  • Purpose: The purpose of this study was to investigate the clinical decision making (CDM) development process throughout the comparison between novice and expert physical therapist as well as develop a CDM model for physical therapists under the fee-for-service (FFS) and physicians' prescriptions. Methods: Purposive sampling techniques were used to select 10 clinical physical therapists paired into five groups (each pair consisted of 1 novice and 1 expert physical therapist). The coding schemes were extracted from interviews and through within- and across-case analyses, cases were summarized. The reliability of coding schemes was confirmed by checking of case summaries by the participants. Results: Novice and expert physical therapists were influenced by two themes, internalized theme and external forces or information. Novice clinicians depended more on external forces or information. Although clinicians should care patients under the FFS and physician's prescription, expert clinicians were more likely to rely on internalized knowledge. Conclusion: The findings of the present study may be used by educators or association officials enhance CDM abilities and knowledge pools of student or novices as well as develop a guide to suitable novices or students under the specific context limiting the development of their CDM.

  • The purpose of the study is to examine the use of medication among adults by comparing the pattern of outpatient prescription drug use with the pattern of long term taking lifestyle drug use. Furthermore, the study investigates factors associated with the use of medication, particularity focusing on socioeconomic factors. Korea Health Panel data of 2008 was used to conduct the study analysis. By performing four different logistic regression models, the study noticed different patterns of the medication use between prescription drugs and lifestyle drugs. More specifically, the study showed that adults with lower education level tend to more frequently receive prescriptions while adults with higher education as well as income level tend to more use lifestyle drugs than their counterparts. Furthermore, other control factors such as age and gender were statistically significant for the use of both prescription and lifestyle drugs in different patterns. The study findings expect that reimbursement structure of drugs may be significantly associated with the different patterns and accordingly the accessability of medicine in particularly vulnerable population. Therefore, these policy factors should be considered in future study to more comprehensively understand about the diverse patterns in the medication use.

For example, standard Romanized nomenclature for acupuncture points, herbal medicines, prescriptions and classic books have been achieaved. However, studies on standard Romanized nomenclature for persons who was involved in traditional medicine are rare. Methods : Following up all articles in Journal of Korean Medical Classics[J. KMC], which have Romanized name for persons and analyzing problems of those. Results : There were several methods to write in Romanized character even to an unique and replaceable name. Conclusions : Romanized nomenclature for persons' name who were involved in traditional medicine as a terminology must have unique expression. • Objectives : This paper studies the Tangaek-Unhoei(湯液韻彙) index of herbal medicine in the Inje-Ji(仁濟志) of the Imwon-Gyeongje-Ji(林園經濟志), which contains about 4,800 formulas. Created by 19th-century Joseon scholar Seo, Yugu, it not only lists the formulas according to their names, but also provides index by topic, which enabled the collection and effective application of massive medical information. Methods : We quantitatively examined the nearly 4,800 herbal medicines in the Tangaek-Unhoei and their categorization. Any uncommon or particular categorization was examined further by analyzing the original text. Results & Conclusions : The prescriptions contained in the Inje-Ji are categorized under 26 headings. They are listed according to the 106 units of the Chinese character dictionary and organized by double headings. This unique index makes it easy to browse the contents of such a vast book containing massive medicinal knowledge. In addition, the fifty or so remedies called 'Fang(方)' exemplify the author's attitude toward medicinal knowledge, which is both rational and inclusive. This is an attitude that should be recognized beyond tradition.

  • Although Oriental medicines have long been used effectively in treating many diseases throughout the world, the pharmacological mechanisms of most Oriental medicines used have not been defined. As part of our continuing search for biologically active antiallergic drugs from natural sources, Oriental medicines were analyzed. Some Oriental medicines have been used against various allergic diseases for generations, and still occupies an important place in traditional medicine in Korea. It is also still unclear how Oriental medicine prevents allergic disease in experimental animal models. Some Korean folk medicines inhibited the mast cell-mediated allergic reaction. This review summarizes the effective folk medicine in experimental effect of allergic reaction. Potential antiallergic folk medicines include: Poncirus trifoliata; Siegesbeckia glabrescence; Solanum lyratum; Aquilaria agallocha; Ulmi radicis; Polygonum tinctorium; Hwanglyun-Haedok-Tang; Rehmannia glutinosa; Kum- Hwag-San; Syzygium aromaticm; Spirulina platensis; Sosiho-Tang; Sinomenium acutum; Schizonepta tenuifolia; Shini-San; Magnoliae flos; Sochungryoung-Tang; Oryza sativa; Cryptotympana atrata; Salviae radix; Rosa davurica; Asiasari radix; Chung-Dae-San; and Cichorium intybus. Understanding the mechanisms of action for these Oriental medicines can permit drug development and laying of the ground-work for evaluating potential synergistic effects by addition and subtraction of prescriptions.

  • Objectives : Acupoints are divided into three categories: classical acupoints, extra-acupoints, and Ashi points. The aim of this study was to understand the meaning and features of Ashi points. Methods : We examined the original meaning of Ashi points from the classical medical texts, including the Beijiqianjinyaofang, the Essential Prescriptions Worth a Thousand Gold for Emergencies, and the Huangdi Neijing, the Yellow Emperor's Canon of Internal Medicine. Results : First, the Ashi method is to locate the points for the acupuncture and moxibustion based on the patients' reactions mainly manifested by sensations of comfort and pain, which can help identify not only Ashi points but also classical and extra-acupoints. Thus, Ashi points may or may not match to classical or extra-acupoints, and we propose that Ashi points should not be classified mutually exclusive to classical or extra-acupoints. Second, there are several similarities between Ashi points and myofascial trigger points. They are located by palpation and have no fixed anatomical positions. Patients experience painful but pleasant feeling when Ashi and myofascial trigger points are pressed, and stimulation of these points have treatment effects. Conclusions : We suggest that Ashi method be used to identify the acupoints based on how patients react when these points are pressed. Ashi points may or may not correspond to classical or extra-acupoints, and share traits with myofascial trigger points.
