Individualized Medical Nutrition Therapy Improved Nutritional Status and Quality of Life in Hemodialysis Patients

  • A case-controlled, 12 week follow-up, study was designed to investigate the effect of dietician-delivered medical nutrition therapy (MNT) on the nutritional status and quality of life in hemodialysis patients. Subjects were recruited at Kyung-Hee Medical Center and were randomly assigned to two groups : the control and the MNT group. The MNT group received individualized MNT for 12 weeks. The results were as follows: 1) The mean ages of the control (n = 20) and MNT (n = 24) groups were 50.6 $\pm$ 14.8 and 45.7 $\pm$ 14.0 years, and the mean durations of dialysis were 2.3 $\pm$ 2.3 and 1.7 $\pm$ 1.9 years, respectively. The interdialysis weight gain of the subjects was higher than that recommended. 2) The indicators of the subjects nutritional status showed that S to 25% of the subjects had some degree of malnutrition, with most of them in the mild malnutrition category. 3) After 12 weeks of the experiment, the percentage of the ideal body weight (% IBW) of the control group decreased, but that of the MNT group increased. Changes in other anthropometric parameters in both groups during the study period were not significantly different. 4) At the beginning of the study, the 54% of the MNT group consumed more than 28 kcal/kg body weight/day and 50% consumed more than 1.0g protein/kg body weight/day. However, these percentages rose to 71% and 75%, respectively, after 12 weeks of the individualized MNT. 5) The serum albumin and blood urea nitrogen (BUN) levels of the control group decreased significantly artier 12 weeks of the experiment, while those of the MNT group did not change. 6) After 12 weeks of individualized MNT, the mean score of nutrition knowledge and total mean score of quality of life (QL) of the MNT group were significantly higher than that of th\ulcorner control group. Body pain and social functioning scores of the MNT group were significantly higher than those of the control group. The positive effect of individualized MNT on the hemodialysis patients consisted of their improved nutritional status, nutritional knowledge, and the quality of life. These results suggest that individualized MNT continuously performed by a dietitian can be helpful for hemodialysis patients. However, larger and longer term studies are needed to confirm these positive effects of MNT. In addition, the development of nutritional education programs for MNT is needed to increase the positive impact of MNT.

Associations of dietary intake and metabolic syndrome risk parameters in Vietnamese female marriage immigrants in South Korea: The KoGES follow-up study

  • BACKGROUND/OBJECTIVES: This study was conducted to compare the overall changes in dietary intake and metabolic syndrome risk parameters in Vietnamese marriage-based female immigrants over time. SUBJECTS/METHODS: The subjects of this study were 581 Vietnamese marriage-based female immigrants, who were recruited from local clinical centers in Korea. Baseline data were collected from 2006-2011 and the follow-up data were collected from 2012-2014. Dietary food intake was assessed by a 1-day 24-hour recall method. RESULTS: Compared to the baseline, the mean body weight, body mass index, waist circumference, high density lipoprotein (HDL)-cholesterol, systolic blood pressure and diastolic blood pressure increased while the fasting blood sugar, triglycerides and low density lipoprotein-cholesterol decreased at follow-up. The total consumption of foods such as vegetables/fruits/seaweeds/mushrooms, oil and fat, and eggs decreased during the follow-up period, whereas the consumption of sugars, beverages and meats increased. Partial correlation between the rate of change [(Follow-up - Baseline) / Baseline ${\times}100$] in metabolic syndrome risk parameters and food intake after controlling for confounding factors showed that the waist circumference was positively correlated with either the total plant food intake (r = 0.1042, P = 0.0129) or the total food intake (r = 0.0880, P = 0.0359). The plasma levels of total cholesterol (r = -0.1918, P = 0.0289) and HDL-cholesterol (r = -0.1424, P = 0.0007) were negatively correlated with the percentage of total intake from plant food, and HDL-cholesterol levels were positively correlated with the intake of total animal food (r = 0.0980, P = 0.0217). The serum C-reactive protein levels were positively correlated with the total intake of animal food (r = 0.2374, P < 0.0001) or the percentage of total intake from animal food (r = 0.1346, P = 0.0016). CONCLUSIONS: These results suggest that the change rate of dietary intake such as total plant food or animal food is associated with the change rates of metabolic syndrome risk parameters.

The Effect of Nutrition Education on Weight Control and Diet Quality in Middle-Aged Women (영양교육에 의한 식행동 향상이 중년여성의 체중조절 및 식사의 질에 미치는 영향)

  • We examined the effect of dietary behavior improvement on diet quality, body composition and blood lipid profiles in 68 middle-aged women. Dietary behavior intervention was consisted of counselling in the problem areas of portion control, food selection for low carbohydrate and high fiber food items, and education on the improvement of micronutrient intakes and diet quality. The subjects were divided into two groups; Improved or Not-Improved Group according to the level of changes in dietary behavior scores. After 6 months, body weight, BMI, waist-hip ratio, and visceral fat area were significantly decreased in the Improved Group compared to the Not-Improved Group. Calorie-adjusted protein, fiber, calcium, iron and vitamin C intakes were significantly increased in the Improved Group. The index of nutritional quality (INQ) and mean adequacy ratio (MAR) were increased only in the Improved Group. We observed a significantly increase in HDL-cholesterol and a decrease in total cholesterol, triglyceride and LDL-cholesterol, and an improvement of atherogenic index in the Improved Group. These results showed that dietary behavior intervention improved dietary behavior scores, and those whose dietary behavior score improved showed more improvement in diet quality, body composition, and serum profiles than those whose dietary behavior score did not improve.

Effect of Carotenoids on the Growth of HT-29 Human Colon Cancer Cells (Carotenoids가 인체의 대장암 세포인 HT-29 세포의 증식에 미치는 영향)

  • Epidemiological studies have observed a negative association between increased consumption of green and yellow vegetables and cancer incidence. These vegetables contain carotenoids, which are reported to exhibit anticarcinogenic effects. Overexpression of ErbB2 and ErbB3 genes is a frequent event in several human cancers. The present study was performed to determine whether $\alpha$-carotene, $\beta$-carotene, lutein, or lycopene inhibits cell growth and to assess such an effect is related to changes in the levels of the ErbB receptor family and tile ErbB3 receptor signaling pathway in HT-29 cells. HT-29 cells were cultured in serum-free medium in the presence of various concentrations (0~100 $\mu$M) of the individual carotenoids. $\alpha$ -Carotene and lycopene significantly inhibited cell growth in a dose-dependent manner, whereas lutein slightly inhibited cell growth and $\beta$-carotene increased cell growth. Lycopene is more potent than $\alpha$ -carotene in inhibiting HT-29 cell growth. Lycopene inhibited DNA synthesis and induced apoptosis of HT-29 cells. The ErbB3 ligand heregulin (HRG) increased cell growth but did not prevent the lycopene-induced inhibition of cell growth. Lycopene decreased ErbB2 protein levels in a dose-dependent manner. Immunoprecipitation/Western blot studies revealed that lycopene inhibited HRG-induced phosphorylation of ErbB3, recruitment of the 985 regulatory subunit of phosphatidylinositol 3-kinase (PI3K) to the ErbB3 receptor, and phosphorylation of Akt. These results indicate that downregulation of ErbB2/ErbB3/PI3K/Akt signaling may be one of the mechanisms by which lycopene inhibits HT-29 cell pro-liferation and induces apoptosis.

  • In order to study the effect of aqua-acupuncture with Radix Angelicae gigantis extract solution by acuncture point. Radix Angelicae gigantis aqua-acupuncture was carried on every day for 3 days and 5 days 7 days respectively on the corresponding loci of Chondol(CV22) and non-acupuncture points (loci at the root of tail), after inducing hypothyroidism by thiourea administration and thereafter the contents of serum triiodothyronine$(T_3)$, thyroxine$(T_4)$, triiodothyronine uptake, free $T_3$, free $T_4$, total protein, albumin, creatinine, creatine, BUN, cholesterol, triglyceride, and total lipid were measured in hypothyroidism rats. The following results have been obtained : 1. $T_3$ was increased with statistical significance by Radix Angelicae gigantis aquq-acupuncture for 3 days, 5 days and 7 days Chondol(CV22) and non-acupuncture points as compared with the control group. 2. $T_3$ uptake was increased with statistical significance by Radix Angelicae gigantis aquq-acupuncture for 3 days, 5 days and 7 days to Chondol(CV22) as compared with the control group. 3. Free $T_4$ was increased with statistical significance by Radix angelicae gigantis aqua-acupuncture for 3 days, 5 days and 7 days to Chondol(CV22), and non-acupuncture points respectively as compared with the control group. 4. Albumin was decreased with statistical significance by Radix Angelicae gigantis aqua-acupuncture for 3 days, and 5 days and 7 days to Chondol(CV22), and non-acupuncture points respectively as compared with the control group. 5. Creatine was decreased with statistical significance by Radix Angelicae gigantis aqua-acupuncture for 3 days, and 5 days and 7 days to Chondol(CV22), and non-acupuncture points respectively as compared with the control group. 6. Cholesterol was decreased with statistical significance by Radix Angelicae gigantis aqua-acupuncture for 3 days, and 5 days and 7 days to Chondol(CV22) and non-acupuncture points respectively as compared with the control group. 7. Triglyceride was decreased with statistical significance by Radix Angelicae gigantis aqua-acupuncture for 3 days, and 5 days and 7 days to Chondol(CV22) and non-acupuncture points respectively as compared with the control group. 8. Total lipid was decreased with statistical significance by Radix Angelicae gigantis aqua-acupuncture for 3 days, and 5 days and 7 days to Chondol(CV22) and non-acupuncture points respectively as compared with the control group.

Effects of Selenium, Vitamin E, and Their Combination on Growth, Hematological Changes, and Biological Blood Parameters in Orchidectomized Rat Model (Vitamin E와 Selenium이 정소적출 포유동물모델의 성장, 혈액 및 생화학적인 변화에 미치는 영향)

  • The present study was devised to determine the effects of vitamin E and selenium (Selevit) on body weight, organ weight, hematological values and biochemical parameters in the orchidectomized (Orch) rats. Intact group (n=15) received no treatment and operation. Orch+Selevit received operation and Selevit. The body weights of each group increased, but that of the Orch+Selevit group were significantly lower than those of all the other groups. There were significantly different decreased (p<0.001) of body weights between Orch+Selevit group and all the other groups. Also, organ weights such as heart, liver, spleen, kidney, lung and skeletal muscle were measured. The heart and liver weights in the Orch+Selevit group were significantly different decreased (p<0.001) in comparison with those in the Intact and Sham groups. The kidney weights in the Orch+Selevit group were significantly different decreased (p<0.01, p<0.001) in comparison with those in all the other groups. The number of white blood cell (WBC) was significantly higher (p<0.05) in the Orch+Selevit group than in all the other groups. The hematological values of 12 parameters were not significantly different in any of the groups. The concentrations of serum total protein, albumin and alkaline phosphatase only increased significantly (p<0.05, p<0.01) in the Orch+Selevit group as compared to that in the Orch group. We conclude that Selevit was significantly decreased the body weight in the Orch rats. Our findings suggest that Selevit may influence the process of lipid packaging and absorption in the Orch rats.

Effect of Dietary Supplementation of Yellow Loess on Physiological Performance and Carcass Grade in Pigs (황토의 식이 내 첨가에 의한 돼지의 생리적 특성과 등급판정에 미치는 영향)

  • This study was conducted to examined the effect of dietary supplementation of yellow loess on the growth performance and blood component profiles in pigs. A total of one hundred fifty three pigs (60 one weeks old piglets and 93 adult pigs). Piglets were randomly divided into 3 groups which were control, control + 0.5% yellow loess, control + 1% yellow loess. We measured body weight and weight gain among treatment group. Indeed, we obtained hematological data with WBC, RBC, Hb, Hct, MCV, MCH, MCHC, RDW, and PLT in all loess supplemented group compared to the control. Adults were divided into 2 group (control and control + 1% yellow loess) and serum albumin levels in 1% yellow loess supplemented group was significantly higher than the control group (p<0.01). Amount of crude fat in feces was significantly reduced in yellow loess supplemented group (p<0.001), but no differences were observed at crude protein and crude ash in both groups. In carcass weight and back fat thickness, no significant differences were observed between control and yellow loess supplemented groups. The carcass grade was improved by the dietary supplementation of yellow loess compared to the pig fed control. As a results, dietary yellow loess improves nutrient absorption and carcass grade.

  • Objective : This experiment was designed to investigate the effect of the CBD hot water extract & ultra-fine Powder on Alzheimer's Disease Model Induced by ${\beta}A$. Method : The effects of the CBD hot water extract on expression of interleukin-1 beta($IL-1{\beta}$), $TNF-{\alpha}$ mRNA and production of IL-6, $TNF-{\alpha}$ in BV2 microglial cell line treated by lipopolysacchaide(LPS). The effects of the CBD hot water extract & ultra-fine powder on (1) the behavior (2) expression of $IL-1{\beta}$, tumor necrosis factor-alpha($TNF-{\alpha}$), (3) the infarction area of the hippocampus in Alzheimer's diseased mice induced with ${\beta}A$ were investigated. Result : The CBD hot water extract suppressed the expression of $IL-1{\beta}$, $TNF-{\alpha}$ mRNA in BV2 microglia cell line treated with LPS. The CBD hot water extract significantly suppressed the production of $IL-1{\beta}$, $TNF-{\alpha}$ in BV2 microglial cell line treated with LPS. The CBD hot water extract & ultra-fine powder a significant inhibitory effect on the memory deficit was shown for the mice with Alzheimer's disease induced by ${\beta}A$ in the Morris water maze experiment, which measured step-through latency and distance movement-through latency. The CBD hot water extract & ultra-fine powder significantly suppressed the expression of $IL-l{\beta}$ and $TNF-{\alpha}$ protein in the microglial cell of mice with Alzheimer's disease induced by ${\beta}A$. The CBD hot water extract & ultra-fine powder suppressed the over-expression of AChE activity in the serum of the mice with Alzheimer's disease induced by ${\beta}A$. The CBD hot water extract & ultra-fine powder reduced infarction area of hippocampus, in the mice with Alzheimer's disease induced by ${\beta}A$. Conclusions : These results suggest that the CBD hot water extract & ultra-fine powder may be effective for the prevention and treatment of Alzheimer's disease. Investigation into the clinical use of the CBD hot water extract & ultra-fine powder for Alzheimer's disease is suggested for future research.

The SSJHT hot water extract & ultra-fine powder reduced infarction area of hippocampus. and controlled the injury of brain tissue in the mice with Alzheimer's disease induced by ${\beta}A$. The results suggest that the SSJHT hot water extract & ultra-fine powder may be effective for treatment of Alzheimer's disease. Investigation into the clinical use of the SSJHT hot water extract & ultra-fine powder for Alzheimer's disease is suggested for future research.

Gene Cloning, Expression and Immunogenicity of the Protective Antigen Subolesin in Dermacentor silvarum

  • Subolesin (4D8), the ortholog of insect akirins, is a highly conserved protective antigen and thus has the potential for development of a broad-spectrum vaccine against ticks and mosquitoes. To date, no protective antigens have been characterized nor tested as candidate vaccines against Dermacentor silvarum bites and transmission of associated pathogens. In this study, we cloned the open reading frame (ORF) of D. silvarum 4D8 cDNA (Ds4D8), which consisted of 498 bp encoding 165 amino acid residues. The results of sequence alignments and phylogenetic analysis demonstrated that D. silvarum 4D8 (Ds4D8) is highly conserved showing more than 81% identity of amino acid sequences with those of other hard ticks. Additionally, Ds4D8 containing restriction sites was ligated into the pET-32(a+) expression vector and the recombinant plasmid was transformed into Escherichia coli rosetta. The recombinant Ds4D8 (rDs4D8) was induced by isopropyl ${\beta}$-D-thiogalactopyranoside (IPTG) and purified using Ni affinity chromatography. The SDS-PAGE results showed that the molecular weight of rDs4D8 was 40 kDa, which was consistent with the expected molecular mass considering 22 kDa histidine-tagged thioredoxin (TRX) protein from the expression vector. Western blot results showed that rabbit anti-D. silvarum serum recognized the expressed rDs4D8, suggesting an immune response against rDs4D8. These results provided the basis for developing a candidate vaccine against D. silvarum ticks and transmission of associated pathogens.
