A Clinical Research of Atopic Dermatitis treated by Yeongyuseungmatang in cosmetics (연교승마탕(連翹升麻湯)이 함유된 한방화장품이 아토피 피부염에 미치는 임상 연구)

  • Objective : This clinical research was conducted to test patients with Atopic Dermatitis by external application with Yeongyuseungmatang in cosmetics. Methods : We gave scores to 33 patients who visited the Dept. of Oriental Medical Opthalmology & Otolaryngology & Dermatology of Semyung Jecheon Oriental Medical Hospital from April 17th, 2008 to June 12th. Seventeen patients were treated with Yeongyuseungmatang(Experimental group) and sixteen patients were treated with Atopico skin care cream(Control group) for 4weeks. We observed change of total IgE, eosinophil count, Skin Temperature, Transepidermal Water Loss(TEWL), Skin Hydration and Skin pH. Also Clinic Index of Atopic Dermatitis(SCORAD Index) and global assesment of efficacy were used to evaluate the effects of Yeongyuseungmatang. Statistical analysis was performed by using frequency analysis and descriptive analysis. Statistical significance was achieved if the probability was less than 5%(p<0.05). Results : 1. After 4 weeks of external application treatment, SCORAD Index in Yeongyuseungmatang was significant statistically decreased compared with control group. 2. After 4 weeks of external application treatment, eosinophil count in Yeongyuseungmatang was significant statistically decreased compared with control group but total IgE was very increased both Yeongyuseungmatang and control group. 3. Statistically, Yeongyuseungmatang and control group didn't showed significant effect on Skin Temperature was measured by Thermometer SM815. 4. Transepidermal Water Loss(TEWL) in Yeongyuseungmatang was significant statistically decreased compared with control group measured by Tewameter TM300. 5. Skin Hydration in Yeongyuseungmatang was significant statistically increased compared with control group measured by Corneometer CM825. 6. Statistically, Yeongyuseungmatang and control group didn't showed significant effect on Skin pH was measured by Skin-pH-Meter PH905. 7. After 4 weeks of external application treatment, Yeongyuseungmatang showed significant effect on global assesment of efficacy. Conclusion : Considering the above results, we can speculate that cosmetics with Yeongyuseungmatang has some therapeutical effects in mitigating the symptoms of atopic dermatitis.

Effect of Aromatherapy on Skin Xerosis and Pruritus in Patients Undergoing Maintenance Hemodialysis (향 요법이 혈액투석 환자의 피부건조와 소양증에 미치는 효과)

  • This study was designed to investigate the effect of aromatherapy on skin xerosis and pruritus in patients undergoing maintenance hemodialysis. Twenty one subjects of this study were selected from St. Paul's Hosipital in Seoul. All the subjects were received the mineral oil massage at the arm without fistular three timesper week for 4 weeks. After 2 weeks period of wash-out, the subjects were received the aromatherapy of lavender and tea tree essence oil in the same way. This study was carried out from March 20 to June 13, 1998. Pruritus score, skin pH and stratum corneum hydration were measured before and after each treatment. But, biochemical parameters were measured before the treatment of the mineral oil massage, after the treatment of the mineral oil massage and the aromatherapy. Data of this study were analyzed by paired t-test, repeated measures ANOVA, Bonferroni multiple comparisons and Pearson correlation coefficient. The results were as follows ; 1. Pruritus score was significantly decreased after the aromatherapy, while no significant change after the treatment of the mineral oil massage. Therefore, there was a significant difference in the priritus score between the two treatments. 2. Stratum corneum hydration was significantly increased after the aromatherapy, while no significant change after the treatment of the mineral oil massage. Therefore, there was a significant difference in the stratum corneum hydration between the two treatments. 3. Skin pH was significantly increased after the treatment of the mineral oil massage, while no significant change after the aromatherapy. Therefore, there was no significant difference in the skin pH between the two treatments.4. After the aromatherapy, the serum calcium was significantly increased. Whereas the serum parathyroid hormone intact was significantly decreased compared with the treatment of the mineral oil massage. But the level of the serum Ca and PTH-intact were within the normal range. 5. Stratum corneum hydration was decreased corresponding to the duration of hemodialysis, while pruritus score and skin pH showed no change corresponding to the duration of hemodialysis and the age of the subjects. The correlation of pruritus score on skin pH, stratum corneum hydration and biochemical parameters was not significant. In conclusion, this findings indicate that aromatherapy may be effective in decreasing skin xerosis and pruritus score in uremic pruritus patients undergoing maintenance hemodialysis.

Inhibition of Melanin Synthesis by Enhanced Cytosolic Delivery of N-glycosylation Inhibitors Using pH-Sensitive Nano-carrier (pH 감응형 나노입자를 이용한 멜라닌 합성저해 연구)

  • Inhibition of the early N-glycosylation process in the endoplasmic reticulum prevents the activation of tyrosinase, a key enzyme for melanin biosynthesis. This work aims at evaluating the increased activity of N-glycosylation inhibitors in vitro b, employing a nano-sized pH-sensitive liposome as a delivery carrier. Melexsome, a pH-sensitive nano carrier loaded with glycosylation inhibitos, was prepared by the hydration method with phospholipids and cholresterol-based amphiphiles. Inhibitory effects of Melexsome on the N-glycosylation process were evaluated by EndoH & PNGaseF digestion and the western blotting. Melanin synthesis was also monitored after treatment with Melexsome Interestingly, Melexsome effectively increased the efficacy of N-glycosylation inhibitors. Melexsome was also much more efficiently translocated into the cytoplasm as observed in CLSM. These results demonstrated that the amphiphilic lipid-based pH-sensitive nano-carriers could be, used as an efficient delivery system for N-glycosylation inhibitor to enhance the effects of skin whitening cosmetics.

The Comparison of Skin Physical Parameters in Asian Women (아시아 여성의 피부 물리적 변수들의 비교)

  • Skin properties are influenced by a variety of internal or external factors. This study was conducted to investigate and compare the skin characteristics in Asian women. We evaluated values of skin physical parameters, and then assessed the correlations among the parameters, and regional differences in 1,500 Asian subjects in seven countries (Malaysia, Indonesia, Vietnam, Thailand, India, China, and South Korea). The values of transepidermal water loss (TEWL), pH, skin hydration, sebum content, skin sensitivity, elasticity, crow's feet wrinkles, and skin brightness were measured. New Delhi's women were observed to have a lower level of hydration in stratum corneum, sebum content, stinging scores, and darker skin color than others. Furthermore, their skin surface pH on all sites is the highest among seven countries. Vietnamese had the lowest pH values in skin surface. Korean and Chinese had generally higher elasticity and fewer wrinkles. Both the skin hydration and TEWL had positive correlations with the stinging test, and there was a negative correlation between wrinkles and stinging test. These results represent that there was no clear trend related to the area or region. Skin physical parameters should be studied along with understanding of regional differences and various factors (skin care habits, lifestyle and personal sensitivity).

Targeted and sustained delivery of hydrocortisone to normal and stratum corneum-removed skin without enhanced skin absorption using a liposome gel

  • Judging from hydrocortisone concentration in dosing area, the extent of absorption was reduced in the liposome-gel formulation. However, higher and sustained skin concentrations of hydrocortisone were achieved for the liposome-gel as compared to the ointment. Drug concentration in both viable and deep skin reached its maximum within 0.5 h after application of both formulations to both skin types. Drug concentrations in both skins from the ointment declined with time, while those from the liposome-gel were greatly sustained. The sustainment by the liposome-gel was more remarkable in the viable skin than in the deep skin. Drug concentration in the viable skin could be maintained at a nearly constant level for over 8 h by applying the liposome-gel. As a result, a 5-fold higher viable skin drug concentration was obtained from the liposome-gel than from the ointment at 8 h after the application to the SC-removed skin. However, the plasma concentration of hydrocortisone at 4 h from the liposome-gel was only one-fourth (p<0.01) the value from the ointment when the drug was applied to the SC-removed skin, consistent with. the lower urinary (one-third, p<0.05) and fecal (one-half, p<0.05) excretion. Conclusions : Retarded diffusion of the drug from the skin to the systemic blood stream appears to be a potential factor in the sustained skin concentration of hydrocortisone from the liposome-gel, Interaction of hydrocortisone in the skin with phosphatidylcholine, a component of the liposomes and skin, may well be a factor in retarding the diffusion of the drug in the skin.

Effect of pH-Sensitive P(MAA-co-PEGMA) Hydrogels on Release and Stability of Albumin (pH 감응성 P(MAA-co-PEGMA) 수화젤이 알부민의 방출과 안정성에 미치는 영향)

  • pH-sensitive P(MAA-co-PEGMA) hydrogel particles were prepared and their feasibility as smart delivery carriers for cosmetic ingredients was evaluated. P(MAA-co-PEGMA) hydrogel particles having an average size of approx. $2{\mu}m$ were synthesized via dispersion photopolymerization. There was a drastic change in the swelling ratio of P(MAA-co-PEGMA) particles at a pH of around 5 due to the ionization of MAA in the hydrogel and as the amount of MAA in the hydrogel increased, the swelling ratio increased at a pH above 5. The P(MAA-co-PEGMA) hydrogel particles showed a pH-sensitive release behavior. Thus, at pH 4 almost none of the albumin permeated through the skin while at pH 6 relatively high skin permeability was obtained. The albumin loaded in the P(MAA-co-PEGMA) hydrogel particles was hardly degraded in the presence of pepsin and its stability was maintained.

Physicochemical Characteristics of Filefish and Cod Skin Collagen (말쥐치피 및 대구피 콜라겐의 물리화학적 특성)

  • The collagens from filefish (Novoden modestus) and cod (Gadus macrocephalus Tilesius) skin were isolated and their physicochemical properties were investigated. Glutamic acid, hydroxyproline, valine and phenylalanine in the filefish skin collagen (FSC) were presented at higher levels than those of cod skin collagen (CSC), but the contents of glycine, proline and serine were contrary. The content of essential amino acids of FSC (265 residues/1000 residues) was higher than CSC (229 residues). The solubilities of both collagens were the lowest at pH 7.0, but precipitously increased at acid zone(below pH 5.0). FSC has lower viscosity than CSC. Furthermore, while the viscosities of both collagens were the lowest at pH 7.0, the viscosities of FSC and CSC were the highest at pH 4.0 and pH 2.0, respectively. The denaturation temperature of $FS(25^{\circ}C)$ was higher than $CSD\;(17^{\circ}C)$. The free hydrophobic residue contents of FSC and CSC tended to increase till $60^{\circ}C,\;and\;50^{\circ}C$ respectively, and to decrease thereafter. Hydration capacities of both collagens were the lowest at pH 7.0, and CSC had the superior hydration capacity to FSC. In addition, emulsifying and emulsifying stability of CSC was also superior to FSC.

Dietary effect of green tea extract on epidermal levels of skin pH related factors, lactate dehydrogenase protein expression and activity in UV-irradiated hairless mice (자외선 조사와 병행된 녹차 식이 공급이 무모생쥐의 표피 산도 관련 인자 함량 및 젖산 탈수소 효소의 단백질 발현과 활성에 미치는 영향)

  • Purpose: Skin pH, an indicator of skin health, is maintained by various organic factors, which include lactate, free amino acid (FAA), and free fatty acid (FFA). As skin ages or with illness, skin pH becomes less acidic, and functional food has been developed to maintain the acidic pH of skin. In this study, we determined the dietary effect of green tea extract (GTE) on skin pH of photo-aged mice, as measured by epidermal levels of lactate, FAA, and FFA. The protein expression and activity of lactate dehydrogenase (LDH), an enzyme of pyruvate reduction for lactate generation, was further determined. Methods: Albino hairless mice were fed a control diet (group UV+) or a diet with 1% GTE (group GTE) in parallel with UV irradiation for 10 weeks. A normal control group was fed a control diet without UV irradiation for 10 weeks (group UV-). Results: Skin pH was higher (less acidic) in group UV+ than in group UV-. In parallel, epidermal levels of lactate and FFA, as well as of LDH protein expression and activity, were reduced in group UV+. Dietary supplementation of GTE (group GTE) reduced skin pH to similar to the level of group UV-, and inversely increased epidermal levels of lactate, LDH protein expression and activity, but not of FFA. Although epidermal levels of FAA were similar in groups UV- and UV+, it was increased in group GTE to a level higher than in group UV-. In further analysis of major FFA, epidermal levels of palmitic acid [16:0], oleic acid [18:1(n-9)], and linoleic acid [18:2(n-6), but not of stearic acid [18:0] in group GTE were similar to or lower than those in group UV+. Conclusion: Dietary GTE normalized skin pH with increased levels of lactate and FAA, as well as with increased protein expression and activity of LDH in the epidermis of UVB irradiated hairless mice.

Skin Permeation of Indomethacin from Gels (겔제제로부터 인도메타신의 피부투과)

  • In order to reduce the systemic side effects and the gastrointestinal irritation of indomethacin following its oral administration, the drug was formulated as a transdermal gel using poloxamer 407. In vitro diffusion cells fitted with excised rat skins were used to evaluate the effects of formulation variables on skin permeation of indomethacin from poloxamer gels. The formulation variables were the concentrations of indomethacin, poloxamer 407 and ethanol, and the gel pH. The increase of the drug amount in the gel from 0.5% to 2.0% induced a direct but nonlinear increase in the skin permeation rate of indomethacin. The increase of poloxamer concentration from 17.5% to 25% in the gel resulted in a decrease of skin permeation rate of indomethacin, which was due to a reduction in the amount of free drug molecules available for permeation through skin by entrapping more drug molecules within the micelles formed by poloxamer. The increase of ethanol concentration from 10% to 20% in the gel resulted in a linear increase of permeation rate of indomethacin through skin, possibly due to the penetration enhancing effect of ethanol. The skin permeation of indomethacin was substantially influenced by the gel pH, exhibiting a maximum at pH 4.
