대학생의 호흡기감염 예방행위에 영향을 미치는 요인 (Factors Influencing the Respiratory Infection Preventive Behavior among College Students)

  • 본 서술적 조사연구의 목적은 대학생의 호흡기감염 예방행위에 영향을 미치는 요인으로 건강신념과 호흡기 감염관리 자기효능감을 알아보고자 시도되었다. 대상자는 경상북도 K 시 일개 대학에 재학 중인 학생 178명으로 2020년 9월 1일부터 10월16일까지 구조화된 설문지로 자료를 수집하였다. 연구결과는 다음과 같다; 건강신념은 참여자의 성별(t=-2.86, p=.005), 전공계열(F=2.95, p=.034), 처방된 약물 복용 유무(t=2.18, p=.030)와 유의한 차이가 있었다. 호흡기 감염관리에 대한 자기효능감은 대학생의 성별(t=-3.56, p=<.001), 전공계열(F=4.59, p=.004)과 유의한 차이가 있었다. 건강신념(r=.276, p<.001)과 호흡기 감염관리에 대한 자기효능감(r=.195, p<.001)은 호흡기감염 예방행위에 정적 상관이 있었다. 다중회귀분석 결과 호흡기 감염관리에 대한 자기효능감(β=.66, p<.001)이 호흡기감염 예방 행동에 유의한 영향을 미치는 것으로 나타났고, 모형의 설명력은 43%로 나타났다. 따라서, 대학생의 호흡기감염을 예방하기 위한 행동을 촉진하기 위해서는 호흡기 감염관리에 대한 자기효능감을 강화할 수 있는 프로그램이 개발되어야 할 것이다.

Influence of time-of-day on respiratory function in normal healthy subjects

  • Purpose: Human body have biological rhythmic pattern in a day, which is affected by internal and external environmental factors. We investigated whether respiratory function was fluctuated according to the influence of time-of-day (around at 9 am, 1 pm, and 6 pm) in health subjects, using pulmonary function test (PFT). Methods: Eighteen healthy volunteers (8 men, mean ages; $22.4{\pm}1.6$, mean heights; $166.61{\pm}9.60$, mean weight; $59.3{\pm}10.3$) were recruited. Pulmonary function test (PFT) was measured at three time points in day, around 9 am, 1 pm, and 6 pm in calm research room with condition of under 55dB noise level, using a spirometer (Vmax 229, SensorMecis, USA). Forced vital capacity (FVC), forced expiratory volume at one second (FEV1), FVC/FEV1, and peak expiratory flow (PEF) were acquired. Results: In comparison of raw value of PFT among three time points, subjects showed generally better respiratory function at 9 am, than at other points, although no significance was found. In comparison of distribution of ranking for respiratory function in each individual, only PEF showed significant difference. In general, distributional ratio of subjects who showed best performance of respiratory function in a day was high. Conclusion: These findings showed that circadian rhythm by diurnal pattern was not detected on respiratory function throughout all day. But, best performance on respiratory function was observed mostly in the morning, although statistical significance did not exist.

피하기종과 종격기종을 동반한 홍역성 폐렴 1예 (A Case of Measles Pneumonia Complicated by Subcutaneous Emphysema and Pneumomediastinum in an 18-year-old Patient)

  • Background : A 18 year-old man presented with dyspnea and a swelling of the neck. On physical examination, maculopapular rashes were noted on the face and the whole body and crepitus was noted at the thorax and upper arms. His chest X -ray showed bilateral interstitial infiltrates of the lung, pneumomediastinum and subcutaneous emphysema. On serologic examination, measles IgM was positive. Under the diagnosis of measles pneumonia, the patient was treated with oral ribavirin, which resulted in a complete resolution of the pneumomediastinum, subcutaneous emphysema, pneumonic infiltrate, and subjective symptoms of dyspnea and swelling of the neck in 7 days. Here we report this case with a brief review of the relevant literature.

Novel respiratory infectious diseases in Korea

  • Respiratory infections are very common and highly contagious. Respiratory infectious diseases affect not only the person infected but also the family members and the society. As medical sciences advance, several diseases have been conquered; however, the impact of novel infectious diseases on the society is enormous. As the clinical presentation of respiratory infections is similar regardless of the pathogen, the causative agent is not distinguishable by symptoms alone. Moreover, it is difficult to develop a cure because of the various viral mutations. Various respiratory infectious diseases ranging from influenza, which threaten the health of mankind globally, to the coronavirus disease 2019, which resulted in a pandemic, exist. Contrary to human expectations that development in health care and improvement in hygiene will conquer infectious diseases, humankind's health and social systems are threatened by novel infectious diseases. Owing to the development of transport and trading activity, the rate of spread of new infectious diseases is increasing. As respiratory infections can threaten the members of the global community at any time, investigations on preventing the transmission of these diseases as well as development of effective antivirals and vaccines are of utmost importance and require a worldwide effort.

  • 결핵균은 공기를 매개로 전파되고 결핵균에 노출된 시간과 최초의 노출 정도가 전파의 위험을 증가 시키는 중요한 인자로 알려져 있다. 활동성 폐결핵 환자와 함께 생활하는 가족 내에서 감염이 현저히 증가하는데 이는 결핵균에 노출되는 기회 및 시간이 증가하기 때문이다. 하지만 고식적인 접촉자 조사만으로는 흔히 결핵 발생률을 과소평가할 수 있기 때문에 접촉자 조사와 결핵균의 유전형 조사를 병행하는 것이 감염의 위험이 높은 집단을 선별해내고 발생빈도 및 전염경로를 밝히는 데 보다 효과적인 것으로 보고되고 있다. 이에 접촉자 조사와 결핵균의 유전형 조사를 통해 가족 내 결핵균 전파를 증명한 1예를 보고하는 바이다.

  • This study was designed to analyze normal nasal respiratory resistance in prepubertal children. The subjects consisted of 30 prepubertal children (male: 15, female: 15). The mean age was 11.4 years in male children and 11.5 years in female children. The results were as follows: I. The normal nasal respiratory patency was lower than the normal values from RION corp. 2. The normal nasal respiratory airflow rates showed no sexual differences. And there were no differences between inspiration and expiration. 3. Before and after use of nasal decongestants, there were no significant differences of normal nasal respiratory airflow rates and after the administration of nasal decongestants, nasal respiratory patency manifested lower variability. 4. The normal nasal respiratory resistance without nasal decongestants at 150 Pascal in inspiration was $0.30Pa/cm^3/sec({\pm}0.07)$ and peak nasal inspiratory airflow rate was $1016.83cm^3/sec({\pm}223.89)$. 5. The normal nasal respiratory resistance with nasal decongestant at 150 Pascal in inspiration was $0.25Pa/cm^3/sec({\pm}0.05)$ and peak nasal inspiratory airflow rate was $1148.33cm^3/sec({\pm}234.29)$.

Different Responses to Clarithromycin in Patients with Cryptogenic Organizing Pneumonia

  • Cryptogenic organizing pneumonia (COP) is an idiopathic interstitial pneumonia characterized by a subacute course and favorable prognosis with corticosteroids. However, some patients show resistance to steroids. Macrolides have been used with success in those patients showing resistance to steroids. A few reports showed treatment failure with macrolides in patients with COP who were resistant to steroids. In this report, we described two cases of COP who showed different responses to clarithromycin. One recovered completely, but the other gradually showed lung fibrosis with clarithromycin.

A Comparative Study on the Pulmonary Function between Smoking Soldier and Non-smoking Soldier

  • Smoking can be a significant cause of lung diseases and reduced respiratory functions. Among soldiers, smoking may have a negative impact on their health (physical strength) and well being. Information on differences in the respiratory functions of smokers and nonsmokers in the military services and the effects of the smoking duration and amount (i.e., the number of cigarettes smoked per day) would be useful. This study investigated smoking durations and smoking amounts among young male soldiers (N = 61). The forced vital capacity (FVC), forced expiratory volume in 1 sec (FEV1), and forced expiratory volume in 1 sec/forced vital capacities (FEV1/FEC) were measured FVC, FEV1, or FEV1/FEC of smokers and nonsmokers were not significantly different, and FVC and FEV1 were inversely proportional to smoking duration. Besides, the number of cigarettes smoked per day was not correlated with respiratory functions. These findings may be attributed to the effect of the strenuous physical activity (e.g., military drills) undertaken by soldiers on their respiratory functions. Despite the lack of evidence for a difference in the respiratory functions of smokers and nonsmokers, this study recommends ongoing respiratory function management through smoking cessation programs and respiratory physiotherapy to manage the respiratory functions of Korean smoking soldiers.

Correlations between the Respiratory Function and Cervical and Lumbar Range of Motion in Elderly People

  • PURPOSE: The respiratory function and range of joint mobility deteriorate due to aging, respectively, and the respiratory function may be related to the range of joint mobility. To the best of the authors' knowledge, there has been little research on how the deterioration of the range of joint mobility caused by aging is related directly to the respiratory function. This study investigated the relationship between the respiratory function and the range of joint motion in the cervical and lumbar regions of older adults. METHODS: Thirty-two older adults participated in the study. The forced vital capacity (peak expiratory flow: PEF and forced expiratory volume in 1 second: FEV1) and the range of cervical joint motion were assessed in all subjects. The results were analyzed using the Pearson's correlation coefficient. RESULTS: The respiratory function moderately correlated with age. A moderate correlation was observed between FEV1 and right cervical lateral flexion. PEF was moderately correlated with the lumbar lateral flexion. In addition, FEV1 correlated strongly with lumbar lateral flexion. CONCLUSION: The respiratory function was correlated with the cervical and lumbar lateral flexion. Therefore, rehabilitation of the respiratory function should involve improving the limited range of motion of the joints, and a stretching program is needed for older adults.

The Effect of Dynamic Neuromuscular Stabilization (DNS) on the Respiratory Function of Subjects with Forward Head Posture (FHP)

  • PURPOSE: The purpose of this study was to apply dynamic neuromuscular stabilization (DNS) to subjects with forward head posture (FHP) and to compare its effects on respiratory function as against the conventional neck stabilization exercise and neck stretching and extensor strengthening exercises. METHODS: The whole-body posture measurement system was used to measure the degree of FHP, and a spirometer and a respiratory gas analyzer were used to measure the respiratory function. After the intervention was completed, the changes over time were analyzed in the DNS group, the neck stabilization exercise group, and the neck stretching and extensor strengthening exercise group. The inter-group difference in the changes was also analyzed. A repeated ANOVA was performed to compare the respiratory function according to the period between the three groups, and the least significant difference (LSD) method was used for the post hoc test. RESULTS: After the 6-week exercise period, respiratory functions, such as forced vital capacity (FVC), forced expiratory volume for 1 second (FEV1), forced expiratory volume for 1 sec/forced vital capacity (FEV1/FVC), maximum oxygen intake (VO2max), and the volume of expired gas (VE), significantly improved according to the period (p < .05), but no inter-group differences were found. CONCLUSION: DNS is an effective training method, and can be applied along with neck stabilization exercise and neck stretching and extensor strengthening exercises, which are widely used in clinical practice, to people with FHP who cannot directly perform neck exercises to improve their respiratory function.
