The Reharvesting of Iliac Crest Cancellous Bone for the Repair of the Alveolar Cleft (치조열 교정을 위한 장골 능선 해면골 재이식술)

  • Purpose: The anterior iliac crest is a common source for autologous cancellous bone graft. For patients who have previously received cancellous bone grafts from bilateral anterior iliac crests, there may be concerns of whether a sufficient quantity of autologous cancellous bone remains for additional grafts without harvesting it from other sites, such as the posterior iliac crest. Methods: We experienced 3 cases of reharvesting in 2 patients. The diagnosis of the first patient was bilateral facial cleft number 3. This patient received bilateral side cleft alveoloplasty with corticocancellous bone graft from the both anterior iliac crest respectively by a previous surgeon. This patient then needed reharvesting of the anterior iliac crest cancellous bone to correct an ongoing skeletal problem for the bilateral cleft. The other patient had bilateral incomplete cleft of the primary palate. This patient received left side cleft alveoloplasty with cancellous bone graft from the right anterior iliac crest. Before the patient could receive the alveoloplasty on the other side, a radial head osteotomy and cancellous bone graft was performed by orthopedic surgeons who then used the remaining left iliac crest in order to treat a pulled elbow. For the completion of the right side cleft alveoplasty, the anterior iliac crest cancellous bone needed to be reharvested. Prior to the reharvesting, a preoperative computed tomography scan of the pelvis was obtained to assess the maturity of the donor site regeneration. The grafts were then taken from site where a greater amount of regeneration was evident. Results: Long term follow ups showed that the grafts were successfully taken. This sufficient volume was obtainable 14 months after the first harvest. Conclusion: Satisfactory results were achieved after the reharvesting of iliac cancellous bone. Thus, it appears that the reharvesting of the iliac bone is a possible alternative to multiple site grafting, use of allograft or bone substitute materials.

Radiological Study of Interphalangeal Sesamoid Bones on Hallux in Korean Subjects (한국인에서 족무지 지관절의 종자골에 대한 방사선학적 연구)

  • Purpose: Frequency of sesamoid bone on hallucal interphalangeal joint has been described to be low probability in orthopaedic and anatomical literature. We have, however, experienced two cases of interphalangeal joint dislocation giving difficulty to usual manipulative reduction because of presence of sesamoid bone recently. In order to ascertain existence of sesamoid bone on interphalangeal joint of hallux in Korean adults, radiological study have been performed with feet of patients Materials and Methods: Between May 2003 and October 2006, 974 patients with 1098 radiographs of feet which were reached skeletal maturity over 18-year-old were examined. Unilateral or bilateral anteroposterior, lateral and oblique radiographs were observed by one same person and presence was recorded if there was sesamoid in films. Distance of long and short axes were measured in lateral view and cases of two sesamoids in interphalangeal joint were recorded. Statistical differences between left and right side or between men and women were evaluated by chi-square test. Results: Frequency of sesamoid was 980 cases (89.3%) and no occurrence in 118 cases (10.7%). Two sesamoids were observed in 3 cases. Average distance of long axis was 4.9 mm (range, 0.5-11.4) and average distance of short axis was 3.5 mm (range, 0.3-9.3). Unilateral sesamoid was observed in 7 patients (5.6%), bilateral absence was 7 patients (5.6%) and bilateral sesamoids in 110 patients (88.8%) out of 124 patients who took bilateral feet radiographs. Men has less frequency than women significantly (p=0.014) while there was no significant difference in frequency according to side(p>0.05). Conclusion: Sesamoid bone was seen in 980 feet (89.3%) out of 1098 normal Korean radiological studies of feet. We report 3 cases of two seamoids which was extremely rarely reported in literature. Korean frequency is similar with Japanese, but much higher than Caucasians and black Africans.

Comparison of the Effects of Barefoot, Kinesio Tape, and Dynamic Tape on Static and Dynamic Balance in Subjects With Asymptomatic Flexible

  • Background: Flat-footed persons with collapsed medial longitudinal arch lose flexibility after skeletal maturity, resulting in several deformities and soft tissue injuries. Although arch support taping is usually applied in the clinic to support the collapsed arch, research on the use of different types of tape for more efficient arch support in flat-footed persons is lacking. Objects: The purpose of this study was to examine three conditions (barefoot, kinesio tape, and dynamic tape) and compare their effects on static and dynamic balance in persons with asymptomatic flexible flatfoot. Methods: Twenty-two subjects (9 females and 13 males) with asymptomatic flexible flatfoot participated in this study. The subjects performed the Y-balance test to measure the composite reach score. The subjects also performed a 30-second standing test to measure the center of pressure (COP) path length and a walking test to measure anteroposterior and lateral variability using the Zebris FDM system. One-way repeated-measures analysis of variance compared the three conditions applied to the subjects' feet for each balance variable. Results: The composite reach score significantly increased following the application of dynamic tape compared with barefoot and that of kinesio tape compared with barefoot. There was no significant difference in the COP path length during standing among the three conditions. Anteroposterior and lateral variability during walking significantly with dynamic tape application compared with barefoot. Conclusion: The results of this study suggest that, in persons with asymptomatic flexible flatfoot, application of kinesio tape and dynamic tape may be effective in increasing the composite reach score in Y-balance test, whereas application of dynamic tape may be effective in reducing anteroposterior and lateral variability during walking.

The Study on Correlations of Risser Sign with the Chronological Age, Bone Age, Menarche, and Adult Height Prediction according to TW3 Method (Risser 증후와 역연령, 골연령, 초경 시기 및 성인 예측신장 (AHP-TW3)과의 관계)

  • Objectives The purpose of this study was to find out the clinically reliable relationships between the Risser sign and chronological age, bone age, menarche, and adult height prediction (AHP) and to evidence the reliability of the Risser sign. Methods This study had been carried out with 50 children who had their growth checked in an oriental medical hospital from January 2015 to February 2017. We investigated Risser sign in AP X-rays with iliac crest, bone age, AHP for all 50 children and the timing of menarche from the 22 girls in the study subjects. We also investigated a correlation between the Risser stage and the other indicators to analyze statistical data. Results The mean chronological ages of Risser 1, 2, 3 and 4 were 11.2, 12.6, 14.4, and 15.5 years respectively for the boys and 10.8, 12.2, 13.8 and 14.8 years respectively for the girls. The mean bone ages of Risser 1, 2, 3 and 4 were 12.3, 13.6, 15.7 and 16.5 years respectively for the boys and 11.7, 13.8, 14.3 and 14.9 years respectively for the girls. We analyzed 22 girls' Risser stages in accordance with the duration from menarche. The result showed that in the first six months after menarche, all girls were in Risser 1 and 2; in the next six months, the girls were in Risser 2 on average; in the next 12 months, all girls were in Risser 3 and 4; after more than two years from menarche, all girls were in Risser 4. The mean remaining growth height of Risser 1, 2, 3 and 4 were 27.8, 17.3, 4.4 and 1.0 cm respectively for the boys and 14.5, 5.1, 3.1 and 1.1 cm respectively for the girls. The Risser stage was correlated strongly with chronological age (Spearman's rho=0.707 (boy), 0.841 (girl)), bone age (Spearman's rho=0.869 (boy), 0.875 (girl)), duration from menarche (Spearman's rho=0.909) and remaining growth height (Spearman's rho=-0.784 (boy), -0.878 (girl)). Conclusions This study showed that the Risser sign can be useful in assessing skeletal maturity and predicting remaining growth height based on the Risser stage and the other growth indicators.

The Relationship between Food Habit, Nutrient Intakes and Bone Mineral Density and Bone Mineral Content in Adult Women (성인여성의 식습관과 영양섭취상태와 골밀도 및 골무기질함량과의 관계)

  • Bones are important parts in sustaining the shape of the body, but they are also metabolic organs which undergo bone remodeling by constant bone resorption and formation. Osteoporosis, the typical metabolic bone disease, is characterized by a reduction in bone mineral density (BMD). Women more than men are at risk fir osteoporosis-related fractures, especially in the lumbar spine, wrist, and hip region. Risk of fracture depends on one's BMD, which open determined by the peak bone mass value achieved at skeletal maturity and followed by subsequent age-and menopause-related bone loss. Genetic and environmental factors are known to play a key role in bone metabolism and diet is considered as one of the important environmental factors. The purpose of the present study was to assess the status of BMD and bone mineral content(BMC) to clarify the relationships between dietary intakes and the risk of osteoporosis in adult women in Taegu. Subjects were 130 healthy females in between 20 and 69 years of age. BMD and BMC of the lumbar spine(venebrae L2-4) of the subjects were measured by dual energy X-ray absorptiometry. The average age of the subjects was 47.4${\pm}$11.7 years old, the average weight was 57.2${\pm}$8.4kg, the average age of menarche was 16.6${\pm}$1.9 years old and the average age of menopause was 48.4${\pm}$5.3 years old. The nutrient intakes of the subjects measured by the convenient method were generally lower than the level of RDA. The result of nutrient intake assessed has shown that the average energy intake was 1701${\pm}$316kca1 which is 85.1% of the RDA and the average calcium intake was 485.4${\pm}$172.3mg which is 69.3% of the RDA. The intakes of protein, vitamin A, vitamin B$_1$, niacin were greater than the RDA, whereas the remaining nutrient intakes were lower than the RDA. The average BMD of the subjects was shown to be 1.06${\pm}$1.09g/$\textrm{cm}^2$. The highest BMD of 1.24${\pm}$0.14g/$\textrm{cm}^2$ was noticed in the subjects of 30s compared to 20s, 40s, 50s, 60s. The BMD values were compared by the relative body weight(R3W) of the menopause subjects, and it was found that the underweight group had significant lower BMB while the rest of the groups did not have any differences in BMD. The most strongly correlated nutrient with BMD among the menopause subjects appeared to be calcium. The women whose Ca intakes were higher than 500mg showed the significantly higher BMD than those with Ca intakes lower than 500mg. This study suggests that the most effective way to prevent osteoporosis and to reduce the incidences of fractures seems to be minimizing bone loss through the adequate intake of calcium as well as avoiding underweight, especially in menopausal women.   • They evaluate the bone age using the GP-BP (Greulich-Pyle and Bayley-Pinneau) and TW3 (Tanner-Whitehouse 3) in clinical. The skeletal maturity in Hand AP is evaluated by clinical experience of physicians and this is qualitative evaluation not same in every physicians. In order to devise and evaluate new methods not using TW3 method in this situation. The study was conducted with 70 (Male 35, Female 35) children who visited Yangsan P Hospital from March 2014 to March 2015. The study measured the length of growth plate and distal proximal phalanx and conducted regression analysis for statistical significance test of bone age length difference. The study found average and standard deviation corresponding to certain ranges each bone age. The more bone age increase, the more the length of growth plate and distal proximal phalanx decreased. The girls have less average rather than the boys because bone grows fast. The girls have first period age of 12 to 14, it appears length variation significantly. The study conducted regression analysis and this has statistical significance.

  • Three dimensional analysis of malocclusion and craniofacial deformation is essential for the successful orthodontic treatment. But the orthodontists are not familiar with diagnosis and treatment plane based on lateral cephalometric analysis. Since orthodontists do not posses a sufficient knowledge in standard value of posteroanterior cephalometric anaysis and of clinical importance for transverse jaw growth. In this study male(n=130) and female(n=171) aged from 6 to 16 and diagnosed as Class I malocclusion were selected to analysis width of cranium, maxilla and mandible on the posteroanterior cephalogram. The changes as a function of chronologic age and cervical vertebrae maturity index(CVXI) were examined. The Proper regression model was selected by sex with polynominal regression models and method of variable selection. Mean of each measurements and 70% confidence interval of individual measurement according to age was assesed and a graphs were made. Results are as follows :1. All the measurements for the width are gradually incresed as increase in chronologic age and CVMI. From the total amount of change between age 6 and 16, there is a tendency that mandibular width is broader than maxillary width and the width of male is broader than female. 2. There is no statistically significant sexual difference in Mx-Mn difference, Mx-Mn width differential, Mx/Mn ratio according to age and CVMI. 3. Mean of each measurement and 70% confidence interval of individual measurement according to age and sex were assessed and graphs were made for maxillary width, mandibular width, Mx-Mn difference, Mx/Mn ratio. 4. The width of maxilla and mandible in Korean children are broader than Western children during growth period.

A Comparison of the Results of BPTB and Hamstring ACL Autograft - Function, Stability & Tunnel Expansion - (골-슬개건-골 및 슬괵건을 이용한 전방 십자 인대 재건술의 비교 - 술후 슬관절의 기능, 안정성 및 대퇴, 경골터널 확장정도에 대하여 -)

  • Purpose : The purpose of this study was to compare the results of ACL reconstruction using bone-patellar tendon-bone to hamstring. Materials and Methods : Thirty-two ACL reconstructions with autogenous BPTB and eighteen reconstructions with hamstring were compared in terms of functional outcome, stability and tunnel expansion. Results : The functional score of BPTB group was higher than hamstring group in OAK(Orthopadishe Arbeitsguppe Knie) and IKDC(International Knee Documentation Committee) system. In BPTB group, OAK scores were 71.6$({\pm}10.0)$ preoperatively and 88.5$({\pm}7.9)$ finally. In hamstring group, OAK scores were 73.9$({\pm}11.5)$ and 82.5$({\pm}12.9)$ respectively. There was no difference in stability checked by either $KT-1000^{TM}$ or stress view. Anterior tibial translation measured by $KT-1000^{TM}$ were 2.4$({\pm}1.8)$mm in BPTB and 2.3$({\pm}2.4)$mm in hamstring group. Anterior tibial translation in stress view were 2.8$({\pm}3.4)$mm in BPTB and 2.8$({\pm}2.5)$mm in hamstring group. There was no difference in tibial tunnel expansion but femoral tunnel was more enlarged in hamstring group than BPTB (P=0.03). Conclusion : As there was no difference in stability between two groups, it seems prudent to select either graft defend on such factors as anterior knee pain, skeletal maturity and cosmetic concern. Tunnel expansion seems not to affect stability, but further study is needed to confirm that decrease of which might improve stability.

Effect of Backfat Thickness on the Carcass Grade Factors and Carcass Price in Hanwoo Cows and Steers (등지방 두께가 한우 암소와 거세우의 도체 등급 요인 및 도체가격에 미치는 영향)

  • Analyses were conducted to estimate the effect of backfat thickness (BFT) on carcass grade factors and carcass price in Hanwoo cows and steers. Data on grade factors and prices were collected from 69,159 Hanwoo carcasses (n = 13,376 cows, n = 55,783 steers) abtained at the Nonghyup Seoul slaughter house in 2009. Carcass BFTs were divided into nine categories from ${\leq}$ 3 mm to ${\qeq}$ 25 mm. The average BFTs were 11.93 mm in cows and 12.57 mm in steers. An increase in the BFT resulted in an increase in carcass weight, loin-eye area, skeletal maturity, fat color, but decreases in the yield index and yield grade (from grade A to C) in cows and steers. Carcass unit price (Won/kg) improved if the carcass was sold with a BFT of 13-15 mm in cows and 10-12 mm in steers. Marbling score and quality grade increased significantly to 21 mm BFT but decreased from a BFT of 22 mm in cows and steers. BFT was significantly correlated with yield index (cow, r = $-0.97^{**}$; steer, r = $-0.96^{**}$), marbling score (cow, r = $0.20^{**}$; steer, r = $0.14^{**}$), and carcass price (cow, r = $0.03^{**}$; steer, r = $-0.11^{**}$). These results could be used as a fundamental dataset for further research to increase the productivity of Hanwoo carcasses.

Long-Term Results of Surgical Treatment for the Idiopathic Clubfoot (선천성 만곡족의 장기 수술적 치료 결과)

  • Purpose: This study evaluated the results of surgical treatment for residual or recurrent deformity after the conservative treatment of idiopathic clubfoot. Materials and Methods: Fifty-one cases (32 patients), who were followed up to skeletal maturity, were reviewed retrospectively. The mean age at the last follow-up was 18.7 years. The surgical options included selective or comprehensive soft tissue release, tendon lengthening and transfer, and various types of osteotomy. The radiology measurements included the talocalcaneal angle and talo-first metatarsal angle in the anteroposterior (AP) view, and the talocalcaneal angle and calcaneal pitch in the lateral view. The radiology measurements were compared with the normal values for adults. The clinical evaluations were made using the ankle-hindfoot score and the midfoot score of the American Orthopaedic Foot and Ankle Society (AOFAS): excellent (>85), good (71-85), pair (56-70), and poor (<56). Results: At the last follow-up, the percentages of the 51 cases, whose parameter values fell within the normal ranges were as follows: in the AP view, 41.2% (talocalcaneal angle); and 90.2% (talo-first metatarsal angle). In the lateral view, the percentage was 84.3% (talocalcaneal angle). For the calcaneal pitch, the percentages were 61%. The mean AOFAS score was 88.1±10.7 on the ankle-hindfoot score and 86.7±11.5 on the midfoot score. Conclusion: The long-term outcome of patients with idiopathic clubfoot, who underwent surgical treatment after conservative treatment, was found to be 43%-90% of the normal range of radiographic indices. Clinically, the mean AOFAS scores were "excellent". Therefore, a satisfactory result can be obtained by analyzing the elements of deformity more accurately and then using the selective operation method, even if the non-surgical correction method fails.
