Objective: Controlling the incisal inclination is fundamental in orthodontics. However, the relationship between the inclination prescription and its clinical outcome is not obvious, and the incisal inclination changes generated by different bracket prescriptions were investigated. Methods: Twenty-eight non-extraction dental Class II patients (15 females, 13 males; mean age = 12.9) were retrospectively analyzed. Patients were treated using passive self-ligating fixed appliances with three inclination prescriptions for maxillary incisors (high, standard, low), and two for mandibular incisors (standard, low). Clinical outcomes were compared among different prescriptions, and regression analysis was used to explain the effects of bracket prescriptions and to understand the prescription selection criteria (α = 0.05). Results: For maxillary central incisors, low and high prescriptions were related to linguoversion (p = 0.046) and labioversion (p = 0.005), respectively, while standard prescription maintained the initial dental inclination. Maxillary lateral incisors did not show significant changes. For mandibular incisors, low prescription led to linguoversion (p = 0.005 for central incisors, p = 0.010 for lateral incisors), while standard prescription led to labioversion (p = 0.045 for central incisors, p = 0.005 for lateral incisors). The factors affecting inclination changes were the imposed change and selected prescription, while prescription selection was influenced by the initial dental inclination and initial intercanine distance. Conclusions: The direction of correction of incisal inclination can be controlled by choosing a certain prescription, but the final inclination may show limited consistency with it. The amount of imposed inclination change was the most relevant predictor of the clinical outcome.
The objectives of this study is to examine the German mandatory prescription system in terms of the applicability and restriction of the pharmaceutical policy in order to obtain some useful implications for solving the problems in Korean prescription system. Recently, in Germany, an issue about the security of the prescription, the price control of the pharmaceuticals and the containment of the increasing prescription expenditure has been intensively discussed. Similar problems are also occurred in Korea. So, the policy measurements of Germany could be used in Korea. But it could not easy to introduce the German policy measurements in Korea because of the social-institutional differences between the two countries, which are following; (1) Korea has a short experience with the mandatory prescription system, (2) the German concept of the management differs from that of the Korea, (3) the subscribers and the patients are excluded from the decision making process, (4) the medical service providers often resist against reform plans. For the stable development of the Korean prescription system the principle of self-government, the collective bargaining concept for cost containment, and social consensus about optimal expenditure of the pharmaceuticals are expected to be needed.
연구목적 자살시도로 응급실에 내원한 환자들 중 음독이 가장 높은 빈도를 보이고 있다. 본 연구에서는 음독 의약품에 따른 자살 시도자들의 특성을 분석하여 향후 임상 진료 현장에서 도움이 될 제언과 자살 예방 정책을 마련하는데 기초자료를 제공하고자 하였다. 방 법 2011년부터 2019년까지 서울 소재 1개 종합 병원 응급실에 음독 자살 시도 후 내원한 환자 574명의 의무기록을 후향적으로 검토하였다. 의무기록을 통해 인구통계학적 정보 및 자살 과거력, 정신건강의학과 과거력, 정신질환 외 기저질환력, 음독 약물의 종류 및 양, 획득 경로, 단일 혹은 혼합 음독여부 등에 대해서 조사 후 카이제곱 검정 및 독립표본 t-검정을 사용하여 통계 분석하였다. 유의한 결과에 대해서는 위험률 산출을 위해 오즈비를 구하였다. 결 과 일반의약품 음독군이 전문의약품 음독군에 비해 연령은 유의하게 낮았으며 국민건강보험 외 비율은 낮고 배우자가 있는 비율은 유의하게 높았다. 전문의약품 음독군은 일반의약품 음독군에 비해 비자발적 응급실 내원, 정신질환력 및 정신질환 외 기저질환력에서 높은 비율을 보였다. 전문의약품 중 벤조디아제핀 음독군은 비음독군에 비해 본인 처방 비율이 높은 반면 졸피뎀 음독군은 타인처방 비율이 높았다. 벤조디아제핀, 졸피뎀, 항우울제 단독 음독군이 복합 음독군에 비해 유의하게 정신질환력이 없는 비율이 높았다. 결 론 일반의약품 판매 규제에 대한 가이드라인이 필요할 것이며 임상 현장에서 자살 고위험군의 특성을 유념한 면담이 필요하다. 또한 자살 위험성이 높은 환자들에게 전문의약품 처방 시 추적 관찰 기간 및 처방 기간, 포장 방식에 대한 적극적인 고려가 필요할 것으로 보인다.
Purpose: The purpose of this study was to develop a progressive exercise program based on clinical guidelines for exercise prescription for older adults with osteoarthritis, which was planned to improve the existing arthritis self-help program. Method: We analyzed the contents of the arthritis self-help program and closely reviewed the guideline of exercise prescription for elderly with osteoarthritis. Results: The major contents of the revised arthritis self-help program are as follows: 1) The weekly education is composed of one-hour exercise and one-hour health education about various subjects. 2) The weekly main exercise is composed of muscle strengthening and endurance exercise. 3) Endurance exercise consists of three steps. 4) Muscle strengthening exercise is performed by gravity at first, and then the Thera-Band muscle strengthening exercise is followed on the 4th week. Conclusion: It is necessary to verify the effect of the progressive exercise program for older adults with osteoarthritis. It is thought that this revised program could be adopted as a rehabilitation program for older adults with osteoarthritis.
Objective: The purpose of this study was to evaluate the patterns of Over-the-Counter (OTC) drugs and their interactions with prescription drugs in adults visiting a community pharmacy. Method: The subjects were 151 adults aged over 20 years visiting a community pharmacy in Asan-si from December 16th 2011 to February 1st 2012. We used a survey questionnaire. The survey inquired about the prevalence and the details of any OTC drug use and the characteristics of the study subjects. The drug interaction classification system from Lexicomp's Lexi-interact data fields was used to identify OTC drugs likely to have clinically significant interactions with prescription drugs. Results: The patterns of OTC drug use were related to thirties (from 30 to 40 years old), female gender, higher education, non-smoking, sometimes use of alcohol, and self-perceived normal health status. The most commonly used OTC drug category was antipyretic-analgesics (n=104, 53.3%), and the most commonly used ingredient was acetaminophen (n=67, 64.4%). The biggest motivation for taking OTC drugs was suggestion by pharmacists, reported by 55.6%. After reviewing each patient's prescription drugs and OTC drugs, 14 patients (36.8%) of 38 patients using prescription drugs were taking drug combinations with potential for clinically significant interactions. The concomitant use of OTC drugs with prescription drugs may lead to increased potentially harmful interactions. Conclusion: It is suggested that health-care professionals should be more aware of the potential and possible interactions and take into better account their patients' OTC drug use.
The purpose of this study was to contribute to the development of clinical treatment for psoriasis through analyzing and investigating the result of the process of the treatment used for the treatment for psoriasis with prescription of Gamisoyosan, prescription for psoriasis No.1, and No.2. This clinical treatment approached by using 3 steps. In Step 1 7 patients with psoriasis were classified based on the severity of the disease and the types of psoriasis. Step 2, Proper treatment was applied to patients in accordance with typical types of symptoms and types of diagnosis in Oriental Medicine. In step 3, all the process of the treatment was recorded with pictures and the patients' signs of self-consciousness from initial treatment to final treatment determined the result. As a result of the treatment, it showed that prescription of Gamisoyosan applied to pustular psoriasis on the palms, pustular psoriasis on the sole of feet and geographic psoriasis on the whole bodies worked effectively and prescription for psoriasis No.1 applied to nummular psoriasis and prescription for psoriasis No.2. applied to nummular psoriasis made progress for the patients. Side effects and any other abnormal symptoms were not found. In conclusion, therefore, it is anticipated that selected treatment and prescription should be given according to types of diagnosis in Oriental Medicine referring the types of psoriasis. It is considered that more attention, deep and thorough study in mechanism of pathogenesis mechanism of treatment according to the types of psoriasis may make it possible for doctors to treat and prescribe patients.
The effects of characteristics of community pharmacists on consultation time for prescription (RX) and nonprescription drugs (nRX) were investigated. A crosssectional descriptive design was established with a self-administered anonymous mail survey. Response rate was 52%. Significant pharmacists' factors related to having less than 3 minutes consultation for RX were one's dispensing duties exceeding 50%, low satisfaction with one's own level of consultation, working at the pharmacies nearby clinics, or shorter duration of consultation time for nRX. Consultation promoting conditions should be built up for the provision of sufficient consultation.
Purpose: The purpose of this study was to describe role conflict and job satisfaction of Neurosurgery Clinical Nurse Specialist(NCNS) and to identify associated factors. Method: The target populations was 77 NCNSs from 30 general hospitals. Data were collected with self-administered questionnaires and analyzed using t-test and ANOVA. Results: The overall mean scores of role conflict and job satisfaction were $3.60({\pm}0.54)$ and $3.04({\pm}0.46)$ respectively. Significant variables affecting role conflict were the working period as NCNS, the number of colleague NCNSs in working hospital, and assignment of prescription rights. The significant variable affecting job satisfaction was assignment of prescription rights. There was no statistically significant correlation between job and role conflict. Conclusion: The variable effecting both role conflict and job satisfaction was the extent of prescription rights. In order to improve the quality of patient care and to protect Neurosurgery Nurse Clinical Specialist, the enactment of law on prescription practice is needed.
Purpose: This study was performed to examine predictors of allied health science students' exercise behavior using Pender's Health Promotion Model. Method: The subjects were 203 university students majoring in nursing, emergency medical service, and exercise prescription. The hypothetical model was based on the HPM(Pender, 1996). Exogenous variables of the model were exercise habit and role belief. Endogenous variables were exercise-related affect, exercise self-efficacy, exercise barrier, exercise intention, and exercise behavior. The data were analyzed by SAS PC program and LISREL 8.12 program. Result: The degree of exercise behavior was low(mean 1.86, range 1~4). The overall fit of the model to the data was acceptable. Exercise habit, exercise self-efficacy and exercise-related affect were significant predictors of exercise intention and exercise behavior. Conclusion: This study shows the necessity of the program to increase the level of exercise participation of university students majoring in allied health science. The model constructed in this study is applicable to explain exercise behavior of university students majoring in allied health science, and suggests that we should focus on exercise habit, exercise self-efficacy and exercise-related affect to increase the level of exercise behavior of this group.
