Effects of the Educational Program for Patients of T.K.R.A. (슬관절 치환술 환자의 교육 프로그램 효과에 관한 연구)

  • This study is aimed to evaluate the effects of the educational program that is designed to help the patients of T.K.R.A. to improve their self-nursing ability. The research will show how well this program can contribute to raising of their adaptability of everyday life by decreasing the difficulties that the patients had before in their daily life. The sample persons for this research were 40 adult patients who had T.K.R.A. in our hospital, and the data were collected through the questionnaires which were given to them. For the surveying tool, Jette's scale was applied to measure the degree of uncomforting that the patients experienced in their daily life. The materials of report had been gathered from May 1st to December 10th. The collected data were analyzed with real numbers, percentage, average, the standard deviation, TorF test and Pearsons correlation. The results of this study can be summarized like follows: The survey of self-caring exercise part were processed with two groups containing experiment group and contrast group. The exercise which took the highest points in experiment group was repetition of stretching and bending knees with laid down position while taking a walk, repetition of stretching and bending knees with seated on chair position and repetition of stretching and bending knees with laid down position got high points in contrast group. There were distinctive differences in statistics between repetition of stretching and bending knees in laid down position and taking a walk. There was no significant statistical gap between the degrees of inconveniences of daily life that each two groups experienced when we compared the average of the two groups, but 12 out of 20 questions got considerable differences. According to common characters of self-caring, there was a distinctive gap in the group who left the hospital in 14days and the ones who spend more than 30 minutes with exercise. The patient group who left the hospital in fourteen days after T.K.R.A. spent more time doing self-caring exercise. In the relationship between common characters and the degree of difficulties of daily life, Exercising is effective and Spending more that 30 minutes for exercise showed visible differences. The patients who answered that exercise are effective and put more time for it experienced less pain in their everyday life. The data proved by Pearson's correlation showed the relationship between self- caring and the degree of pain of daily life. According to the result, the increasing of self-caring activities helped to down the degree of inconveniences of daily life. This data show that T.K.R.A. reduced self-caring exercise ability and daily life activities. Therefore, the medical staffs who would take care the T.K.R.A. patients should offer stable nursing through well scheduled and education materials for patients. It is also important to encourage the patients to participate to the exercise by showing them how exercise after T.K.R.A. The education program is expecting to guarantee not only the right of patients to get enough information but also effective result to decrease uncomfortable of daily life.

The Effects of Radix aconiti on the Contraction of Rat's Spinal Cord Injury (부자(附子)의 경구투여(經口投與)가 RAT의 척수손상(脊髓損傷)에 미치는 영향(影響) 및 비교(比較))

  • Purpose In this study, We observed the recovery process in the ability to move in the hind limbs of the rats whose spinal cord injuries were treated by Radix acouniti(RA). The purpose of this study is to see the effects of Radix acouniti(RA) water extract on the contraction of rat's spinal cord injury. Procedure First, the rats were subjected to hemisectional spinal cord injuries by a scalpel blade. Those rats, then, were divided into three groups: Sham operated rats group and the experimental group, which received the Radix acouniti(RA) water extract, and the control group, which had no treatment. Their recovery in the ability to walk was observed by the Open Field Test (OFT) for 14 days after the injuries. Method The OFT was applied at four points: the hip, knee, ankle joint, and the tail. Each joint was given a movement rating of from 0 to 3, depending on the amount of movement. A movement rating of 0 designates no movement, a 1 designates slight movement, a 2 designates increased movement, and a 3 designates active movement. Slight movement is defined as a joint displaying less than or equal to 30% of that joint range, increased movement is displaying less than or equal to 60% of that joint's range, and active movement is greater than or equal to 90% of that joint's range. Tail movement is also graded on a scale of 0 to 3. A rating of 0 indicates that the tail is down 100% of the time, one of 1 indicates that the tail is down more than 10%, one of 2 shows that the tail is down less than 50% but more than 10% of the time, and one of 3 shows that the tail is down less than 10% of the time. All four ratings were added together and then averaged to arrive at a single score. Results The sham group which did not go through spinal cord injuries showed near normal results on all 3 joints and tail from right after the operation, which one would expect. The RA oral application group showed more effective recovery of movememt function than the control group around 4 days after the spinal cord injuries. However, after 14 days, both groups displayed almost the same degree of movement recovery. The results of this study are summarized as follows: 1. After 14days the spinal cord injuries, movement was recovered in sham operated group, control group, and experimented group in the hip, the knee, the tail and then the ankle of rats, in that order. 2. Around 7 days after the spinal cord injuries, the experimental group proved the effectiveness of the therapy in terms of movement recovery. 3. The level of ALT, ALP, AST in RA treated group was slightly increased. 4. The level of BUN and creatinine in RA treated group was slightly increased. The above results indicate that RA therapy at an early stage can bring about better movement recovery in patients with spinal cord injuries from traffic accidents or industrial disasters. But there is apparent side effect of RA on clinical, therefore the study on this should be continued.

Dynamic Test of a Full Scale Model of Five-Story Stone Pagoda of Sang-Gye-Sa (쌍계사 오층석탑 실물 크기 모델의 동적 거동 시험)

  • There occurred a moderate size earthquake of Magnitude 5 in Whagae-Myun, Hadong-GUn, Kyongsangnam-Do on July 4, 1936. It caused severe damage to the buildings and other structures in Sang-Gye-Sa, a Buddhist Temple. The top component of a five-story stone pagoda was tipped over and fell down to the ground during the earthquake. In order to have accurate and quantitative estimate of the peak acceleration level of that earthquake, a full-scale model was constructed through rigorous verification process. The complete model was mounted on a shaking table and subjected to the dynamic tests. Two kinds of tests were performed: exploratory test and fragility test. The exploratory test was done with low acceleration level. In the fragility test, the behavior of the model was carefully monitored while increasing the acceleration level. The construction details of the model are provided and test procedures are reported. Finally important test results are presented and their implications are discussed.

  • Objective : The purpose of this study is to investigate the effect of mental practice on increasing balance and fall index & fall efficacy scale of persons with post stroke. Method : As a single subject research design with multiple baseline across individuals, the patients were four stroke patients, employed in this study. The employed program included receiving mental practice, and measuring balance score changes using FRT for 4 weeks, including baseline and intervention periods. The subject's fall danger were measured by Fall Index Test(Tetrax) and Fall Efficacy Scale for balance ability were analyzed. The analyses were performed using visually and Two Standard Deviation Band Method. Result : Using FRT, the participants improved their standing balance. During the intervention periods, Their improvement of balance skills results in decreasing Fall Index increasing Fall Efficacy. Conclusion : As a result of this study, mental practice can be an effective method to improve the balance of stroke patien.

Numerical Study of Surface Heat Transfer Effects of Multiple Fan-Shaped Small-Scale Fins (다중 미세 날개구조의 표면 열전달에 미치는 영향분석)

  • In this work, we study a heat transfer enhancement technology using fan-shaped small-scale fins. Fins having a thickness of 10 ${\mu}m$ move up-down by a pulsating flow. Owing to these motions, the heat transfer on a surface increases dramatically. The two-way FSI (fluid-structure interaction) method was applied for the analysis, and the analysis model was evaluated using a single fin model by comparing the experimental results. In summary, a maximum 40% increase in heat transfer capacity using a single and multiple small-scale fins was obtained in comparison with the results obtained without using fins. From this work, we believe that the proposed method can be a promising method for heat transfer enhancement in real applications.

The Validity and Reliability of Reflux Symptom(RSI) Index and Reflux Finding Score(RFS) (역류증상지수와 역류소견점수의 타당성과 신뢰도)

  • Laryngopharyngeal reflux (LPR) is the retrograde movement of gastric contents into the larynx, pharynx, and upper aero-digestive tract. LPR differs from gastroesophageal reflux in that it is often not associated with heartburn and regurgitation symptoms. Otolaryngological manifestations of acid reflux include a wide range of pharyngeal and laryngeal symptoms. Belafsky et al. developed a useful self-administered tool, the reflux symptom index (RSI), for assessing the degree of LPR symptoms. Patients are asked to use a 0 to 5 point scale to grade the following symptoms: 1) hoarseness or voice problems; 2) throat clearing; 3) excess throat mucus or postnasal drip ; 4) difficulty swallowing; 5) coughing after eating or lying down; 6) breathing difficulties ; 7) troublesome or annoying cough; 8) sensation of something sticking or a lump in the throat; 9) heartburn, chest pain, indigestion or stomach acid coming up. A RSI score greater than 13 is considered abnormal. As there is no validated instrument to document the physical findings and severity of LPR, Belafsky et al. developed an eight-item clinical severity scale for judging laryngoscopic finding, the reflux finding score (RFS). They rated eight LPR-associated findings on a scale from 0 to 4 : subglottic edema, ventricular obliteration, erythema/hyperemia, vocal-fold edema, diffuse laryngeal edema, posterior commissure hypertrophy, granuloma/granulation tissue, and thick endolaryngeal mucus. A RFS score of greater than 7 was found to suggest LPR-associated laryngitis. Although both indices (RSI and RFS) are widely used, there is some controversy about their validity (sensitivity and specificity) and reliability (intra-rater and inter-rater) in LPR diagnosis and treatment. We discuss the validity and reliability of RSI and RFS with literature review.

Full-scale Fire Suppression Test for Application of Water Mist System in Road Tunnel (미분무수 소화시스템의 도로터널 적용을 위한 실물 화재 실험)

  • The full-scale experiments are carried out to investigate the fire suppression characteristics of water-based fire fighting systems in a road tunnel. Applied systems are the low-pressure water spray system at 3.5 bar and the high-pressure water mist system at 60 bar. The water flow rate of the high-pressure system is one sixth only of the water spray system. A passenger car and a heptane fuel pan with area of $1.4m^2$ are used as fire sources. A blower system is installed at the tunnel exit to realize the longitudinal ventilation conditions (0.9~3.8 m/s) in the tunnel. Temperatures from the fire source to the down-stream direction are measured by K-type thermocouple trees. The experimental results show that the cooling effect of the high pressure water mist system in the test conditions were equivalent to that of the low pressure water spray system for B-class fire.

  • While growth of domestic CM market has slowed down due to economic recession, it recently shows sign of scale as some public agencies are relocating their headquarters. Since the introduction of CM system, a number of client organizations have acquired construction project management capabilities, however, sufficient efforts are not made to estimate the size of construction project management organization. This study drew a method for estimating the size of the organization managing construction project by utilizing statics analysis and project costs per member based on 10 public construction projects and tried to review applicability through case studies. With this study, we would like to make a contribution to achieving smoother work performance and success of the construction project by estimating and allocating proper scale of management workforce.

An Exploratory Study on the Ethical Attitudes of Consumers in Consumption Situations;based on the Consumer Ethics Scale (소비 상황에서 소비자의 윤리적인 태도에 관한 탐색적 연구)

  • The purpose of this study was to examine the ethical attitudes of consumers in consumption situations by using the consumer ethics scale(CES). Self-administrated questionnaires were completed by 492 consumers, and the resulting data were analysed by frequency, chi-square, one-way ANOVA, factor analysis, reliability analysis, and cluster analysis. We found four different groups of consumers were identified according to their ethical attitudes in consumption situations: 1) the unethical onlooker consumer group, 2) the socialized unethical consumer group, 3) the ethical consumer group, and 4) the severely unethical consumer group. The unethical onlooker consumer group was most widespread prevalent among university students who were in their 20s and down or under, and who received spending money from their parents, and the socialized unethical consumer group was most prevailing prevalent among the higher educated consumers. The women who consumers slightly outnumbered men in an the ethical consumer group, slightly outnumbered the men who did, and many of the university-educated large income earners fell into this group consumed in that way as well. The severely unethical consumer group was quite uncommon among those who were at the age of 41 and up over, and not many of the higher educated people and larger income earners consumed in an the severely unethical consumer group, either. Thus, it's proven this study proves that in terms of the propensity to consume, the consumers' were different from one another in ethical attitudes and behaviors differ according to their demographic characteristics.

AThe Effects of Public Loan Programs in Fishery Industry on Management Performance and Credit Rating Change from a BSC perspective (BSC관점에서 수산정책자금이 경영성과와 신용등급 변화에 미치는 영향)

  • This study investigated the difference of the effects of public loan programs in fishery industry on management performance from a balanced score card (BSC) perspective depending on the type of loan, scale of fund, period of support and business category, using the financial data of fisheries firms having the balance of loan at the end of 2014. The key factors influencing credit rating change were also analyzed after public loan support. From a integrative perspective, results show that the firms supported by working fund have higher management performance than the firms supported by facility fund. The firms received large scale fund showed higher management performance than the firms received small scale fund. While management performance was decreasing or slowing down over time after financial support, management performance of the firms supported by facility fund improved over time. From a non-financial perspective, the firms received facility fund invested more in education and growing perspective than the firms received working fund. As the size of fund increased, the investment in education, growing, internal process and customer increased. Personnel expenses and employee benefits for education and growing has increased over time. However, the firms with facility fund restricted the expenses of education, personnel expenses and employee benefits as time goes by. Because the effects of public loan on credit rating of fisheries corporations have no statistical significance, it has become known that the financial support of public loan program has no influence on the change of credit rating of fisheries corporations. This study attempted performance analysis from a BSC perspective which combine factors of non-financial perspective with factors of financial perspective. Findings from this study suggest the direction of microscopic performance analysis of public loan in fishery industry.
