Analysis of the Effectiveness of Tai Chi Exercise in Elderly (노인에게 적용한 타이치운동의 논문분석)

  • Purpose: This study aims to conduct a systematic review of the physical, psychosocial and physiological effects of Tai Chi exercise in elderly. Method: 37 articles from Medline search of foreign journals (1966-2006) were surveyed by the key words 'Tai Chi', 'Tai-chi program' and 'Tai-chi and elderly' limiting the range to age 65 or older, choosing clinical trial and randomized controlled trial research in English articles. Four articles were excluded due to methodological study, pilot study and review. The research was analyzed according to health status of the subjects, styles and forms of Tai Chi exercises, factors for physical, psychosocial and physiological measures. Result: A short forming Yang style was commonly used in chronic diseases and health for elderly. Variable outcome measures were used in Tai Chi studies relating to balance, muscle strength, walking and mobility, flexibility and cardiorespiratory function in physical measure, quality of life, depression, self-efficacy, health status, cognition and impact questionnaire in psychosocial measure, lipids, insulin resistance and hormone in physiological measure. Conclusion: Tai Chi exercise appears to have physical and psychosocial benefits and also appears to be safe for elderly and chronic diseases. It is suggested that future studies analyze statistical part of systematic reviews through meta analysis.

Smart syringe pumps for drug infusion during dental intravenous sedation

  • Dentists often sedate patients in order to reduce their dental phobia and stress during dental treatment. Sedatives are administered through various routes such as oral, inhalation, and intravenous routes. Intravenous administration has the advantage of rapid onset of action, predictable duration of action, and easy titration. Typically, midazolam, propofol or dexmedetomidine are used as intravenous sedatives. Administration of these sedatives via infusion by using a syringe pump is more effective and successful than infusing them as a bolus. However, during intravenous infusion of sedatives or opioids using a syringe pump, fatal accidents may occur due to the clinician's carelessness. To prevent such risks, smart syringe pumps have been introduced clinically. They allow clinicians to perform effective sedation by using a computer to control the dose of the drug being infused. To ensure patient safety, various alarm features along with a drug library, which provides drug information and prevents excessive infusion by limiting the dose, have been added to smart pumps. In addition, programmed infusion systems and target-controlled infusion systems have also been developed to enable effective administration of sedatives. Patient-controlled infusion, which allows a patient to control his/her level of sedation through self-infusion, has also been developed. Safer and more successful sedation may be achieved by fully utilizing these new features of the smart pump.

Dose-Dependent Inhibition of Melanoma Differentiation-Associated Gene 5-Mediated Activation of Type I Interferon Responses by Methyltransferase of Hepatitis E Virus

  • Hepatitis E virus (HEV) accounts for 20 million infections in humans worldwide. In most cases, the infections are self-limiting while HEV genotype 1 infection cases may lead to lethal infections in pregnant women (~ 20% fatality). The lack of small animal models has hampered detailed analysis of virus-host interactions and HEV-induced pathology. Here, by employing a recently developed culture-adapted HEV, we demonstrated that methyltransferase, a non-structural protein, strongly inhibits melanoma differentiation-associated gene 5 (MDA5)-mediated activation of type I interferon responses. Compared to uninfected controls, HEV-infected cells display significantly lower levels of $IFN-{\beta}$ promoter activation when assessed by luciferase assay and RT-PCR. HEV genome-wide screening showed that HEV-encoded methyltransferase (MeT) strongly inhibits MDA5-mediated transcriptional activation of $IFN-{\beta}$ and $NF-{\kappa}B$ in a dose-responsive manner whether or not it is expressed in the presence/absence of a tag fused to it. Taken together, current studies clearly demonstrated that HEV MeT is a novel antagonist of MDA5-mediated induction of $IFN-{\beta}$ signaling.

Scombroid Fish Poisoning and Histamine Food Poisoning (스콤브로이드 생선 중독과 히스타민 식중독)

  • Scombroid fish poisoning (SFP) is a form of histamine food poisoning caused by the ingestion of improperly stored fish. The term "scombroid" derives from the family name of the fish family first implicated, such as tuna and mackerel. On the other hand, non-scombroid fish species, such as sardine and herring, can also cause histamine poisoning. The histamine is converted from histidine by a bacterial enzyme in the causative fish. Because the symptoms of SFP can easily be confused with food allergies, it is believed to have been significantly under-reported. In 2016, an outbreak of SFP occurred among primary school students who had eaten yellowtail steak in Korea. The most common findings consisted of a rapid onset of flushing of the face and trunk, erythematous and urticarial rash, diarrhea, and headache occurring soon after consuming the spoiled fish. Usually, the course is self-limiting and antihistamines can be used successfully to relieve symptoms, but several life-threatening SFP cases have been reported. Clinical toxicologists should be familiar with SFP and have competency to make a differential diagnosis between fish allergy and histamine poisoning. SFP is a histamine-induced reaction caused by the ingestion of histamine-contaminated fish, whereas a fish allergy is an IgE-mediated reaction. This review discusses the epidemiology, pathophysiology, diagnosis, treatment, and preventive measures of SFP.

Current perspectives on atypical pneumonia in children

  • The major pathogens that cause atypical pneumonia are Mycoplasma pneumoniae, Chlamydophila pneumoniae, and Legionella pneumophila. Community-acquired pneumonia (CAP) caused by M. pneumoniae or C. pneumoniae is common in children and presents as a relatively mild and self-limiting disease. CAP due to L. pneumophila is very rare in children and progresses rapidly, with fatal outcomes if not treated early. M. pneumoniae, C. pneumoniae, and L. pneumophila have no cell walls; therefore, they do not respond to β-lactam antibiotics. Accordingly, macrolides, tetracyclines, and fluoroquinolones are the treatments of choice for atypical pneumonia. Macrolides are the first-line antibiotics used in children because of their low minimum inhibitory concentrations and high safety. The incidence of pneumonia caused by macrolide-resistant M. pneumoniae that harbors point mutations has been increasing since 2000, particularly in Korea, Japan, and China. The marked increase in macrolide-resistant M. pneumoniae pneumonia (MRMP) is partly attributed to the excessive use of macrolides. MRMP does not always lead to clinical nonresponsiveness to macrolides. Furthermore, severe complicated MRMP responds to corticosteroids without requiring a change in antibiotic. This implies that the hyper-inflammatory status of the host can induce clinically refractory pneumonia regardless of mutation. Empirical macrolide therapy in children with mild to moderate CAP, particularly during periods without M. pneumoniae epidemics, may not provide additional benefits over β-lactam monotherapy and can increase the risk of MRMP.

Self Updatable Pseudonym System for VANET (VANET를 위한 차량자체 갱신가능 익명ID 시스템)

  • Since message forgery or alteration in VANET may cause severe consequences, authentication of critical messages must be provided. However, using normal digital signature may infringe privacy of drivers. Therefore, VANET requires authentication systems that provide conditional anonymity. In this paper, we propose a new authentication system for VANET. In our proposed system, each vehicle can update its pseudonym using re-encryption technique and digitally sign messages using representation problem on the pseudonym. By limiting the usage period, revocation of individual pseudonym is not required. Moreover, we also provide a way to revoke the vehicle itself. Secureness of our system partially rely on the usage of tamper-resistance hardware.

Hemophagocytic lymphohistiocytosis with recurrent Kikuchi-Fujimoto disease

  • Kikuchi-Fujimoto disease (KFD), also known as histiocytic necrotizing lymphadenitis, is a self-limiting lymphadenitis. It is a benign disease mainly characterized by high fever, lymph node swelling, and leukopenia. Hemophagocytic lymphohistiocytosis (HLH) is a life-threatening disease with clinical symptoms similar to those of KFD, but it requires a significantly more aggressive treatment. A 19-year-old Korean male patient was hospitalized for fever and cervical lymphadenopathy. Variable-sized lymph node enlargements with slightly necrotic lesions were detected on computed tomography. Biopsy specimen from a cervical lymph node showed necrotizing lymphadenitis with HLH. Bone marrow aspiration showed hemophagocytic histiocytosis. The clinical symptoms and the results of the laboratory test and bone marrow aspiration met the diagnostic criteria for HLH. The patient was diagnosed with macrophage activation syndrome-HLH, a secondary HLH associated with KFD. He was treated with dexamethasone (10 mg/m2/day) without immunosuppressive therapy or etoposide-based chemotherapy. The fever disappeared within a day, and other symptoms such as lymphadenopathy, ascites, and pleural effusion improved. Dexamethasone was reduced from day 2 of hospitalization and was tapered over 8 weeks. The patient was discharged on day 6 with continuation of dexamethasone. The patient had no recurrence at the 18-month follow-up.

Recent progress on Performance Improvements of Thermoelectric Materials using Atomic Layer Deposition (원자층 증착법을 이용한 열전 소재 연구 동향)

  • Atomic layer deposition (ALD) is a promising technology for the uniform deposition of thin films. ALD is based on a self-limiting mechanism, which can effectively deposit thin films on the surfaces of powders of various sizes. Numerous studies are underway to improve the performance of thermoelectric materials by forming core-shell structures in which various materials are deposited on the powder surface using ALD. Thermoelectric materials are especially relevant as clean energy storage materials due to their ability to interconvert between thermal and electrical energy by the Seebeck and Peltier effects. Herein, we introduce a surface and interface modification strategy based on ALD to control the performance of thermoelectric materials. We also discuss the properties of the interface between various deposition materials and thermoelectric materials.

Eclectic rehabilitation for bell's palsy: A case report

  • Bell's Palsy is one of the most common mononeuropathies or disorders affecting a single nerve and is associated with facial nerve weakness and paralysis. Though self-limiting the disorder may leave its long-lasting residual manifestations in the form of abnormal facial symmetry, inability to close the eyes, and other poor outcomes leading to disability and impairment in societal functioning among patients. Treatment strategies include pharmacological, surgical, and therapeutic options and to limit the long-term devastating effects therapeutic options play a vital role. Physiotherapeutic techniques have been widely used among patients with Bell's Palsy but not all techniques are performed in combination. One is compared with the other and also for short durations. We planned this study to see the outcome of combined techniques available in Physiotherapy on a patient with long-term follow-up. This is a case of 38 years old male patient diagnosed with right-sided bell palsy who received 7 weeks of rehabilitation in the form of electrotherapy, facial exercises, facial PNF, massage, and education on eye care. The implementation of 7 weeks of physiotherapeutic rehabilitation led to improved facial functions and a reduction in the level of disability in the patient.

  • Expectations for artificial intelligence technology are increasing, and its utility value is growing, leading to active use in the public sector. The use of artificial intelligence technology in the public sector has a positive impact on aspects such as improving public work efficiency and service quality, enhancing transparency and reliability, and contributing to the development of technology and industries. For these reasons, major countries including Korea are actively developing and using artificial intelligence in the public sector. However, artificial intelligence also presents issues such as bias, inequality, and infringement of individuals' right to self-determination, which are evident even in its utilization in the public sector. Especially the use of artificial intelligence technology in the public sector has significant societal implications, as well as direct implications on limiting and infringing upon the rights of citizens. Therefore, careful consideration is necessary in the introduction and utilization of such technology. This paper comprehensively examines the legal issues that require consideration regarding the introduction of artificial intelligence in the public sector. Methodological discussions that can minimize the risks that may arise from artificial intelligence and maximize the utility of technology were proposed in each process and step of introduction.
