Study on the Environmental Factors and Symptoms of VDT Syndrome (VDT 증후군의 환경적 요인과 증상에 대한 연구)

  • Purpose: Recently incidence of VDT syndrome has gradually increased as extensive use of computers. VDT syndrome reported by VDT workers include musculoskeletal disorder, neuropsychiatric disoders and eye symptoms such as eye strain, tired eyes, irritation and blurred vision. The environmental factors of VDT syndrome include electromagnetic waves, size, brightness and lighting of computer screen, height of a monitor and a worktable, working hours, kind of task, distance between screen and workers, indoor humidity and temperature, indoor air contamination and ventilation. In this study, we investigated the environmental factors related to body symptoms and health effects included in VDT syndrome. Methods: Study subjects were total 120 persons (54 male, 66 female) with age from 19 to 28. We surveyed the body symptoms and physical discomfort when doing an activity in a short distance such as reading book or paper, computer work. The questionnaire included main body symptoms, self-consciousness symptoms of eye, satisfaction of working environment, pain of the wrist when using keyboard and mouse. Results: Most of people (70%) felt physical pain from long time work of computer, paper, electrical apparatus. They mainly complained pain of neck and low back (57.1%), eye (45.2%) and head (31%). With the environmental factors, 78.3% of the subjects complaint pain of eye from inappropriate illumination. Most of the symptoms included 'eye fatigue'(38.3%), 'dryness of eye'(31.9%) and 'blurred vision'(23.7%). Subjects in this study complained discomfort of their chairs and most of them experienced pain in the wrist when using keyboard or mouse. Conclusions: When people use electrical apparatus or work with paper, people would get their eye fatigue and feeling of physical fatigue because of not harmonizing various environmental factors such as light, space, posture, worktable with theirselves. Therefore, workers should develop preventive method such as self-control of adequate break time to avoid fatigue while VDT work. Work environment should be changed to ergonomic design for optimal visual environment to prevent musculoskeletal disorder through constant research.

Self-Esteem and Depression of the Elderly People Living Alone (독거노인의 자아존중감과 우울)

  • This study aims to investigate the related variables of self-esteem and depression among the Korean elderly people living alone and examine the relationships or interactions between those variables. Participants were 676 elderly men and women who were at least 65 years of age(M=76.17, sd=7.60) and lived in Seoul, Korea. Three hundred seventy eight participants of this study were living alone. Participants completed questionnaires and psychological tests including: Rosenberg's Self-Esteem Scale, Zung's Self-Rating Depression Scale, The Index of Activities of Daily Living(IADL), Social Support Index(SSI), and Life Satisfaction in the Elderly Scale(LSES). Main statistical designs were 2(gender)×2(residential types), Pearson-product moment and regression analysis. Results indicated that the elderly living alone recognized their health poorer, have lower economic status, and received less social supports than the elderly lived with others. The elderly men who had children were more likely to lived alone. And the elderly men living alone received less social supports than the elderly women living alone, and significant gender difference was found in the reason of living alone. The elderly men living alone had lower self-esteem than the elderly women living alone, while the elderly living alone showed more depressive symptoms than the elderly living with others. There were 2-way interactions both in self-esteem and depression by gender and residential types. There was highly significant gender difference in self-esteem only for the elderly living alone, and it was found that there was no significant difference in depression between elderly men living alone and women living with others. Regression analysis revealed that physical function and self-reported health are predictors of self-esteem, and physical function, self-reported health, and social support are predictors of depression for the Korean elderly living alone. These findings reiterate the role of physical function, social support, health in self-esteem and depession among the elderly and suggest the gender role for quality of life among the Korean elderly living alone.

Association Between Air Conditioning Use and Self-reported Symptoms During the 2018 Heat Wave in Korea

  • Objectives: The purpose of this study was to investigate the health effects of air conditioning use during the 2018 heat wave in Korea, included the highest temperature ever recorded in the nation. Methods: The participants in this study were 1000 adults aged 19 years and older recruited from across Korea. The participants were asked about their experience of symptoms of various diseases, disruptions of their daily lives, and use of air conditioning during the heat wave. The associations between air conditioning use during the heat wave and health outcomes were analyzed using the chisquare test and multiple logistic regression models. Results: Among participants who lacked air conditioning in the main space where they spent time outside the home, 33.9%, 8.1%, 43.5%, and 19.4% experienced symptoms of heat-related, cardiovascular, nervous system diseases, and air-conditioningitis, respectively. In comparison, participants who did have air conditioning outside the home experienced the same symptoms at proportions of 21.0%, 1.9%, 26.8%, and 34.2%, respectively (p=0.027, 0.007, 0.007, and 0.023, respectively). Among participants who had no air conditioner at home, 10.0% were absent from school or work due to the heat wave. In contrast, among participants who had an air conditioner at home, only 3.7% were absent as a result of the heat wave (p=0.007). Conclusions: When air conditioning was not used at home or in the main space where participants spent time outside the home during the 2018 heat wave, adverse health effects were more prevalent, but the risk of air-conditioningitis was reduced.

Efficacy and Safety of Balloon Kyphoplasty in the Treatment of Osteoporotic Vertebral Body Compression Fractures : Compared with Vertebroplasty

  • Objective : Kyphoplasty and vertebroplasty are two minimally invasive procedures for osteoporotic vertebral compression fractures. The purpose of this retrospective study was to compare the radiological findings and clinical outcomes between two procedures. Methods : Osteoporotic vertebral fractures were treated in 76 vertebrae, using kyphoplasty (n=35 vertebrae) and using vertebroplasty (n=41 vertebrae). Fractured vertebral bodies were diagnosed by correlating the clinical symptoms with radiologic study. The responses of pain symptoms were measured by a self-reported Visual Analog Scale (VAS) score. Plain X-rays were checked preoperatively and postoperatively at admission and 6 months. The vertebral body height and kyphotic angle were measured to assess the reduction of the sagittal alignment. Results : The mean pain scores were decreased significantly for both procedures postoperatively, but there were no significant differences between two groups. Kyphoplasty led to a significant reduction of the vertebral body height and improvement of kyphotic angle. There were no neurological deficits after kyphoplasty, but one patient experienced paraparesis after vertebroplasty. During the 6 months follow-up both procedures provided stabilization of the sagittal alignment. Conclusion : Kyphoplasty and vertebroplasty are considered effective minimally invasive techniques for the stabilization of osteoporotic vertebral body fractures, leading to a statistically significant reduction in pain. Kyphoplasty significantly restore sagittal alignment. Also, complications and the incidence of bone cement leakage are significantly lesser than vertebroplasty. Therefore, kyphoplasty seems to be reasonable procedure for osteoporotic vertebral body compression fractures when medical treatment fail.

Treatment and diagnosis of oral pain without identifiable oral mucosa lesion (정상 구강 점막 소견의 구강 통증 환자의 진단 및 치료)

  • Objectives : Oral pain without identifiable oral mucosa lesion is probably multifactorial origin, which include burning mouth syndrome (BMS), oral candidiasis and so on. The aim of this study was to analyze the characteristics of oral pain without identifiable oral mucosa lesion and to evaluate treatment outcome of those patients. Materials and Methods : We reviewed 50 patients without identifiable oral mucosa lesion who were complaint of oral pain. The patients were analyzed according tothe sites, associated symptoms, laboratory tests and fungus culture. The questionnaire included questions on their current diseases, smoking and alcoholic history, psychological factors, and symptoms. Results : The average age of patients was 60 years old. The most frequently involved site was tongue (92%), followed by palate, lower lip, oropharynx, and gingiva. 60% of the patients has psychological disorder as self reported. Culture for Candida was positive in 36% of patients and serum zinc deficiency was present in 60% of patients. Serum iron, vitamin B12, hemoglobin, folic acid deficiency were present in 6-2% of patients. Seventeen patients (65%) with BMS and twelve patients (66%) with oral candidiasis were improved after treatment. Conclusion : We recommend oral candida culture to oral pain patients without oral mucosa lesion. Zinc supplementation of zinc depletion patients may be helpful whereas other laboratory tests have no diagnostic values.

Effects of Quality of Sleep and Related Factors for Fatigue Symptoms of Nurses in an University Hospital (대학병원 간호사들의 수면의 질과 관련요인이 피로에 미치는 영향)

  • Purpose: The present study was intended to measure fatigue symptoms according to the quality of sleep among nurses working for an university hospital and to reveal its related factors. Methods: The self-administered questionnaires were given to 397 nurses employed in an university hospital located in Daejeon City during the period from June 1st to July 31st, 2007. Results: The group with higher quality of sleep accounted for 78.1% of all the subjects and the one with lower quality, 21.9%. According to qualities of sleep, the fatigue symptom was determined to be significantly higher in the group with lower quality of sleep than its counterpart. On adjustment for quality of sleep, the level of fatigue symptom was significantly higher in the groups, than in each respective counterparts, who don't take regular exercises, who have few leisure time, who have lower level of subjective health status, who range ${\geq}$ 1 and < 10 years in job careers, who work in shifts, who reported that it is hard to get physical burden of work, who are satisfied with the job, who find themselves unfit to the job, who are considering quitting the job, who have higher job demand, who have lower job control, who have higher level of supervisor support, who have type A behavior pattern. Conclusion: The study results may explain that the fatigue symptom of nurses is highly associated with the quality of sleep independently of other variables and also with job-related characteristics independently of the sleep quality. In conclusion, to reduce the fatigue symptom of nurses, strategic efforts should be directed for programs to improve the quality of sleep and a proper job analysis.

Clinical Study on the Correlation between Retention of Fluid, namely Damum(痰飮) and Dark Circles under Eyes (다크 서클(Dark Circles under Eyes)과 담음(痰飮)의 연관성에 관한 연구)

  • Objectives: The aim of this study was to investigate correlation of the phlegm-retained fluid, namely Damum(痰飮) and dark circles under eyes. Methods: 2 males and 11 females aged 26-60 years were participated in this study. They were asked the intensity of 29 symptoms related with Damum(痰飮) including dark circle with the questionnaire published by the Journal of the Korea Institute of Oriental Medical Diagnostics. Three skin researchers including OMD graded the dark circles by inspection. We took the pictures around eyes by Facial Stage (Moritex, Japan) and analyzed the skin color by Image-Pro Plus (Media Cybernetics, USA). Results: There was statistically significant correlation between Damum(痰飮) and dark circles measured by self, inspectors and image analysis. Conclusions: In the Oriental Medicine, It is reported that the shadows under eyes are the sign of the retention of fluid, Damum(痰飮). In our study, that shadows, namely dark circles, has correlation with the symptoms of Damum(痰飮). Especially, the correlation between inspected grade and image analysis was very high. According to the above results, it is proved that dark circles under eyes are important diagnostic sign of Damum(痰飮).

  • Cyclic vomiting syndrome(CVS) is a disorder of unknown etiology that is characterized by its clinical pattern of intermittent episodes of severe vomiting, similar in time of onset and duration, with no symptoms during the intervening period. By definition, CVS is an idiopathic disorder that requires exclusionary laboratory testing. Not only can it be mimicked by many specific disorders, eg, surgical, neurologic, endocrine, metabolic, renal, but within idiopathic CVS there may be specific subgroups that have different mechanisms. It has been reported that CVS usually begins in toddlers and resolves during adolescence. Migraine is also self-limiting episodic condition of children and the clinical features of migraine and CVS show considerable similarity. It is proposed that CVS is a condition related to migraine. This paper reports clinical courses of long term follow-up and reversible EEG changes in three patients whose history included CVS. Clinical situations of attack interval, duration and associated symptoms had changed variablely in each patients through long term follow-up period. Cyclic vomiting subsided in two cases. Abnormal delta activity was seen during episodes and resolved at follow-up, when the patient asymptomatic. The brain wave changes support the interpretation of CVS as a migraine variant.

Factor Analysis of Standardized Depression Scales in Suicidal Attempters among Patients with Depressive Disorders : CRESCEND Study (우울증 자살시도자들에서 표준우울평가척도들의 요인분석 : CRESCEND 연구)

  • Objective : The aim of this study was to find associations between specific symptoms and suicidal behavior by using global severity on depression scales. Methods : Data were obtained from 1,183 patients of CRESCEND study. Factor analysis of the 17-item Hamilton Rating Scale for Depression (HAM-D) and the Beck Depression Inventory-II (BDI-II) was used to compare symptom clusters between the past suicide attempters and the non-suicide attempters, among the patients with depressive disorders. Results : Factor analyses of HAM-D and BDI-II extracted three factors. Suicide attempters had significantly higher scores on core factor of HAM-D, including depressed mood, feeling of guilt, suicide, work, and activities. The higher scores on all factors of BDI-II or scores correlated with a total of 7 suicide attempts and with known risk factors for suicidal behavior. Conclusion : These findings suggest that the suicide attempters complained of more cognitive, affective, somatic symptoms on BDI-II, but the core factor on HAM-D was the only differentiated factor between the two groups. Clinician-rated scales as well as self-reported questionnaires were valid in measuring suicidal attempts, and the clinical profile may help in guiding the studies of biological correlates and the treatments to reduce suicide risk.

Oral Cancer Knowledge and Practice among Dental Patients and their Attitude Towards Tobacco Cessation in Iran

  • Background: One of the main causes of delay in diagnosis of oral cancer is lack of awareness about aetiology and symptoms among the general population. The aim of this study was to assess the knowledge and practice of patients regarding oral cancer and their attitude towards tobacco cessation. Materials and Methods: This study was carried out in Isfahan-Iran in 2014. A 29-item self-administrated questionnaire was designed and piloted and distributed to patients attending dental clinics. Questions were focused on awareness about oral cancer risk factors, signs and symptoms, places in the mouth which are more susceptible and attitude toward tobacco cessation. Chi-square, T-test, ANOVA and logistic regression tests were used for statistical analysis. Results: A total of 546 valid completed questionnaires were obtained. The mean knowledge score of patients was 4.1(${\pm}2.7$) out of 13. Some 80% of patients did not know about early manifestations of oral cancer. Only 18% knew the most likely sites of oral cancer. Only 43.1% and 65.2% of patients reported alcohol and tobacco consumption as the main risk factors but they had a fair knowledge about other risk factors. There was no significant difference in Knowledge level between patients regarding their sex, educational levels and age. Most patents (90%) expected their dentists to warn them about the harmful effects of smoking and showed willingness to quit if recommended. Conclusions: Knowledge about oral cancer was found to be quite low. It seems necessary to increase the level of public awareness using educational programs with cooperation of dentists in tobacco cessation programs.
