mISC-Q) was used to determine potential changes in patients' QoL as a result of switching from MUC to SUC. The mISC-Q consists of questions within four categories: ease of use, convenience, discreetness, and symptomatic benefit. Answers were graded as Strongly agree (+2), Agree (+1), Not sure (0), Disagree (-1), and Strongly disagree (-2). Overall patient QoL, as well as by sex, disease (presence of augmentation cystoplasty), and catheterization route (via urethra or urinary diversion), were analyzed. Results: Thirty-eight patients (21M:17F; mean age: $21.7{\pm}5.3y$) submitted questionnaires. For ease of use, SUC was significantly better than MUC (score: 0.364, P=0.002) in all patients. Patients with catheterization via the urethra showed significant favor for SUC in ease of use (score: 0.512, P<0.001) and convenience (score: 0.714, P=0.011), but patients with catheterization via the abdominal stoma of urinary diversion gave negative scores in all categories, though no categories were significant. Conclusion: This study suggested that changing from MUC to SUC may lead to improvements in QoL, especially regarding ease of use. This benefit was clearly found in patients with catheterization via urethra rather than abdominal stoma of urinary diversion.
