Revised Piper Fatigue Scale의 구성타당도 : 한국여성 유방암 환자를 중심으로 (Construct Validity of the Revised Piper Fatigue Scale in Korean Women with Breast Cancer)

  • Revised Piper Fatigue Scale (Piper et al., 1998)은 미국에서 개발된 암환자의 피로를 측정하는 자가보고서 형식의 도구이다. 이 도구는 총 22문항으로 네 개의 구성요소로 이루어져 있으며, 미국인 유방암 환자를 대상으로 신뢰도와 타당도가 검증되었다. 본 연구의 목적은 한국 유방암 환자를 대상으로 Revised Piper Fatigue Scale의 구성타당도를 재평가하기 위함이다. 총 122명의 유방암 환자가 번역-역번역 과정을 거쳐 한국어로 번역된 Revised Piper Fatigue Scale에 응답을 하였고 그 자료는 요인분석 (principal axis analysis with oblique rotation)에 의해 분석되었다. 요인분석 결과, 총 19문항으로 이루어진 네 개의 구성요소가 도출되었다. 총문항의 Cronbach's alpha는 .93이었고, 도출된 네 요인들의 alpha는 .84에서 .91이었다. 한국어로 번역된 총 19문항의 Revised Piper Fatigue Scale(revised PFS-K)은 한국인 유방암 환자에게도 적용될 수 있는 신뢰도와 타당도가 검증된 도구라 할 수 있다. 앞으로 다양한 한국인 암환자를 대상으로 계속해서 도구 검증이 이루어지기를 제언한다.

비진행성 갑상선암 환자에 대한 제주워터의 항피로 효능에 대한 유효성과 안전성 평가 예비 임상연구 - 무작위배정, 이중맹검, 삼군위약대조군 연구 (Effects of Vanadium Water on Cancer-related Fatigue of Non Advanced Thyroid Cancer Patients: Randomized, Three armed, Triple Blinded Controlled Trial)

  • Objective : This study was conducted to evaluate of efficacy and safety of Vanadium Water (VW) on Cancer related Fatigue of Non-advanced Cancer Patients. Methods : Experimental Group Administration. Participants in treatment group consumed three 500 ml/bottle bottles of Jeju Island's VW, which contains 40.2 ${\mu}g/L$ of vanadium, three times a day (morning, afternoon, evening) for 4 weeks (28 days) without any additional treatments for fatigue improvement. The control group participants consumed three 500 ml/bottle bottles of ordinary water (0 ${\mu}g/L$ of vanadium) three times a day for the same period of time without any additional treatments for fatigue improvement. Results : After 4 weeks of VW consumption, total score of Revised Piper Fatigue Scale (RPFS) was reduced 30% and basal score after Fisher's Exact Test resulted statistical borderline significance (p=0.0799) and result of t-test of comparison between two groups' RPFS total score and fatigue cognitive/mood changes resulted statistical significance (p=0.0112). In detail, only 36.4% of control group (N=11) showed improvement while 77.8% of treatment group (N=9) showed fatigue improvement and thyroid hormone level changes. No other lab measures indicated any significant differences between two groups. Conclusion : Daily consumption of 1.5 L of Vanadium water for 4 weeks showed improvement of non-advanced cancer patients' fatigue.

항암 화학요법을 받는 유방암 환자의 피로 관련요인 (Factors Related to Fatigue in Breast Cancer Patients Receiving Chemotherapy)

  • Purpose: The purpose of the study is to investigate the degree of fatigue and its related factors in patients with breast cancer. Method: The subjects of this study consisted of 62 breast cancer patients receiving chemotherapy. The Revised Piper Fatigue Scale, Disruption of Usual Activity Scale, Linear Analogue Self Assessment Scale and Self-Care Diary Scale were employed to measure fatigue, disruption of usual activity, mood state and physical symptom respectively. Collected data were analyzed using SPSS-PC to obtain summary statistics for the descriptive analysis, Pearson Correlation, and Stepwise Multiple Regression. Result: The results are as follows. 1. The mean score of the degree of fatigue was 4.82 points indicating moderate level. 2. The mean score of the degree of disruption of usual activity, mood state and physical symptom were 2.66, 5.00, and 4.69 points. 3. Fatigue of the subjects was significantly correlated with disruption of usual activity(r=.517, p<.01), mood state(r=.420, p<.01) and physical symptom(r=.463, p<.01). 4. With the result of stepwise multiple regression, disruption of usual activity, mood state and physical symptom being the three variables which could explain fatigue by 39.1%. The results of this study suggest that comprehensive intervention strategy for fatigue should be developed to maintain quality of life during and following chemotherapy considering these factors.

항암화학요법을 받는 위암 환자의 피로 변화 양상과 관련요인 (Fatigue and Related Factors in Patients with Stomach Cancer during Chemotherapy)

  • Purpose: This study was intended to identify the patterns of fatigue and its related factors in patients with stomach cancer during chemotherapy. Methods: Thirty participants (24 males and 6 females) were recruited for this study which utilized a longitudinal and descriptive approach. The research instruments included the Revised Piper Fatigue Scale, Symptom Distress Scale, and Linear Analogue Self Assessment Scale. The participants received 5-FU and Adriamycin at the first week and 5-FU only at the second and third week. The instruments were measured six times in total. The data were analyzed using SPSS 17.0. Results: It was found that fatigue scores in patients with stomach cancer, receiving 5-FU and Adriamycin (FA) regimen, reached the highest level on the third day (F=9.37, p=.024) after the initial infusion, and decreased gradually afterward. The symptom and psychological distress scores illustrated very similar pattern. The concept of multidimensionality of fatigue in patients with stomach cancer was supported in this study, showing that all four dimensions of the scale were positively correlated. Conclusion: The results of this study provided useful information of patients with stomach cancer on fatigue and other related symptoms which they experienced during weekly scheduled chemotherapy with FA regimen.

암환자의 신체활동과 피로 (Physical Activity and Fatigue in Patients with Cancer)

  • Purpose: The purpose of this study was to identify the relationship between physical activity and fatigue in patients with cancer. Methods: The participants of the study were 118 patients who had been diagnosed with cancer at least more than three months before at a university hospital. Physical activity was measured by IPAQ Korean version (2006), and fatigue by the Revised Piper Fatigue Scale. The collected data were analyzed using frequency, percentage, t-test, ANOVA, Pearson correlation coefficient. Results: Continuous physical activity overall in this study was on average $2,154.03{\pm}2,845.86$ MET, and the overall degree of fatigue of the participants was $4.47{\pm}1.80$. There were significant negative correlations between fatigue and moderate physical activity. Conclusion: The findings of the study suggested that moderate physical activity is effective for improving fatigue in cancer patients.

유방암 생존자의 피로와 디스트레스가 자기효능감에 미치는 영향 (The Effects of Fatigue and Distress on Self-efficacy among Breast Cancer Survivors)

  • Purpose: The purpose of this study was to describe the impact of fatigue and distress on self-efficacy among breast cancer survivors and to provide a base for development of nursing intervention strategy to improve self-efficacy. Methods: A descriptive research design was used. The subjects were 158 patients who were either being treated or were receiving follow-up care at a university breast center in D City from May 30 to August 30, 2014. Structured questionnaires, Revised Piper Fatigue Scale, Distress Thermometer, and Self-Efficacy Scale for Self-Management of Breast Cancer were used to measure fatigue, distress, and self-efficacy. Data were analyzed using t-test, ANOVA, Pearson's correlation coefficients, and multiple regressions. Results: The mean scores of fatigue, distress, and self-efficacy were 3.83, 4.31, and 3.77, respectively. There were significant differences among participants in terms of educational background, current treatment methods, perceived health status, economic burden for fatigue and perceived health status for distress. Self-efficacy was impacted by age, educational background, marital status, average monthly income, perceived health status, and medical expenses. Fatigue, age, and the burden on medical expense had the most impact on self-efficacy, accounting for 17% of the variance. Conclusion: Fatigue should be managed to improve self-efficacy of breast cancer survivors. Therefore, nursing programs designed to decrease fatigue may be helpful.

  • Purpose: This study aimed to evaluate nutritional status and fatigue, and to identify the relationship between nutritional status and fatigue in gynecological cancer patients receiving chemotherapy. Methods: Participants were 106 outpatients who had uterine, ovary, or breast cancer and a descriptive cross-sectional design was used. Nutritional indicators including biochemical and anthropometric measures, the Subjective Global Assessment (SGA) scale, and the Piper's Revised Fatigue Scale were completed. Results: About thirty-nine percent of the patients were malnourished based on the SGA. Serum hemoglobin and total lymphocyte levels were less than the normal ranges, although anthropometrics were normal. The women experienced moderate fatigue with a mean score of $5.84{\pm}2.00$. Nutritional status was significantly different by age, education, employment status, and monthly income. The women also had different nutritional status and fatigue depending on the type of cancer and chemotherapy. Fatigue was significantly related to SGA scores, serum hemoglobin, triceps skinfold thickness and mid-arm muscle circumference. Conclusion: The results of this study highlight an urgent need to provide adequate nutritional support and fatigue management with individualized strategies for gynecological cancer patients on chemotherapy.

  • Purpose: The purpose of this study was to identify the relationship between energy intake and fatigue in cancer patients receiving chemotherapy. Methods: A total of 106 subjects had participated in this study. Data were collected at a university hospital in D city from September 1st to November 10th, 2010. Energy intake including carbohydrates, protein and fat was measured by scale and analyzed using Can Pro 3.0 program. Fatigue level was measured by the Revised Piper Fatigue Scale. Data were analyzed using descriptive analysis, t-test, ANOVA and Pearson correlation coefficient with SPSS/WIN 15.0. Results: The mean calorie intake during chemotherapy was $906.53{\pm}201.28Kcal/day$ which was 45.3% of the recommended daily calorie intake. The mean of protein intake level was $43.62{\pm}11.13g/day$, and it was low compared to the recommended daily protein intake. Calories, carbohydrates, and protein levels on 3rd day after chemotherapy were significantly lower than those of 2nd day after chemotherapy (p<.001). The fatigue level during chemotherapy was $5.77{\pm}0.77$ which was moderate level of fatigue. There was a significant negative interrelation between energy intake and fatigue. Conclusion: Theses results suggested that nursing approaches to encourage dietary intake may be helpful to reduce fatigue for cancer patients receiving chemotherapy.

만성 간 질환자의 피로, 우울 및 수면장애 (A study of Fatigue, Depression and Sleep Disorders in Patients with Chronic Liver Disease)

  • The purpose of this study is to investigate the level of fatigue, depression and sleep disorders and to find the correlation between them in patients with chronic liver disease. Methods: The participants consisted of 130 patients with chronic liver disease who visited the C University Hospital in Daegu, Korea from July to August, 2011. Data were collected by a self-reporting questionnaire of the Revised Piper Fatigue Scale, Depression Inventory (BDI) and Sleep Scale. Collected data were analyzed by using PASW Statistics 18.0 program for descriptive statistics, T-test, one-way ANOVA and Correlation Analysis. Results: There were significant differences in fatigue scores (t=8.415, $p$=.004) and depression scores (t=10.08, $p$=.002) between subjects with symptoms of liver disease and those with no symptoms. There was no significant difference in sleep disorder scores. In addition, there is a significant correlation between fatigue and depression (r=.641), linking fatigue and sleep disorders (r=.578), and between depression and sleep disorders (r=.572). Conclusion: It is necessary to develop multidisciplinary intervention programs to relieve patientsuffering.

폐암환자의 피로와 자기효능의 관계 (The Relationship between Fatigue and Self-efficacy in Patients with Lung Cancer)

  • Purpose: The purpose of this study was to identify the relationship between fatigue, and self-efficacy in patients with lung cancer. Method: The data was collected from January 21th to April 8th 2001. The study subjects were recruited from K hospital in Pusan, Korea. Their fatigue was measured using the 22-item Revised Piper Fatigue Scale developed by Piper et al. (1998) and translated by the investigators, and self-efficacy was measured using the 10-item General Self-efficacy Questionnaire developed by Lee, Schwarzer & Jerusalem. The data were analyzed by SPSS 10.0 program using frequency, percentage, mean and standard deviation, t-test, ANOVA, Post-hoc test (Scheffe's test), and Pearson Correlation Coefficients. Result: 1) The mean fatigue score was $118.48{\pm}33.02$ (range: 22-220). The scores for sub-dimension were $35.40{\pm}10.06$ in behavior/severity, $25.98{\pm}9.06$ in affective score, $27.88{\pm}10.06$ in sensory score, and $29.22{\pm}8.27$ in cognitive/mood. The mean self-efficacy score was $28.80{\pm}5.85$. 2). There were significant differences in the fatigue of patients with lung cancer on income per month (F=4.651, p= .014), 'present pain' (F=2.601, p= .012), 'change of weight' (F=5.911, p= .005), by general characteristics. 3) There were significant differences in the self-efficacy of patients with lung cancer on 'religion' (F=3.732, p= .031), 'employment status' (F=5.525, p= .003), 'past therapy' (F=2.869, p= .034), by general characteristics 4) There was a significant negative correlation between fatigue and self-efficacy (r=- .528, p= .000). Conclusion: Patients with lung cancer experience fatigue. Increased fatigue is associated with decreased self-efficacy. Nurses must provide patients with nursing care for the less occurrence of fatigue and interventions to manage self-efficacy for them.
