High energy photon beams from medical linear accelerators produce large scattered radiation by various components of the treatment head, collimator and walls or objects in the treatment room including the patient. These scattered radiation do not provide therapeutic dose and are considered a hazard from the radiation safety perspective. Scattered dose of therapeutic high energy radiation beams are contributed significant unwanted dose to the patient. ICRP take the position that a dose of 500mGy may cause abortion at any stage of pregnancy and that radiation detriment to the fetus includes risk of mental retardation with a possible threshold in the dose response relationship around 100 mGy for the gestational period. The ICRP principle of as low as reasonably achievable (ALARA) was recommended for protection of occupation upon the linear no-threshold dose response hypothesis for cancer induction. We suggest this ALARA principle be applied to the fetus and testicle in therapeutic treatment. Radiation dose outside a photon treatment filed is mostly due to scattered photons. This scattered dose is a function of the distance from the beam edge, treatment geometry, primary photon energy, and depth in the patient. The need for effective shielding of the fetus and testicle is reinforced when young patients ate treated with external beam radiation therapy and then shielding designed to reduce the scattered photon dose to normal organs have to considered. Irradiation was performed in phantom using high energy photon beams produced by a Varian 2100C/D medical linear accelerator (Varian Oncology Systems, Palo Alto, CA) located at the Yonsei Cancer Center. The composite phantom used was comprised of a commercially available anthropomorphic Rando phantom (Phantom Laboratory Inc., Salem, YN) and a rectangular solid polystyrene phantom of dimensions $30cm{\times}30cm{\times}20cm$. the anthropomorphic Rando phantom represents an average man made from tissue equivalent materials that is transected into transverse 36 slices of 2.5cm thickness. Photon dose was measured using a Capintec PR-06C ionization chamber with Capintec 192 electrometer (Capintec Inc., Ramsey, NJ), TLD( VICTOREEN 5000. LiF) and film dosimetry V-Omat, Kodak). In case of fetus, the dosimeter was placed at a depth of loom in this phantom at 100cm source to axis distance and located centrally 15cm from the inferior edge of the $30cm{\times}30cm^2$ x-ray beam irradiating the Rando phantom chest wall. A acryl bridge of size $40cm{\times}40cm^2$ and a clear space of about 20 cm was fabricated and placed on top of the rectangular polystyrene phantom representing the abdomen of the patient. The leaf pot for testicle shielding was made as various shape, sizes, thickness and supporting stand. The scattered photon with and without shielding were measured at the representative position of the fetus and testicle. Measurement of radiation scattered dose outside fields and critical organs, like fetus position and testicle region, from chest or pelvic irradiation by large fie]d of high energy radiation beam was performed using an ionization chamber and film dosimetry. The scattered doses outside field were measured 5 - 10% of maximum doses in fields and exponentially decrease from field margins. The scattered photon dose received the fetus and testicle from thorax field irradiation was measured about 1 mGy/Gy of photon treatment dose. Shielding construction to reduce this scattered dose was investigated using lead sheet and blocks. Lead pot shield for testicle reduced the scatter dose under 10 mGy when photon beam of 60 Gy was irradiated in abdomen region. The scattered photon dose is reduced when the lead shield was used while the no significant reduction of scattered photon dose was observed and 2-3 mm lead sheets refuted the skin dose under 80% and almost electron contamination. The results indicate that it was possible to improve shielding to reduce scattered photon for fetus and testicle when a young patients were treated with a high energy photon beam.

Background Implant-based breast reconstruction is being performed more frequently, and implants are associated with an increased risk of infection. We reviewed the clinical features of cases of implant infection and investigated the risk factors for breast device salvage failure. Methods We retrospectively analyzed 771 patients who underwent implant-based breast reconstruction between January 2010 and December 2016. Age, body mass index, chemotherapy history, radiation exposure, and smoking history were assessed as potential risk factors for postoperative infection. We also evaluated the presence and onset of infection symptoms, wound culture pathogens, and other complications, including seroma, hematoma, and mastectomy skin necrosis. Additionally, we examined the mastectomy type, the use of acellular dermal matrix, the presence of an underlying disease such as hypertension or diabetes, and axillary node dissection. Results The total infection rate was 4.99% (58 of 1,163 cases) and the total salvage rate was 58.6% (34 of 58). The postoperative duration to closed suction drain removal was significantly different between the cellulitis and implant removal groups. Staphylococcus aureus infection was most frequently found, with methicillin resistance in 37.5% of the cases of explantation. Explantation after infection was performed more often in patients who had undergone 2-stage expander/implant reconstruction than in those who had undergone direct-to-implant reconstruction. Conclusions Preventing infection is essential in implant-based breast reconstruction. The high salvage rate argues against early implant removal. However, when infection is due to methicillin-resistant S. aureus and the patient's clinical symptoms do not improve, surgeons should consider implant removal.

Purpose: The most basic conditions of radiation therapy is to prevent unnecessary exposure of normal tissue. The risk factors that are important o evaluate the dose emitted to the lung and heart from radiation therapy for breast cancer. Therefore, comparing the dose factors of a normal tissue according to the radion treatment position and Seeking an effective radiation treatment for breast cancer through the analysis of the correlation relationship. Materials and Methods: Computed tomography was conducted among 30 patients with left breast cancer in supine and prone position. Eclipse Treatment Planning System (Ver.11) was established by computerized treatment planning. Using the DVH compared the incident dose to normal tissue by position. Based on the result, Using the SPSS (ver.18) analyzed the dose in each normal tissue factors and Through the correlation analysis between variables, independent sample test examined the association. Finally The HI, CI value were compared Using the MIRADA RTx (ver. ad 1.6) in the supine, prone position Results: The results of computerized treatment planning of breast cancer in the supine position were V20, $16.5{\pm}2.6%$ and V30, $13.8{\pm}2.2%$ and Mean dose, $779.1{\pm}135.9cGy$ (absolute value). In the prone position it showed in the order $3.1{\pm}2.2%$, $1.8{\pm}1.7%$, $241.4{\pm}138.3cGy$. The prone position showed overall a lower dose. The average radiation dose 537.7 cGy less was exposured. In the case of heart, it showed that V30, $8.1{\pm}2.6%$ and $5.1{\pm}2.5%$, Mean dose, $594.9{\pm}225.3$ and $408{\pm}183.6cGy$ in the order supine, prone position. Results of statistical analysis, Cronbach's Alpha value of reliability analysis index is 0.563. The results of the correlation analysis between variables, position and dose factors of lung is about 0.89 or more, Which means a high correlation. For the heart, on the other hand it is less correlated to V30 (0.488), mean dose (0.418). Finally The results of independent samples t-test, position and dose factors of lung and heart were significantly higher in both the confidence level of 99 %. Conclusion: Radiation therapy is currently being developed state-of-the-art linear accelerator and a variety of treatment plan technology. The basic premise of the development think normal tissue protection around PTV. Of course, if you treat a breast cancer patient is in the prone position it take a lot of time and reproducibility of set-up problems. Nevertheless, As shown in the experiment results it is possible to reduce the dose to enter the lungs and the heart from the prone position. In conclusion, if a sufficient treatment time in the prone position and place correct confirmation will be more effective when the radiation treatment to patient.

Purpose: The goal of radiation treatment is to deliver a prescribed radiation dose to the target volume accurately while minimizing dose to normal tissues. In this paper, we comparing the dose distribution between three dimensional conformal radiation radiotherapy (3D-CRT) and helical tomotherapy (TOMO) plan for partial breast cancer. Materials and Methods: Twenty patients were included in the study, and plans for two techniques were developed for each patient (left breast:10 patients, right breast:10 patients). For each patient 3D-CRT planning was using pinnacle planning system, inverse plan was made using Tomotherapy Hi-Art system and using the same targets and optimization goals. We comparing the Homogeneity index (HI), Conformity index (CI) and sparing of the organs at risk for dose-volume histogram. Results: Whereas the HI, CI of TOMO was significantly better than the other, 3D-CRT was observed to have significantly poorer HI, CI. The percentage ipsilateral non-PTV breast volume that was delivered 50% of the prescribed dose was 3D-CRT (mean: 40.4%), TOMO (mean: 18.3%). The average ipsilateral lung volume percentage receiving 20% of the PD was 3D-CRT (mean: 4.8%), TOMO (mean: 14.2), concerning the average heart volume receiving 20% and 10% of the PD during treatment of left breast cancer 3D-CRT (mean: 1.6%, 3.0%), TOMO (mean: 9.7%, 26.3%) Conclusion: In summary, 3D-CRT and TOMO techniques were found to have acceptable PTV coverage in our study. However, in TOMO, high conformity to the PTV and effective breast tissue sparing was achieved at the expense of considerable dose exposure to the lung and heart.

The APR1400 reactor may be operated for a long time under high temperature and pressure conditions, causing damage to the stud holes and causing stud bolts and holes to stick. The present practice is to manually remove the anti-sticking agent and foreign matter remaining in the APR1400 reactor stud hole and to visually check the surface condition of the thread to check the damage status of the threads. In the case of the APR1400 reactor stud holes, manually cleaning the threads increases the risk of radiation exposure and operator's fatigue. To avoid this, the autonomous mobile robot is used to automatically clean the reactor stud holes. The purpose of this study is to optimize the cleaning performance of the mobile robot by looking at the behavior of the surface roughness of the stud surface cleaned by the brush attached to the mobile robot due to changes in brush material, thickness of wire, and rotation speed. A microscopic approach to the surface roughness of the flank is needed to investigate the effects of the newly proposed brush of the autonomous mobile robot on the thread holes. According to this experiment, it is reasonable to use STS brush rather than Carbon one. Optimal operating conditions are derived and the safety of APR1400 reactor stud holes maintenance can be improved.

Dental caries is an important dental disease among children and adolescents that can continue for a lifetime. Early detection of dental caries in deciduous dentition is significant because it can influence the permanent teeth. It is also critical to prevent dental caries by performing fluoride treatment and pit-and-fissure sealant for high-risk children. Various methods have been developed for the early detection of dental caries; however, many studies are still seeking to discover more effective methods. In general, visual examination and radiographic images are used, but these techniques have several limitations such as errors and radiation exposure. In this study, clinical application of the newly developed DIAGNOcam caries identification device and its possible applications were examined. DIAGNOcam was applied to diagnose dental caries in the posterior teeth of patients in the Department of Pediatric Dentistry, and it was confirmed that it could be used to detect proximal caries, the margin of restoration, and the extent of dental caries lesions.
Magnetic resonance imaging(MRI) uses strong magnetic field to image the cross-section of human body and has excellent image quality with no risk of radiation exposure. Because of above-mentioned advantages, MRI has been widely used in clinical fields. However, the noise generated in MRI degrades the quality of medical images and has a negative effect on quick and accurate diagnosis. In particular, examining a object with a detailed structure such as brain, image quality degradation becomes a problem for diagnosis. Therefore, in this study, we acquired T2 weighted 3D data of multiple sclerosis disease using BrainWeb simulation program, and used quantitative evaluation factors to find appropriate slice thickness among 1, 3, 5, and 7 mm. Coefficient of variation and contrast to noise ratio were calculated to evaluate the noise level, and root mean square error and peak signal to noise ratio were used to evaluate the similarity with the reference image. As a result, the noise level decreased as the slice thickness increased, while the similarity decreased after 5 mm. In conclusion, as the slice thickness increases, the noise is reduced and the image quality is improved. However, since the edge signal is lost due to overlapped signal, it is considered that selecting appropriate slice thickness is necessary.
To ensure adequate protection from the risk of burns, fire fighter's turnout has a composite of more than three components and air gaps between layers of materials. During the flame exposure, radiation and convection heat transfer occurs in the air gap, thus the air gap acts as a thermal resistance with non-linear characteristics. Therefore, in this study, the experiments were performed to identify the effect of various air gap width (0~7 mm) on the thermal protective performance of fire fighter's clothing. The temperatures on each layer and RPP (Radiant Protective Performance, the most effective index representing the thermal protective performance) were measured with various incident radiant heat fluxes. The temperature at the rear surface of the garment decreased and RPP increased with increasing air gap width because the thermal resistance increased. Especially, it could be found that RPP value and air gap width has almost linear relation for the constant incident heat flux conditions. Thus relatively simple RPP predictive equation was suggested for various incident heat flux and air gap conditions.

Low-dose X-ray fluoroscopic image sequences to avoid radiation exposure risk are contaminated by quantum noise. To restore these noisy sequences, we propose a 3D nonlocal means (NLM) filter based on stochastic distancesed can be applied to the denoising of X-ray fluoroscopic image sequences. The stochastic distance is obtained within motion-compensated noise filtering support to remove the Poisson noise. In this paper, motion-adaptive weight which reflected the frame similarity is proposed to restore the noisy sequences without motion artifact. Experimental results including comparisons with conventional algorithms for real X-ray fluoroscopic image sequences show the proposed algorithm has a good performance in both visual and quantitative criteria.
