Intravascular papillary endothelial hyperplasia (IPEH) has appeared in the literature under a variety of names, including Masson's tumor, Masson's hemangioma, and Masson's pseudoangiosarcoma. It is a benign lesion of the skin and subcutaneous tissue characterized by reactive proliferation of vascular endothelial cells with papillary formations. The clinical picture is not specific and the lesion resembles malignant angiosarcoma clinically and histopathologically. Therefore, it is often mistaken for angiosarcoma and a group of other benign and malignant vascular lesions. We report on a case of IPEH adherent to peripheral nerve treated with operative excision.
Cutaneous horn is a morphologic designation for a projectile, conical, dense hyperkeratotic nodule that resembles the horn of an animal. The lesion varies in size from only a few millimeters to several centimeters, in color(white or yellowish) and in form (straight, curved, or twisted). It arises from a wide range of epidermal lesions, which include benign lesions, premalignant lesions and malignant lesions. An 83-year-old women came to our clinic with a giant cutaneous horn on the right chin and a small horn on the left upper eyelid. The patient had no palpable cervical lymph node. A wide elliptical skin incision was made and the horn was totally excised. In pathology, the giant cutaneous horn on the right chin revealed a moderately differentiated squamous cell carcinoma with subcutis invasion at its base. "Giant cutaneous horns" have often been associated with invasive squamous cell carcinoma. Cutaneous horns are common lesions usually found on the face, rarely larger than 2 cm. As large cutaneous horns are often associated with underlying malignancy, histopathologic examination of the base of the lesion is necessary to rule out carcinoma and full excision is recommended.
Xanthelasma palpebrarum is the most common cutaneous xanthoma found on the medial side of the eyelid. The typical lesion is usually a flat and yellowish plaque on the skin. However, we report on a unique case of intramuscular xanthoma found during blepharoplasty for the correction of ptosis. A 53-year-old male patient visited our department with a complaint of a ptotic eyelid. He was concerned about the cosmetic appearance and the uncomfortable feeling while opening his eyes, and wanted these problems to be solved. A yellowish plaque of about $0.3{\times}0.3cm$ in size was found in the orbicularis oculi muscle during the surgery. The lesion was excised and xanthelasma was confirmed with biopsy. We have found this specific case of xanthelasma palpebrarum in the only muscle. Therefore, a careful approach to clinical and histologic examination and imaging is required for patients with these lesions.

Psoriasis is a common chronic inflammatory skin disease that is histopathologically characterized by abnormal differentiation and hyperproliferation of keratinocytes, infiltration of T lymphocytes, and alternations of the dermal vasculature. Pulsed dye laser (PDL) is commonly used to treat cutaneous vascular lesions, and it has been studied for the treatment of localized psoriatic lesions. A 30-year-old female patient with refractory psoriasis on her forehead was treated using PDL. After two sessions, the size of the psoriatic lesion increased, which might have occurred due to koebnerization. As the patient continued to receive PDL treatment, the lesion showed significant improvement with no recurrence on the one-year follow-up. We present here a case of refractory psoriasis treated with PDL and demonstrates an association between a delayed therapeutic effect and the Koebner phenomenon.
Facial massages are frequently performed to achieve a feeling of freshness, rejuvenation, skin tightening, and delayed onset of wrinkles. However, vigorous massages can induce unexpected symptoms. Here, we present a case of a woman who complained of an asymmetric facial appearance and a mass-like lesion following a long-term facial massage intervention. A facelift incision was performed. Platysma muscle rupture was observed intraoperatively, which was then repaired. To our knowledge, this is the first report of a vigorous facial massage-induced ipsilateral platysma rupture.

Various kinds of positive effects of green tea extracts had been studied for long time which included anti-inflammation, anti-aging, and cardiometabolic effects. Although topical steroid and non-steroidal calcineurin inhibitors may control clinical symptoms of allergic contact dermatitis, some of patients also present allergic reaction to these topical agents. Therefore, we have tried green tea extracts for managing this skin disorder with expectation of anti-inflammatory effect without potential side effects including skin irritation and toxic responses. The toxicity test of green tea extract also did not show any sign of irritation in the skin throughout the test period. Moderate severity of allergic contact dermatitis presented satisfactory clinical outcome at second week follow-up which was final visit of outpatient. This result mean that green tea extract has a positive effect for managing allergic contact dermatitis but its potency and efficacy seem to be so not strong enough to control moderate severity allergy skin lesion. In this pilot study, we were able to conclude that green tea cell extracts might be applied for potential anti-inflammatory soaking without skin toxicity.
Atopic dermatitis is characterized by chronically pruritic and inflammatory dermatitis. In this study, we investigated the effect of Platycodon grandiflorum including platycodin D (PG-Platycodin D) in an atopic dermatitis-like mouse model. An atopic dermatitis-like skin lesion was induced by repeated treatment of 2,4-dinitrochlorobenzene (DNCB) on the dorsal skin of ICR mice. The efficacy of PG-Platycodin D was tested by observing scratching behavior, the skin severity score, and histopathologic analysis. PG-Platycodin D reduced the DNCB-induced increase in scratching behavior and the skin severity score. In addition, histopathologic analysis revealed a reduction in the thickening of the epidermis in the PG-Platycodin D group. These results may contribute to the development of a therapeutic drug for the treatment of atopic dermatitis.
Purpose: Tumor ablation and traumatic intractable ulceration of the plantar surface of the foot results in skin and soft tissue defects of the weight-bearing sole. Simple skin grafting is not sufficient for reconstruction of the weight-bearing areas. Instead, the island medial plantar flap (instep flap) and distally-based island medial plantar flap was used for proper reconstruction of the weight bearing area. However, there are some disadvantages. In particular, an island medial plantar flap has a short pedicle limiting the mobility of the flap and the distally-based island medial plantar flap is based on a very small vessel. We investigated whether good results could be obtained using a reverse island medial plantar flap based on the lateral plantar vessel as a solution to the above limitations. Methods: Three patients with malignant melanoma were cared for in our tertiary hospital. The tumors involved the lateral forefoot, the postero-lateral heel, and the medial forefoot area. We designed and harvested the flap from the medial plantar area, dissected the lateral and medial plantar artery and vena comitans, and clamped and cut the vessel 1 cm proximal to the branch from the posterior tibial artery and vena comitans. The medial plantar nerve fascicles of these flaps anastomosed to the sural nerve, the 5th interdigital nerve, and the 1st interdigital nerve of each lesion. The donor sites were covered with skin grafting. Results: The mean age of the 3 subjects was 64.7 years (range, 57 - 70 years). Histologically, all cases were lentiginous malignant melanomas. The average size of the lesion was $5.3\;cm^2$. The average size of the flap was $33.1\;cm^2$. The flap color and circulation were intact during the early postoperative period. There was no evidence of flap necrosis, hematomas or infection. All patients had a normal gait after the surgery. Sensory return progressively improved. Conclusion: Use of an island medial plantar flap based on the lateral plantar vessel to the variable weight-bearing sole is a simple but useful procedure for the reconstruction of any difficult lesion of the weight-bearing sole.

Background: Red-skin root of Asian ginseng (Panax ginseng) significantly reduces the quality and limits the production of ginseng in China. The disease has long been thought to be a noninfectious physiological disease, except one report that proved it was an infectious disease. However, the causal agents have not been successfully determined. In the present study, we were to reveal the pathogens that cause red-skin disease. Methods: Ginseng roots with red-skin root symptoms were collected from commercial fields in Northeast China. Fungi were isolated from the lesion and identified based on morphological characters along with multilocus sequence analyses on internal transcription spacer, β-tubulin (tub2), histone H3 (his3), and translation elongation factor 1α (tef-1α). Pathogens were confirmed by inoculating the isolates in ginseng roots. Results: A total of 230 isolates were obtained from 209 disease samples. These isolates were classified into 12 species, including Dactylonectria sp., D. hordeicola, Fusarium acuminatum, F. avenaceum, F. solani, F. torulosum, Ilyonectria mors-panacis, I. robusta, Rhexocercosporidium panacis, and three novel species I. changbaiensis, I. communis, and I. qitaiheensis. Among them, I. communis, I. robusta, and F. solani had the highest isolation frequencies, being 36.1%, 20.9%, and 23.9%, respectively. All these species isolated were pathogenic to ginseng roots and caused red-skin root disease under appropriate condition. Conclusion: Fungal complex is the causal agent of red-skin root in P. ginseng.

Scalp psoriasis affects at least two thirds people who have psoriasis. Generally, the scalp will have plaque psoriasis characterized by elevated, inflamed, lesions covered with silvery white scales. The lesion of psoriasis is head, elbow and knee that are yang-channels in Oriental medicine. The syndrome falls with in the purview of the dryness-metal(燥金). In time, psoriasis takes a turn for the worse in the winter season. In space, many of people in comparison to the average in the world have psoriasis in high latitude that is dry and cold, for instance, the Scandinavian peninsula. Deficiency of yin-fluid and blood course often the dryness-syndrome, for instance, psoriasis, etc. So psoriasis is one of the dryness-syndrome in Oriental medicine.
