Effective communicable disease surveillance systems are the basis of the national disease prevention and control. Following the increase in emerging and re-emerging infectious diseases since late 1990s, the Korean government has strived to enhance surveillance and response system. Since 2000, sentinel surveillance, such as influenza sentinel surveillance, pediatric sentinel surveillance, school-based sentinel surveillance and ophthalmological sentinel surveillance, was introduced to improve the surveillance activities. Electronic reporting system was developed in 2000, enabling the establishment of national database of reported cases. Disweb, a portal for sharing communicable disease information with the public and health care workers, was developed. In general, the survey results on usefulness and attributes of the system, such as simplicity, flexibility, acceptability, sensitivity, timeliness, and representa-tiveness, received relatively high recognition. Compared to the number of paid cases of national health insurance, reported cases by national notifiable disease surveillance system, and various sentinel surveillance system, the result of the correlation analysis was high. According to the research project conducted by KCDC, the reporting rate of physicians in 2004 has also greatly improved, compared with that in 1990s. However, continuous efforts are needed to further improve the communicable disease surveillance system. Awareness of physicians on communicable disease surveillance system must be improved by conducting education and information campaigns on a continuous basis. We should also devise means for efficient use of various administrative data including cause of death statistics and health insurance. In addition, efficiency of the system must be improved by linking data from various surveillance system.
The surveillance and epidemiological investigation systems for companion animals in South Korea are significantly underdeveloped compared to those for humans and livestock. Recent outbreaks, such as idiopathic neuromuscular syndrome and highly pathogenic avian influenza among cats, have highlighted the need for reliable systems. This short review conducts situation analysis regarding disease surveillance and epidemiological investigation for companion animals in South Korea. The current challenges include an absence of administrative leadership, a lack of legal support, and unreliable medical data. The recommendations for future directions include clear leadership by the Animal and Plant Quarantine Agency, amending the Act on the Prevention of Contagious Animal Diseases to include companion animals, and enhancing the quality of medical data through standardized coding systems, such as Systematized Nomenclature of Medicine Clinical Terms. In addition, sentinel surveillance rather than universal systems should be established to provide adequate incentives for local practitioners to provide data and develop sustainable public-private networks. These recommendations could be important for developing a comprehensive and sustainable system for disease surveillance and epidemiological investigation in the companion animal field.
Purpose: The purpose of this study was to examine the effect of patient surveillance on nurses' adherence to the guidelines on peripheral vascular catheter management. Method: A one group time series design was used. Seventy six nurses participated in the study. The data were collected from October 1 to November 4, 2008. Result: The effects of patient surveillance were significant on nurses' average adherence rates to the guidelines estimated by observers and by themselves, and the levels of nurses self recognition on the importance of the guidelines. In addition, overall occurrence rates of phlebitis tend to decrease after patients surveillance. Even though 33 nurses didn't follow the guidelines perfectly, most patients did not provide feedbacks to the nurses' non-adherence. Among the nurse participants, 36.8% answered that patient surveillance was acceptable. Conclusion: This study found that patients with peripheral catheters could positively influence nurses' adherence to the catheter management guidelines. Therefore, inviting patients in nursing activities could benefit to the quality of patients care. However, it seems essential to create an environment fostering nurses' acceptance of the patients' surveillance. Further research warrants the potential benefits of the patient's participations in nursing activities.
Due to heightened concerns regarding possible bioterrorist attacks, the Korea Center for Disease Control and Prevention introduced syndromic surveillance systems, which have been run by emergency departments in hospitals throughout Korea since 2002. These systems are designed to identify illness clusters before diagnoses are confirmed and reported to public health agencies, to mobilize a rapid response, and thereby to reduce morbidity and mortality. The Korea Center for Disease Control and Prevention performed drop-in syndromic surveillance successfully during the World Cup Football Games in 2002, the Universiad games in 2004, and the Asian Pacific Economic Cooperation meeting in 2005. In addition, sustainable syndromic surveillance system involving the collaborative efforts of 125 sentinel hospitals has been in operation nationwide since 2002. Because active data collection can bias decisions a physician makes, there is a need to generate an automatic and passive data collection system. Therefore, the Korea Center for Disease Control and Prevention plans to establish computerized automatic data collection systems in the near future. These systems will be used not only fur the early detection of bioterrorism but also for more effective public health responses to disease.
Background: As the habitat changes in Korea due to climate change, the emergence of disease-mediated vectors is increasing rapidly. Thus for the surveillance of mosquito- and chigger mite-borne disease, their seasonal prevalence and species composition were investigated at seven locations in Daegu, Gunwi and Sangju. Methods: Mosquitoes were collected twice every month from five collection sites using a black light and BG sentinel traps in Daegu and Gunwi from April through November. Chigger mites were investigated twice per month from wild rodents caught with Sherman live traps in Gunwi and Sangju from April through May and September through November. Results: A total of 2,361 female mosquitoes were collected. Cowshed (626 individuals, Trap index (TI) 44.7) and Kyungpook National University campus (846 individuals, TI 60.4) in Daegu had the highest number of mosquitoes in the black light and BG sentinel trap, respectively. The mosquitoes were collected more by BG sentinel trap than the black light trap. Nine mosquito species were trapped, and the Culex pipiens complex was the most commonly mosquito (1,397 individuals, 59.2%), followed by Anopheles sinensis (554 individuals, 23.5%). Anopheles sinensis (531 individuals, 51.9%) and Culex pipiens complex (1,142 individuals, 85.4%) were the most mosquitoes from black light and BG sentinel trap, respectively. In terms of seasonal prevalence, the highest abundance was in July, with 824 individuals collected. In chigger mites, eighty-one wild rodents of five species that are hosts of chigger mites were collected; among them, 53 and 25 individuals of Apodemus agrarius and Crocidura suaveolens, respectively were trapped. Leptotrombidium pallidum was a dominant species, with 2,467 individuals collected (67.8%). Conclusions: The mosquito was the dominant species in Culex pipiens complex and the highest in July and August. Apodemus agrarius was most abundant in wild rats and Leptotrombidium pallidum was dominant in the collected chigger mites.
지구온난화로 인해 촉발된 기후변화가 홍수와 같은 수재해의 빈도와 규모를 증가시키며 국내 또한 장마와 집중호우로 인한 수재해가 증가하는 추세를 보인다. 이에 광범위한 수재해에 대해 효과적인 대응 및 기후 변화에 따른 선제적 대처가 필수적이며 이는 위성레이더 영상을 통해 가능하다. 본 연구에서는 Sentinel-1 위성 레이더 영상으로부터 국내 수체의 특성을 반영하기 위해 한강권역과 낙동강 권역의 일부 수체 영역에 대해 수체 학습 데이터셋 1,423장을 구축하였다. 정밀한 데이터 어노테이션(Annotation)을 위해 다양한 상황에 따른 구축 기준 문서를 작성한 뒤 진행하였다. 구축이 완료된 데이터셋을 딥러닝 모델 중 U-Net에 적용하여 수체 탐지 결과를 분석하였다. 최종적으로 학습된 모델을 학습과에 활용되지 않은 수체 영역에 적용하여 결과를 분석함으로써 전 국토 수체 모니터링의 가능성을 확인하였다. 분석 결과 구축된 수체 영역의 대해서는 F1-Score 0.987, Intersection over Union (IoU) 0.955의 높은 정확도로 수체를 탐지할 수 있었으며, 학습 및 평가에 활용되지 않은 다른 국내 수체 영역에 대해서도 동일하게 F1-Score 0.941, IoU 0.89의 높은 수체 탐지 결과를 나타냈다. 두 결과 모두 전반적으로 일부 그림자 영역과 폭이 좁은 하천에서 오류가 관찰되었으나, 그 외에는 정밀하게 수체를 탐지하였다. 이러한 연구 결과는 수재해 피해 규모 및 수자원 변화 모니터링에 중요한 기여를 할 것으로 기대된다. 추후 연구에서는 보다 다양한 수체 특성을 가진 데이터셋을 추가 구축한다면 오분류한 영역을 개선할 수 있을 것으로 기대되며, 전 국토의 수체를 효율적으로 관리 및 모니터링하는데 활용될 것으로 사료된다.

As animal welfare issue becomes important, the European Union bans conventional cages for laying hens from 2012. So the alternative housing systems like floor pens, aviaries or free range systems have been suggested. From 2011 to 2014, we monitored 20 welfare-oriented laying hen farms in South Korea to figure out serological status of major viral diseases. During this period, total 3,219 blood samples were collected from the randomly selected chickens to test and evaluate the hemagglutination inhibition titers for low pathogenic avian influenza, Newcastle disease and egg drop syndrome '76. A total of 2,926 blood samples were tested through enzyme linked immunosorbent assay (ELISA) to assess the serological status of infectious bronchitis (IB). The distribution of ELISA titers for IB was various from almost 0 to 20,000 through the all weeks of age. Also, the antibody coefficient of variation for most of the diseases in this study was higher than those of typical cage layers. As this study was the first surveillance for major avian viral diseases of the animal welfare-oriented farms in South Korea, the results obtained from this study will help to determine what information and resources are needed to maintain better biosecurity and to improve the health and welfare of laying hen flocks.
Erwinia amylovora에 의해 발생하는 화상병은 2015년에 국내에서 처음으로 보고된 이후 2021년 기준으로 전국 22개 지역으로 전파되었다. 우리나라는 식물방역법에 따라 화상병이 발생한 사과 및 배 과원은 발생주율을 기준으로 모든 기주식물들을 완전히 제거하여 구덩이에 매몰처리한다. 이후, 3년간 화상병균 전파를 막기 위해 매몰지 위에 기주식물 식재를 금하고 있다. 매몰처리법에 의한 화상병균 박멸 효과를 확인하기 위하여, 화상병 감수성 식물을 미끼식물로 이용하여 매몰지에 식재 후 화상병 재발 유무를 확인하였다. 이를 위해, 2019년부터 2021년에 매몰처리한 경기도 안성시와 충북 충주시 소재 매몰지 3곳을 선정하여 미끼식물 감시시설을 설치하였다. 화상병 감수성 식물인 사과(부사)를 미끼식물로 선정하고, 각 감시시설당 5주를 식재하였다. 감시시설은 울타리와 그물로 격리하였다. 또한, 실시간 모니터링을 위한 CCTV와 동작감지기, 그리고 현지 기상상황을 기록하는 센서를 설치하였다. 감시시설을 주기적으로 방문하여 육안으로 화상병 발병 유무를 확인하였다. 미끼식물로부터 화상병 의심 증상을 나타내는 표본을 채취하고 화상병균 특이적 프라이머를 이용하여 loop-mediated isothermal amplification polymerase chain reaction (PCR)과 conventional PCR을 통해 화상병균 감염 유무를 확인하였다. 그 결과, 현재까지 어떠한 미끼식물에서도 화상병균은 검출되지 않았다. 따라서, 현재 시행되고 있는 매몰 후 화상병 기주식물 3년 식재 금지 조항을 완화하는 근거로 본 연구 결과를 제시하고자 하며, 이를 통해 국내 과수산업과 농가의 피해를 최소화하는 데 기여할 수 있을 것이라 생각된다.
