Objectives : It examines influence of satisfaction with dental esthetics upon quality of life related to oral health after examining the satisfaction with dental esthetics and the quality of life related to oral health according to subjective perception. Methods : The subjects who have experience of dental esthetics from June 2010 to August 2010 in 50 places of hospitals and clinics with dental esthetics in Seoul, which were selected with convenience sampling, the structured questionnaire was widely distributed to 315 people who agreed to research. After making the subjects respond with self-reported method, data was collected. 298 copies except 17 copies were used in the final analysis. Results : The better in the perceived general health group led to the higher satisfaction with orthodontic treatment(p=0.001). There was significant difference even in satisfaction with dental bleaching(p=0.030). The better in the perceived general health group led to the higher quality of life related to oral health(p=0.002). The subjective perception was indicated to have relationship with satisfaction at dental esthetics or quality of life related to oral health. The satisfaction with orthodontic treatment(${\beta}$=2.142, p=0.038) was indicated to be relevant factor of having influence upon quality of life related to oral health. Conclusion : As a result of analyzing partial correlation analysis by having subjective perception as control variable, the satisfaction with orthodontic treatment and the life quality related to oral health showed positive correlation.
Purpose: The aim of this study is to assess the influence exerted by the observer's dental specialization and compare patients' opinion with observers' opinion of the esthetics of maxillary single-tooth implants in the esthetic zone. Methods: Forty-one adult patients, who were treated with a single implant in the esthetic zone, were enrolled in this study. Eight observers (2 periodontists, 2 prosthodontists, 2 orthodontists and 2 senior dental students) applied the pink esthetic score (PES)/white esthetic score (WES) to 41 implant-supported single restorations twice with an interval of 4 weeks. We used a visual analog scale (VAS) to assess the patient's satisfaction with the treatment outcome from an esthetic point of view. Results: In the PES/WES, very good and moderate intraobserver agreements were noted between the first and second rating. The mean total PES/WES was $11.19{\pm}3.59$. The mean PES was $5.17{\pm}2.29$ and mean WES was $6.02{\pm}1.96$. In the total PES/WES, the difference between the groups was not significant. However, in the WES, the difference between the groups was significant and prosthodontists were found to have assigned poorer ratings than the other groups. Periodontists gave higher ratings than prosthodontists and senior dental students. Orthodontists were clearly more critical than the other observers. The statistical analysis revealed statistically significant correlation between patients' esthetic perception and dentists' perception of the anterior tooth. However, the correlation between the total PES/WES and the VAS score for the first premolar was not statistically significant. Conclusions: The PES/WES is an objective tool in rating the esthetics of implant supported single crowns and adjacent soft tissues. Orthodontists were the most critical observers, while periodontists were more generous than other observers. The statistical analysis revealed a statistically significant correlation between patients' esthetic perception and dentists' perception of the anterior tooth.
STATEMENT OF PROBLEM: While patient-centered outcomes are usually not reported, these may represent major aspects of the implant success for the patient. Use of a well-designed patient survey form can be an invaluable asset to the implant practitioners. PURPOSE: The objective of this study was to investigate patient satisfaction after implant therapy by means of a questionnaire. MATERIAL AND METHODS: South Korean patients (n = 100), who visited the dental examination center of Soon Chun Hyang university hospital, were asked to fill out the satisfaction questionnaire regarding aspects of cost, comfort, esthetics, chewing, gingival health, food impaction, phonetic aspect, screw loosening, and general satisfaction. Responses to statements were given on the Likert response scale. Four experimental groups of patients were distinguished with various location ($A_1,\;A_2,\;A_3$), year ($B_1,\;B_2,\;B_3$), number of implant replacements ($C_1,\;C_2,\;C_3$), and treatment cost ($D_1,\;D_2,\;D_3$). The reliability of the response scales was measured by calculation of its internal consistency, expressed as Cronbach's ${\alpha}$. The scales were distinguished by means of factor analysis method. Possible differences in scale scores among the groups were assessed by One-way ANOVA (${\alpha}$= 0.05). RESULTS: Patients responded to most of the statements with high satisfaction. But the mean scale score of statement about cost was low. After the verification of internal consistency and factor analysis, five components, e.g. general satisfaction, comfort, chewing efficiency, esthetics, and phonetic aspect were grouped together. These components could be explained with common meaning and the first factor was named as 'general satisfaction'. Differences in patient satisfaction on the scale with esthetics were present between patients who have been wearing the implant prosthesis less than three years and those more than seven years ($B_1<B_3$). CONCLUSION: The patients were generally satisfied with the outcome of implant treatment. But the patients' major complaint was high cost and while the statistically significant difference was not shown, the satisfaction scale about food impaction and esthetics was low. So the continuing efforts to make improvements about these problems are needed for the implant practitioners.
Purpose: The anterior region is a challenge for most clinicians to achieve optimal esthetics with dental implants. The provisional crown is a key factor in the success of obtaining pink esthetics around restorations with single implants, by soft tissue and inter-proximal papilla shaping. Provisional abutments bring additional costs and make the treatment more expensive. Since one of the aims of the clinician is to reduce costs and find more economic ways to raise patient satisfaction, this paper describes a practical method for chair-side fabrication of non-occlusal loaded provisional crowns used by the authors for several years successfully. Methods: Twenty two patients (9 males, 13 females; mean age, 36,72 years) with one missing anterior tooth were treated by using the presented method. Metal definitive abutments instead of provisional abutments were used and provisional crowns were fabricated on the definitive abutments for all of the patients. The marginal fit was finished on a laboratory analogue and temporarily cemented to the abutments. The marginal adaptation of the crowns was evaluated radiographically. Results: The patients were all satisfied with the final appearance and no complications occurred until the implants were loaded with permanent restorations. Conclusions: The use of the definitive abutments for provisional crowns instead of provisional abutments reduces the costs and the same results can be obtained.
목적: 이 연구의 목적은 하악 임플란트 지지 피개의치와 전통적인 총의치의 환자 만족도를 비교하기 위함이다. 재료 및 방법: 하악 임플란트 지지 피개의치 치료가 계획된 40명의 상하악 완전 무치악 환자(51세 - 82세)를 대상으로 하였고, 모든 환자는 저작기능, 발음, 심미, 통증, 전반적인 만족도를 기존의 총의치 장착 시와 임플란트 지지 피개의치 장착 1주 후, 12주 후에 visual analog scale법을 이용하여 답하였다. 각 시기의 만족도 비교를 위해 반복측정 분산분석(repeated-measures ANOVA)을 시행하였다(P<.05). 결과: 모든 설문 항목에서 임플란트 지지 피개의치 장착 1주 후, 12주 후에 총의치를 장착했을 때보다 높은 만족도를 보였다(P<.05). 임플란트 지지 피개의치 장착 12주 후에 저작기능, 발음, 통증, 전반적인 만족도는 장착 1주 후에 비해 증가된 결과를 보였고, 심미에 대한 만족도와 전체적인 만족도는 감소함을 나타냈으나, 통계적인 유의성을 보이지는 않았다. 결론: 임플란트 지지 피개의치는 기존의 총의치에 만족하지 못하는 환자들의 만족도를 높임으로써 환자의 삶의 질을 개선시킬 수 있는 효과적인 치료 방법 중 하나가 될 수 있으리라 생각된다.

The aim of this clinical report is to describe the complex treatment of an adult Class III malocclusion patient who was disappointed with the outcome of a previous oral rehabilitation. Interdisciplinary treatment planning was performed with a primary indication for implant removal because of marginal bone loss and gingival recession, followed by orthodontic and surgical procedures to correct the esthetics and skeletal malocclusion. The comprehensive treatment approach included: (1) implant removal in the area of the central incisors; (2) combined orthodontic decompensation with mesial displacement and forced extrusion of the lateral incisors; (3) extraction of the lateral incisors and placement of new implants corresponding to the central incisors, which received provisional crowns; (4) orthognathic surgery for maxillary advancement to improve occlusal and facial relationships; and finally, (5) orthodontic refinement followed by definitive prosthetic rehabilitation of the maxillary central incisors and reshaping of the adjacent teeth. At the three-year follow-up, clinical and radiographic examinations showed successful replacement of the central incisors and improved skeletal and esthetic appearances. Moreover, a Class II molar relationship was obtained with an ideal overbite, overjet, and intercuspation. In conclusion, we report the successful esthetic anterior rehabilitation of a complex case in which interdisciplinary treatment planning improved facial harmony, provided gingival architecture with sufficient width and thickness, and improved smile esthetics, resulting in enhanced patient comfort and satisfaction. This clinical case report might be useful to improve facial esthetics and occlusion in patients with dentoalveolar and skeletal defects.
임기존 hader bar의 재위치 문제를 해결하고 무치악 환자에서 부적합한 악간관계 채득으로 인한 비심미적인 안모와 부적절한 교합문제를 해결하기 위해서 치아 상실 전의 자연치의 위치와 비슷한 위치로 교합고경 설정과 치아 배열을 수행했다. 부적합한 환자의 안모를 회복하여 심미적인 개선을 이루었고, 더불어 의치의 이차적인 심미에 대한 만족감을 최대한으로 높이기 위하여 컴포지트 레진을 사용하여 마무리하였다.
Dental clinicians commonly encounter extrusion of the anterior teeth due to localized alveolar bone loss in adult patients with chronic periodontitis. This pathologic migration of the anterior not only may cause loss of functional anterior guidance and continuous periodontal destruction, but also can lead to esthetic disaster. In fact, most frequently reported chief complaint of the patients with extruded anterior teeth is the 'loss of esthetics', not the periodontal problem itself. The utilization of the expertise and skills in the various dental disciplines is thought to be the best treatment modality for the ultimate functional and esthetic attainment. Interdisciplinary therapy, involving periodontics, orthodontics, and restorative dentistry, was performed for the highest levels of patient and provider satisfaction in this case.

PURPOSE. The aim of this study was to assess the esthetic outcome of maxillary anterior single implants by comparing the esthetic perception of dental professionals and patients. MATERIALS AND METHODS. Twenty-three patients with single implants in the esthetic zone were enrolled in this study. Dentists of four different dental specialties (Three orthodontists, three oral surgeons, three prosthodontists, and three periodontists) evaluated the pink esthetic score (PES)/white esthetic score (WES) for 23 implant-supported single restorations. The satisfactions of the patients on the esthetic outcome of the treatment have been evaluated according to the visual analog scale (VAS). RESULTS. The mean total PES/WES was $12.26{\pm}4.76$. The mean PES was $6.45{\pm}2.78$ and mean WES was $5.80{\pm}2.82$. There was a statistically significant difference among the different specialties for WES (P<.01) and Total PES/WES (P<.01). Prosthodontists were found to have assigned poorer ratings among the other specialties, while oral surgeons gave the higher ratings than periodontists, orthodontists, and prosthodontists. CONCLUSION. Prosthodontists seemed to be stricter when assessing aesthetic outcome among other specialties. Moreover, a clear correlation existed between dentists' and patients' esthetic perception, thereby providing rationales for involving patients in the treatment plan to achieve higher levels of patient satisfaction.
This case study was to report the possible increase in the denture retention and psychological relief using the implant-supported fixed prostheses in a completely edentulous patient. The implants were placed in the anterior portion of the mandible in a patient who had completely edentulous state following the extraction of residual abutment teeth, and consequently a distal extension removable partial denture was fabricated. The patient's adaptation and satisfaction to the new prosthesis was monitored and confirmed in terms of masticatory function and esthetics, by restoring the oral condition similar to initial status before the residual teeth extraction. After 6 months, radiographic examination confirmed that both the abutment teeth and the implants were stable and well maintained. Considering the relatively short clinical follow-up period, however, continuous long-term monitoring was required.
