사전정보제공과 진정제투여가 기관지내시경 검사 대상자의 불안 감소에 미치는 영향 (The effects of Preparatory Information and According to Premedication on the Level of Anxiety of Patients undergoing Bronchoscopy)

  • 본 연구는 기관지내시경 검사 시 사전정보제공과 진정제 투여가 환자의 불안 감소와 생리적 변수에 미치는 영향을 확인하기 위하여 2007년 6월 4일부터 12월 28일까지 기관지내시경 검사를 받는 환자 102명을 대상으로 시행되었다. 불안척도는 Spielberger의 STAI를 이용하였고, 사전정보제공소책자를 제작하여 사용하였다. 수집된 자료는 SPSS 14.0을 이용하여 분석하였다. 그 결과, 기관지내시경 검사에 대한 사전정보제공군, 진정제 투여군, 사전정보제공과 진정제를 함께 적용한 세 실험군은 대조군에 비해 수축기(p=.062), 이완기 혈압(p=.189) 변화에 영향을 주지 못하였으나, 환자의 불안(p=.005)과 맥박(p=.033), 호흡(p=.006)의 변화에는 효과적이었으므로 임상에서 적용할 가치가 있을 것으로 본다.

Selection and identification of single-domain antibody against Peste des Petits Ruminants virus

  • Background: Peste des petits ruminants (PPR) is an infectious disease caused by the peste des petits ruminants virus (PPRV) that mainly produces respiratory symptoms in affected animals, resulting in great losses in the world's agriculture industry every year. Single-domain variable heavy chain (VHH) antibody fragments, also referred to as nanobodies, have high expression yields and other advantages including ease of purification and high solubility. Objectives: The purpose of this study is to obtain a single-domain antibody with good reactivity and high specificity against PPRV. Methods: A VHH cDNA library was established by immunizing camels with PPRV vaccine, and the capacity and diversity of the library were examined. Four PPRV VHHs were selected, and the biological activity and antigen-binding capacity of the four VHHs were identified by western blot, indirect immunofluorescence, and enzyme-linked immunosorbent assay (ELISA) analyses. ELISA was used to identify whether the four VHHs were specific for PPRV, and VHH neutralization tests were carried out. ELISA and western blot analyses were used to identify which PPRV protein was targeted by VHH2. Results: The PPRV cDNA library was constructed successfully. The library capacity was greater than 2.0 × 106 cfu/mL, and the inserted fragment size was approximately 400 bp to 2000 bp. The average length of the cDNA library fragment was about 1000 bp, and the recombination rate was approximately 100%. Four single-domain antibody sequences were selected, and proteins expressed in the supernatant were obtained. The four VHHs were shown to have biological activity, close affinity to PPRV, and no cross-reaction with common sheep diseases. All four VHHs had neutralization activity, and VHH2 was specific to the PPRV M protein. Conclusions: The results of this preliminary research of PPRV VHHs showed that four screened VHH antibodies could be useful in future applications. This study provided new materials for inclusion in PPRV research.

Botulinum Toxin Therapy in a Patient with HHH Syndrome with Gait Disturbance: A Case Report

  • Background : Hyperornithinemia-hyperammonemia-homocitrullinuria (HHH) syndrome is a rare, autosomal recessive metabolic disorder which is caused by genetic mutations that disrupt the urea cycle. It is characterized by variable clinical presentation and the age of onset. Patients may present with gait disturbance and progressive paraplegia and muscle tightness in the lower extremities. The use of botulinum toxin in metabolic disease has rarely been discussed. We describe a case of a 14-year-old-boy with HHH syndrome, who presented with a several - month history of gait disturbance and lower extremity weakness. Case presentation : A 14-year old male had a history of recurrent upper respiratory tract infections, occasional vomiting, loss of appetite, and general weakness, all of which started since he was 10 months old. He was diagnosed with HHH syndrome at one year of age. At the age of 14, he was referred for the assessment and treatment of his gait disturbance and aggravated weakness of the lower extremities. Brain MRI, electrodiagnostic study and blood test were performed to exclude any lesions related to neurologic dysfunction. Botulinum toxin type A were injected into muscles of adductor longus, adductor magnus, lateral and medial hamstring, and lateral and medial gastrocnemius muscle heads under needle electromyography guidance to reduce lower limb spasticity. Intensive physical therapy including gait training and stretching exercise of adductor and calf muscles were also provided. After intensive physical therapy and botulinum toxin injection to reduce lower limb spasticity, he was able to ambulate for 20 meters independently without any walking aids. There were no adverse events after the injection. Conclusion : Botulinum toxin injection is a safe and effective therapy for patients with HHH syndrome who suffer from gait disturbance.

Genotype-phenotype correlations in pediatric patients with myotonic dystrophy type 1

  • Purpose: Myotonic dystrophy, also known as dystrophia myotonica (DM), is an autosomal dominant disorder with 2 genetically distinct forms. DM type 1 (DM1) is the more common form and is caused by abnormal expansion of cytosine/thymine/guanine (CTG) repeats in the DM protein kinase (DMPK ) gene. Our study aimed to determine whether the age of onset is correlated with CTG repeat length in a population of pediatric patients with DM1. Methods: We retrospectively identified 30 pediatric patients with DM1 that underwent DMPK testing, of which the clinical data of 17 was sufficient. The cohort was divided into 2 subgroups based on the clinical phenotype (congenital-onset vs. late-onset) and number of CTG repeats (<1,000 vs. ${\geq}1,000$). Results: We found no significant difference between the age of onset and CTG repeat length in our pediatric patient population. Based on clinical subgrouping, we found that the congenital-onset subgroup was statistically different with respect to several variables, including prematurity, rate of admission to neonatal intensive care unit, need for respiratory support at birth, hypotonia, dysphagia, ventilator dependence, and functional status on last visit, compared to the late-onset subgroup. Based on genetic subgrouping, we found a single variable (poor feeding in neonate) that was significantly different in the large CTG subgroup than that in the small CTG subgroup. Conclusion: Clinical variables exhibiting statistically significant differences between the subgroups should be focused on prognosis and designing tailored management approaches for the patients; our findings will contribute to achieve this important goal for treating patients with DM1.

Metabolomics comparison of serum and urine in dairy cattle using proton nuclear magnetic resonance spectroscopy

  • Objective: The aim of the study was to conduct metabolic profiling of dairy cattle serum and urine using proton nuclear magnetic resonance (1H-NMR) spectroscopy and to compare the results obtained with those of other dairy cattle herds worldwide so as to provide a basic dataset to facilitate research on metabolites in serum and urine. Methods: Six dairy cattle were used in this study; all animals were fed the same diet, which was composed of total mixed ration; the fed amounts were based on voluntary intake. Blood from the jugular neck vein of each steer was collected at the same time using a separate serum tube. Urine samples were collected by hand sweeping the perineum. The metabolites were determined by 1H-NMR spectroscopy, and the obtained data were statistically analyzed by performing principal component analysis, partial least squares-discriminant analysis, variable importance in projection scores, and metabolic pathway data using Metaboanalyst 4.0. Results: The total number of metabolites in the serum and urine was measured to be 115 and 193, respectively, of which 47 and 81, respectively were quantified. Lactate (classified as an organic acid) and urea (classified as an aliphatic acylic compound) exhibited the highest concentrations in serum and urine, respectively. Some metabolites that have been associated with diseases such as ketosis, bovine respiratory disease, and metritis, and metabolites associated with heat stress were also found in the serum and urine samples. Conclusion: The metabolites measured in the serum and urine could potentially be used to detect diseases and heat stress in dairy cattle. The results could also be useful for metabolomic research on the serum and urine of ruminants in Korea.

도라지(Platycodon grandiflorum)의 임상적 효과에 대한 체계적 문헌 고찰 (The Clinical Effects of Platycodon grandiflorum: A Systematic Review)

  • Platycodon (Platycodon grandiflorum) is used as a food and medicinal ingredient worldwide, particularly in Northeast Asia. Although a large number of studies have been conducted to determine its medicinal efficacy, there is still a paucity of literature summarizing the clinical activity of Platycodon. In this systematic review, we aimed to summarize the clinical efficacy of Platycodon. All literature describing the clinical efficacy of Platycodon was collected from international databases, and relevant papers were carefully evaluated for eligibility. Data were extracted from the selected papers and quality evaluation was conducted in accordance with the standards provided by the National Institutes of Health (NIH), with all procedures being conducted in accordance with PRISMA 2009 checklist guidelines. After the removal of 342 papers, 644 studies were fully screened for their titles and abstracts, among which, 19 studies were finally selected for full summarization. Extraction of data indicated evidence of the effectiveness of Platycodon in the treatment of respiratory system-related diseases and cancer, and in the provision of immunity. However, we found that the quality and objectivity of evaluation, management of variable factors, sample size, and reproducibility were generally poor. Although Platycodon has shown potential antiviral, antibacterial, anticancer, and antidepressant effects, and promotion of immunity in clinical settings, it was not possible to integrate objective indicators to conduct a meta-analysis due to the lack of literature and insufficient studies. More investigations in the clinical setting should be conducted to confirm the functional effects of Platycodon.

High-performance computing for SARS-CoV-2 RNAs clustering: a data science-based genomics approach

  • Nowadays, Genomic data constitutes one of the fastest growing datasets in the world. As of 2025, it is supposed to become the fourth largest source of Big Data, and thus mandating adequate high-performance computing (HPC) platform for processing. With the latest unprecedented and unpredictable mutations in severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the research community is in crucial need for ICT tools to process SARS-CoV-2 RNA data, e.g., by classifying it (i.e., clustering) and thus assisting in tracking virus mutations and predict future ones. In this paper, we are presenting an HPC-based SARS-CoV-2 RNAs clustering tool. We are adopting a data science approach, from data collection, through analysis, to visualization. In the analysis step, we present how our clustering approach leverages on HPC and the longest common subsequence (LCS) algorithm. The approach uses the Hadoop MapReduce programming paradigm and adapts the LCS algorithm in order to efficiently compute the length of the LCS for each pair of SARS-CoV-2 RNA sequences. The latter are extracted from the U.S. National Center for Biotechnology Information (NCBI) Virus repository. The computed LCS lengths are used to measure the dissimilarities between RNA sequences in order to work out existing clusters. In addition to that, we present a comparative study of the LCS algorithm performance based on variable workloads and different numbers of Hadoop worker nodes.

Effects of Muscle Thickness of Masseter and Sternocleidomastoid, Forward Head Posture and Breathing in Subjects With and Without Dentures

  • Background: For the elderly, masticatory function is one of the most important oral functions and the masticatory ability is related to the wearing of dentures. Many older people wear dentures for their masticatory function, but a significant number of older people who use dentures have found that they feel uncomfortable when performing their daily activities, such as performing masticating functions or talking. Objects: The purpose of this study is to investigate how the forward head posture (FHP), respiratory function and thickness of masseter (MS), and sternocleidomastoid (SCM) are affected by the presence or absence of dentures in the elderly, and what kind of correlation there is between these variables. Methods: The study was conducted on 11 patients in the normal group and 13 in the denture group. The participant's cognitive ability was evaluated using Mini-Mental State Examination Korean (MMSE-K), and the FHP was evaluated by measuring the craniovertebral angle (CVA). The thickness of the MS and SCM muscles were measured using ultrasound, and respiration was measured with a spirometry. As for the statistical method, the correlation of each variable was investigated using Spearman's correlation coefficient. Results: In the normal group, there was a significant correlation between forced vital capacity (FVC) and forced expiratory volume in 1 second (FEV1) (p < 0.001), and in the denture group, FVC and FEV1 (p < 0.001), maximal inspiratory pressure (MIP) and maximal expiratory pressure (MEP) (p < 0.001), CVA and Lt. MS (p = 0.012). Conclusion: CVA and Lt. MS of the denture group have a high negative correlation, it is related that the thickness of MS may be thick when the elderly wearing dentures are FHP.

Application of Topical Rocuronium Bromide Dosing by Ocular Size in Four Species of Wild Birds

  • Background: Rocuronium bromide has been evaluated as a mydriatic agent in birds, but the species applied were limited and the dose and effect were variable. Objective: This study aims to evaluate the efficacy of topical rocuronium bromide as mydriatics in 4 species according to horizontal palpebral fissure length: Feral pigeon (Columba livia), Common kestrel (Falco tinnunculus), Northern boobook (Ninox japonica), and Eurasian eagle owl (Bubo bubo). Methods: A total of 32 birds (8 for each species) were included as pre-releasing examination. Rocuronium bromide was instilled in one randomly selected eye of each bird based on palpebral fissure length criteria (0.5 mg/50 µL for pigeons, 1 mg/100 µL for kestrels and boobook owls, and 2 mg/200 µL for eagle owls). The contralateral eye was used as control and treated with normal saline. After instillation of the drug, pupil diameter, pupillary light reflex, intraocular pressure, heart rate, and respiratory rate were evaluated at 10 min intervals up to 180 min and at 30 min intervals up to 360 min. Results: Statistically significant mydriasis was obtained in all birds (p < 0.001). However, in boobook and eagle owls, marked mydriasis persisted until 360 min. Side effects including corneal erosion and lower eyelid paralysis were common, which was observed in 26/32 birds. Blepharospasm was also noted during this study. No systemic adverse signs were observed. Conclusions: Rocuronium bromide could be a good mydriatics option for 4 species of birds, however, further studies are needed to find lowest effective dose to reduce drug-related side effects.

결핵성 흉벽농양의 흉부 전산화 단층 촬영 소견 (CT Findings and Types of Tuberculous Chest Wall Abscess)

  • 배 경 : 결핵성 흉벽 농양은 결핵의 드문 합병증이다. 결핵성 흉벽 농양은 다양한 병인에 따라 CT 소견에 차이가 있을 것으로 생각되며 이러한 차이를 밝히는 것이 결핵성 흉벽농양의 진단 뿐만 병인을 이해하는 것에도 도움이 될 것으로 생각된다. 대상 및 방법 : 결핵성 농양으로 확진된 20명의 환자를 대상으로 하였다. 20명 환자의 29개 농양을 활동성 폐실질 혹은 흉막 병변의 유무와 이들 병변과 농양의 접촉 여부에 따라 3가지 유형으로 분류하였다. l형은 폐실질 또는 흉막에 활동성 결핵성 병변이 없는 경우로, 2형은 활동성 폐결핵이나 흉막병변이 흉벽농양과 접하고 있는 경우로, 3형은 폐결핵이나 흉막병변이 있으나 농양과 접하고 있지 않는 경우로 각각 정의하였다. CT 유형에 따른 농양의 크기, 모양 및 위치 등의 차이를 분석하였다. 결 과 : l형에는 6명 환자의 6개의 농양이 속하였다. 이틀 농양은 늑골이나 늑연골의 파괴를 동반하며, 이것을 중심으로 크기가 크고 비교적 구형에 가까운 모양이었다. 2형은 10명 환자의 13개의 농양이 속하였다. 흉막 병변과 접하고 있는 7예와 종격동 림프절염과 접하고 있는 1예는 농양의 중심이 외근육층에 있고, 상하 길이가 긴 타원형의 모양이었다. 폐실질 병변과 접하고 있던 5예는 농양의 중심이 흉막외공간에 있고 크기가 작고 횡축이 긴 타원형이었다. 3형에는 6명 환자에서 10개의 농양이 속하였다. 이것들은 주로 흉막외공간에 생기며 크기가 작고 늑골의 장축을 따라 횡측의 길이가 긴 타원의 모양이고, 다발성의 빈도가 높았다. 결 론 : 결핵성 흉벽 농양은 농양의 모양과 동반된 폐실질 및 흉막 병변에 따라 몇개의 유형으로 나눌 수 있었다. 이러한, 유형의 차이는 다양한 병인에 따른 것으로 추정되며, 결핵성 흉벽 농양의 다양한 형태를 이해하는 것 이 진단 뿐만 아니라 병인을 이해하는데 도움이 될 것으로 생각된다. CT는 결핵성 농양의 진단 뿐만 아니라 농양의 범위, 동반된 폐실질및 흉막병변을 보는데 유용하다.
