Objectives : This study was to evaluate the effectiveness of Bee Venom acupuncture therapy on patients with cervical spondylosis. Methods: We investigated 29 cases of patients with cervical spondylosis. We divided patients into two groups: Bee venom Acupuncture was performed at one group, and the other group we did acupuncture therapy. To evaluate the effectiveness of treatment applied for two groups, we used visual analog scale(VAS) and neck disability index(NDI). We compared the VAS and NDI score of two groups statistically. Results : As a result of evaluation by using visual analog scale(VAS) and neck disability index(NDI), treatment score at final was significantly different from that at the baseline in each groups. Treatment at final, Bee venom acupuncture therapy group showed significant difference on visual analog scale(VAS) and neck disability index(NDI) compared with acupuncture therapy group. Conclusions : Bee Venom acupuncture therapy can be used with acupuncture therapy for highly effective in the patient with cervical spondylosis.
Objective: The aim of this study was to objectively evaluate sensory disturbance in cervical spondylosis using grip force and investigate the relationship between the grip force and upper extremity function. Design: Cross-sectional study. Methods: Eleven cervical spondylosis patients with paresthesia conducted grip and lift tasks using a precision grip with the tips of the thumb and index finger on either side. The sum of the grip force used during the first four seconds was calculated and defined as the total grip force. The cutaneous pressure threshold of the fingers, the pinch power, the grip power and three subtests of the Simple Test for Evaluating Hand Function (STEF) were also assessed. Correlations between the total grip force and cutaneous pressure threshold, pinch power, grip power, and STEF subtest times were evaluated. Results: We found that the total grip force correlated with the cutaneous pressure threshold (p<0.05). Moreover, the total grip force of the dominant thumb correlated with the results of the three STEF subtests (p<0.05). There were no significant correlations between the total grip force and pinch/grip powers. Conclusions: We found that the total grip force correlated with cutaneous pressure threshold and upper extremity function. The results suggest that the total grip force could serve as an objective index for evaluating paresthesia in cervical spondylosis patients, and that the impaired ability of the upper extremity function is related to grip force coordination.
Objectives : To study that lateral epicondylitis(tennis elbow) is related with cervical spondylosis. Objective : The purpose of this study was to investigate the prevalence of and the relevant risk factors for lumbar spondylosis (LS) among middle-aged and elderly rural Korean residents and to explore the association between radiographic LS and lower back pain (LBP) in relation to age and gender. Methods : This community-based, cross-sectional study evaluated 1512 subjects with available radiograph. The prevalence of LBP was obtained using a questionnaire and disability resulting from LBP was measured using a validated Korean version of the Oswestry disability index (ODI). In lumbar spine radiographs, vertebral levels from L1/2 to L4/5 were evaluated for the presence of osteophytes and joint-space narrowing (JSN), and Kellgren-Lawrence (KL) grading was applied. Results : Of 4261 subjects aged 40-79 years, data from 1512 subjects were included. The prevalence of radiographic LS indicated by grade ${\geq}2$ osteophytes and JSN were 53.9 and 15.8%, respectively. Seventy-three percent of subjects had KL grade ${\geq}2$ spondylosis and LBP was present in 36.5% of subjects. Although LS was more common among males, the prevalence of LBP was higher among females. Age, male gender and history of hand or knee arthritis were risk factors for LS. LS was significantly associated with LBP mostly among females over 60 years old and correlated with the ODI after adjusting for age and gender. Conclusion : Our study among rural Korean residents revealed a high prevalence of LS and LBP. The association between LS and LBP was observed mostly among females and LS was significantly correlated with the severity of back pain.

Objective : The aim of this study was to identify the susceptibility genes responsible for lumbar spondylosis (LS) in Korean patients. Methods : Data from 1427 subjects were made available for radiographic grading and genome wide association studies (GWAS) analysis. Lateral lumbar spine radiographs were obtained and the various degrees of degenerative change were semi-quantitatively scored. A pilot GWAS was performed using the AffymetrixGenome-Wide Human single-nucleotide polymorphisms (SNPs), 500K array. A total of 352228 SNPs were analyzed and the association between the SNPs and case-control status was analyzed by stepwise logistic regression analyses. Results : The top 100 SNPs with a cutoff p-value of less than $3.7{\times}10^{-4}$ were selected for joint space narrowing, while a cutoff p-value of $6.0{\times}10^{-4}$ was applied to osteophytes and the Kellgren-Lawrence (K-L) osteoarthritis grade. The SNPs with the strongest effect on disc space narrowing, osteophytes, and K-L grade were serine incorporator 1 (rs155467, odds ratio [OR]=17.58, $p=1.6{\times}10^{-4}$), stromal interaction molecule 2 (STIM1, rs210781, OR=5.53, $p=5{\times}10^{-4}$), and transient receptor potential cation channel, subfamily C (rs11224760, OR=3.99, $p=4.8{\times}10^{-4}$), respectively. Leucine-rich repeat-containing G protein-coupled receptor 4 was significantly associated with both disc space narrowing and osteophytes (rs1979400, OR=2.01, $p=1.1{\times}10^{-4}$ for disc space narrowing, OR=1.79, $p=3{\times}10^{-4}$ for osteophytes), while zinc finger and BTB domain containing 7C was significantly and negatively associated with both osteophytes and a K-L grade >2 (rs12457004,OR=0.25, $p=5.8{\times}10^{-4}$ and OR=0.27, $p=5.3{\times}10^{-4}$, respectively). Conclusion : We identified SNPs that potentially contribute to the pathogenesis of LS. This is the first report of a GWAS in an Asian population.
Objective : In the present study, we evaluated the effect, safety and radiological outcomes of cervical hybrid surgery (cervical disc prosthesis replacement at one level, and interbody fusion at the other level) on the multilevel cervical degenerative disc disease (DDD). Methods : Fifty-one patients (mean age 46.7 years) with symptomatic multilevel cervical spondylosis were treated using hybrid surgery (HS). Clinical [neck disability index (NDI) and Visual Analogue Scale (VAS) score] and radiologic outcomes [range of motion (ROM) for cervical spine, adjacent segment and arthroplasty level] were evaluated at routine postoperative intervals of 1, 6, 12, 24 months. Review of other similar studies that examined the HS in multilevel cervical DDD was performed. Results : Out of 51 patients, 41 patients received 2 level hybrid surgery and 10 patients received 3 level hybrid surgery. The NDI and VAS score were significantly decreased during the follow up periods (p<0.05). The cervical ROM was recovered at 6 and 12 month postoperatively and the mean ROM of inferior adjacent segment was significantly larger than that of superior adjacent segments after surgery. The ROM of the arthoplasty level was preserved well during the follow up periods. No surgical and device related complications were observed. Conclusion : Hybrid surgery is a safe and effective alternative to fusion for the management of multilevel cervical spondylosis.
Although Chronic tension-type headache(CTTH) is one of the most common symptom in primary headache, there is no definite mechanisms. But muscular factors and psychological factors is supposed to be related with CTTH according to many other studies and pressure pain threshold(PPT) is used to measure muscular factors. Methods 1. We performed this study with 63 patients fulfilling the International Headache Societ criteria for chronic tension-type headache and 20 healthy control group and measured the PPT of temporal muscle and trapezius muscle. 2. We investigated the correlation between clinical characteristic and PPT. 3. Each of the CTTH groups and Control group is divided to four group again - HNP, Spondylosis, Sprain, Normal according to Cervical spine X-ray. Results 1. The PPT of temporal muscle and trapezius muscle in the CTTH is significantly lower than that of Control. 2. In CTTH group, the PPT has significant positive relation with duration of headache and continued time of headache. And the PPT has significant inverse relation with Frequency of headache and Level of headache. 3. In CTTH group, spondylosis group has the highest PPT and normal group is second. And there are significant difference between spondylosis group and the others. Conclusion : We found that PPT is strongly significant to measure muscular factor in CTTH.
Objectives : Recently work-related musculoskeletal disorders have increased in various workplaces and many reports have been confirming this, but a comparative study on incidence of spinal disease by occupation has not been established yet. So this study was designed to clarify the distinction on incidence of spinal disease between white and blue collar workers. Methods : The subjects who visited with spinal disease to Ja-Seng oriental hospital from January to April in 2020 were selected and divided into white collar workers(n=844) and blue collar workers(n=333). We counted the number of patients with each spinal disease in both groups and carried out comparative analysis in five items(herniated nucleus pulposus(HNP), stenosis, spondylosis, sprain, etc). All data were analyzed by chi-square test. Results and Conclusions : Whit collar workers had higher incidence in HNP(p=0.030), sprain(p=0.016),etc(p=0.035) than blue collar workers, while blue collar workers had higher incidence in stenosis(p=0.002). But we think that the confidence is low in stenosis because the number of cases are very small. There were no significance between two groups with spondylosis.
Posterior cervical foraminotomy is an attractive therapeutic option in selected cases of cervical radiculopathy that maintains cervical range of motion and minimize adjacent-segment degeneration. The focus of this procedure is to preserve as much of the facet as possible with decompression. Posterior cervical inclinatory foraminotomy (PCIF) is a new technique developed to offer excellent results by inclinatory decompression with minimal facet resection. The highlight of our PCIF technique is the use of inclinatory drilling out for preserving more of facet joint. The operative indications are radiculopathy from cervical foraminal stenosis (single or multilevel) with persistent or recurrent root symptoms. The PCIFs were performed between April 2007 and December 2009 on 26 male and 8 female patients with a total of 55 spinal levels. Complete and partial improvement in radiculopathic pain were seen in 26 patients (76%), and 8 patients (24%), respectively, with preserving more of facet joint. We believe that PCIF allows for preserving more of the facet joint and capsule when decompressing cervical foraminal stenosis due to spondylosis. We suggest that our PCIF technique can be an effective alternative surgical approach in the management of cervical spondylotic radiculopathy.
Objective : We investigated association between excessiveness and deficiency of the visceral and twelve merdians and low back pain, by checking Yangdorak. Methods : Clinical studies were done 62 patients who were treated with low back pain to Dept. of Acupuncture & Moxibustion, Hospital of Oriental Medicine in Semyung University from August 2, 2002 to August 20, 2002. We divided low back pain patients into lumbar vertebra strain, herniated nucleous pulposus(H.N.P.) degenerative spondylosis(D.J.D) and tested the potentiality of skin resistance(Yangdorak) to them. Results: 1. H.N.P. groups were more than another groups in comparing with the States over Physiological Limits and the and excessiveness of merdians. 2. In degenerative spondylosis groups, excessiveness of the F3(kidney) was to be superior. In H.N.P. groups deficiency of the H5(triple energiger) was to be superior. In lumbar vertebra strain groups, deficiency of the H5(triple energiger) and excessiveness of the F2(Liver) was to be very superior. Conclusions: We could investigate the relationship of the excessiveness and deficiency of the visceral and twelve merdians to low back spain patients by checking Yangdorak. Specially, Deficiency of the H5(triple energiger) and H4(Small intestine) may be helpful in diagnosis H.N.P..
